Nd:YAG 레이저 후낭절개술 후 유리체탈출에 의해 발생한 동공차단 및 안압상승

Purpose: To report a case of pupillary block and increased intraocular pressure caused by vitreous prolapse after Nd:YAG laser posterior capsulotomy. Case summary: A 70-year-old male visited the hospital for decreasing visual acuity and ocular pain in the left eye. Two days earlier, he had undergone Nd:YAG laser posterior capsulotomy in the left eye. Best corrected visual acuity (BCVA) was 0.63 in the right eye and FC 60 cm in the left eye. Intraocular pressure (IOP) was 14 mm Hg in the right eye and 64 mm Hg in the left eye. Slit-lamp examination revealed diffuse corneal stromal edema and iris bombe with vitreous prolapse in the anterior chamber. Gonioscopy confirmed a closed angle. Pupillary block and increased intraocular pressure were diagnosed. A Nd:YAG laser iridotomy was performed. Three days after the iridotomy, BCVA was 0.2 and IOP was 11 mm Hg in the treated eye. Slit-lamp examination revealed reduced vitreous prolapse in the anterior chamber. Conclusions: This is the first report of pupillary block and increased IOP caused by vitreous prolapse after Nd:YAG laser posterior capsulotomy in Korea. Physicians should be aware that pupillary block could be caused by prolapsed vitreous after Nd:YAG laser posterior capsulotomy and Nd:YAG laser iridotomy could be performed effectively in such cases.

폐의 저악성 점액성 낭종은 보고된 예가 매우 드물며 기관지성 낭종 및 뮤신을 분비하는 기관지폐포암 형태의 선암과 감별이 필요하다. 63 세의 여자환자로서 흉부단순촬영 사진에서 우측 폐하엽에서 종괴가 관찰되었다. 술전 경피생검상에서 기관지폐포암으로 진단되었고 우측 폐하엽 절제술 및 임파절 제거술을 시행하였다. 절제된 폐내에서 다양한 크기의 다발성 낭성 종괴들이 보였고 종괴내에는 점액으로 채워져 있었다. 현미경상에서 낭종은 키가 큰 주상구조의 점액성 상피세포들로 구성되어 있었고 한정된 일부분에서 기관지폐포암양 병소가 관찰되었으나 기관지폐포암과 비교하여 세포의 비정형성(atypism)은 부족하였다. 수술후 환자는 합병증없이 퇴원하였고 12개월 추적관찰에서 재발이나 원격전이 소견은 보이지 않았다. 폐의 저악성 점액성 낭종은 술후 예후가 상당히 좋은 것으로 보고 되고 있으며 폐에서 발생하는 다른 신생물과는 감별이 필요할 것이다. Pulmonary mucinous cystic tumor of borderline malignancy is very rare and distinguished from bronchogenic cyst or adenocarcinoma of bronchoalveolar type. We present the case of a 63-year-old woman with a right lower lobe mass, found by chest radiographs. The preoperative diagnosis was made as bronchoalveolar cancer by percutaneous needle aspiration of mass. Right lower lobectomy and lymph node dissections were performed. The lobectomy specimen contained variable sized multilocular cystic mucous masses, filled with mucus. Microscopically, the cystic masses are lined with tall columnar mucinous epithelium but some area contains focal cellular atypism and bronchoalveolar cancer like foci. This foci are lack of cellular atypism consistent with bronchoalveolar cancer cell. After lobectomy the patient has remained free from recurrence and distant metastasis for following 12 months period. Pulmonary mucinous cystic tumor of borderline malignancy appears to have a favorable prognosis and should be distinguished from other lung neoplasms.
근시 환자에서 노안교정 비구면 미세단안시 라식 수술의 장기적인 임상 결과

Purpose: To evaluate the efficacy, safety, and satisfaction of patients who underwent LASIK for presbyopia correction in myopic patients using aspheric micro-monovision.<br /> Methods: LASIK for presbyopic correction using aspheric micro-monovision was performed in 18 patients between December 2010 and December 2011. Distance, intermediate, and near visual acuity, refractive change, and patient’s satisfaction were evaluated for at least 12 months after the surgery.<br /> Results: Among dominant eyes, 100% achieved uncorrected distance and intermediate visual acuity of 0.8 or better and 100% of the eyes achieved 0.8 or better binocularly. In the non-dominant eyes, 83% achieved uncorrected near visual acuity of J3 or better, and 94% of the eyes achieved J3 or better binocularly. Postoperatively, the mean manifest refraction spherical equivalent (MRSE) of the dominant eyes were -0.09 ± 0.35D, -0.17 ± 0.42D, and -0.17 ± 0.47D at 1, 6 and 12 months, respectively. The MRSE of the non-dominant eyes were -0.94 ± 0.53D, -1.03 ± 0.56D, and -1.02 ± 0.50D at postoperative 1, 6, and 12 months, respectively, without significant regression. After surgery, the patient’s overall satisfaction score was good (4.2 out of 5).<br /> Conclusions: The aspheric micro-monovision protocol showed good distance, intermediate, and near visual outcomes, and was a well-tolerated, stable, and effective procedure for treating patients with myopic presbyopia.<br /> J Korean Ophthalmol Soc 2013;54(12):1893-1901

