        응급실에 내원한 환자에서 어지럼증의 원인과 임상양상의 분석

        Background: Recently, The number of patients who have been complaining of the vertigo or dizziness has been increasing due to rapid growth of elderly population and senile disease. The differentiation of dizzy patients is not familiar to most emergency physicians. This study was designed to differentiate true vertigo and to investigate the clinical difference among central vertigo, peripheral vertigo, and other causes of dizziness. Methods: The authors analyzed the cases of 237 dizzy patients who visited the emergency department of Chonnam University Hospital during the recent 2 years. For the base of dizziness, associated past illnesses, severity, nystagmus type, and the causes of central and peripheral vertigo. Results: Female patients were 142(59.9%) and male patients were 95(40.1%). The most common age group was the 6th decade. The origin of dizziness was classified as peripheral cause(25.3%), central cause(32.9%), and others(41.8%). As to nature of the dizziness, the rotatory sense was dominant in peripheral vertigo and the floating sense and blurred vision in central vertigo. Peripheral vertigo was triggered by position change of head and body, and central vertigo by the eye movement. Audiograms I showed that most patients with central vertigo had normal hearing, but 46.7% of thoswith peripheral vertigo had an abnormal hearing disturbance. Nystagmus was more prevalent in peripheral vertigo Conclusion: History taking and physical examination played an important role in the diagnosis of dizzy patients. An important part of the diagnosis of a dizzy patient could be to evaluate the peripheral origin, the central origin and others causes. Because central vertigo may be associated with a significant neurological pathology, spectral radiological studies, including MRI and CT, are mandatory to rule out a devastating brain lesion.

      • HBx형질전환 생쥐에서 발생한 간세포암종에서 H-ras 및 c-myc의 발현에 관한 연구

        <연구목적> HBx형질전환 생쥐에서 발생한 간세포암종의 발암과정에 종양유전자(H-rgs, c-myc)의 발현 정도를 조사하고자 하였다. <연구방법> 정상생쥐 12마리(4-18개월) 및 HBx 형질전환 생쥐 44마리(4-18개월)를 대상으로 포르말린에 고정하고 파라핀에 포매한 간 조직을 이용하여 면역조직화학적염색을 실시하였다. 실험군은 정상 부위, 이형성 부위 및 종양 부위로 구분하였으며, 종양 부위는 소결절성병변 부위와 간세 포암종 부위로 구분하였고, 이형성병변 부위는 이형성병변만 발견되는 부위, 소결절성병변과 동반된 이형성병변 부위 및 간세포암종과 동반된 이형성병변 부위로 구분하였다. <연구결과> H-rgs의 발현은 정상 간조직에 비하여 이형성병변 부위(P<0.05) 및 종양 부위(P<0.01)에서 증가하였으며, 소결절성병변 부위과 간세포암종 부위 사이에서는 간세포암종 부위에서 증가된 경향이었으나 통계학적으로 유의한 차이가 없었으며, 각 이형성병변 부위 사이에서도 유의한 차이는 없었다. c-myc의 발현은 정상 간조직 및 이형성병변 부위에 비해 종양 부위에서 증가하였으며(P<0.001), 소결절성병변 부위와 간세포암종 부위에서는 비슷하였고, 각 이형성병변부위 사이에서도 비슷하였다. <결론> HBx형질전환 생쥐에서 발생하는 간세포암종의 발생에 H-rgs는 이형성 변화를 일으키는 시기에 관여하며, c-myc은 이형성병변에서 암으로 이행하는 시기에 관여할 것으로 생각한다. Background: This experiment was designed for the expression of H-ras and c-myc in hepatocarcinogenesis of the HBx transgenic mice. Methods: Immunohistochemical stains in the paraffin embedded tissue of the liver were used for the detection of H-ras and c-myc in the 12 normal mice and 44 HBx transgenic mice of the 4-18 month old. Results: Expression of the H-ras was significantly increased in the dysplastic area (P<0.05) and tumor area (P<0.01) than in the normal liver. But there were no differences of H-ras expression between areas of microscopically identified hepatocellular carcinoma (MI-HCC) and grossly identified hepatocellular carcinoma (GI-HCC) and dysplastic areas among the only dysplastic areas, dysplastic areas with MI-HCC and GI-HCC. Expression of the c-myc was significantly increased in the tumor area (P<0.001) than in the normal liver and dysplastic area. But there were no differences of c-myc expression between areas of MI-HCC and GI-HCC, and dysplastic areas among only dysplastic areas, dysplastic areas with MI-HCC and GI-HCC. Conclusions: Our study suggests that H-ras is related to the dysplastic change and c-myc is related to the neoplastic change in the hepatocarcinogenesis of the HBx transgenic mice.

      • 약물유도 추체외로증상 발현에서의 혈중칼슘이온과 마그네시움 이온의 역할

        Objects : The purpose of this study is to evaluate the role of serum Ca and Mg ions on the occurrence of drug-induced extrapyramidal symptoms among schizophrenic patients Methods : The subjects of this study were composed of 22 schizophrenic patient those who with extrapyramidal symptoms(N=14) and without extrapyramidal symptoms(N=8). We examined relationships between drug free cacium and magnesium ion level and EPS onset calcium and magnesium ion level among patients with extrapyramidal symtoms. And we compare drug free calcium and magnesium levels between EPS group and nonEPS gruop. additionally, we evaluated the relationships among halopendol dosage and drug free and halopendol average and EPS onset calcium and magnesium ion levels by correlation analysis. Results : Drug free calcium and magnesium levels in both group were not significantly different, there are no significant corelation among halopendol dosage, drug free, EPS onset calcium and magnesium ion levels in both groups. As halopendol administerd, calcium and magnesium levels were not significantly change Conclusion : we can not find significant relation between drug free calcuim and magnesium ion levels and the occurrence of extrapyramidal symptoms in out study design.

