      • Urapidil, Labetalol의 투여가 기관내 삽관시 심혈관계에 미치는 영향

        We have examined the comparative efficacy of small doses of intravenous urapidil and labetalol in blunting hemodynamic response to endotracheal intubation and surgical incision in 30 patients without cardiovascular diseases. After intravenous urapidil 0.2 mg/kg or labetalol 0.2 mg/kg anesthesia was induced with thiopental 5mg/kg. Endotracheal intubation was facilitated by vecuronium 0.15 mg/kg with priming principle and anesthesia was maintained with enflurane and nitrous oxide in oxygen. Systolic, diastolic and mean arterial pressure and heart rate were measured before administration of the drugs, 5 minute after administration, just prior to endotracheal intubation and 1, 3, 5, 10 minute after intubatin. Also the peak blood pressures and heart rate within 10 minutes after surgical incision were measured. Endotracheal intubation and surgical stimulation were associated with significant increases in blood pressures and heart rate in both urapidil and labetalol group. Comparison of the changes in systolic, diastolic, and mean artrial pressures and heart rate between urapidil and labetalol group showed no significant difference except peak systolic pressure after surgical incision. It is concluded that the pressor response to endotracheal intubation and surgical stimulation are not influenced significantly by urapidil 0.2 mg/kg or labetalol 0.2 mg/kg. However, urapidil and labetalol preloading may be similarly effective in the blunting of the increases in blood pressures with larger doses of the durgs during anesthetic induction.

        We isolated a toluene degrading bacterium from the bioactive foam reactor(BFR) which designed to remove volatile organic compounds(VOCs) from the contaminated air. In order tofind the taxonomic and phylogenetic relationship with known bacteria, the partial sequence of 16S ribosomal DNA gene was analyzed. The isolated bacterium was classified as Pseudomonas sp. based on the close relationship to bacteria belonging to this genus. From the sequence, we designed the appropriate DNA probe which will be useful to detect this bacterium with the molecular methods such as FISH. The isolated bacterium and designed probe will be helpful to optimize the operating conditions and to improve the efficiency of BFR in detoxification of VOCs.

      • 성인에서 Haemophilus influenzae 감염증의 임상양상과 미생물학적 특성

        목적 : H. influenzae는 성인에서 흔히 호흡기 감염을 일으키는 균주로, 최근 ampicillin 내성 β-lactamase 생성 균주들이 지속적인 증가를 보이고 있어, 이에 국내 성인에서 H. influenzae에 의한 감염의 임상양상과 분리된 균주들의 미생물학적 특성을 조사하고자 하였다. 방법 : 1998년 3월부터 1999년 9월까지 고려대 구로병원에 내원한 환자중에서 H. influenzae가 분리된 68명을 대상으로 임상양상을 조사하고 항균제 감수성 검사를 시행하였다. 분리된 33주에 대해서 혈청형, 생물형, 세포외막 단백분석을 시행하였고, β-lactamase 생성여부에 따른 감염 환자군간 임상상 및 예후의 차이를 비교하였다. 결과 : 환자의 평균연령은 68세였고, 지역사회 획득감염이 전체의 85%를 차지하였다. 폐렴과 기관지염과 같은 하기도 감염이 전체환자의 75%를 차지하였고 분리 검체 역시 객담이 84%로 가장 많았다. 환자의 71%가 기저질환을 가지고 있었으며, 그 중에 만성 폐쇄성 폐질환, 심혈관 질환, 악성종양, 당뇨병이 대부분이었다. 혈청형으로는 nontypeable 형이 68%로 가장 많았고 b형은 한 예도 없었다. β-lactamase 생성율은 63.2%였고, β-lactamase 양성균주의 항균제 내성율이 음성균주에 비하여 높았으나 β-lactamase 생성 여부에 관계없이 cefotaxime, azithromycin, ciprofloxacin에 대하여 100%의 감수성을 나타내었고 cefaclor, cefuroxime에 대해서는 10% 이하의 낮은 내성율을 보여 이들 항균제들이 치료에 효과적일 것으로 사료된다. 세포외막 단백분석에서는 C형과 D형이 전체의 64%로 가장 많았으며, 혈청형과의 유의한 연관성은 보이지 않았다. 결론 : 성인에서 H. influenzae는 주로 만성 기저질환자에서 급성 호흡기감염 또는 만성 폐쇄성 폐질환의 급성 악화를 유발한다. 기존의 보고에 비해 β-lactamase 양성율이 증가하였고, 이에 따른 항균제 내성율도 증가하여 항균제의 선택에 유의하여야 할 것으로 생각된다. 세포외막 단백분석상 특정한 유형의 균주에 의해 감염이 집중적으로 발생하고 있어 향후 역학 조사의 도구로 이용할 수 있겠다. Purpose : H. influenzae is one of the common causative microorganisms of respiratory tract infections in adults. Recently. β-lactamase producing, ampicillin-resistant H. infiuenzae has become worldwide problem as well as in Korea. We studied to investigate the clinical features and microbiologic characteristics of H. infiuenzae infections in adults. Materials & Methods : From March 1998 to September 1999, 68 patients were enrolled in this study. Clinical features of H. infiuenzae infections were investigated, 68 isolates were subjected to the β-lactamase test, and 33 isolates were used for serotyping. biotyping, antibiotic susceptibility and outer membrane protein (OMP) analysis. Results : Mean age of study patients was 68 years-old and the acquisition rate in community was 85%. Pneumonia and bronchitis were the most common type of infection such as 80%, sputum was the most common clinical specimen for H. mQuenzae isolation. 70% of patients had chronic underlying diseases. Non-typeable strains were 68% and β-lactamase producing rate was 63%. There was no difference in the clinical features and prognosis of H. influenzae infection between the β-lactamase positivie and -negative groups. The susceptibility of cefotaxime, azithromycin, and ciprofloxacin was 100% and that of cefaclor, cefuroxime was under 10% in spite of β-lactamase positivity. In OMP analysis, C and D types were dominant (64%). Conclusion : H. influenzae caused acute respiratory infections in adults with chronic underlying disorders. This study shows higher β-lactamase producing rate and antibiotic-resistance rate than that were reported previously. OMP analysis shows that two major types of strains cause clinical infections intensively.

