        초기 균상 식육종 진단에서 T 세포 감마 수용체 유전자 재배열 검사의 의의

        N/A Background: The diagnosis and differential diagnosis of mycosis fungoides(MF) is often difficult, clinically and histologically. A variety of inflammatory dermatoses may be seen with similar clinical and histological features. Given the limitation of histologic diagnosis in the early MF, an ancillary method to demonstrate a clonal T cell proliferation would be helpful. Recent attempts to enhance diagnostic sensitivity have involved T-cell receptor(TCR) gene rearrangement studies, using polymerase chain reaction(PCR) technique. Objective : The purpose of this study was the value of PCR detection of TCR y gene rearrangement in the diagnosis of early MF. Materials and Methods . Thirty-two cases of paraffin embedded tissue(PET) from patients of early MF were investigated for the presence of TCR y gene rearrangement using a nested FCR technique and analyzed by polyacrylamide gel electrophoresis(PAGE). As a control, PET from patients of 6 psoriasis, 3 lichen planus, 1 neurodermatitis, and 1 allergic contact dermatitis were tested. Results : Monoclonal TCR γ gene rearrangement was detected in 28 out of 32 (88%) patients with early MF and in none out of 11 patients with inflammatory skin diseases. Conclusion : TCR γ gene rearrangement studies on lesional skin using PCR may be helpful as an adjunct to the histopathologic diagnosis of early MF. ( Korean J Dermatol 2001;39(11) : 1231~1237)

        세프테졸 부티로락톤 에스텔의 합성 및 생물약제학적 특성

        N/A Butyrolactone ester of ceftezole (CFZ-BL) was synthesized by esterification of ceftezole (CFZ) with α-bromo-γ-butyrolactone. The synthesis was confirmed by spectroscopic analysis. CFZ-BL was more lipophilic than CFZ when the lipophilicity was assessed by partition coefficients between n-octanol and water at various pH. CFZ-BL itself did not show any microbiological activity in vitro, but serums taken after oral administration of CFZ-BL showed substaintial microbiological activity indicating that CFZ-BL is converted to microbiologically active metabolite, probably CFZ, in the body. The conversion was confirmed by in vitro incubation study, in which CFZ-BL was incubated in some body tissues of rabbit. Liver homogenate showed fastest conversion of CFZ-BL among the tissues tested (blood and intestine). Thus, CFZ-BL appeares to be rapidly metabolized in the liver to CFZ following oral administration. The metabolism process appears to be hydrolysis of the ester to CFZ, the parent drug of CFZ-BL. In vivo metabolism of CFZ-BL to CFZ was confirmed by analying CFZ by HPLC. CFZ concentration in the serum samples taken after oral administration of CFZ-BL were higher than those in the serum samples taken after oral administration of equivalent amount of CFZ. Oral bioavailability of CFZ-BL, a prodrug of CFZ, was 1.45-fold higher than that of CFZ in rabbits possibly due to enhanced lipophility and absorption of the prodrug.

        요양보호사의 불안정 노동과 이직을 둘러싼 제도적 관계

        본 연구의 목적은 요양보호사의 ‘잦은 이직’이 제도적 관계(사회정책, 노동시장정책)안에서 어떻게 조직화되는지를 살펴보고 이를 발생시킨 지배론적 담론을 살펴보는데 있다. 기존 연구들은 이직의 원인을 인간관계나 직무만족, 근로조건과 같은 개인이나 조직차원에서 찾는다. 이에 비해 본 연구는 요양보호사의 불안정 노동이 어떤 시퀀스를 통해 이직이 발생하는지를 제도적 문화기술지 방법으로 풀어냈다. 자료는 연구참여자의 심층인터뷰, 장기요양시설에서의 현장관찰, 제도적 텍스트 등이다. 연구결과, 장기요양정책이 근로기준법과 부정합하여 요양보호사는 과도한 업무량을 감내하면서 이직이 활성화되었다. 또한 요양보호사는 사회보험에서 배제되지 않았지만, 몸이 아파도 휴직제를 쓸 수 없어 퇴직을 종용당하는 등 사회보장제도가 실직적인 재분배 역할을 하지 못하였다. 특히 정부가 추진한 시장화정책이 이윤을 추구하는 자본과 결합되면서 노동자를 보호하기 위해 만든 고용보험제도, 퇴직금제도, 적극적 노동시장정책이 오히려 이직을 유도하는 정책이 되기도 하였다. 요양보호사의 재취업은 삶을 지탱할만큼의 노동조건과 사회적 보호를 받지 못하는 불안정노동의 복합적 위기상황에서 받은 실업급여로 노동과정에서 축난 몸을 스스로 재활시키고 일터로 복귀하는 과정(return to work)이라고 볼 수 있다. 이는 신자유주의적 시장자유화 전략, 소비자 선택모델, 신공공관리론에 근원을 두고 있다. The aim of this study is to examine how the return to work of care workers are organized within institutional relations(social security system, labor market policy) and then examine the dominant discourse. While previous studies find the cause of job quit at a personal or organizational level such as human relationships or job satisfaction, this study describes by what sequence does job quit occur from the institutions relevant to the precarious work through institutional ethnography. The data includes in-depth interviews, field observations at long-term care fercilities, institutional texts, etc. As a result of the study, there is an evident incongruence between the Labor Standards Act and the long term care policy, so an excessive workload created turnover of care workers. In addition, although care worker was not excluded from social insurance, we observe that care workers who are eligible for Industrial Accident Compensation Insurance could not benefit and had to quit the job because the social security system did not work or function for redistribution. In particular, the marketization policy combined with profit-seeking capital has led the employment insurance system, retirement allowance system, and active labor market policy to protect workers. It can be seen as a process of rehabilitating the body itself and returning to work. The turnover issues of care workers had its roots in the neorealism market liberalization strategies, consumer choice models, and new public management theories.