      • ATM상의 서로 다른 LIS에 속한 IP 호스트간의 직접 연결

        The existing local area networks are primarily based on shared media interconnects, which are likely to become potential bottlenecks not only because of new multimedia applications but also because of the rapid growth of services employing simple data transfers. Asynchronous transfer mode(ATM) is viewed as an emerging technology capable of removing this bottleneck. Therefore it requires implementing the IP in the evolving ATM local area networks. Implementing IP directly over ATM require translating an IP address to an ATM address. This paper suggests a new path control server and one-to-one connection set-up type to minimize such limitations which come from connection set-up. The suggesting model, which has independent server on connection set-up required at each logical IP subnetwork, provides faster routing information by managing path control informations to establish direct connections across IP subnets in the ATM domain.

      • 정신분열병 환자에서 Olanzapine이 인지기능에 미치는 영향

        Objects : The purpose of this study was to test the effects of olanzapine on cognitive functions in schizophrenic patients. Methods : The subjects were 16 haloperidol-medicated schizophrenic patients and 15 risperidone-medicated schizophrenic patients, who participated in an eight-week crossover study from haloperidol and risperidone to olanzapine. Results : The patients in the group of crossover from haloperidol to olanzapine showed significant improvement in Parkinson scale of ESRS and other cognitive functions except for verbal fluency. Negative scale of PANSS and cognitive functions such as verbal memory, verbal fluency, visual memory and executive function were significantly improved in patients In the group of crossover from risperidone to olanzapine. Conclusions : These data suggest that olanzapine has some superior cognitive benefits relative to haloperidol and risperidone, but a larger sample replication study is necessary.

      • Clozapine이 인체내에서 인간조직적합성항원에 미치는 영향

        The histocompatibillty antigen (human leukocyte antigen : HLA) has been used for organ transplantation, discrimination of one's real children and transfusion, etc. The objective of this study was to find out how dozapine affects HLA class I and II type in the humanbody when therapeutic doses are applied. The subject consists of 3 persons for lass I and class II They were administered for about 3 months with clozapine 157-250mg/day (mean dally dose 200mg). Analysis of HLA lass I type was the microcytotoxicity test of Terasaki and class II type was PCR method of Erhich. The results were as followings: Two of 3 were changed in HLA-A, B, C type. All three were not chanced in HLA-DR type. In conclusion, the short-term application, about 3 months, of therapeutic dose of clozapine is not considered to bring about changes in DNA level.

        Schizophrenia can alter a wide range of cognitive abilities, ranging from attention to memory to language production. schizophrenia has been associated with functional and anatomic disturbance in many different brain regions. but there may be no single cognitive ability or brain area that somehow holds the key to understanding schizophrenia. we have reviewed many articles that discussed about neural network theory and schizophrenic symptom. many authours conclude that schizophrenia originates from pathological interactions of brain areas and cognitive potentials. so we have reviewed in this article about PDP concept and brain functioning, computer simulations of PDP network, and interactional pathology in PDP network including relationship between schizophrenic symptoms and parasitic focus. and then, we described a theroy of dopamine function and its role in cognitive deficits in schizophrenia as an example case of PDP.

      • 림프구에 대한 clozapine의 생체내 유전독성

        최근 난치성 정신분열증 환자에서 가장 효과적인 약으로 알려진 clozapine은 골수에 작용하여 심각한 부작용인 무과립 구증을 일으킬 수 있다. 무과립구증의 원인 및 기전에 대한 연구는 매우 많으나 현재까지 확실한 결론이 나지 않은 상태이다. 저자들은 clozapine의 이러한 골수 독성에 근거하여 clozapine이 인체내에서 염색체에 어떤 변화를 주는지 아닌지를 알아보기 위해 clozapine 단독치료를 시행한 환자에게서 치료전과 치료중에 얻어진 말초혈액 림프구를 배양하여 염색체에 어떤 변화가 생기는지를 조사하였으나, clozapine에 의한 염색체의 자발적인 변이는 관찰되지 않았다. Objective : Many drugs and environmental chemicals can induce aneuploidy or chromosome instability in vivo or in vitro. Modification of the fidelity of cellular division may occur via a variety of mechanisms and chemical interactions. The clozapine is new antipsychotic drug with superior effect than classic antipsychotics. But, it may produce agranulocytosis and seizure in some patients. Agranulocytosis is a serious side effect of this drug but the cause and mechanism of this side effect is not clearly explained. Some study suggested that oxidative stress can induce chromosome aberration and the other study suggested that clozapine produce free radical and it can induce oxidative stress on some kind of cells. Therefore we decided to investigate whether clozapine and its metabolites can induce genotoxic effect in vivo. Method : The patients were chronic schizophrenic patients without a treatment at least one year. Diagnosis was made by DSM-IV criteria and agreed by both of two psychiatrists. Blood sampling was done before the treatment and during the treatment with considerable time interval. We used peripheral blood lymphocyte culture method and we counted the numerical aberrations, sister chromatid exchanges. Results : Chromosome aberration was not detected in all samples. Conclusion : Clozapine did not induce spontaneous chromosome aberration to human lymphocytes in vivo.
