        중등도 무지외반증에서 시행한 단축 스카프 절골술

        Purpose: The aim of this study was to evaluate the radiographic and clinical results of short scarf osteotomy that has minimized longitudinal cut for moderate hallux valgus. Materials and Methods: Total 12 patients (12 feet) were reviewed by medical records and radiographs. All patients were female and the mean age at the time of operation was 41.5 years. The mean followup time was 21.2 months. We modified original scarf osteotomy by shortening the longitudinal cut to 15~20 mm in length. Additionally, Akin osteotomy of the first proximal phalanx was done in 7 feet and Weil osteotomy of the second metatarsal was done in 4 feet. First-second intermetatarsal and hallux valgus angles were analyzed radiographically before and after the operation. And the clinical result was assessed by AOFAS (American Orthopaedic Foot and Ankle Society) hallux score. Results: First-second intermetatarsal and hallux valgus angles were reduced from the mean preoperative values of $14.6^{\circ}$ and $32.8^{\circ}$ to $6.5^{\circ}$ and $11.2^{\circ}$, respectively. The mean AOFAS hallux score was increased from 52.4 points preoperatively to 88.2 points at followup. Three complications were found: metatarsal fracture during the operation, painful scar around second metatarsal head after Weil osteotomy and postoperative neuralgia. There was no transfer metatarsalgia or recurrence of hallux valgus during followup. Conclusion: Short scarf osteotomy would be an effective surgical procedure for moderate hallux valgus with the benefits of minimized soft tissue dissection and stable fixation.

      • 고속스핀에코기법을 이용한 MRI검사에서 ETL 변화에 따른 영상 blurring의 평가

        The purpose of this study is to evaluate image blurring according to variation of the ETL and propose the clinically appropriate ETL range. SIEMENS MAGNETOM Skyra 3.0T and 20 channel head coil were used for the study. MRI phantom was kept the lines horizontally to three direction(X,Y,Z) of the coil and T1, T2 weighted images that used the fast spin echo technique acquired. The ETL with increase of 10 was applied from 10 to 80. In addition, the ETL with increase of 1 was applied in the interval statistically significant differences occurred. And T1, T2 weighted images that used the conventional spin echo technique acquired to compare image blurring of the images that used the fast spin echo technique. The slope of lattice in the images was measured using Image J 1.47v program to evaluate image blurring. And image blurring was determined by the degree of the slope. The statistical significance of both techniques was evaluated by the Kruskal-Wallis test of the SPSS 17.0v. And the correlation of the ETL and image blurring was evaluated quantitatively by regression analysis. The slope of the T1, T2 weighted images that used fast spin echo technique decreased as contrasted with conventional spin echo technique. In the result of the Kruskal-Wallis test, the T1, T2 weighted images that used fast spin echo technique made a significant difference with conventional spin echo technique. Particularly, in the Tomhane' T2 test, the T1, T2 weighted images made a significant difference from ETL 22 and 31 respectively. In the result of the regression analysis, the R-squared of the T1, T2 weighted images are 0.762 and 0.793. It is difficult to apply the long ETL in the T1 weighted image caused by the short TR and multi-slices study. Therefore, clinical impact according to variation of the ETL is very slight in the T1 weighted images. But the application of the proper ETL is demanded in T2 weighted images using the fast spin echo technique in order to prevent image blurring.

        This study was carried out to evaluate the zinc phosphatibility of steel, Zn-Ni coated steel and aluminum alloy<br/> sheets. Paint adhesion tests shows that the zinc phosphating conversion coatings formed on steel, Zn-Ni coating and aluminum surface performed excellent and satisfied the requirements, except<br/> one type of aluminum sheets. The inferiority of this aluminum sheet was found to be caused by a thick MgO oxide layer and insufficient degreasing of the surface. It should be noted that scratches on the surface inhibited the zinc phosphating of aluminum sheets.<br/> Key Word: Zinc Phosphating, Aluminum, Steel, Zn-Ni Coated Steel, Paint Adhesion, Oxide Film, Scratch.<br/>

        Evaluation of Reproducibility According to Variation of Coil Channel in Quantitative Evaluation Using Synthetic MR