The most widely used piezoelectric ceramics are lead oxide based ferroelectrics (PZT). However, the toxicity of lead oxide and its high vapor pressure during processing have led to a demand for alternative lead-free piezoelectric materials. We synthesized Lead-free piezoelectric ceramics of $0.96(K_{0.5}Na_{0.5})NbO_3-0.04SrZrO_3+x$ mol% $ZrO_2$ [KNN-SZ+$xZrO_2$; x= 0~0.10] doped with 0.1 wt% $MnO_2$ by a conventional solid state method. We investigated the piezoelectric properties and microstructures of these disk samples sintered in reduced atmosphere in order to evaluate the possibility of the multilayered piezoelectric ceramics having the base metal such as Ni as a internal electrode. All of these KNN-SZ samples sintered in 3%$H_2-97%N_2$ atmosphere at $1,140^{\circ}C$ exhibit pure perovskite structure irrespective of the content of $ZrO_2$. Meanwhile, the sintering denisty and piezoelectric properties such as $K_p$, $Q_m$ and $d_{33}$ of KNN-SZ samples as a function of $ZrO_2$ content show the maxima ($k_p$=28.07%, $Q_m$= 101.34, $d_{33}$= 156 pC/N) at x= 0.04 and it is likely that there is some morphotropic phase boundary(MPB) in this KNN-SZ+$xZrO_2$ composition system. These results indicate that the ceramic composition is a promising candidate material for applications in lead free multilayer piezoelectric ceramics.
CANoe를 이용한 ISO11783 기반 자율주행 트랙터를 위한 제어시스템 개발 기초연구

유럽과 미국 등 해외 선진국에서는 농업의 생산성 향상, 작업의 안정성을 위해 자동화 제어기술을 이용한 농용 작업기 및 트랙터의 활용이 증가하고 있다. 다양한 농작업에 필요한 작업기를 효율적으로 작동시키기 위해서는 트랙터와 작업기 간의 통신과 다양한 작업기들 간의 통신기술이 필요하다. ISOBUS는 농업용 트랙터와 작업기들 간의 통신 및 제어을 위해 국제 표준 규격 중의 하나인 ISO11783과 J1939 표준을 기초로 하여 트럭, 버스, 건설 및 농업용 응용프로그램에 공통으로 적용될 수 있는 국제적 통신 규약이다. 본 연구에서는 ISO11783 기반 자율주행 트랙터에 적용되는 조향각, 작업기의 각도를 실시간으로 감지하고 주행 중인 트랙터의 조향각과 작업기 자동 제어기술 개발을 위한 통신프로토콜 정리 및 응용을 목표로 한다. 자율주행 트랙터 조향각과 작업기 제어를 위해 CANoe 8.0 소프트웨어를 이용하여 CAN 통신 기반 가상 ECU를 가상터미널로 구성하여 CAN 메시지를 실시간으로 수신하도록 제작되었고, 하드웨어 인터페이스로 임베디드 보드(VN1630A, Vector)를 사용하였다. 가상 조향, 작업기 ECU 개발은 Vector社의 CANoe를 이용하여 C언어와 유사한 Vector社에서 개발한 CAPL 코드를 사용하여 개발하여 실험을 진행하였다. 실험은 가상 ECU로 개발된 알고리즘을 CANoe 시뮬레이션에서 실험하였으며, CAN 메시지의 실시간 수신을 확인함으로써 추후 실제 환경에서 자율주행 트랙터에 적용 가능함을 확인했다.
한국어 거절 화행에 대한 중국인 학습자와 러시아인 학습자의 인식 연구

The purpose of this study is to analyze Korean refusal speech acts in various situations and contexts and to clarify Korean language cultural characteristics and their communicational components. Besides, by collecting and analyzing refusal expressions of Chinese speaking Korean learners and Russian speaking Korean learners in given situations, we tried to understand Chinese and Russian language cultural characteristics from a cross-cultural point of view. The result of this study can be utilized as fundamental materials for textbooks or data for explanation, references for discussion. Refusal is to express the willingness to ignore the other’s suggestion or to turn down the other’s request. It can be offensive sometimes. Therefore refusal is often conducted through special language strategies or certain expressions. Koreans’ refusal speech acts are also often performed without clear expression of refusals. Because of this many foreign learners have experienced the actual failure in communications with Korean native speakers even though they have reached high level of Korean language competence. This kind of communicational failure is not because they do not know certain supposition expressions nor certain interrogative expressions. Misunderstanding of Korean language culture and transferring their own pragmatic knowledge into the speaking situation in Korea bring communication failure. We think that the fundamental solution for this kind of problem is to let foreign students understand language culturalmechanism working in Korean refusal speech acts. To extract the language cultural characteristics of Korean refusal speech acts we designed a set of DCT and developed structured interview questions. We conducted these surveys of Korean native speakers, Chinese speaking learners and Russian speaking learners. As a result, we could deduce 5 Korean language cultural characteristics from refusal speech acts, which are modesty, grant, bond, face, sympathy. And also we could notice that China has similar language culture from Korean while Russians show quite different features in terms of directness, politeness and clearness.This study can be used as basic materials for textbooks or explanatory data, references for discussions, hopefully.
