        성인요추 척추전방전위증의 외과적 치료 : 치료방법간의 비교 Comparision of Various Method

        In retrospective study of 35 cases(male=17. female=18) of lumbar spondylolisthesis(isthmic 24 degenerative 6 iatrogenic 5) treated with different surgical management. we elucidate the result of follow-up radiologic changes and surgical outcome Percent-slip was reduced and percent-disc height was increased in all three groups at 1 month after the operation But S months follow-up after the operation revealed that percent-slip was increased to around the value of preoperation in PPLF group(Pedicle screw flxation with posterolateral fusion. 13cases) and PLIF group(Posterior lumbar interbody fusion only. 11cases) and that percent-disc height was decreased compared to postoperative 1 month in PPLF group. Although PPlF group(Pedicle screw fixation with posterior lumbar interbody fusion, 11cases) showed better correction and maintenance But there was no significant differences in overall outcome among the three surgical groups(PPLF 76.9% PLIF : 81.8% PPIF 81 8%, % of satisfaction) and the outcome of surgery was not related with bone union rate(PPLF=76.9% PLIF=72.7%, PPIF=81.8%) or accompanied NIC and HNP. There were S patients with complications(17.1%), such as two wound infection. two screw fractures and two others. Further longterm follow-up and evaluation is needed for clarifying the efficiency and superiority in surgical options associated with type. degree of slip, complications. bone fusion and patient satisfaction.

        Occurrence and Biotransformation of Ochratoxin A during Pepper Sauce Fermentation

        Gochujang, a savory and pungent fermented sauce, is a Korean ethnic food composed primarily of fermented soybean cake, red pepper powder, glutinous rice, and salt. Industrial gochujang is fermented following inoculation with $Aspergillus$ $oryzae$ and $Bacillus$ $subtilis$ as starter microorganisms. In contrast, a wide variety of natural fungi and bacteria are used in fermentation and aging of artisanal gochujang, the aging period of which is longer than 6 months. We monitored ochratoxin A (OTA) contamination of red pepper and gochujang available in the Korean retail market and explored the possibility that microorganisms in gochujang might degrade OTA present in contaminated red pepper during soybean fermentation and aging. We report here that OTA can be easily biotransformed serially to several products by $B.$ $subtilis$ (KTCT 1021) under the conditions of our experiments. The dynamic microbial activity in gochujang might contribute to the low occurrence of ochratoxin A in this product despite the use of highly contaminated pepper powder, one of the main ingredients of gochujang. At the same time, however, an inappropriate fermentation process and the long aging period are assumed to be sources of contamination. Studies to identify critical control points and to prevent fungal spoilage by the development of high quality starters should be followed up to enable the artisan to produce OTA-free gochujang.

        하부 요추 방출 골절의 수술방법 결정시 고려 요인들

        Objectives : Burst fracture of the lower lumbar spine(L3-L5) is rare and has some different features compare to that of thoracolumbar junction. Lower lumbar spine is flexible segments located deeply, and has physiologic lordosis. All of these contribute to making surgical approach difficult. Generally, lower lumbar burst fracture is managed either anteriorly or posteriorly with various fixation and fusion methods. But there is no general guideline or consensus regarding the proper approach for such lesion. We have tried to find out the influencing factors for selecting the surgical approach through the analysis of lower lumbar burst fractures treated for last 4 years(1994.3-1998.3). Method : This study includes 15 patients(male : 10, female : 5, age range 20-59 years with mean age of 36.7 years, L3 : 8 cases, L4 : 5 cases, L5 : 2 cases). Patients were classified into anterior(AO) and posterior operated(PO) groups. We investigated clinical findings, injured column, operation methods, and changes in follow-up radiologic study (kyphotic angle) to determine the considerable factors in selecting the surgical approaches. Results : There were 5 AO and 10 PO patients. Anterior operation were performed with AIF with Kaneda or Z-plate and posterior operation were done with pedicle screw fixation with PLIF with cages or posterolateral fusion. Canal compression was 46.6% in AO and 38.8% in PO. The degree of kyphotic angle correction were 10.7 degree(AO) and 8.5 degree(PO), respectively. There was no statistical difference between anterior and posterior operation group. All patients showed good surgical outcome without complications. Conclusion : Anterior operation provided good in kyphotic angle correction and firm anterior strut graft, but it difficulty arose in accessing the lesions below L4 vertebra. While posterior approach showed less correction of kyphotic angle, it required less time and provided better results for accompanied adjacent lesion and pathology such as epidural hematoma. The level of injury, canal compression, biomechanics, multiplicity, and pathology are considered to be important factors in selection of the surgical approach.

