        하수의 유기물 분류에 미치는 동력학적 영향인자의 민감도 분석

        The effects of 6 kinetic parameters such as Y_H (yield coefficient of heterotrophs), b_H (decay rate of heterotrophs, /d), μ_HR (heterotrophic maximum specific growth rate on RBCOD, /d), μ_HS (heterotrophic maximum specific growth rate on SBCOD, /d), K_S (heterotrophic half saturation coefficient for RBCOD, ㎎COD/L) and K_X (heterotrophic half saturation coefficient for SBCOD) on the variation of RBCOD (readily biodegradable COD) concentration were analyzed during the COD fractionation of primary settled municipal wastewater by simulation of respirometric tests. Variation of RBCOD concentration was the highest when Y_H was changed within the acceptable range and standard deviation was about 48% of the averaged RBCOD concentration. Standard deviations of RBCOD concentration were about 10.4 and 6.3% of the averaged values when μ_HS and K_X were changed, respectively and it was about 6.5% when b_H was changed. The effects of μ_HR and K_S on RBCOD estimation were lower than the other kinetic parameters and the variations of RBCOD concentration were only about 2.2 and 3.5% of the averaged value when those two kinetic parameters were changed, respectively. Accordingly, hydrolysis or oxidation of SBCOD (slowly biodegradable COD) was more important than RBCOD oxidation during the analysis of respirometric test. It means that the variation of RBCOD concentration was more sensitive to the change of μ_HS and K_X than that of μ_HR and K_S because μ_HR and K_S did not directly affect the RBCOD concentration in wastewater. However, μ_HS and K_X could affect the RBCOD concentration through hydrolysis of SBCOD.

        Under the pressure to open the service market as proposed by World Trade Organization, we can easily expect our educational systems for allied health professions will experience a lot of changes in near future. Accordingly, we compared our current educational systems for allied health professions with the foreign ones and mapped out some strategies predicting the forthcoming enormous changes. Major investigations and analysis are summed up as follows, The educational programs for allied health professions which was started in 1963 in Korea are stilled remained as 2 to 3 school years, while the systems in advanced countries have already been changed or been changing from 2 year to 4 year college levels. We can hardly find the uniform educational system or school years for the allied health professions in the world except for Korea. Fortunately, university level four-year programs for the profession was established in 1979 in Won-joo for the first time in Korea. However, there are still only few four-year programs in this country. To meet the rapidly changing needs we have faced in the health areas we should supply appropriate health personnel through various efforts. Now we would like to suggest followings, <Educational goals for the allied health professions> 1) reconfirming characteristics for the educational goals of health professions 2) extending school years for the allied health professions 3) strengthening clinical training and internship 4) expanding opportunities for the continuing education through lifelong education 5) upgrading from old-fashioned technologies to advanced technologies 6) cultivating competencies of health professionals for the global competition <Suggestions for the Development of Allied Health Professions> 1) reforming the school years of educational programs for the allied health professions to meet the global standards 2) expanding the role of junior college programs for the allied health professions through the development of intensive courses in the professions. 3) diversifying and specializing the allied health professions mainly through 4 year B.S. degree programs.

        연속회분식 반응조를 이용한 슬러지 소화에서 포기비율과 포기빈도가 슬러지 소화에 미치는 영향

        Lab-scale sequencing batch experiments were carried out to investigate the effects of aeration ratio and aeration interval on nitrogen and phosphorus transformation during the sludge digestion by intermittent aeration. The highest nitrification and denitrification efficiency could be obtained when the aeration ratio was 0.25. Nitrification inhibition caused by alkalinity deficit was not observed at the aeration ratio of 0.75, however denitrification was limited at this level of aeration ratio. Biological phosphorus accumulation was possible at the aeration ratio of 0.25, however it was not observed when the aeration ratio was over 0.5. Under the same condition of aeration ratio, aeration interval played an important role in phosphorus release. Soluble phosphorus concentration of digested sludge was about 1.5㎎/L when the aeration interval was 4 hr and that was much lower than the concentration (12-18㎎/L) when the aeration interval was 20-44 hrs.

