        악교정 수술후 항생제 투여의 효용성 : 술후 항생제 투여 여부에 따른 창상감염 빈도

        This study observed the frequency of post-surgical infection according to post-surgical application of antibiotics in order to evaluate the benefits of the use of antibiotics after the orthognathic surgery. 349 patients without any specific medical history were divided into two groups depending on whether or not antibiotics had been applied after the surgery. The mean(SD) age of the 349 patients was 22.7(${\pm}4.25$) with a male-female ratio of 168:181. 226 patients received only 1.0g of a third-generation cephalosporin(Cefpiramide) intravenously 30 minutes prior to the surgery. Likewise, 123 patients received 1.0g of Cefpiramide 30 minutes prior to the surgery and twice daily longer than the third day after surgery. The mean(SD) duration of administration was 4.75(${\pm}0.89$) day. The patients were evaluated after surgery for any postoperative infections according to the criteria: purulent drainage from a wound, spontaneous wound dehiscence accompanied by swelling, pain, and fever around the wound. However, 14 patients of 226 patients received antibiotics only prior to the surgery developed postoperative infection, 2 patients of 123 patients received antibiotics longer than postoperative 3 days developed postoperative infection. Postoperative infection frequency showed no significant difference between the two groups(p=0.094). Also, bi-maxillary operation and mandibular operation alone, showed no significant difference in the frequency of post-surgical infection when antibiotics had been continuously used after the surgery. From this study, postoperative use of antibiotics seems to be unnecessary with view of the little significance of the factors that could affect the wound infection.

        하악 제3대구치 발치를 위한 예방적 항생제의 효용성

        This study analyzed the incidence of wound infection after the operation of mandibular third molar extraction in relation with antibiotic prophylaxis with the object of young and healthy patients. The study object was 1,177 mandibular third molars of 850 men of 20 to 25 years old without any specific systemic disease. Three methods of preventive antibiotic medication were selected according to the preventive antibiotic medication previously reported; three experimental groups were selected based on them, and the antibiotic used was amoxicillin($^{(R)}Kymoxin$, Yuhanyanghaeng, Seoul). The group 1 includes the patients that took the antibiotic orally before the operation(one hour earlier, 500mg) and for three days after the operation(250mg per time, three times/day), the group 2 is the ones that took the same antibiotic orally only once about one hour before the operation(500mg), and the group 3 did not take any antibiotics before and after the operation. And to compare the difficulties and the degrees of extraction during operations which can be possibly related to the wound infection after the operations, the mandibular third molars' impacted depths and extraction methods were investigated as well. To check if the wound was infected, observations with an internal of one week were performed twice after the operation, and the meaningfulness of the infection incidence was verified through Chi-square test using SPSS program(SPSS Inc., IL, USA). There was no statistically significant difference between the antibiotic medication methods and the wound infection incidence after the operation among the experimental groups. As examining the relations between the mandibular third molar operation methods and the wound infection incidence after the operation, there existed a statistically meaningful difference in the infection incidence according to the operation methods(p=0.020). And there was no statistically significant difference in the wound infection incidence according to the impacted depth of the mandibular third molar. Therefore, it is thought that there exists little necessity of prophylatic antibiotics medication when extracting the mandibular third molar of young and healthy men without any systemic disease in general; however, in case when it is expected that the possibility of infection will be high or the wound on the tissue will be severe, it is sure that the prophylactic antibiotics medication will be necessary.

