Background: Information concerning the cardiopulmonary effects of pneumoperitoneum in children is lacking. Methods: Twenty eight patients were assigned to receive diagnostic laparoscopy (n=12) or laparoscopic surgery (n=16). Before insufflation of CO2, tidal volume was set at 10 ml/kg and respiratory rate was adjusted to achieve an end-tidal CO2 (P(ET)CO2) of 30-35 mmHg. Abdominal pressure was maintained at 10-15 mmHg by a CO2 insufflator. We measured the changes of systolic arterial pressure (SAP), heart rate (HR), P(ET)CO2 and peak airway pressure (PAP) at 5 min before (control value) and after CO2 insufflation and 5 min after CO2 deflation. Results: SAP and PAP were increased significantly after pnemoperitoneum compared with the control both in diagnostic laparoscopy and laparoscopic surgery (P<0.05). P(ET)CO2 was increased significantly after pneumoperitoneum and after CO2 deflation in laparoscopic surgery compared with the control and also with diagnostic laparoscopy (P<0.05). Driving pressure (the difference between peak airway pressure and abdominal pressure) was increased significantly after pneumoperitoneum in laparoscopic surgery compared with diagnostic laparoscopy (P<0.05). Conclusions: SAP, PAP and P(ET)CO2 increases during diagnostic laparoscopy and laparoscopic surgery, but this effect appears to be of smaller magnitude in diagnostic laparoscopy compared to laparoscopic surgery. We found that these changes had no clinically deleterious effects in healthy children. (Korean J Anesthesiol 2009;56:31~5)

H type of congenital tracheoesophageal fistula (TEF) is rare, occurring approximately once in 100,000 births. The presentation of this anomaly in adults is indeed uncommon. We report a case of a 47-year-old male with congenital TEF dectected during epidural hematoma removal under general anesthesia. Intermittent disappearance of normal capnography, bubbling sound at substernal area, and air leakage at oral cavity observed during manually assisted ventilation, especially during inspiration. We observed a H-type of TEF and tracheal intubation via esophagus in chest CT after operation. (Korean J Anesthesiol 2009; 56: 698~702)

A $LiNi_{0.8}Co_{0.15}Al_{0.05}O_2$ (LNCAO/C) active material composite cathode was coated with carbon. The conductive carbon coating was obtained by addition of surfactant during synthesis. The addition of surfactant led to the formation of an amorphous carbon coating layer on the pristine LNCAO surface. The layer of carbon coating was clearly detected by FE-TEM analysis. In electrochemical performance, although the LNCAO/C showed similar capacity at low C-rate conditions, the rate capability was improved by the form of the carbon coating at high current discharge state. After 40 cycles of charge-discharge processes, the capacity retention of LNCAO/C was better than that of LNCAO. The carbon coating is effectively protected the surface structure of the pristine LNCAO during Li insertion-extraction.
Multiple Psoas Abscess Formation after Pharmacopuncture -A Case Report-

Acupuncture has been widely used in alternative medicine for pain relief but may have many complications due to lack of appropriate cares. Pharmacopuncture is a sort of acupuncture that injects a herbal ingredient through a thin tube for the purpose of combining the effects of the herb and acupuncture and it has many pitfalls. The agents, used in pharmacopuncture are not refined for a desired effect and not produced by sterile standard processes under strict medical surveillance. We, report a case of a 44-yr-old male patient who had multiple abscesses in the psoas region with fever, right low back and hip pain that began after the pharmacopuncture treatment. This case shows that although pharmacopuncture has been practiced widely, it is important that the appropriate aseptic technique should be used to prevent severe infections and other complications.

Sludge reduction by biofilm processes in sewage treatment plants was studied. This resulted in a sharp reduction in the amount of sludge and improved water quality. This process maintains stable treatment independent of variations in temperature and pollution loading. Laboratory experiments and pilot plants tests were processed with raw waste water through the improved contact oxidation biofilm by biological endogenous phase. The obtained results were satisfactory. When the amount of sludge production, treatment efficiency, and economical efficiency are considered, 2.5 hours were the most efficient and economical. Also, stable and excellent treatment efficiency was maintained in spite of temperature change. The amount of sludge was sharply reduced by controling biological endogenous phase.

