        Allergen-specific immunotherapy (AIT) can alter the natural course of an allergic disease. It is widely held that the efficacy of a given therapeutic modality should be proven by well-designed double-blind placebo-controlled randomized clinical trials and meta-analyses. In this article, the therapeutic efficacy of AIT for allergic respiratory diseases, such as allergic rhinitis and asthma, is reviewed. In addition, potential clinical and experimental predictors of the treatment response are reviewed for AITs requiring long-term treatment, and for some allergic rhinitis and asthma patients who show no response to AIT.         Toxocariasis is an infection in a human host caused by Toxocara canis. Most human infections occur in younger persons who eat dirt containing embryonated eggs or larvae in raw or uncooked foods, particularly raw liver. Toxocariasis can produce various clinical manifestations associated with hypereosinophila and eosinophilic infiltration into the internal organs in the form of abscess or granuloma. We report the case of a 63-year-old man with hypereosinophilia caused by Toxocara infection, who had a history to eat raw ostrich liver 6 months before. He had general weakness and weight loss after ingestion of raw ostrich liver and was then referred to our hospital for the evaluation of hematologic malignancies because he had hypereosinophilia, anemia and a reversed albumin-globulin (A/G) ratio. Toxocara infection was confirmed by enzyme-linked immunosorbent assay for Toxocara canis. He was treated with albendazole for 2 weeks and was discharged from the hospital without any complications. At the 4 month and 1 year follow-up visits, his clinical symptoms improved, the total eosinophil counts were markedly reduced, and the A/G ratio.         국내 성인 알레르겐 감작률과 Geographic Information System 기반 공간분석을 활용한 지역별 감작률 연구

        Purpose: To evaluate allergen sensitization in Koreans and assess regional differences in it using Geographic Information System (GIS)-based modeling techniques. Methods: We retrospectively analyzed multiple allergen simultaneous test (MAST) results collected from 2,017 clinics and hospitals nationwide between 2018 and 2020. The AdvanSureTM AlloScreen (LG Chem, Seoul, Korea) was used to test for 47 allergens. Then, we created maps to visualize the sensitization prediction rate by using the ordinary kriging method. Results: The total number of participants were 196,419; 104,371 (53.1%) were sensitized to one or more allergens. The most frequent allergen causing sensitization was Dermatophagoides farinae (34.0%), followed by Dermatophagoides pteronyssinus (32.3%), house dust (26.2%), cat dander (13.6%), Acarus siro (12.5%), rye grass pollen (8.8%), and birch tree pollen (8.2%), respectively. Perennial allergens, such as mites and cat dander, were more common than seasonal allergens. Sensitization rates to grasses were generally higher than those to tree and weed allergens. The sensitization rates to Hymenoptera venoms were 4.8% in honey bee and 2.2% in yellow jacket. The sensitization prediction rate against Dermatophagoides farinae calculated by GIS-based mapping showed geographical differences even within the same administrative district. Conclusion: Dermatophagoides farinae and Dermatophagoides pteronyssinus were found to be the most prevalent sensitizing allergens throughout Korea. GIS-based spatial pattern analysis using MAST revealed different sensitization patterns between geographic areas and at the subdistrict level. These data could assist in prevention and management of allergic diseases and provide valuable evidence for environmental and public health policymakers. (Allergy Asthma Respir Dis 2023;11:135-143)

        북한의 학문분류체계

        본 논문은 분단이후 북한의 제 학문분야에서의 연구성과를 분석 파악하기 위한 기초연구로서 학문분류체계와 특징을 검토하는데 목적을 두고 있다. 학문분류체계는 시대적인 상황에 따라 변화되며 국가마다 특수성을 지니고 있기 때문에 규격화되어 있지 않고 통일성이 없는 것이 특징이다. 북한의 학문체계도 학문으로서의 보편성이란 관점에서 공통성도 지니고 있지만 북한 나름대로의 국가적 특수성을 지니고 있다. 따라서 북한의 학문분류체계를 파악하기 위하여 교육과 연구의 제 영역에서의 분류실태에 대한 분석을 시도하였다. 즉, 교육과 학술기관(대학 학과와 연구소)에서의 분류실태, 문헌(도서)분류와 사전적 개념 및 학위의 종류, 분야별 연구성과 평가과정에서의 분류를 종합 검토하여 이를 토대로 학문분류체계의 시안을 작성하였다. 시안에 대한 타당성을 검토하고 특징을 도출한 결과 우선적으로 학문의 대상인 과학을 자연과학과 사회과학으로 양분하고 인문과학을 사회과학의 틀 속에 포함하여 분류하고 있으며 계급성을 강조하고 있다. 둘째로 사회과학을 주체철학, 혁명력사학, 철학, 력사학, 고고학, 민속학, 민족고전학, 경제학, 법학, 정치학, 언어학, 문예학, 교육학, 군사학 등 총 14개로 분류하고 있다. 셋째로 체제이데올로기와 지도자에 관한 문제를 학문적 차원에서 접근하고 있다. 넷째로 학문영역의 폭이 넓지 못하다. 따라서 분과학문으로 분화하지 못하고 미분화상태에 있어 많은 분야에서 독자적인 학문영역을 확보하지 못하고 있다. 결론적으로 북한의 학문체계의 제 특성은 체제 성격이 반영된 결과라 하겠다. This is a basic study to analysis the accomplishment of North Korea`s academic after the division of Korean peninsula and it aims to investigate the characteristics and system of academic classification. The system of academic classification is changeable by the situation of the times and it has the national distinctiveness. Because of these features, the system of academic classification is not standardized and it does not have a unity. Therefore, this study analyses the actual condition of classification by education and studying for investigating the system of academic classification. That is, the model of the system of academic classification which is suggested in this study is the result of investigating the actual condition of the classification by dept. of Univ. and research institutes, the lexical concept of document classification and types of academic degree, and the classification by evaluating the accomplishment of academic. As a result of examining logicality of the model and drawing the characteristics, first, the system of academic classification of North Korea divides science into natural science and social science and it regards human sciences as a part of social science. Second, it classifies social science into 14 groups; Juche philosophy, revolution history, philosophy, the historical science, archeology, folklore, the ethno classics, economics, law, politics, linguistics, the science of literature, pedagogy, military science. Third, it has an academic approach to the problem of ideology and leader. Fourth, it does not have various academic fields and its own academic field in many parts. In short, the character of the regime is reflected in the system of academic classification of North Korea.

