        정맥주입요법 간호실무지침 개정

        Purpose: This study was conducted to update the existing nursing practice guideline for intravenous infusion guidelines according to the evidence-based practice guideline in South Korea. Methods: Guideline update process was performed using 22 steps according to the manuals developed by NICE and SIGN. Results: Updated nursing practice guidelines for the intravenous infusion were consisted of 23 domains and 322 recommendations. The number of recommendations in each domain were 4 for general instruction, 12 for vascular access device selection, 20 for site selection, 9 for insertion, 54 for stabilization, 21 for maintaining patency, 4 for blood sampling, 33 for exchange and removal, 28 for add-on device selection, 28, 72 for infusion related complications, 56 for infusion therapies, 7 for education, and 2 for documentation and report. There were 15.9% of A, 30.2% of B, 53.9% of C in terms of grade recommendations. A total of 178 (51.6%) recommendations were newly developed and 24 previous recommendations have been deleted. Conclusion: Updated nursing practice guideline for intravenous infusion was expected to be an evidence-based practice guideline for intravenous infusion in South Korea. This guideline is suggested to be disseminated to clinical nursing settings nationwide to improve the efficiency of intravenous infusion practice.

        직무요구-자원모형을 기반으로 한 요양병원 간호사의 소진에 대한 영향요인 분석

        본 연구는 Xanthopoulou 등(2007)의 확장된 직무요구-자원모형을 기반으로 요양병원 간호사의 소진에 미치는 영향요인을 규명하기 위해 시도되었다. 요양병원 간호사 167명을 대상으로 설문지를 이용하여 2018년 8월 25일부터 2018년 9월 6일까지 자료를 수집하였다. 연구도구는 Maslach Burnout Inventory, Copenhagen Psyco-social Questionnaire version II, Connor-Davidson Resilience Scale 및 Thomas & Ganster (1995)가 개발한 도구를 사용하였다. SPSS WIN 23.0 프로그램을 이용하여 빈도, t-test, ANOVA, 피어슨 상관계수, 위계적 회귀분석으로 분석하였다. 연구결과 요양병원 간호사의 소진은 직무요구(r=.24, p=.002)와는 정적 상관관계를 보이고, 회복탄력성(r=-.43, p<.001)과 사회적지지(r=-.41, p<.001)와는 부적 상관관계를 보였다. 요양병원 간호사의 소진에 회복탄력성(β=-.28, p<.001), 사회적 지지(β=-.20, p=.016), 직무요구(β=.17, p=.016) 순으로 유의한 영향을 미쳤고, 이들 변수의 설명력은 32.0%로 나타났다. 따라서 요양병원 간호사의 소진 예방 및 감소를 위해 직무요구를 줄이고 회복탄력성과 사회적 지지를 향상시킬 수 있는 중재의 수행을 적극적으로 권장한다 This study was conducted to investigate the factors influencing burnout of nurses in long-term care hospitals based on the expanded JD-R model (Xanthopoulou et al, 2007). Participants were 167 nurses who worked in long-term care hospitals. The instruments used in this study were the Maslach Burnout Inventory, Copenhagen Psycho-social Questionnaire version II, Connor-Davidson Resilience Scale and the instrument developed by Thomas and Ganster (1995). Data were collected from August 25 to September 6, 2018 and analyzed based on frequency, t-tests, ANOVA, Pearson"s correlation, and hierarchical regression using the SPSS WIN 23.0 program. The results were as follows: Nurses" burnout in long-term care hospitals was positively correlated with job demands (r=.24, p=.002) and negatively correlated with resilience (r=-.43, p<.001) and social support (r=-.41, p<.001). Factors influencing the level of burnout were resilience (β=-.28, p<.001), social support (β =-.20, p=.016), and job demands (β=.17, p=.016) ), which together explained 32.0% of the total variance in burnout. Therefore, we strongly recommend interventions that reduce job demands and enhance resilience and social support to prevent and reduce the burnout of nurses in long-term care hospitals.

