        인천지역에서 분리된 황색포도상구균의 분자생물학적 특성 분석

        황색포도상구균은 주요한 기회 감염균으로, 최근 여러 가지 항생제에 내성을 지닌 메치실린 내성 황색포도상구균 Methicillin-resistant Staphylococcus aureus: MRSA)이 늘어나 문제가 되고 있다. 이에 본 연구에서는 인천지역관내 설사환자에서 분리한 장독소 양성인 S. aureus를 대상으로 항생제 감수성시험 및 PCR을 이용한 tsst, eta, etb mecA 유전자 검사를 실시하여 생물학적인 특성을 조사하였고, tsst 양성인 MRSA를 대상으로 Pulse Field Gel Electrophoresis (PFGE)에 의한 유전자형을 분석함으로서 경시적인 분자역학적 특성을 파악하고자 하였다. 그 결과 2,281건의 대변에서 173주의 장독소 양성인 S. aureus를 분리하였으며 A독소와 C독소가 각각 39%, 58%를 차지하는 것으로 나타났다. 항생제 감수성 결과 장독소 양성주는 모두 MRSA였으며, 이중 51%가 tsst 양성인 것으로 나타났고 eta, etb 유전자는 검출되지 않았다. mecA 내성유전자는 MRSA 균주의 97%가 양성으로 나타났다. tsst 양성인 MRSA 88주를 대상으로 PFGE한 결과, 10개의 유형으로 나뉘었으며 그중 A형, H형 및 F형 이 각각 58%, 10%, 9%로 주요한 형으로 나타났다. Staphylococcus aureus is one of the most significant pathogens and a causative agents of nosocomial infections. The emergence of methicillin resistant S. aureus (MRSA), in particular, has become a major clinical and epidemiological problems worldwide. In this study, we analyzed the toxin genes and investigated molecular epidemiological characteristics of S. aureus isolated from stools of diarrheal patients at the hospitals in Incheon. Of the 609 strains from 2,281 specimens, 173 strains retained enterotoxin; 68 isolates (39.30%), 100 isolates (57.80%) were classified to A and C type, respectively. In the antibiotic susceptibility, all of enterotoxin positive isolates were resistant to oxacillin. Eighty eight strains (50.86%) of 173 MRSA isolate possessed tsst gene, but eta and eth genes were not detected at all. In the detection of MRSA associated genes by PCR method, mecA genes were detected in 167 strains (96.53%). From the result of PFGE analysis, we classified tsst-positive MRSA to 10 types and 24 subtypes. Type A, H and F were the major strains comprised of 57.95% (51 strains), 10.22% (9 strains) and 9.09% (8 strains) respectively.

        강화 교동면 주민과 들쥐의 쯔쯔가무시병 및 발진열에 대한 혈청역학 조사

        Scrub typhus and murine typhus are common endemic febrile illness in the fall in Korea. Scrub typhus is caused by Orientia tsutsugamushi, murine typhus is caused by Rickettsia typhi. Trombiculid mites are known as both the vector and the reservoir host of O tsutsugamushi, the mites which transmit O tsutsuga,nushi have been reported to be Leptotrombidium jxzllidum and L scutellare. The author carried out an epidemiological study of scrub typhus and murine typhus in Kyodong-Myeon, Kanghwa-Gun, Incheon in relation to the residents and the host rodents, such as their distribution, seroepideniiology, and population density of chigger mites. 1. Out of 900 residents, 33(3.7%) showed positive reaction to 0 tsutsuga`nushi, 24(2.7%) to R typhi. 2. In the seropositives to O tsutsugamushi or R typhi, between the sixties and the seventies of the age were dominant. 3. In the seropositives to O tsutsutzmushi serotypically Gilliarn was dominant. 4. Among the total 42 field rodents trapped by the sherman traps, 18 rodents were Apodemus agrarius(42.9%), 13 rodents were Crocidura lasiura(31.O%), 5 rodents were Mus musculus(11.9%), 2 rodents(4.8%) were Crocidura suaveolens, Rattus norvegicus, Tscherskia triton, respectively. 5. Out of 42 field rodents, 25 were parasitized by 4,149 chigger mites, showing 59.5% of the infestation rate and 98.8 of the chigger index. L jxillidum parasitized in A agrarius, C lasiura, M musculus, R norvegicus and T triton, and L scutellare parasitized only C lasiura. 6. Antibodies in the sera of field rodents against O tsutsugamushi and R typhi were investigated by indirect immunofluorescent antibody technique. Positive rate of antibody against 0 tsutsugamushi were 11.9%(5 of 42) and all of the positive is A agrarius. Antibody against R typhi was not detected. These results might provide the basic information for the management of scrub typhus and murine typhus in Kyodong-myeon, where the epidemiological studies on scrub typhus and murine typhus was not carried out enough.