<P><B>Abstract</B></P> <P>Activated carbon (AC) is an inexpensive catalyst for oxygen reduction in an air cathode of microbial fuel cells (MFCs). In the AC-based cathode, carbon black (CB) is used as a conductive supporting material. In this study, it was hypothesized cathodic performance would increase if reduced graphene oxide (rGO) replaces CB in an optimum ratio. rGO replaced CB in the four different weight ratios of rGO toCB: 0:30 (rGO0); 5:25 (rGO5); 15:15 (rGO15); 30:0 (rGO30). Maximum power density was the best in rGO15 (2642 mW/m<SUP>2</SUP>) followed by rGO5 (2142 mW/m<SUP>2</SUP>). In the optimum external resistance operation, rGO5 and rGO15 showed similar power (∼1060 mW/m<SUP>2</SUP>), higher than the others. Linear sweep voltammetry, cyclic voltammetry, and impedance spectroscopy also showed that the optimal rGO additions improved cathodic performance and reduced cathodic internal resistance. Due to the flatter and wider shape of rGO and 5 times higher electrical conductivity than CB, the rGO addition improved the cathodic performance, but the complete replacement of CB with rGO decreased the cathodic performance due to the increased thickness and the morphological crack. The optimum rGO addition is a simple and effective method for improving cathodic performance.</P>
Identification of molecular mechanism controlling P34 gene expression in soybean

P34 has long been known as one of major allergenic proteins in soybean. Recently, two low-P34 soybean accessions, PI603570A and PI567476, were identified. In order to understand the molecular mechanism of a significant reduction of P34 protein level in PI567476, we performed comparative analysis of the P34 cDNAs and genomic sequences from low-P34 and normal soybean accessions. While no sequence variation was detected in P34 coding regions of the soybean accessions tested, the P34 gene from PI567476 contains 'ATGT' 4-bp insertion in front of the start codon. We also found the length polymorphism of 'TA' repeats in the P34 promoter regions among accessions. Northern and western blot analysis revealed that, even though mRNA levels of P34 are similar in low-P34 and normal soybean accessions, lower accumulation of P34 protein is only detected in the low-P34 accession, suggesting that the genetic polymorphisms in P34 promoters have a more significant effect on translation efficiency than transcription of the P34 gene. Transient expression analysis showed that the P34 promoter of PI567476 has significantly reduced activity compared to that of normal accession, cv. Clark. In addition, the chimeric promoter generated by introducing the 4-bp insertion in front of the start codon of the P34 gene from normal Clark accession showed markedly reduced activity. The results suggest that lower accumulation of P34 protein in low-P34 accession is largely due to the reduced translation efficiency caused by the 4-bp insertion in the P34 promoter, thus providing direct evidence for the molecular mechanism to explain the regulation of P34 gene expression in various soybean accessions.

Background: Since the morning fluoride level of 10 uM is recommended for adults patients being treated for osteoporosis so far, measurement of serum fluoride level is important to detect abnormally high levels or to detect levels below the therapeutic windows. Aims of this study are to determine the normal range of serum ionic fluoride levels in Korean female adults (from 5th to 7th decade), and to evaluate the in vivo fluoride pharmacokinetics of monofluorophosphate in Korean adults. Methods: Serum level of fluoride was measured from blood samples of 72 female subjects(age 43-69years) using an ion selective electrode. For pharrnacokinetics of monofluorophosphate-calcium(MFP-Ca), 6 subjects(age 27~45 years) were included to be withdrawn the blood hourly for the first S hours and the blood was withdrawn at 24 hours after a single dose of MFP-Ca. Results: Mean level of serum fluoride was 1.64+-0.12uM in 5th, 6th, 7th decades adults, and there was no difference of serum fluoride levels among age groups. Peak serum fluoride level exhibited 5.02+-0.67pM, and returned to basal level on 24 hours after a single dose of MFP-Ca. Conclusion: This study shows that mean serutn fluoride of Korean female adults(from 5th to 7th decade) is not different from that of other reports, and a single dose of MFP-Ca does not cause serum fluoride levels above the recommended therapeutic windows of 5-10uM for 24 hours.