        The enzyme, squalene synthase, represents a branch point in the isoprenoid pathway capable of diverting carbon flow specifically to the biosynthesis of phytosterol and Triterpenoid. Transgenic Eleutherococcus senticosus plants were prepared by introducing the genes for squalene synthase derived from Panax ginseng (PgSS1), hygromycin phosphotransferase (HPT) and green fluorescent protein (GFP) through Agrobacterium-mediated transformation. Establishment of transgenic plantlets were confirmed by the presence of PgSS1 and HPT bands in the genomic DNA preparation using a PCR method and a Southern blotting. In addition, expression of the introduced GFP DNA was confirmed by observing green fluorescence of GFP from the embroyos. In the in vitro analysis of the PgSS1 enzymatic activities the transgenic plants showed to have 1.5 to 3 times higher than wild type plant, indicating that the PgSS1 genes was well over-expressed in the transgenic plants. Especially the PgSS1 acivities of embroyogenic cell was 120 times higher than those of plantlet.

        A new device, muscle clamping system was developed to facilitate exact quatifying technique A strabismus surgery, and reduce the risk of complications. The device is composed of a lower fixing body with three jaws and an upper supporting body. They are used to clip an extraocular muscle and fix it to the sclera with a single bite. Superior rectus recession on 16 rabbit eyes were performed with this new device. Conjunctival injection, muscle adhesion strength, and light microscopic findings were examined at 1, 2, 4 and 8 weeks postoperatively. The Conjunctival injection were minimal, adhesion power ranged from 420 to 600 gram gravity, which is sufficient in withstand the normal pull of human extraocular muscle. In microscopic exam, some Inflammatory cells and fibrosis were found. The new device was technically easy, fast, and accurate, so it may be useful in stabismus surgery.

        Biodegradable retinal fixation devices obtain mechnical fixation of the retina with desirable chorioretinal scarring and with the potential for local, sustained release of antimetabolites and steroids to inhibit proliferative vitreoretinopathy. We manufactured a biodegradable retinal tack with barb that was designed in order to prevent intrusion from implantation of retinal tacks. This study was carried to evaluate the efficacy for retinal fixation and the capability for sustained release of drugs with a newly designed biodegradable retinal tack Biodegradable retinal tacks were made of polymers of glycolic acids and were designed with barbs in a shape to prevent the disinsertion. Biodegradale retinal tacks are divided into 3 parts, a conical portion that is inserted into the sclera, a cylinder portion that remains in the vitreous, and a neck portion between the pin and the cylinder. The tapered conical end was manufactured to allow easy insertion through the retina and choroid into the sclera. A cylinder portion was manufactured with a tapered angle that fixes firmly into the orifice of 19 gauge spinal needle. A neck portion, 0.4 mm in diameter, was designed to prevent disinsertion from following implantation of retinal tack. The applicator was a 19 gauge spinal needle and its orifice was prepared to 15°angle to accept the tapered cylinder portion of the retinal tack. The retinal tacks, secured in the needles, were passed through the formed vitreous and inserted into the retina, choroid, and sclera and were released by pushing the internal needle, usually within 2-3mm of the medullary ray of the posterior rabbit retina A retinal tack was placed in each of 8 pigmented rabbit eyes. Slit-lamp biomicroscopy, indirect ophthalmoscopy and fundus photography were performed periodically from 1 day to 8 weeks after surgery. Eight eyes were enucleated and studied by light microscopy at 8 weeks. Biomicroscopic evaluation of the animals revealed edemas adjacent to the retinal surfaces immediately after insertion of the biodegradable retinal tacks in all the animals. These edemas disappeared after 1 week. The first noticeable change in the size of retinal tacks was shown after 2weeks. The size of the retinal tacks gradually got smaller, decreasing to about one-half at 4 weeks and about one-third at 8 weeks. All retinal tacks remained in inserted places without any movement for an 8 week period. On light microscopy, epiretinal proliferations were seen to extend into the vitreous cavity. Cellular capsules that lined the inner aspect of the scleral defect caused by tack insertion were found. However the adjacent retina had a normal cytologic appearance and architecture in all specimens. We manufactured a biodegradable retinal tack that is designed to prevent intrusion from implantation of retinal tacks. All biodegradable retinal tacks reduce in size with time, but no retinal tacks extruded from the inserted place. The newly designed biodegradable retinal tack can be used for retinal fixation and may be used as a vehicle for the introduction of pharmacologic agents to prevent the cellular events that promote proliferative vitreoretinopathy.