        Recently, there is increasing interest in indoor air pollution. Fungal spores are one of the pollutants that is the causes of hypersensitivity reactions, bronchial asthma, allergic rhinitis. This study was performed to assess the levels of ungi concentration in public facilities and airborne fungi were collected with the air sampler using DRBC(Dichloran rose bengal chloramphenicol) agar. It was found that airborne fungi concentration were related to the humidity and the people movement. 10 genera of fungi was isolated and identified and the most common culturable fungi were Penicillium, Cladosporium, and Aspergillus.

        인천지역에서 분리된 비브리오 패혈증균의 특성

        2006년도 인천지역 해양환경에서 분리된 Vibrio vulnificus에 대한 생화학적, 분자생물학적 특성 및 항생제 감수성 결과를 알아보았다. 본 실험에 사용된 균주는 총 233주로 해수, 갯벌, 어패류, 수족관수에서 분리되었다. API 20E kit 실험 결과 15개 profile로 분류되었으며, 모든 균주가 ONPG와 Amygdalin 양성이었다. 209주를 대상으로 vvhA와 viuB 유전자 부위에 대해 PCR 실험결과 vvhA는 206주(98.6%) 양성, viuB는 110주(52.6%)가 양성이었으며, 특히 viuB 유전자부위에 대한 PCR 결과는 해수, 어패류, 갯벌에서 분리한 균주의 48%, 48.5%, 61.1%가 양성인 것으로 나타났다. 시험균주 175주에 대해 항생제 감수성 실험 결과 Imipenem (100.0%), Sulfamethoxazole/trimethoprim (98.9%), Tetracycline, Ciprofloxacin (98.3%), Ampicillin/sulbactam (97.1%), Chloramphenicol (96.6%), Cefepime (94.9%), Ceftriaxone (94.8%)이 감수성을 나타내었고, 항생제 하나 또는 그 이상의 약제에 대해 내성을 나타낸 것은 56주(32%)로 해수 28주(31.5%), 갯벌 21주(34.4%), 어패류 7주(29.2%)였다. V. vulnificus 233주에 대해 PFGE를 실시하여 dendrogram으로 분석한 결과 90%이상의 상동성 기준으로 126개의 유형으로 분리되었고, 58%이상의 상동성을 기준으로 13개의 cluster로 분류되었다. Cluster I는 104주(44.6%)로 가장 많은 균주들이 분포하였고 채취시기 대부분 I에 가장 많은 균주들이 분포하였으나 10월과 6월에 채취한 검체는 J에 16주(69.6%)와 13주(36.1%)로 가장 많은 균주들이 분포하였다. 7월에 채취한 검체에서 분리된 균주는 9개의 cluster에 속하였고 8월은 8개, 6월은 7개, 9월은 6개, 10월은 5개, 5월은 3개, 3월은 1개를 나타냈다. We performed the biochemical characteristics, molecular epidemiologocal analysis, and drug susceptibility test on V. vulnificus isolated from environmental sources in Incheon. For this study, 233 strains were isolated from seawater, sediment, shellfish. V. vulnificus isolates were divided into 15 biochemical groups, which were positive for ONPG and Amygdalin test. Among the 209 strains, 206 (98.6%) strains and 110 (52.6%) strains revealed positive for vvhA and viuB gene, and the viuB gene detection rates of V. vulnificus from seawater, shellfish and sediment were 48%, 48.5% and 61.6%, respectively. From disc diffusion test on 175 isolates, most of strains were sensitive to Imipenem (100.0%), Sulfamethoxazole/trimethoprim (98.9%), Tetracycline, Ciprofloxacin (98.3%), Ampicillin/sulbactam (97.1%), Ohloramphenicol (96.6%), Cefepime (94.9%) and Ceftriaxone (94.8%), multi-drug resistance rates was 31.5% of seawater, 34.4% of sediment and 29.2% of shellfish. PFGE was performed on 233 V. vulnificus isolates with the objective of investigating the extent of genetic diversity of these isolates in our region. We could find that at least 126 different PFGE patterns were generated according by 90% of similarity and 13 clusters by 58% of similarity. The major cluster was type I (44.6%) during the most of the year, and type J was frequent pattern in June and October. There were 9 distinct PFGE types in July, 8 types in August, 7 types in June, 6 types in September, 5 types in October 3 types in May and 1 type in March.