        RI 폐기물 내 방사성요오드 분석을 위한 분리 및 회수

        Various kinds of RI wastes are discharged from licensed organizations of radioisotopes les such as hospitals and clinic organizations, educational organizations, research institutions, and public organizations. Radioiodines such as $^{125}I\;and\;^{131}I$ are radioisotopes mainly used in nuclear medicine and industry. A method for the determination of radioiodines in RI wastes has been applied to measure low level activity using acid decomposition method and HPGe gamma ray spectrometer. Prior to analysis of real samples, $^{131}I$ reference solution and 10 g of yellow tissue paper was added to flask in mantle and was heated in 100 mL of 0.4 N $K_2Cr_2O_7$ and 100 mL of 9 M $H_2SO_4$, and then distilled after adding 10 mL of 30% $H_2PO_3$ and 1 mL of 30% $H_2O_2$. The condensed iodine by circulator was extracted into $CCl_4$, then back-extracted into the aqueous phase with 10 mL of 5% $K_2SO_2$ solution. Finally, $^{131}I$ was measured at 364.48 keV using HPGe gamma ray spectrometer after precipitation and filtration. Chemical yield of three steps such as acid decomposition process, chemical separation process, and precipitation and filtration process was more han 94% respectively, MDA(Minimum Detectable Activity) of $^{131}I$ at this analytical condition was 0.6 Bq/g.

        일제강점기 박람회 건축을 통해 본 건축양식의 상징성

        This research examines symbolic meanings of architectural style of Japanese ruling era of Korea through analysis of Expo buildings. Expo buidings of Joseon Product Evaluation Expo(1915), Joseon Expo(1929), and Joseon Grand Expo(1940) are chosen as subjects of this research. Expos held in Korea since 1945 were creatures of colonial reign and their objectives are advertising the advancement of Japan. New and latest architectural styles of Western countries were used as a symbol of advancement in non-western countries. Renaissance style and Secession style in Joseon Product Evaluation Expo and Modernism style in Joseon Expo were introduced as architectural styles that symbolize advancement. On the contrary, the traditional architectural style of Joseon Dynasty was distorted as symbol of backwardness. Latest Western Architectural Styles were used elaborately and Intentionally to symbolize advancement and industrialization by Japanese ruling power and companies. Specially, Modernism style operating as 'symbol of advancement' is characteristic of non-western society in attempt to proceed Modernization through Westernization. Also, it can be suggested that architectures in Modernism style are used in ways to symbolize the advancement of the colonial reign authorities within the colonial society.

        악교정 수술시 술후 환자의 증상변화와 항생제 투여와의 관계

        This study compared and evaluated the variation of patients' symptoms depending on whether or not antibiotics had been used in the recovery period after the orthognathic surgery. 28 patients, with no particular medical history, were divided into two groups on the basis of whether antibiotics had been applied after the orthognathic surgery. All patients had been operated on maxilla and mandible simultaneously and were investigated with any symptom changes within the postoperative 4 days. Among the symptoms that could follow after the orthognathic surgery, 6 symptoms were checked every 24 hours on 5 points basis. Phases of each symptom improvements were checked and analyzed by ANOVA statistical method. Aspects of symptom changes according to application of antibiotics showed no statistical significance. Symptoms, except for swelling, showed continuous improvements after the surgery. Swelling increased until postoperative $2^{nd}\;day$, and then started to decrease. This study shows that post-surgical use of antibiotics has no significance provided that post-surgical infection had not occurred.

        자초(紫草)가 HaCaT 세포의 사이토카인 유전자 발현에 미치는 영향

        Objective : Propionibacterium acnes (P. acnes) is a major pathogenic bacteria for acne vulgaris. This study was performed to evaluate the effects of Lithospermum erythrorhizon extracts on the inflammatory cytokines gene expression by P. acnes in human keratinocytes, HaCaT cell line. Methods : Anti-bacterial activity and cytotoxicity of LE extracts was analyzed by agar plate culture and XTT assay. The cytokines gene expressions were assessed by real time RT-PCR for IL-8, MCP-1 and TNF-${\alpha}$. During the cell culture and treatments, amounts of secreted TNF-${\alpha}$ were measured by ELISA. Translocation of transcription factor NF-${\kappa}B$ from cytoplasm into nucleus was observed by immunocytochemistry and confocal microscopy. Results : There were no anti-bacterial effects and cytotoxicity as high as $1,000{\mu}g/ml$ of LE extracts in XTT assay. Transcription levels of inflammatory cytokines, IL-8, MCP-1 and TNF-${\alpha}$ were increased by P. acnes in HaCaT. LE extracts decreased the upregulated gene transcription levels. However, amounts of secreted TNF-${\alpha}$ were similar in HaCaT cells with P. acnes and LE extracts. Translocation of NF-${\kappa}B$ into nucleus by P. acnes was significantly inhibited by LE extracts. Conclusions : From the results of this study, LE extracts have anti-inflammatory effects on HaCaT cells by P. acnes that decreased the mRNA expressions of IL-8, MCP-1 and TNF-${\alpha}$. This anti-inflammatory effects of LE extracts could provide the potential of therapeutic substance for acne vulgaris.