        최신 영상 기법인 MAGiC sequence의 RF coil channel 수 변화에 따른 T1, T2 이완 시간 및 T1, T2 이완율 을 측정하여 분석하였고 그에 따른 재현성을 평가하였다. Channel 수가 각각 (1, 8, 16, 32)channel 인 RF coi l에 조영제 phantom(1.0, 0.6, 0.2, 0mM)을 위치시키고 MAGiC sequence를 이용하여 T1, T2, R1, R2 map을 획득하였다. 그리고 각각의 channel 수와 농도별로 T1, T2, R1, R2 value 그리고 relaxation rate를 측정하였고 이를 기반으로 각 농도에 따른 Relaxivity를 구하였다. 각각의 coil에서 T1, T2, R1, R2 value를 측정한 결과 T1, R1 value는 coil간의 유의한 차이가 없었다(p>0.05). 하지만 T2, R2 value는 유의한 차이가 있는 것으로 나타났다(p<0.05). 사후분석 결과에서는 T2 value의 경우 1 channel coil에서 측정한 결과가 8, 16, 32 channel coil에서 측정한 결과와 유의한 차이가 나타났고(p<0.05) 8, 16, 32 channel coil간의 차이는 나타나지 않았다 (p>0.05). R2 value의 경우도 마찬가지로 1 channel coil에서 측정한 결과가 8, 16, 32 channel coil에서 측정한 결과와 유의한 차이가 나타났으며 또한 8 channel coil에서 측정한 결과와 16 channel coil에서 측정한 결과 가 유의한 차이가 나타났다(p<0.05). 결론적으로 T1, R1 value는 coil의 channel 수 변화에 따른 차이가 크지 않지만 T2, R2 value는 큰 차이를 나타냈다. 따라서 MAGiC sequence를 이용하여 T2, R2 value를 정량적으로 측정할 경우 재현성을 위해 동일한 channel 수의 coil을 사용해야 할 것으로 사료된다. T1, T2 relaxation time and relaxation rates were measured and analyzed according to the change of RF coil channel number of MAGiC sequence. T1, T2, R1 and R2 maps were obtained by using MAGiC sequence with phantom (1.0, 0.6, 0.2, 0 mM) on the RF coil with channel number of 1, 8, 16 and 32 respectively. T1, T2, R1, R2 values and relaxation rates were measured for each channel number and concentration, and Relaxivity was calculated according to each concentration. T1, T2, R1, and R2 values were measured in each coil. There was no significant difference between T1 and R1 values (p> 0.05). However, T2 and R2 values were significantly different (p <0.05). In the post-analysis results, T2 value was significantly different from that measured on 1, 8, 16, and 32 channel coils (p <0.05) and There was no difference between 8, 16, and 32 channel coils (p> 0.05). The R2 value was significantly different from that measured on the 8, 16, and 32 channel coils in the 1 channel coil, and the results on the 8 channel coils and the 16 channel coils showed a significant difference (P <0.05). In conclusion, T1 and R1 values were not significantly different according to the number of channels in the coil, but T2 and R2 values were significantly different. Therefore, when quantitative measurement of T2 and R2 values using the MAGiC sequence, the same number of coils should be used for reproducibility.