        인천지역 설사환자에서 분리한 Salmonella enterica serovar Enteritidis의 항생제 내성 및 다제내성 양상

        본 연구에서는 2004년 1월부터 2008년 12월까지 인천지역 5개 종합 병 의원에 설사증상으로 입원 또는 내원한 환자의 대변에서 분리한 115주의 S. Enteritidis의 항생제 내성 및 다제내성 양상을 모니터링하였다. 그 결과, 가장 내성율이 높은 항생제는 nalidixic acid (54.8%)이었고, ampicillin (40.0%), ticarcillin (38.3%), chloramphenicol (20.9%), tetracycline (17.4%) 순이었다. 분리균의 91.3%는 1제 이상의 항생제에 내성이 있었다. 항생제 내성 유형을 살펴본 결과 nalidixic acid 단일 내성이 44.3%로 가장 높았으며, ampicillin-chloramphenicol-ticarcillin (11.3%) 및 ampicillin-ticarcillin (10.4%) 순이었다. 전체적으로 1제 내성이 51.9%, 2제 내성 18.2%, 3제 내성 14.5%, 4제 내성 5.8% 그리고 5제 이상 내성은 9.7%이었다. This study describes the antimicrobial susceptibility and multidrug resistance patterns in 115 isolates of Salmonella enterica serovar Enteritidis from diarrhea patients in Incheon from Jan. 2004 to Dec. 2008. Overall, the highest rate of resistance was found to the following antimicrobial agents: nalidixic acid 54.8%, ampicillin 40.0%, ticarcillin 38.3%, chloramphenicol 20.9%, and tetracycline 17.4%. 91% (104 isolates) were resistant to one or more antimicrobial agents tested. Among the described antimicrobial resistant patterns, the predominant patterns were only nalidixic acid (44.3%), ampicillin-chloramphenicol-ticarcillin (11.3%,) and ampicillin-ticarcillin (10.4%). Overall, the resistant rate to one drug was 51.9%, two drugs 18.2%, three drugs 14.5%, four drugs 5.8%, and five or more drugs 9.7%.

      • 인천연안 해역의 Vibrio속 세균분포 및 증식특성

        This study was conducted to investigate the effects of environmental factor such as temperature, salinity, turbidity, pH and dissolved oxygen on the growth of Vibrio spp.. In this survey, total 56 seawater samples were obtained from 8 different sites of the Incheon coastal area during the periods from april 2008 to october 2008. Enumeration of Vibrio spp. was determined by using the most probable number(MPN) procedure. Isolation rates of V. parahaemolyticus, V. vulnificus, V. cholerae in all samples tested were 44.0%, 21.4% and 13.1%, respectively. The enumeration of Vibrio spp. was very low correlated with water temperature and pH and negatively correlated with salinity, dissolved oxygen and turbidity. We found salinity to be the parameter most highly correlated with the enumeration of Vibrio spp. The highest rate of antibiotic resistance of V. vulnificus and V.parahaemolyticus was Cefazolin(11.5%), Ampicillin(70.8%), respectively.

        We investigated the distribution and drug susceptibility test of V. vulnificus isolated from environmental sources in Incheon. In this survey, total 4,302 samples were obtained from different sites of the Incheon coastal area during the periods from march 2004 to November 2006. Among the 4,302 samples, 310 strains were isolated. The isolation rates of Vibrio vulnificus from fish, shellfish, estuarine water and sediment were 6.7%, 4.7%, 12.4% and 23.2%, respectively. The highest isolation rate was 14.6% in September and in spite of low temperature the isolation rate was 5.1% in November. The highest rate of antibiotic resistance was observed against streptomycin(15.3%), oefazclin(8.5%), oephalcthin(8.3%), amikacin(8.3%), oefoxitin(6.7%) and nalidixic acid(6.7%). Seventeen peroent of isolates were observed to be resistant to two or more of the antibiotics tested.

        In order to investigate water pollution of spring, we carried out the experiments on mineral water, rainwater and surrounding soil of several springs collected in Incheon area from February to August in 2007. The results were as follows. 1. Soil : This study investigated the pH and organic matter contents in the soil which effect on microorganisms, The pH value was from 4.5 to 5.3 on average and organic matter contents ranged from 1,09 % to 5,6% and E. coli, K. pneumoniae, S.faecalis and other microorganisms were isolated. 2. Natural mineral water : As the rainfall increased, the concentration of No?-N, consumption of KMnO₄ and the number of microorganisms was increased while the other physicochemical items were not affected. The microorganism isolated in the natural mineral water were as follows: E. coli. K. oxytoca S. faecalis, S. facium, and so on from evacuation; E. agglomerans, E. amnigenus, E. cloacae, E. sakazakii and so on from the environments and evacuation. Detection of these microorganisms shows that the environment around the spring had been exposed to excrements of the warm blood animal. 3. Rain : The coliforms in the rain were the identical one detected from the mineral water and the soil. In conclusion the quality of natural mineral water is influenced by rain, resident bacteria of soil and surrounding environment. This study will be the basic data of characteristics of spring in Incheon area and also this result will give useful aids for the efficient control of spring.