        백서 편측 설신경 손상 후 미각 및 설유두의 변화에 대한 연구

        Purpose of study: Lingual nerve damage can be caused by surgery or trauma such as physical irriatation, radiation, chemotherapy, infection and viral infection. Once nerve damage occurred, patients sometimes complain taste change and loss of taste along with serious disturbance of tongue. The purpose of this study was to evaluate the effects of unilateral lingual nerve transection on taste as well as on the maintenance of taste buds. Materials & Methods: Male Sprague-Dawley rats weighing 220-250g received unilateral transection of lingual nerve, subjected to the preference test for various taste solutions (0.1M NaCl, 0.1M sucrose, 0.01M QHCl, or 0.01M HCl) with two bottle test paradigm at 2, 4, 6, or 8 weeks after the operation. Tongue was fixed with 8% paraformaldehyde. After fixation, they were observed with scanning electron microscope(JSM-840A, JEOL, JAPAN) and counted the number of the dorsal surface of the fungiform papilla for changes of fungiform papilla. And, Fungiform papilla were obtained from coronal sections of the anterior tongue(cryosection). After cryosection, immunostaining with Gαgust(I-20)(Santa Cruz Biotechnology, USA), PLCβ2(Q-15)(Santa Cruz Biotechnology, USA), and T1R1(Alpha Diagnostic International, USA) were done. Immunofluorescence of labeled taste bud cells was examined by confocal microscopy(F92-300, Olympus, JAPAN). Results: The preference score for salty and sweet tended to be higher in the operated rats with statistical significance, compared to the sham rats. Fungiform papilla counting were decreased after lingual nerve transaction. In 2 weeks, maximum differences occurred. Gustducin and T1R1 expressions of taste receptor in 2 and 4 weeks were decreased. PLCβ2 were not expressed in both experimental and control group. Conclusion: This study demonstrated that the taste recognition for sweet and salty taste changed by week 2 and 4 after unilateral lingual nerve transection. However, regeneration related taste was occurred in the presence of preserving mesoneurial tissue and the time was 6 weeks. Our results demonstrated that unilateral lingual nerve damage caused morphological and numerical change of fungiform papilla. It should be noted in our study that lingual nerve transection resulted in not only morphological and numerical change but also functional change of fungiform papillae.

        Coexistence of Subcutaneous Dermoid Cyst and Lipomyelomeningocele

        Although lipomyelomeningocele and dermoid cyst are formed during a similar embryologic developmental stage of 'neural tube formation', they are caused by entirely different embryologic mechanisms. We encountered a case, which has not been previously reported, that had a lumbar subcutaneous dermoid cyst associated with lipomyelomeningocele. A 52-year-old man presented with a slowly growing lumbar mass of 3 years duration, which had been present since birth. Lumbosacral magnetic resonance imaging[MRI] showed lipomyelomeningocele at the L3, 4 and 5 levels and a subcutaneous cystic mass of high signal intensity on the T2 weighted image and iso or low signal intensity on the T1 weighted image. He underwent total resection of the lumbar subcutaneous mass. Intraoperative findings and histological examination were consistent with the preoperative diagnosis of a 'dermoid cyst'. We demonstrate that the formation of a dermoid cyst might coexist with lipomyelomeningocele during the embryologic developmental stage.

        Thoracic and lumbar laminoplasty using a translaminar screw: morphometric study and technique

        <B>Object</B><P>The aim of this study was to describe a novel technique for laminoplasty in which translaminar screws are used in the thoracic and lumbar spine.</P><B>Methods</B><P>The authors first performed a morphometric study in 20 control individuals using 3D reconstructed CT scans and spine simulation software to measure the lengths and diameters of the spaces available for translaminar screw placement from the T-1 to S-1.</P><P>Based on the results of the morphometric study, the authors then attempted translaminar screw fixation in 5 patients (April 2007-July 2007) after en bloc laminectomy in the thoracic and lumbar regions. All patients had intradural lesions: 3 schwannomas, 1 cavernoma, and 1 arachnoid cyst.</P><B>Results</B><P>The morphometric study in control individuals revealed that the safe trajectories for simulated screws measured 25-30 mm in length and 8-11 mm in diameter in the thoracic region (T1-12) and 26-34 mm in length and 6-7 mm in diameter in the lumbosacral region (L1-S1). This morphometric and simulation study showed that translaminar screw placement would be possible in practice.</P><P>Five patients underwent en bloc laminoplasty and translaminar screw fixation in which the screws measured 2.7 mm in diameter and 24 or 26 mm in length. Sixteen attempts at translaminar fixation were made in 8 vertebrae. Fourteen translaminar screws were successfully placed at the thoracic and lumbar levels. Two microplates had to be used because the laminae were too thin and narrow after further laminectomy with undercutting. There were no complications associated with the translaminar screws.</P><P>The mean follow-up period was 14.5 months. There was no screw breakage or displacement. Solid osseous fusion was documented in 2 patients who underwent CT scanning 15 months postoperatively.</P><B>Conclusions</B><P>The authors found that the laminoplasty and translaminar screw technique is feasible in the thoracic and lumbar regions, but further studies are needed to analyze the biomechanical effects and long-term outcomes in a large number of patients.</P>
