      • HIV 감염자에 대한 달팽이 엑기스의 혈액면역학적 효과

        A unique sialic-acid-binding lectin from giant African snail Achatina fulica(AF) has a T cell mitogenicity. It was also noted that acetone extracts of AF have a anti-HIV effect in vitro. Thirty HIV-infected individuals(HIV+) were treated with AF extract(40% Chunho extract) for average 5. 5 months. Peripheral blood sampling and questionnaire were done every month after take. There was a significant decrease of WBC from 4 months after take. As a result, lymphocyte counts also decreased(P<0.01). However, there was a significant increase of CD4+T cell percentage from 1 month(P<0.01). The mean increase of CD4+T cell count was significant for first one month(39/pl)(P<0.05). Body weight also increased significantly(P<0.05). Over 90% HIV+ ex-pressed symptomatic improvement and wanted to be treated with AF extract continuously after the end of this study. In one HIV+ treated with AF for 19 months, the increases of CD4+T cell percentage and counts over 11 times after AF extract take were mean 67% and 85%, respectively, compared to baseline value. These data strongly suggest that aqueous AF extract has a immune enhancing effect on HN+.

      • Adipocyte-Specific Deficiency of De Novo Sphingolipid Biosynthesis Leads to Lipodystrophy and Insulin Resistance

        <P>Sphingolipids have been implicated in the etiology of chronic metabolic diseases. Here, we investigated whether sphingolipid biosynthesis is associated with the development of adipose tissues and metabolic diseases. SPTLC2, a subunit of serine palmitoyltransferase, was transcriptionally upregulated in the adipose tissues of obese mice and in differentiating adipocytes. Adipocyte-specific SPTLC2-deficient (aSPTLC2 KO) mice had markedly reduced adipose tissue mass. Fatty acids that were destined for the adipose tissue were instead shunted to liver and caused hepatosteatosis. This impaired fat distribution caused systemic insulin resistance and hyperglycemia, indicating severe lipodystrophy. Mechanistically, sphingosine 1-phosphate (S1P) was reduced in the adipose tissues of aSPTLC2 KO mice, and this inhibited adipocyte proliferation and differentiation via the downregulation of S1P receptor 1 and decreased activity of the peroxisome proliferator-activator receptor gamma. In addition, downregulation of SREBP (sterol regulatory element-binding protein)-1c prevented adipogenesis of aSPTLC2 KO adipocytes. Collectively, our observations suggest that the tight regulation of de novo sphingolipid biosynthesis and S1P signaling plays an important role in adipogenesis and hepatosteatosis.</P>

        말초형 간내담관암의 수술 후 예후에 관한 고찰

        Background/Aims: Prognostic factors related to the postoperative survival of patients with peripheral cholangiocarcinoma (PCC) are not well documented. The purpose of this study was to evaluate the clinical and pathologic factors that might influence the postoperative survival of patients with PCC. Methods: Twenty-nine patients with PCC underwent laparotomy between September 1994 and September 1999. Fourteen clinicopathologic factors influencing postoperative survival were analyzed in nineteen patients who underwent hepatic resection (resection group). Ten patients had unresectable advanced tumor (unresectable group). Results: In the resection group, the overall 1-, 2-, and 3-year survival rate was 51.4%, 45.0%, and 28.1%, respectively (median survival of 17.6 months). The median survival in the unresectable group was 4.1 months and no patient lived longer than 1 year. In univariate survival analysis, tumor size and perineural invasion were correlated significantly with overall survival. The survival of patients who underwent resection with positive margin was significantly longer than that of the unresectable group. Conclusions: An aggressive surgical approach provides the best chance for long-term survival of patients with PCC. Tumor size and perineural invasion may be considered as prognostic factors for postoperative survival, but a large, long-term study is needed.

        GOTHIC-3D APPLICABILITY TO HYDROGEN COMBUSTION ANALYSIS

        Severe accidents in nuclear power plants can cause hydrogen-generating chemical reactions, which create the danger of hydrogen combustion and thus threaten containment integrity. For containment analyses, a three-dimensional mechanistic code, GOTHIC-3D has been applied near source compartments to predict whether or not highly reactive gas mixtures can form during an accident with the hydrogen mitigation system working. To assess the code applicability to hydrogen combustion analysis, this paper presents the numerical calculation results of GOTHIC-3D for various hydrogen combustion experiments, including FLAME, LSVCTF, and SNU-2D. In this study, a technical base for the modeling oflarge- and small-scale facilities was introduced through sensitivity studies on cell size and bum modeling parameters. Use of a turbulent bum option of the eddy dissipation concept enabled scale-free applications. Lowering the bum parameter values for the flame thickness and the bum temperature limit resulted in a larger flame velocity. When applied to hydrogen combustion analysis, this study revealed that the GOTHIC-3D code is generally able to predict the combustion phenomena with its default bum modeling parameters for large-scale facilities. However, the code needs further modifications of its bum modeling parameters to be applied to either small-scale facilities or extremely fast transients.