        간호대학 저학년생의 신체활동정도와 영향요인

        본 연구의 목적은 간호대학 저학년생의 신체활동 정도와 신체활동에 미치는 영향요인을 파악하여 간호대학 저학년생의 신체활동의 증진 및 유지 전략을 개발하는데 기초자료를 제공하기 위함이다. 연구대상은 U시에 소재한 일개 간호학과에 재학 중인 1, 2학년 여자 간호대학생 234명이었고, 자료수집 기간은 2016년 9월 5일부터 9월 15일까지였다. 연구도구는 한국판 신체활동 수준 질문지, 계획된 행위이론 변수 질문지, 신체활동 사회적지지 질문지, 신체활동 습관 질문지를 사용하였다. 자료 분석은 IBM SPSS/WIN 22.0을 사용하여 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, 사후검증, Pearson’s correlation coefficient, Stepwise multiple regression을 이용하였다. 연구결과 간호대학 저학년생의 신체활동은 평균 9.09점(범위 0-80)이었으며, 신체활동을 leisure score index(LSI)에 따라 분류한 결과, 활동군은 25명(10.7%), 중정도 활동군은 31명(13.2%) 및 비활동군은 178명(76.1%)으로 나타났다. 간호대학 저학년생의 신체활동에 영향을 미친 변수는 신체활동 의도, 신체활동 습관으로 나타났으며, 설명력은 총 21.2%(F=31.03, p<.001)였다. 따라서, 간호대학 저학년생을 대상으로 신체활동의도와 신체활동 습관을 증진시키는 신체활동 프로그램을 개발하고 효과를 검증하는 연구가 필요하다. This study was conducted to investigate the level of physical activity and identify factors influencing physical activity of first and second year nursing students. The participants were 234 female first and second year nursing students at one nursing college located in U city. Data were collected from September 5 to 15 in 2016 and analyzed by descriptive statistics, t-test, ANOVA, Scheff? test, Pearson’s correlation coefficient, and stepwise multiple regression using SPSS/WIN 22.0. The average level of physical activity measured by the Korean version of the Godin-Shephard Liesure-Time Exercise Questionnaire was 9.09(range 0-80). According to the physical activity group classified by leisure score index, there were 25 students(10.7%) in the active group, 31 students (13.2%) in themoderately active group, and 178 students(76.1%) in theinsufficiently active group. Factors influencing the level of physical activity were intention to participate in physical activity and habit to physical activity, which together explained 21.2% of the total variance inphysical activity. Therefore, this study suggests that it is necessary to develop and apply a physical activity program enhancing intention and habit to physical activity for first and second year nursing students.

        간호대학생을 위한 복합운동과 운동습관형성 프로그램 개발 및 효과검증

        Purpose: This study was conducted to develop and test the effects of a combined exercise and exercise habit formation program for undergraduate nursing students. Methods: A non-equivalent control group pre-post test design was used. The participants were 44 students (experimental group: 23, control group: 21) who had not done regular exercise for over 3 months before this study. The program was developed based on theory of planned behavior and the habit formation model. The program consisted of 24 sessions for 8 weeks and included combined exercise (aerobic exercise 30 minutes, resistance exercise 20 minutes, and flexibility exercise 10 minutes), exercise intention promotion strategy and exercise habit formation strategy (30 minutes, weekly). Data collection was done before, and immediately after the program: September 4 to October 29. Data were analyzed using t-test, paired t-test, x² test, Mann-Whitney U test, and Wilcoxon signed rank-sum test with SPSS/ WIN 21.0 program. Results: Exercise intention, exercise habit strength, exercise behavior, and physical fitness (cardiopulmonary endurance, muscle strength, and muscle endurance) were significantly better and stress and fatigue were significantly lower in the experimental group compared to the control group. Conclusion: The study findings indicate that the combined exercise and exercise habit formation program for undergraduate nursing students was effective and can be recommended as an intervention for improving healthy lifestyle of undergraduate nursing students.

        슬관절 전치환술 노인 환자를 위한 정맥혈전색전증 예방교육의 효과

        Purpose: This study was conducted to develop and test the effects of venous thromboembolism prevention education for elderly patients with total knee arthroplasty. Methods: A non-equivalent control group posttest only non-synchronized design was used. The participants were 43 older adults (experimental group: 21, control group: 20) who had experienced total knee arthroplasty surgery at *university hospital located in *city. Venous thromboembolism prevention education consisted of a 20 minute intervention on the evening before the operation and a 5 minute intervention on the operation day including education and strategies for enhancing health belief. Data were collected from October 20, 2017 to March 10, 2018. Data were analyzed using independent t-test and Mann-Whitney U test with SPSS/WIN 24.0. Results: The experimental group showed significantly higher levels of knowledge, self-reported preventive behaviors, and number of ankle movements compared to the control group. Conclusion: The study findings indicate that venous thromboembolism prevention education can be recommended as an effective nursing intervention for preventing venous thromboembolism of elderly patients with total knee arthroplasty.

        노인 당뇨병 환자를 위한 동기강화상담 당뇨병 자기관리 프로그램의 개발 및 효과

        Purpose: This study was conducted to develop and test the effects of a motivational interviewing self-management program for use with elderly patients with diabetes mellitus. Methods: A non-equivalent control group pretest-posttest design was used. The participants were 42 elderly diabetic patients (experimental group: 21, control group: 21). The motivational interviewing self-management program for elders with diabetes mellitus developed in this study consisted of a 12-week program in total (8 weeks for group motivational interviewing and education and 4 weeks for individual motivational interviewing on the phone). Data were collected between February 13 and May 3, 2013 and were analyzed using t-test, paired t-test, and repeated measure ANOVA with SPSS/WIN 18.0. Results: For the experimental group, significant improvement was found for self-efficacy, self-care behavior, glycemic control and quality of life (daily life satisfaction, influence of disease) as compared to the control group. Conclusion: The study findings indicate that the motivational interviewing self-management program is effective and can be recommended as a nursing intervention for elderly patients with diabetes mellitus.