      • 인천항으로 반입되는 휴대농산물에 대한 유해물질 조사

        본 연구는 인천항으로 유입되는 휴대농산물의 안전성 확보를 위하여 휴대량 초과로 인천본부세관에 유치되는 농산물 등을 검사하였다. 수거 검체는 건고추절편 또는 고춧가루, 곡류, 종실류, 콩류, 견과류, 한약재원료, 참기름 등 총 293건을 대양으로, 품목별로 잔류농약, 곰팡이수, 타르색소, 수단색소 Ⅰ, Ⅱ, Ⅲ, Ⅳ, 아플라톡신, 납, 카드뮴, 수은, 비소, 잔류이산화황 분석을 실시하였다. 1. 건고추절편 또는 고춧가루는 잔류농약동시다성분분석, 곰팡이수, 타르색소, 수단색소 Ⅰ, Ⅱ, Ⅲ, Ⅳ 검사를 실시하였고 이중 곰팡이수와 타르색소에서 부적합이 있었다. 2. 종실류는 타르색소 검사에서 불검출이었고, 콩류와 견과류의 아플라톡신 검사결과도 불검출이었다. 3. 곡류는 납, 카드뮴, 수은의 중금속 검사와 흑미의 경우 타르색소 검사를 추가하였고, 쌀 2건, 찹쌀, 율무, 조에서 각 1건씩 납 기준을 초과하였다. 4. 한약재 원료는 납, 카드뮴, 수은, 비소, 잔류이산화황 검사에서 대추 2건과 건강 3건에서 잔류이산화황이 기준을 초과하였고, 건강 1건에서 카드뮴이 부접합하였다. 5. 추석 전에 대거 유입된 참기름의 규격검사 결과는 모두 적합한 것으로 나타냈다. 6. 총 293건의 휴대농산물 가운데 건고추절편 또는 고춧가루 78건, 곡류 5건, 한약재 5건 등 88건에서 규격기준을 초과하여 30% 부적합율을 나타내었다, 시험항목 중에서는 곰팡이수 부적합 76건, 타르색소 1건, 곰팡이수와 타르색소 동시부적합 1건, 납 5건, 잔류이산화황 4건, 잔류이산화황과 카드뮴 동시부적합 1건으로 나타냈다. 7. 중금속 측정결과는 곡류에서 Pb 0.000~0.740㎎/㎏(0.191±0.209), Cd 0.000~0.108㎎/㎏(0.007±0.020), Hg 0.000~4,201㎎/㎏(0.162±0.319)으로 나타났으며, 한약재원료에서는 Pb 0.000~ 2.480㎎/㎏(1.130±0.730), Cd 0.000~0.761㎎/㎏(0.044±0.122), Hg 0.000~0.157㎎/㎏(0.022±0.033), As 0.000~0.560㎎/㎏(0.040±0.082), 잔류이산화황은 2.0~1459.4ppm(303.1±324.2)으로 나타났다. This study was investigated hazardous substances in agricultural products of baggage by Incheon Port from chinese. We tested 293 kinds of dried red pepper, cereal, sesame, bean, nut, herbal medicine and sesame oil for pesticides residue, mold, tar color, sudan Ⅰ, Ⅱ, Ⅲ, Ⅳ, aflatoxin, Pb, Cd, Hg, As and sulfur dioxide residues. Violated samples over standard limits by Korean Food and Drug Administartion (KFDA) were 76 dried red pepper over the number of mold and 1 dried red pepper detected red 102 tar color and 1 dried red pepper over the number of mold and detected red 102 tar color simultaneously. And 2 rice, 1 sticky rice, 1 adlay and 1 millet were over lead. Violated herbal medicine were 2 Zizyphi Furctus and 2 Zingiberis Rhizoma over sulfur dioxide residue and 1 Zingiberis Rhizoma over sulfur dioxide residue and cadmium simultaneously. The contents of heavy metals in cereal were as follow; Pb 0.000~0.740㎎/㎏(0.19±0.209), Cd 0.000~0.761㎎/㎏(0.044±0.122), Hg 0.000~0.157㎎/㎏(0.022±0.033), As 0.000~0.560㎎/㎏(0.040±0.082). Contents of SO₂ ranged 2.0~1459.4ppm(303.1±324.2).
