      • 정적인 스트레칭과 Evjenth-Hamberg 스트레칭이 슬관절 등속성 근력에 미치는 효과

        Purpose: The purpose of this study was to examine the effects of static stretching and Evjenth-Hamberg stretching on isokinetic muscle strength of knee flexors and extensors. Methods: The subjects were composed of eighty healthy males without weight-training experience. ROM of knee joint measured active maximal extension and isokinetic Peak Torque measured $60^{\circ}/sec,\;120^{\circ}/sec$ using an the En-Knee. Three tests(Baseline, 4 weeks, 8 weeks, respectively) was operated to examine change of each variable. Data were analyzed with a $2{\times}3$ analysis of variance ($group{\times}test$) for repeated measures on last factor by SPSS package 10.0. The data analysis revealed muscle strength were dependent on stretching method. Results: The results were as follows. First, Evjenth-Hamberg stretching(E-HS) was more effective than static stretching(SS) on ROM. Second, Peak Torque of knee flexors and extensors was improved in both methods by each time. but E-HS was more improved than SS. Conclusion: In conclusion, This study indicates that E-HS is more efficient than SS on muscle strength improvement.

        Landau-Kleffner Syndrome(Acquired Epileptic Aphasia)에 대한 임상적 연구

        연구배경 : 소아에서의 경련발작과 후천성 감각성 실어증을 특징으로 하는 “Landau-Kleffner증후군”의 임상적 특징을 알기 위하여 본 연구를 시행하였다. 방법 : 1991년 3월부터 1996년 2월까지 만 5년 동안 서울중앙병원 소아과와 서울대학병원소아과에 경련발작과 후천성 실어증을 주소로 입원한 환아중 “Landau-Kleffner 증후군” 으로 진단 받았던 6명의 환아를 대상으로 그들의 임상양상과 뇌파소견 뇌 MRI 소견, 청각유발전위검사, 뇌 PET 소견, 임상 경과 등을 살펴보았다. 결과: 1) 환아는 6례 모두 남아였고, 경련발작 발현 연령은 생후 8개월부터 5년 11개월까지, 후천성 감각성 실어증 발현연령은 생후 12개월부터 6년 2개월까지였다. 2)뇌파검사상 6례 모두에서 양측성 혹은 일측성 측두엽을 포함하는 극파 및 에파가 관찰되었다. 3) 진단 당시에 시행한 뇌 MRI 검사와 청각유발전위검사에서는 특이 소견이 없었고, 2례에서 시행할 수 있었다. 뇌 PET검사에서는 1례에서는 양측 측두-두정엽, 1례에서는 거의 모든 대뇌영역에 걸친 포도당의 대사가 저하된 소견이 발견되었다. 4) 추적관찰기간동안 경련발작은 3례(50%)는 추적관찰 기간까지 난치성 경과를 취하고 있었고, 실어증은 5례(83.3%)에서 언어장애를 가지고 있었다. 결론 : 경련발작과 후천성 감각성 실어증을 보인 “Landau-Kleffner 증후군”은 교육을 많이 받아야할 어린 시기에 발현하고 그 경과가 좋지 않으므로, 정상적인 성인으로 성장하는데 큰 장애가 될 수 있는 질환으로 생각된다. 뇌 PET 소견이 이 질환의 진단 및 병인 이해에 도움을 줄 수 있을 가능성을 확인할 수 있었지만, 향후 이 환자들에 대한 보다 장기적인 관찰 및 이 질환의 조기 진단 및 적극적 치료방법에 대한 연구가 있어야 하겠다. Background : The Landau-Kleffner Syndrome, or acquired epileptic aphasia is a relatively rare disorder characterized by epilepsy and progressive language loss with and apparent loss of auditory verbal understanding(agnosia) and speech. We carried out the clinical study it to know its clinical characteristics. Methods : During the past five years (from March 1991 to February 1996), six paatients were diagnosed as Landau-Kleffner Syndrome at the department of Pediatrics of Asan medical center and the Department of Pediatrics of Seoul National University Hospital in Korea. We reviewed their clinical manifestations and courses, EEG, Brain MRI, Brain-stem Auditory Evoked Response(BAER), and Brain Positron Emission Tomography (PET) finding. Results: Patients were all male children. Their seizures had begun between 8 months and 5 years and 11months of age, and sensory aphasia between 12 months and 6 years and 2 months of age. EEG showed both, left or right temporal spike or sharp wave discharges. The findings of brain MRI and BAER at the time of diagnosis were normal or unexplainable. but brain PET(checked in two patients) disclosed decrease of cerebral glucose utilization involving both temporo-parietal in one patient and diffuse cortical areas in another. During the follow-up period(1 to 5 years), the seizure remains intractable in 3 cases(50%)and aphasia persists in 5 cases(83.3%). Conclusion : As the onset of Landau-Kleffner Syndrome begins in childhood and ots course is not benign, especially in aphasia, so the disease have much possibility of an obstacle to becoming a normal adult. In addition to it, we confirmed the possibility of an utility of brain PET to make an early diagnosis and understand the pathophysiology of Landau-Kleffner Syndrome

