마비성 조음장애 환자에서 언어치료 기능성 게임 적용 후 호전 양상에 대한 연구

Purpose: We developed a serious speech game for rehabilitation of the patients with dysarthria. The Smart Speech Program is composed of a three categories such as continuity, pitch, and loudness. The purpose of this study was to examine the usefulness of smart speech programs in speech therapy of patients with dysarthria by evaluating changes in voice. Methods: Fifteen patients with dysarthria disorder and without aphasia suffered from brain diseases were enrolled. The study started with “pre-evaluation 1, and then, evaluations were repeated pre-evaluation 2 after 6 weeks without any treatment in order to generated the control data. After pre-evaluation, a smart speech program was used to perform training. Post-training evaluation 1 were performed just after treatment. And, after six weeks of daily life, conducted a post-training evaluation 2 to see if treatment remained effective. The evaluation used the multi-dimensional voice program of the computerized speech laboratory to collect voice samples-pitch, intensity, fundamental frequency(F0), maximum phonation time(MPT), jitter, relative average perturbation(RAP), and the articulation parameters were measured by the Urimal Test of Articulation and Phonology(U-TAP) and the 3-Position Articulation Test. Results: In the post-training evaluation 1, pitch range(pitch max.-pitch min.), F0, intensity, MPT, jitter, RAP and U-TAP scores of all subjects showed significant results(p<0.05). In the post-training evaluation 2, significant changes were retained in pitch range and U-TAP scores, both of which had a long-term effect. Conclusions: These preliminary results suggest that the newly developed serious game for speech training has a positive effects on pitch, loudness, and articulation abilities in the patients of dysarthria. Therefore, it was found that smart speech was effective in improving speech ability and articulation of dysarthria patients.
순간수위 변화시험, 단공양수시험 및 단계양수시험을 통한 수리상수 추정연구

본 연구는 연구지역의 수리상수를 추정하여 오염된 지하수의 정화공법 설계에 효과적으로 사용하는데 목적이 있다. 이를 위하여 수행된 수리시험은 순간수위변화시험, 단공양수시험 및 단계양수시험이다. 순간수위변화시험은 대표적인 Bouwer and Rice 직선법과 C-B-P 특성곡선법으로 해석하였다. Bouwer and Rice법으로 해석한 평균 수리전도도 값은 4.48 × 10(-3)cm/sec이며 중앙값은 1.16 × 10(-3)cm/sec이다. C-B-P법으로 구한 평균 수리전도도 값은 2.37 × 10(-3)cm/sec이며 중앙값은 7.09 × 10(-4) cm/sec이다. 두 해석 결과 연구지역의 하부가 화강암으로 이루어져 있어 대체로 투수성이 낮아 Bouwer and Rice법으로 해석한 수리전도도가 높게 나타난다. 단공양수시험은 GW7, GW12 및 MW9 관정에서 수행하였으며 여러 종류의 특성곡선법을 적용하여 해석하였다. GW7 관정은 GW12 및 MW9 관정보다 수리전도도 및 투수량계수가 낮으며 이는 기반암의 파쇄대 및 절리의 여부와 관련된 것으로 판단된다. 단계양수시험은 KDPW1 및 KDPW2 관정에서 수행하였으며 해석방법에 따라 수리상수 값의 차이가 나타나지만 매우 미미하며 본 연구에서 해석한 수리상수 값은 오염지하수 정화설계에 있어 적절히 활용될 것으로 판단된다. The aim in this study is used to develop the remediation technologies for contaminated ground water. Slug, single well pumping and step-drawdown tests have been used to obtain hydraulic parameter estimates in the field. Slug tests yield hydraulic conductivity values using the Bouwer and Rice and C-B-P analysis methods. The mean and median hydraulic conductivity values of Bouwer and Rice method are 4.48 × 10(-3) and 1.16 × 10(-3) cm/sec, respectively. On the other hand, C-B-P method gave mean and median hydraulic conductivity values of 2.37 × 10(-3) and 7.09 × 10(-4) cm/sec, respectively. These analyses show a trend for the Bouwer and Rice method to yield lower hydraulic conductivity values in low permeability zones of granite in the study area. Sing well pumping test data were calculated through type curve in GW7, GW12 and MW9 wells. It could be interpreted that the differences of hydraulic conductivity and transmissivity values between GW7 and GW12, MW9 is related with fault clays and fractures in the bedrock among the wells. Step-drawdown tests were carried out in the KDPW1 and KDPW2 wells. The hydraulic parameter of KDPW1 and KDPW2 showed very litter difference between the values. The study of hydraulic parameter estimates can be used to purify in contaminated groundwater.
항혈소판제제를 포함한 약물상호작용에 미치는 프로톤펌프억제제의 영향: Safe 측면

Clopidogrel prevent atherothrombotic complications following acute coronary syndromes by inhibiting platelet aggregation.However, in case of concomitant use of clopidogrel and proton pump inhibitor (PPI) for prophylaxis of gastrointestinal (GI)bleeding, the anti-clotting of clopidogrel will be decreased because PPI inhibit competitively cytochrome P450 enzymes, especially CYP2C19. Through several pharmacodynomic studies, omeprazole, but not other PPIs, worsens surrogate markers (e.g., platelet reactivity index) of clopidogrel efficacy. And many observational studies show concomitant use of clopidogrel and PPI have increased the risks of cardiovascular (CV) events (hazard/odds ratios=1.25-1.5). However, only one prospective randomized trial (COGENT trial) of omeprazole vs. placebo in clopidogrel users show no difference in CV events (hazard ratio=1.02) and decrease the risk of GI bleeding (hazard ratio=0.13). Further well designed research will need to determine whether a pharmacodynamic effect of clopidogrel affected by PPI will be changed clinical CV events. Clinical decisions about concomitant use of PPI and clopidogrel must be balance overall risks and benefits, considering both CV and GI complications. (Korean J Med 2011;81:26-33)