        First Report of Diaporthe actinidiae, the Causal Organism of Stem-end Rot of Kiwifruit in Korea

        Post-harvest diseases of kiwifruit caused severe damages on the fruits during storage, transportation, marketing and consumption. Phomopsis sp. was reported to be one of the major causal organisms of post-harvest fruit rots of kiwifruit. Symptoms of stem-end rot caused by Phomopsis sp. appeared at the stem-end area of the fruit as it ripened. The brown pubescent skin at the area became soft and lighter in color than the adjacent firm healthy tissues. A watery exudate and white mycelial mats were frequently visible at the stem-end area forming a water-drop stain down the sides on the dry brown healthy skin. When the skin was peeled back, the affected flesh tissue was usually watersoaked, disorganized, soft and lighter green than the healthy tissue. Phomopsis sp. was consistently isolated from the diseased fruits, and its pathogenicity was confirmed by an artificial inoculation test on healthy fruit of kiwifruits. The mycological characteristics of the telemorph state of the fungus produced on potato-dextrose agar were in accordance with those of Diaporthe actinidiae. This is the first report on the occurrence of a telemorph state of D. actinidiae as the causal organism of stem-end rot of kiwifruit in Korea.

        조직학적으로 진단된 담낭용종의 임상적 특징

        Background/Aims: We aimed to clarify clinical factors which suggest the possibility of neoplastic polyps of gallbladder. Methods: Ninety-five resected gallbladders with polyps were included. Size and number of the polyps were obtained by preoperative ultrasonography. Histollogically, the polyps were classified into non-neoplastic (cholesterol and inflammatory or hyperplastic polyp) and neoplastic lesions (adenoma and adenocarcinoma). Clinical features, maximum diameter and number of the polyps were compared between the two groups. Results: Of 95 patients, non-neoplastic polyps were observed in 65.3% and neoplastic polyps were observed in 34.7%. We found cholesterol polyp in 47.4%, inflammatory or hyperplastic polyp in 17.9%, adenoma in 25.2% and adenocarcinoma in 9.5%. Two groups showed no difference in age, sex, body mass index, cholesterol, triglyceride, glucose, ALT, HBsAg carriage and the number of polyps. Among the patients with non-neoplastic polyps, 70.2% was under 60 years, while 72.7% of the patients with neoplastic polyps was over 60 years (p$lt;0.05). The size of polyps was under 10 mm in 88.9% of non-neoplastic polyps, and between 10 and 15 mm in 40% of neoplastic leisions. Moreover, 68.4% of neoplastic polyps exceeds 15 mm in diameter (p$lt;0.05). Conclusions: The size of polyp (≥10 mm) and patient's age (≥60 years) could be indicators for neoplastic polyps.

        Turbulence-induced Heat Transfer in PBMR Core Using LES and RANS

        <P>This paper introduces the results of numerical simulations on flow fields and relevant heat transfer in the pebble bed reactor (PBR) core. In the core, since the coolant passes a highly complicated random flow path with a high Reynolds number, an appropriate treatment of the turbulence is required. A set of simple experiments for the flow over a circular cylinder with heat transfer was conducted to finally select the large eddy simulation (LES) and <I>k</I>-ω model among the considering Reynolds-averaged Navier-Stokes (RANS) models for PBR application. Using these models, the PBR cores, whose geometries were simplified to the body-centered cubical (BCC) and face-centered cubical (FCC) structures, were simulated. A larger pressure drop, a more random flow field, a higher vorticity magnitude and a higher temperature at the local hot spots on the pebble surface were found in the results of the LES than in those of RANS for both geometries. In cases of the LES, the flow structures were resolved up to the grid scales. Irregular distributions of the flow and local heat transfer were found in the BCC core, while relatively regular distributions for the FCC core. The turbulent nature of the coolant flow in the pebble core evidently affected the fuel surface temperature distribution.</P>

        상부 위장관 출혈을 동반한 위결핵 1예

        Gastric tuberculosis is quite rare and usually combined with pulmonary tuberculosis. Its diagnosis is so difficult as it is often unsuspected, We report a patient with gastric tuberculosis who presented with hematemesis in Korea, Upper endoscopy showed large, deep penetrating ulcer containing an exposed vessel and adherent clot in the body. Gastric biopsies revealed only chronic inflammation and no evidence of granuloma or malignancy. Diffuse mural thickening was noted on abdominal CT. The diagnosis was made postoperatively following gastrectomy for bleeding gastric ulcer. Microscopic examination of resected stomach showed peptic detritus and noncaseating granulomas. However, multiple caseating granulomas with Lagerhan's giant cells were found on the examination of lymph nodes. The patient was treated with antituberculous therapy for 12 months without any complications.