        전자의무기록을 이용한 DRESS (drug reaction with eosinophilia and systemic symptoms) 증후군 탐색

        Purpose: Since drug reaction with eosinophilia and systemic symptom (DRESS) syndrome is very rare and difficult to diagnose, its exact epidemiology is still unknown. If screening tools based on laboratory results or electronic medical records are available, the occurrence of DRESS syndrome can be monitored in real time. Methods: To screen cases with DRESS syndrome, all the results of both eosinophil and alanine transaminase (ALT) level from July 2014 to June 2015 were analyzed by 36 searching conditions for the signal detection of 7 definite DRESS cases among 199,924 patients during the study period. Those searching conditions were diverse combinations of different cutoff levels of eosinophil and ALT with or without nursing records presenting skin symptoms. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value were calculated for individual searching conditions. Results: As cutoff levels of eosinophil and ALT for screening DRESS increased from 3% to 5% and 40 U/L to 300 U/L, respectively, the sensitivity decreased from 100% to 42.9% and the PPV increased from 0.06% to 13.0%. A combination of eosinophil >10% and ALT >300 U/L which had the highest PPV among 36 search conditions could detect DRESS syndrome by sensitivity 42.9% and PPV 13.0%. When nursing records for skin symptoms were added, PPV was augmented to 21.4%. Conclusion: A combination of eosinophil and ALT levels is a useful search condition for the screening of DRESS syndrome. Nursing records can provide an additional increment in PPV. (Allergy Asthma Respir Dis 2018;6:149-154)

        에페리손에 의한 즉시형 과민반응 증례 보고

        Eperisone is a widely used muscle relaxant and believed to be relatively free of adverse drug reactions. However, a rare case of fatal anaphylaxis has been reported in the literature. Poor awareness due to its rarity and combined administration with other drugs are the major hurdles in diagnosing eperisone-induced anaphylaxis. We experienced 3 cases of immediate hypersensitivity reaction occurring after eperisone administration. Case 1, a 63-year-old female, was admitted via the Emergency Department with urticaria, generalized erythroderma, sore throat, chest discomfort, and dyspnea within 1 hour after administration of common cold remedy. Case 2, a 58-year-old male, visited our allergy clinic to detect culprit drugs. He experienced itching, urticaria, hypotension for several hours after administration of the pills for back pain in the last 3 years. Case 3, a 58-year-old male developed urticaria and dyspnea after administration of medication for a headache. He also experienced urticaria and facial edema after administration of the common cold remedy. Among the medications, eperisone hydrochloride was proven as the culprit drug and others were excluded through oral provocation tests. We advised them to avoid eperisone and issued drug safety card. Clinicians should be aware that eperisone hydrochloride is a potential culprit agent of fatal anaphylaxis. (Allergy Asthma Respir Dis 2017;5:228-231)

      • 스마트 공간에서의 로봇 격투기 게임 시스템 개발

        본 논문에서는 스마트폰 환경에서 가상의 3D 로봇과 실제 로봇의 동작을 실시간적으로 연동하는 동기화 시스템을 개발하고, 상호간의 정보를 주고받기 위한 무선 프로토콜 및 티칭엔플레이의 티칭프로그램 개발과 이를 이용한 로봇 격투 게임 시스템을 구현하였다. 이를 통하여 실제 로봇의 움직임을 일반사용자가 움직임을 쉽게 구성하고, 조정할 수 있기 때문에 개인용 서비스 로봇의 시장 확대에 큰 기여를 할 것으로 예상된다. 본 논문의 결과로서 가상 로봇과 실제 로봇의 연동 제어를 응용하여 인간의 움직임을 캡처하여 가상로봇과 동기화를 시키고 또 이를 실제로봇과 동기화를 시킬 수 있게 되면, 영화 ‘리얼스틸’에서처럼 주인공의 움직임을 로봇이 보고 따라하며 격투 게임을 할 수 있을 것으로 기대된다.

        게임엔진 기술을 활용한 체감형 게임기 제작

        The recent game products enjoyable with exercise are a good response from consumers. In this paper, arcade-type game is designed and implemented by utilizing the tangible game technologies. Firstly, 3D modeled virtual environment constructed by using 3D Max is introduced to the game space, and game program is made in order to execute through characters point of view. Secondly, by using these kinds of 3D model, game are implemented based on Unity 3D game engine, and synchronization system for detecting display position is designed using infrared sensor. Based on the game scenario, the prototype is completed and the level check of the game is also performed. Finally the finished game prototype is produced.