        스톡옵션의 특성에 따른 장기성과

          본 연구에서는 다양한 장기성과 측정모형들을 이용하여 스톡옵션의 특성에 따른 장기성과의 변화를 분석하였다. 분석기간을 1998년부터 2004년 동안으로 하여 101개 기업을 표본기업을 대상으로 벤치마크 포트폴리오를 종합주가지수, 산업지수, 대응기업(SIZE-BE/ME), 대응기업(BE/ME-SIZE)로 하여 ???? 과 ???? 을 측정하였고 또한 Fama and French(1993)의 3요인 모형도 이용하였다.<BR>  분석결과 스톡옵션 부여 공시일 부근에서 유의적인 양(+)의 초과수익률을 발견할 수 있었다. 하지만 스톡옵션 부여 이후 장기성과는 비유의적인 양(+) 또는 음(-)의 초과수익률을 보이고 있어 시장이나 대응기업과 비교해 유의적인 차이를 보이지 않았다. 또한 3요인 모형에서는 월평균 초과수익률을 나타내는 회귀계수 ? 가 마찬가지로 비유의적인 값으로 나타났다.<BR>  스톡옵션의 특성별로 장기성과를 분석한 결과 행사기간이 4~5년인 경우에 장기성과가 다른 행사 기간에 비해 상대적으로 높게 나타났고, 스톡옵션이 임원 및 직원에게 모두 부여한 경우가 마찬가지로 상대적으로 높았다. 주가차액보상은 옵션의 행사 후 지속적인 인센티브 효과를 기대할 수 없으나 분석결과에서는 오히려 장기성과가 높게 나타났다. 이러한 결과는 우리나라의 경우 스톡옵션이 대리인 문제를 완화하기 위한 성과보상의 성격을 갖고 있다는 것 보다는 확정보수로 인식될 가능성도 있다는 것을 시사하고 있다. 일반선형분석에서도 상기 분석결과들을 재확인할 수 있었으며, 부여대상과 행사방법 간의 상호작용이 유의하다는 것도 발견할 수 있었다.   The main purpose of this study was to investigate the relationship between the market long-term performance and the characteristics of the stock option by using the Korean Stock Market data from 1998 through 2004. We used exercise period, extent of entitlement, exercise method as the characteristics of the stock option. We measured the performance by the event-time portfolio approach and the calendar-time portfolio approach for avoiding the misspecification model problem.<BR>  In the first of our main results, there was no evidence of the improvement in the long-term performance for the firms granting stock option relative to benchmarks. This result was consistent with existing studies representing the stock option do not make the firms perform significantly better. Second, there was insignificant differences of the long term performance between exercise period. However, the long-term performance of the stock option of 4~5year exercise period outperformed relatively the stock option of other exercise period. Third, the stock option entitled to CEOs, directors and general managers had the positive influence on the performance of firms relative to only executives. Finally, stock appreciation right had only the significant long-term abnormal returns following the stock option announcement among the exercise method. The results suggested that the stock option was recognized by the fixed compensation in Korea. In addition, the evidence on the general linear model confirmed the results.

        Osteonecrosis of the Femoral Head Associated with Butane Gas Inhalation

        저자들은 부탄가스 흡입으로 인해 조직 소견상 대퇴골두의 심한 울혈을 보이는 괴사증 2예를 보고하였으며, 이에 그 병인을 밝히기 위하여 본 동물실험을 계획하였다. 특별히 고안된 상자를 통해 15마리의 토끼에 대하여 8주간 부탄가스를 흡입 시켰으며, 혈액학적 검사 및 대퇴골두의 조직학적 검사를 시행하였고, 그 결과를 대조군과 비교하였다. 혈액학적 검사에서 혈중 콜레스테롤과 triglyceride의 증가를 보였으며, 대퇴골두에 대한 조직학적 검사에서 골내혈관들의 심한 울혈과 함께, 혈관벽 기저막의 손상을 관찰할 수 있었다. 그러므로, 부탄가스 흡입은 대퇴골두의 무혈성 괴사에 대한 하나의 새로운 원인 인자로 생각되어 질 수 있을 것이다. We have reported two clinical cases of osteonecrosis of the femoral head caused by butane gas inhalation with the characteristic findings of severe congestion of the intraosseous vessels, and we would like to illustrate the pathologic mechanism in an animal study. Butane gas was administered to fifteen rabbits using a specially designed box. Blood chemistry including triglycerides and lipoprotein electrophoresis were performed. The changes in the of blood vessels and marrow of the proximal femur were subject to histopathologic study. The levels of triglyceride and total cholesterol were increased after butane gas inhalation. Disruption of the basement membrane of veins with thrombosis and thrombus of the small to medium sized vessels were observed in and around femoral head. Severe congestion of intraosseous blood vessels was also observed within the femoral head of the rabbits. In conclusion, butane gas inhalation should be considered as a new etiology for the development of osteonecrosis of the femoral head.