      • 인삼의 산지판별을 위한 전자코의 Conducting Polymer 센서와 Metal Oxide 센서의 이용

        인삼의 원산지를 확인하기위하여 전자코를 사용하였다. 농산물에서 배출되는 가스성분을 아무런 전처리 과정 없이 coducting polymer센서와 metal oxide 센서를 각각 사용하였을 때와 동시에 함께 사용하였을 때 정준판별분석결과 두 가지 형태의 센서를 활용하였을 때 인삼의 산지를 뚜렷하게 판별할 수 있었다. The habitats of ginseng was investigated by using electronic nose without any pretreatment. Metal oxide sensor and conduction polymer sensor as a sensing material were used, respectively and also used both of them. The difference of Canonical discriminant scores in case of using both rather than one sensor was distinctly shown. The separation of Chinese ginseng and the locally produced one could be obtained by using composite sensing systems of electronic nose.

        cMycosis fungoides palmaris et plantaris is an uncommon expression of mycosis fungoides that manifests primarily on the palms and the soles. The lesions may be quite hyperkeratotic and mimic various inflammatory palmoplantar dermatoses. A biopsy is recommended in the evaluation of recalcitrant palmoplantar dermatoses. We report a case of mycosis fungoides palmaris et plantaris in a 36-year-old woman presented with hyperkeratotic plaque on her palms and soles.

        각질형성세포종양의 CD44 표준형과 그 변형의 발현 양상

        Background: The CD44 antigen is a widely distributed cell surface glycoprotein implicated in multiple physiological cellular functions including cell-cell adhesion, cell-substrate interaction and cell activation. In the skin, CD44 is normally expressed in epidermal keratinocytes and hair follicular, sebaceous and eccrine epithelial cells. The precise functions of CD44 are not yet clearly understood, though they appear to be involved in the mechanism of tumor invasion and metastasis. Objective: The purpose of this study is to investigate the immunohistochemical expression of the CD44 molecule in the epidermal keratinocytic tumors with different biologic behaviors, such as squamous cell carcinoma(SCC),basal cell epithelioma(BCE), actinic keratosis(AK), bowen's disease, keratoacanthoma(KA), trichoepithelioma. Methods: Routine paraffin sections of formalin-fixed 33 tissues (15 SCC, 23 BCE, 9 AK, 7 KA, 7 bowen's disease, 5 trichoepithelioma, 2 psoriasis, 2 lichen planus) were labeled with anti-CD44 monoclonal antibody using avidin-biotin- peroxidase complex. Normal skin served as the negative control. Results: The results are as follows. 1. In SCC and KA, peripheral parts of the lesion were prominently stained. CD44v6 showed the strongest expression, followed by CD44s and CD44v4/5. The horn cyst and the keratin region of the horn pearl were not stained. The degree of staining was weaker in KA compared to SCC. 2. The degree of staining was weaker in BCC and TEE compared to SCC. The staining was more prominent at the center of the lesion than in the peripheral region. In TEE, stronger staining was found at the center or in the squamous epithelial region. 3. Among the seven cases of Bowen's disease, 4 cases were diffusely stained with all three CD44 types. In the other three cases, local staining with the standard type and diffuse staining with CD44v4/5 and CD44v6 were seen. 4. In AK, the early lesion showed decreased expression of CD44 in all three types. However, those with mature lesion and progression to SCC showed increased degree of staining in all three types of CD44. Conclusion?These results suggest that expression of CD44 in these keratinocytic tumors is not related to malignant transformation and metastasis, but instead may be related tumor differentiation.