        위상 최적설계를 이용한 APV Module Structure의 설계 및 구조해석

        본 논문은 해양 플랜트에 설치되는 압력 시스템 구조물의 위상 최적화를 통한 경량화 설계와 구조해석을 통한 구조안전성 평가에 대한 연구결과이다. 해양 플랜트에 설치되는 구조물은 기본적인 자중 외에도 해상에서 발생하는 풍하중 및 동하중에 따른 구조 설계가 매우 중요하고 구조 안전성 평가가 필수적이다. 본 연구에서는 DNV 선급 규정에 따라 풍하중 및 동하중을 상황별로 분류해 이를 해석에 적용하여 수행하였다. 경량 화된 형상을 얻기 위한 방법으로 위상 최적화 기법을 구조물에 적용하였다. 위상 최적화 해석을 통해 구조물에서 응력이 집중되는 부분을 확인할 수 있었다. 위상 최적화 해석을 통하여 설계 시 불필요한 요소를 제거한 형상을 얻어 보강대 형태를 가진 형태로 설계 하였다. 위상 최적화를 통해 얻은 설계 형태를 바탕으로 구조해석을 통한 안전성을 평가하여 형상에 대한 적합성을 확인하였다. 이를 통해 본 연구에서는 실제시험을 통해 구조안전성 평가가 어려운 해양플랜트 구조물의 설계 및 안전성평가 방법을 제시하였다. This paper presents the research results of a light weight through topology optimization and structural safety evaluation through structural analysis of a pressure system structure installed in an off-shore plant. Conducting a structure design according to the wind load and the dynamic load at sea in addition to a self-load and structure stability evaluation are very important for structures installed in off-shore plants. In this study, the wind and dynamic load conditions according to the DNV classification rule was applied to the analysis. The topology optimization method was applied to the structure to obtain a lightweight shape. Phase optimization analysis confirmed the stress concentration portion. Topology optimization analysis takes the shape by removing unnecessary elements in the design that have been designed to form a rib shape. Based on the analysis results about the light weight optimal shape, a safety evaluation through structural analysis and suitability of the shape was conducted. This study suggests a design and safety evaluation of an off-shore plant structure that is difficult for structural safety evaluations using an actual test.