        척추관협착증 환자에서 수중보행 훈련이 운동기능 및 보행에 미치는 효과

        Objective: Investigate the effects of Underwater Gait training on the balance in a spinal stenosis patient. Method: A Patient who had suffered from the spinal stenosis performed gait training with a Underwater Gait training. We tested Underwater Gait training program on subject 30 min per course, 5 days a week for 8 weeks. Before and after the experiments, we measured balance and motor functional ability and got the result of this following. After Underwater Gait training, difference of COP moving area and Kpa of both feet increased. Results: The results of this study showed that gait training with an Underwater treadmill, after training, had meaningful difference of COP moving area and Kpa of both feet. Conclusion: This study showed that gait training with a Underwater treadmill increased balance and motor functional ability that resulted in enhancement of motor performance.

        일반 사무직근로자의 근골격계 자각증상 정도에 따른 직무스트레스, 자기효능감, 우울 및 건강관련 삶의 질 변화

        본 연구는 일반 사무직근로자의 근골격계 자각증상 정도에 따른 직무스트레스, 자기효능감, 우울 및 건강관 련 삶의 질에 미치는 영향을 알아보기 위해 시행되었다. 광주광역시 D보건소와 D구청에서 근무하는 근로자 중 구조 화된 설문지를 자발적으로 조사에 응한 83명 중 응답이 불충분한 근로자 3명을 제외한 80명을 최종 분석대상자로 선 정하였다. 실험 결과 근골격계 자각증상 정도에 따른 직무스트레스, 자기효능감, 우울 및 건강관련 삶의 질은 모두 유 의한 차이가 있는 것으로 나타났으며, 건강관련 삶의 질을 제외한 우울, 자기효능감, 직무스트레스 간에 유의한 상관 성을 보였다. 결론적으로 일반 사무직 근로자의 근골격계 자각증상의 정도가 심해질수록 직무스트레스, 자기효능감, 우울 및 건강관련 삶의 질 요인에도 영향을 미치는 것을 알 수 있었다. 향후 근골격계 장애에 따른 불안전한 직무스 트레스, 자기효능감, 우울 및 건강관련 삶의 질의 개선을 위해 다양한 프로그램 개발 및 시행이 필요할 것이다. The study aims to evaluate job stress, self-efficacy, depression and health-related quality of life according to musculoskeletal symptoms of the ordinary white-collar worker. The subjects of the study were 83 workers who were working D Health Center and Dgu office of Gwangju Metropolitan City and they were voluntarily interviewed with a use of organized questionnaire. Except the responses from three subjects whose responses were insincere of 102 ones were decided for the final analysis. The results showed that there were significant differences in all job stress, self-efficacy, depression and health-related quality of life according to musculoskeletal symptoms. Depression, self-efficacy, job stress were significantly correlated; however, health related to quality of life was not. Therefore, as the results indicate that job stress, self-efficacy, depression and health-related quality of life can be affected by musculoskeletal pain for office workers, there is a need for the development of programs focused on the prevention of musculoskeletal discomfort.

        중심핵(CENTRAL CORE)병 3례

        Central core disease is one of benign childhood myopathies, charactorized by its histological feature. Mitochondrial oxidative enzyme activity is absent in this cental core area. Patients shows motor weakness in early childhood and motor developemental delay. The muscle involvement is generalised and symmetrical in the trunk and limbs. The deep tendon reflexes are normal of deminished. Skeletal deformities are not nucommon. Creatine kinase serum levels are normal and the electromyographic abnormalities are nonspecific. The disease course in benign and the electromyographic abnormalities are nonspecific. We experienced 3cases of central core disease. The case 1, a 5 year girl, is the first report in our country. All of the cases showed motor weakness and motor developemental delay. In the case 1 and 3, skeletal deformities were detected such as hip joint contracture, kyphoscoliosis. The were treated with physial therapy and had operative correction. We report 3 cases of Central core disease with brief revew of the litratures.

