근위 대퇴 골수정을 이용한 불안정성 대퇴 전자간 골절의 치료

목 적: 최근 개발된 근위 대퇴 골수정 (proximal femoral nail)을 이용한 불안정성 대퇴 전자간 골절의 치료 후 단기 추시 결과를 분석하여 보고하고자 한다. 대상 및 방법: 불안정성 대퇴 전자간 골절에서 근위 대퇴 골수정을 이용하여 수술적 내고정을 시행한 47예를 대상으로 임상적 및 방사선적 결과와 합병증을 평가 하였고, 수술 당시 평균 연령은 76.8세 (62~96세)였고, 평균 추시 기간은 약 15개월(12~24개월)이었다. 결 과: 술 후 보행능력은 타인의 도움 없이 보행대를 사용하여 혼자 걸을 수 있는 정도를 만족하다고 판단하였으며, 전체 47명 중 43예 (91.5%)에서 만족할 만한 결과를 얻었고, 최종 추시상 모든 예에서 골 유합을 얻었으며, 대퇴 경부 나사의 활강은 평균 3.0㎜활강을 보였고, 대퇴골의 대퇴 경간각의 변화는 평균 2.6도를 보였다. 술 후 합병증으로 1예(2.1%)에서 대퇴 경부 나사의 대퇴 골두 천공 (cut-out)을 보였고, 슬부 표재성 국소 감염이 1예(2.1%), 대퇴 전자부의 통증을 호소한 경우가 1예(2.1%) 있었다. 결 론: 근위 대퇴 골수정은 불안정성 대퇴 전자간 골절의 치료시 내고정 장치와 관련된 합병증이 다른 내고정 장치에 비해 적고 술기 또한 간단하여 대퇴 전자간 골절의 치료시 좋은 내고정 장치로 사료된다. Purpose: To review the clinical and radiographic results of the treatment of unstable intertrochanteric femoral fractures with a proximal femoral nail(PFN). Material and Methods: We reviewed 47 unstable intertrochanteric femoral fracture cases that had been treated with a PFN operatively. The clinical and radiographic results and complications were analyzed. The mean age was 76.8 years old(62~96 years old) and the mean duration of follow-up was 15 months(12~24 months). Results: The postoperative walking ability was regarded as satisfactory when the patient could walk alone using an walking frame without other aids and satisfactory results was achieved in 43 cases(91.5%). In all cases the radiologic bone change of neck-shaft angle was 2.6 degree. There were three cases of postoperative complication which were including 1 case of cut-out of femoral neck screw, local superficial in 1 case and pain complaints over trochanteric area in 1 case. Conclusion: The PFN is an useful implant for the treatment of unstable intertrochanteric femoral fracture because of the simplicity of the surgical technique and the low level of the complications encountered.