        구조변동이 아시아 주식시장과 원유 선물시장의 변동성 전이에 미치는 영향

        최근의 금융위기로 인하여 주식시장과 원유시장간의 상관관계가 높아지고 있으며 이는 포트폴리오 위험 관리에 중요한 요인으로 간주되고 있다. 본 연구는 이변량 DCC(dynamic conditional correlation)-GARCH 모형을 이용하여 구조변동이 아시아 주요주식시장과 원유 선물시장의 변동성 전이에 미치는 영향을 분석하였다. 주요 분석결과는 다음과 같다. 우선, 아시아 주식시장과 원유선물시장간의 높은 정의 상관관계를 발견하였다. 그리고 최근의 금융위기가 그러한 정의 상관관계를 강화시키는 것으로 나타났다. 이는 금융시장 구조변동이 변동성 전이효과에 영향을 주는 것을 의미한다. 또한, 최적포트폴리오 가중치와 헷지 비율을 분석한 결과, 투자자들이 시간에 따라 두 시장간의 최적 포트폴리오 가중치와 헷지 비율을 변경하는 것을 알 수 있었다. 이러한 결과는 금융위기와 같은 시장 구조변동에 따라 투자자들이 최적의 포트폴리오 투자전략을 수립하기 위해서 가중치와 헷지 비율을 조정하는 전략을 취하는 것으로 판단된다. 본 연구의 분석결과를 종합해 볼 때, 금융시장에서 발생되는 구조변동이 금융시장의 전이현상에 영향을 주고 있으며 이러한 관계는 포트폴리오 투자전략에 영향을 미치는 중요한 요인으로 작용하는 것으로 나타났다. This paper investigates the impact of structural breaks on volatility spillovers between Asian stock markets (China, Hong Kong, India, Indonesia, Japan, Korea, Singapore, and Taiwan) and the oil futures market. To this end, we apply the bivariate DCC-GARCH model to weekly spot indices during the period 1998-2015. The results reveal significant volatility transmission for the pairs between the Asian stock and oil futures markets. Moreover, we find a significant variability in the time-varying conditional correlations between the considered markets during both bullish and bearish markets, particularly from early 2007 to the summer of 2008. Using the modified ICSS algorithm, we find several sudden changes in these markets with a common break date centred on September 15, 2008. This date corresponds to the collapse of Lehman Brothers which is considered as our breakpoint to define the global financial crisis. Also, we analyse the optimal portfolio weights and time-varying hedge ratios based on the estimates of the multivariate DCC-GARCH model. The results emphasize the importance of overweighting optimal portfolios between Asian stock and the oil futures markets.

        CFD를 이용한 회전 운동을 하는 이젝터의 최적화

        본 연구는 오존 방식 선박평형수 처리의 주요 장치인 기체-액체 이젝터에 대하여 구동 노즐의 유입부에 회전 운동유도 장치에 의해 발생되는 구동 유체의 회전 운동이 이젝터 효율에 미치는 영향에 관한 연구이다. 먼저 배압에 따른 이젝터의 각 포트별 압력과 흡입 유량을 확인하기 위하여 실험 장치를 구성하고, 회전 운동이 없는 이젝터에 대한 실험 수행 및 데이터를 획득한다. 실험의 데이터를 바탕으로 격자 사이즈 비교를 통해 기체-액체 이젝터에 적합한 유한요소모델을 선정하였으며, 도출된 CFD 모델을 이용하여 구동 유체의 회전 운동이 이젝터의 흡입 효율을 향상시킬 수 있음을 확인하였다. 이를 바탕으로 이젝터의 흡입 유량을 높이기 위하여, 메타 모델을 이용한 크리깅 기법을 사용하여 회전 유도 장치의 내부 형상 인자인 전체 길이  , 내부 직경 , 날개 개수 에 대한 구조 최적화를 수행한다. 최적화된 회전 유도 장치를 적용한 결과 구동 유체의 회전 운동이 없는 이젝터에 비해 이젝터 효율이 약 3% 가량 개선됨을 확인하였다. This paper investigates the effect of the rotational motion of a driving fluid generated by a rotational motion device at the inlet of a driving nozzle for a gas-liquid ejector, which is the main device used for ozonated ship ballast water treatment. An experimental apparatus was constructed to study the pressure and suction flow rate of each port of the ejector according to the back pressure. Experimental data were acquired for the ejector without rotational motion. Based on the data, a finite element model was then developed. The rotational motion of the driving fluid could improve the suction efficiency of the ejector based on the CFD model. Based on the CFD results, structure optimization was performed for the internal shape of the rotation induction device to increase the suction flow rate of the ejector, which was performed using the kriging technique and a metamodel. The optimized rotation induction device improved the ejector efficiency by about 3% compared to an ejector without rotational motion of the driving fluid