        운동에 의해 유발된 발작 1례

        저자들은 운동에 의해 유발되는 발작이 지속되어수년간 간질로 치료받았던 6년 5개월 남아에서, 특징적으로 심전도에서 QTc 간격이 연장되어 있고,운동 유발 검사에서 다형성 심실 빈맥 소견을 보여QT 연장 증후군으로 진단하고 베타 수용체 차단제로 치료하면서 추적 관찰 중인 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Long QT syndrome(LQTS) is characterized by prolongation of the QT interval, frequent episodes of syncope, and ventricular tachycardia leading to sudden cardiac death in adolescents and young adults. Early diagnosis and prompt treatment to reduce the risk of life-threatening cardiac events is crucial, however sudden onset convulsive syncope may be misdiagnosed as epilepsy and lead to antiepileptic drug therapy for many years. We experienced a case of six-year-old boy who were having seizure triggered by exercise and being treated with antiepileptic drugs. He had normal EEG and brain MRI but his ECG revealed prolonged QT interval(QTc= 477 ms) and treadmill test induced polymorphic ventricular tachycardia. After applying β-blocker, he had no more seizure attacks. We report a case of long QT syndrome misdiagnosed as epilepsy with a brief review of related literatures.

        전기자극이 근기능 변화에 미치는 효과

        When any damage or disease occurs, muscular strength and muscular endurance are lowered, and thus, if one is able to be restored from the damage or the disease, appropriate stimulus is required, since the muscles have to restore their proper functions. For such stimulus, the way of exercising and the way using electric stimulus are used in physical therapy. In order to examine the change in muscular strength, muscular endurance and girth of limbs for Biceps brachii, according to the lands of stimulus, in this article a total of 42 healthy male and female adults in their twenties were randomly sampled, and a series of tests were conducted for 6 weeks, for 15 minutes per once, 3 times per week, respectively, dividing them into the group for RUS(Russian Current Stimulation), PRE(Progressive Resistive Exercise) and P+R(RUS + PRE). The findings showed that the change in muscular strength was most significant as for the group for P+R, since it increased from $62.12{\pm}25.30$ before experiment to $95.78{\pm}34.07$ after 6 weeks: the change in muscular endurance was most significant as for the group for P+R, since it increased from $17.57{\pm}6.63$ to $42.86{\pm}10.24$; and the change in the girth of limbs was slightly significant only in the group for P+R, and the remaining two groups showed no significance.

        트레드밀운동이 뇌졸중 환자의 걷기기능향상에 관한 문헌연구

        Background : Treadmill training has been proposed as a useful adjunct to conventional physical therapy to restore ability to walk after stroke. The purpose of this study to inform clinical practise by evaluating the research evidence for the effectiveness of treadmill training after stroke. Methods : We searched to the effectiveness of any form of intervention for effect of treadmill training by Dankook University electronic library databases of Medline, Embase, Cinahl, Amed and PEDro combined with a hand search of papers published in relevant peer-reviewed journals. Any type of study relevant to the topic published in English during time period from 1980 to 2007 was included. Results : The literature search identified 35 studies. The included studies enrolled a total of 374 subjects. 1. Treadmill retraining without partial body weight support might be more effective than no treatment at improving gait velocity, get up and go time, gait endurance and step length symmetry. 2. Treadmill retraining with partial body weight support might be more effective than no treatment in improving step length symmetry, gait velocity, gait endurance and balance. 3. Treadmill retraining might be more effective with partial body weight support than without it at improving gait velocity and motor improvement as measured by The Stroke Rehabilitation Assessment of Movement(STREAM). 4. Treadmill retraining without partial body weight support may be no different from physiotherapy and increase gait velocity to conventional gait therapy. 5. Treadmill retraining with partial body weight support may be no different from physiotherapy for gait velocity, motor recovery and balance. Conclusion The review suggests that although treadmill training of gait, especially with partial body weight support, might Improve gait parameters and functional mobility, unless treadmill training is directed at Improving gait speed it might be no more effective than conventional physical therapy at improving gait parameters.