        Assessment of Disease Burden and Immunization Rates for Vaccine-Preventable Diseases in People Living with HIV: The Korea HIV/AIDS Cohort Study

        Background: Prophylactic immunization is important for human immunodeficiency virus (HIV)-infected patients; however, there are insufficient data on the burden of vaccine-preventable diseases (VPDs), vaccination rates, and factors influencing vaccination. Materials and Methods: The incidence and prevalence of VPDs in HIV-infected patients between 2006 and 2017 were estimated using the Korean HIV/acquired immune deficiency syndrome (AIDS) cohort database. In addition, we evaluated the vaccination rates and influencing factors for vaccination in HIV-infected patients through multilevel analysis of clinico-epidemiological factors, immune status, and psychological status. A questionnaire survey was conducted among experts to determine whether they recommend vaccination for HIV-infected patients. Results: The incidence rates of hepatitis B virus (HBV) infection, herpes zoster, and anogenital warts were 1.74, 7.38, and 10.85 per 1,000 person-years, respectively. The prevalence of HBV infection and anogenital warts at enrollment was 4.8% and 8.6%, respectively, which increased to 5.3% and 12.0%, respectively, by 2017. In HIV-infected patients, HBV (21.7% in 2008, 56.3% in 2013, and 75.4% in 2017) and pneumococcal vaccination rates (3.0% in 2015, 7.6% in 2016, and 9.6% in 2017) increased annually, whereas the influenza vaccination rate remained similar by season (32.7 - 35.6%). In the multilevel analysis, peak HIV viral load (≥50 copies/mL: odds ratio [OR] = 0.64, 95% confidence interval [CI]: 0.44 - 0.93; reference, <50 copies/mL) was an influencing factor for pneumococcal vaccination, while nadir CD4 T-cell counts (200 - 350 cells/mm3: OR = 0.54, 95% CI: 0.38 - 0.76; <200 cells/mm3: OR = 0.89, 95% CI: 0.62 - 1.28; reference, ≥350 cells/mm3) was an influencing factor for HBV vaccination. Influenza vaccination was associated with male sex (OR = 1.94) and the number of antiretroviral therapy (ART) regimen change (OR = 1.16), but was not significantly associated with HIV viral load or CD4 T-cell counts. Most experts responded that they administer hepatitis A virus, HBV, pneumococcal, and influenza vaccines routinely, but not human papillomavirus (12.9%) or herpes zoster vaccines (27.1%). Conclusion: The burden of vaccine-preventable diseases was quite high in HIV-infected patients. Nadir CD4 T-cell counts, peak HIV viral loads, and the number of ART regimen change are significant factors related to vaccination. Considering the low vaccination rates for VPDs, there was a discordance between experts' opinions and real clinical practice in the medical field.

        Chemical Modification of Bovine Brain Succinic Semialdehyde Reductase by Diethylpyrocarbonate

        The NADPH-dependent succinic semialdehyde reductase is one of the key enzymes in the brain GABA shunt, and it catalyzes the formation of the neuromodulator γ-hydroxybutyrate from succinic semialdehyde. This enzyme was inactivated by diethylpyrocarbonate (DEP) with the second-order rate constant of 1.1×10³ M-¹min-¹ at pH 7.0, 25℃, showing a concomitant increase in absorbance at 242 nm due to the formation of N-carbethoxyhistidyl derivatives. Complete inactivation of succinic semialdehyde reductase required the modification of five histidyl residues per molecule of enzyme. However, only one residue was calculated to be essential for enzyme activity by a statistical analysis of the residual enzyme activity. The inactivation of the enzyme by DEP was prevented by preincubation of the enzyme with the coenzyme NADPH but not with the substrate succinic semialdehyde. These results suggest that an essential histidyl residue involved in the catalytic activity is located at or near the coenzyme binding site of the brain succinic semialdehyde reductase.
