        Until now, the Saemaul Campaign has greatly contributed to the development of Korean economy. However, it seems that the movement must be continued on a higher level of spirit and technology in so far as we are now in the international environment of so called science war or trade war. In other words, we must confirm the tradition of our national culture and solidify the pride of our nation in order to launch into the awkward international society. Thus, this study deals with the following: 1. At an early age, our nation had been called the East­foreigner by China and bad been anthropologically called the Manchu­Korean of Tungus origin. Korean and Chinese had coexisted until before the Jin had unified the six countries, and it had been recognized that the East­foreigner’s culture had been superior to the Chinese culture until the beginning of the Eun. 2. The tradition of Korean Buddhism had been fixed with the folkreligion in the United Silla Dynasty. WonHyo’s thought of Hwajaeng that had advocated the unification of Buddhism, peace, reconciliation and equality had been remained in existence through EuyChon, the highest rank of priest in the Goryeo Dynasty to Hye Jang and ChoEuySunSa of the end of the Old Korea. 3. From the Triple period to the Goryeo period, the doctrine of Buddhism workship had been continued, but the real nature of the political road had been put on the doctrine of Confucianism based upon the reasonable politics of royal road. Expecially, the appearance of the principle of IGi had resulted in the recreation of Korean though based upon Sung Confucianism, and it had become the background of SilHak thought.

        Surgical Treatment of Inferior Vena Cava Tumor Thrombus in Patients with Renal Cell Carcinoma

        Radical nephrectomy with inferior vena cava (IVC) thrombectomy remains the most effective therapeutic option in patients with renal cell carcinoma and IVC tumor thrombus. Cephalic extension of the thrombus is closely related to perioperative morbidity. We purposed to design a safe and successful surgical strategy through a review of our surgical experience and treatment results in 35 patients (male:female=28:7,mean age=56 yr [32-77]) who underwent IVC thrombectomy with radical nephrectomy between January 1997 and December 2006. The limit of tumor extension was level I in 10 patients (28.6%), level II in 17 (48.6%), and level III and IV in 4 patients each (11.4%). Liver mobilization with hepatic vascular exclusion was performed in 12 patients and cardiopulmonary bypass in 7. Thirty-two primary closures, 2 patch closures, and 1 graft interposition were performed. One patient underwent simultaneous pulmonary embolectomy because of an operative pulmonary embolism. There was no operative mortality, and the overall survival at 5-yr was 50.8%. Complete thrombus removal without tumor fragmentation under long venotomy on fully exposed involved IVC is recommended for successful result in a bloodless operative field. The applicability of liver mobilization, hepatic vascular exclusion, and cardiopulmonary bypass, can be determined by the level of thrombus.

        The lowest local governing body during Yi Dynasty was Sooryong and there used to be a government-controled autonomous organization Sashim-gwan, Yuhyang-so in towns and villages. This self governing organization, however, was never more than a leading agency of mayorship, sooryong. Since the reign of King Joongjong of Yi Dynasty, such distinguished figures as Yi-Taege, Yi=Yulgok, and An-Soonam had developed the organization into a democratic and edifying one owing to hyangyak, rural community agreements. It is, however, inevitable to call the organization somewhat government-controlled rather than self governed. Influenced by the hyangyak, jonghoi, clan’s counsil, growth through the process of homologous villages in ancient Korea was developed to such an extent as to be called an autonomous organization. The clan’s agreement were provided on the basis of the Chinese book “JooJa-Kare”and Jeongja’s thoughts of Song Dynasty in China. Thanks to the agreements, family clan maintain their dignity among themselves, regardless of class, poverty, wealth etc. The clan community whose structure was somewhat similar to that of feudal society had severer disciplines that those of any other self-gover­ning bodies, and the members of the clan community felt the worst disgrace if excluded from the community. In view of the above fact, jonghoi, whose function was more democratic and spontaneous than any other autonomous organization, has developed from the lowest stratum of Korean society.

        상장간막 동맥 동맥류 박리 1예 : 증례 보고

        Aneurysmal disease of visceral arteries is found in only 0.2% of the general population. Aneurysms that involve the splenic artery account for more than 60% of all visceral aneurysms^(8,10). The hepatic artery is involved in 20%, whereas the celiac trunk is involved in 4.5% and the superior mesenteric artery in 5.5%. The remaining 10% include those of the branches of the superior' mesenteric artery, the gastroduodenal, the pancreaticoduodenal arteries and the inferior mesenteric artery^(5,13). The etiology of visceral aneurysms include atherosclerosis, medial degeneration, infection, fibromuscular dysplasia, dissection and congenital anomalies^(2,12,13). The size and anatomic location of the aneurysm should be considered to determine the correct surgical treatment. The authors report a case of aneurysmal dissection of the superior mesenteric artery in a 56-year-old male patient who had acute abdominal pain and massive melena. Resection of dissecting aneurysm and PTFE interposition was performed, and post-operative course was uneventful.