      • Mo¨ssbauer분광법에 의한 Ca_(0.75)Ba_(0.25)Fe_2O_4의 Fe이온에 대한 연구

        The Mo^ssbauer spectrum of CaFe_2O_4, consists of paramagnetic two doublets absorption peaks at room temperature. But, the replacement of Ca^+2 ions in CaFe_2O_4 with Ba^+2 ions changes the magnetic properties without changing the crystal structure and the Mo^ssbauer spectrum of Ca_0.75Ba_0.25Fe_2O_4 consists of paramagnetic two doublets and ferrimanetic three different Zeeman sextets absorption peaks due to the Fe^57 nuclei in the various sublattices. According to this study, the Fe ions in the Ca_0.75Ba_0.25Fe_2O_4 are trivalent in al sublattices and there are more than three sublattices in this structure.

        야뇨증 환아들의 심리사회적 특성에 대한 다기관 연구 : 행동 및 정서 문제를 중심으로

        Objectives : The aim of this study was to examine the behavioral and emotional problems associated with nocturnal enuresis in Korean children. Methods : Three hundred eighteen children with nocturnal enuresis, together with their parents, completed the Child Behavior Checklist (CBCL), Disruptive Behavior Disorder Scale according to DSM-IV (DBDS), Children's Depression Inventory (CDI), State-Trait Anxiety Inventory for Children (STAIC), and Piers-Harris Children's Self-Concept Scale (PHCSC). Ninety-three normal students were selected as the control group. Results : Compared to the normal control group, the mean scores with regard to the withdrawn, social problems, attention problems, delinquent behavior, aggressive behavior, externalizing problems and total problems profiles were significantly higher in the nocturnal enuresis group according to the CBCL results. The nocturnal enuresis group also scored significantly higher in the ADHD and ODD profiles of the DBDS. The nocturnal enuresis group was more depressed and anxious than the control group according to the results of the CDI and STAI. The mean score of the PHCSC was significantly lower in the nocturnal enuresis group when compared to the normal control group. Conclusion : The results of this study suggest that children with nocturnal enuresis in Korea have clinically relevant behavioral and emotional problems. The findings support the link between nocturnal enuresis and psychopathology in Korean children.

        Biodegradable micro-sized discoidal polymeric particles for lung-targeted delivery system

        <P><B>Abstract</B></P> <P>Various types of particle-based drug delivery systems have been explored for the treatment of pulmonary diseases; however, bio-distribution and elimination of the particles should be monitored for better understanding of their therapeutic efficacy and safety. This study aimed to characterize the biological properties of micro-sized discoidal polymeric particles (DPPs) as lung-targeted drug delivery carriers. DPPs were prepared using a top-down fabrication approach and characterized by assessing size and zeta potential. They were labeled with zirconium-89 (<SUP>89</SUP>Zr), and bio-distribution studies and PET imaging were performed for 7 days after intravenous administration. Their hydrodynamic size was 2.8 ± 6.1 μm and average zeta potential was −39.9 ± 5.39 mV. At doses of 5, 12.5, and 25 mg/kg, they showed no acute toxicity in nude mice. Desferrioxamine (DFO)-functionalized <SUP>89</SUP>Zr-labeled DPPs gave a decay-corrected radiochemical yield of 82.1 ± 0.2%. Furthermore, <SUP>89</SUP>Zr-DPPs, from chelate-free labeling methods, showed a yield of 48.5 ± 0.9%. Bio-distribution studies and PET imaging showed <SUP>89</SUP>Zr-DFO-DPPs to be mainly accumulated in the lungs and degraded within 3 d of injection. However, <SUP>89</SUP>Zr-DFO-DPPs showed significantly low uptake in the bone. Overall, our results suggested micro-sized DPPs as promising drug delivery carriers for the targeted treatment of various pulmonary diseases.</P>

        Effect of Ethanol on Na<SUP>⁢</SUP>-P<SUB>i</SUB> Uptake in Opossum Kidney Cells Role of Membrane Fluidization and Reactive Oxygen Species

        <P> This study was undertaken to examine the effect of ethanol on Na<SUP>⁢</SUP>-dependent phosphate (Na<SUP>⁢</SUP>-P<SUB>i</SUB>) uptake in opossum kidney (OK) cells, an established renal proximal tubular cell line. Ethanol inhibited Na<SUP>⁢</SUP>-dependent component of phosphate uptake in a dose-dependent manner with I<SUB>50</SUB> of 8.4%, but it did not affect Na<SUP>⁢</SUP>-independent component. Similarly, ethanol inhibited Na<SUP>⁢</SUP>-dependent uptakes of glucose and amino acids (AIB, glycine, alanine, and leucine). Microsomal Na<SUP>⁢</SUP>-K<SUP>⁢</SUP>-ATPase activity was not significantly altered when cells were treated with 8% ethanol. Kinetic analysis showed that ethanol increased K<SUB>m</SUB> without a change in V<SUB>max</SUB> of Na<SUP>⁢</SUP>-P<SUB>i</SUB> uptake. Inhibitory effect of <I>n</I>-alcohols on Na<SUP>⁢</SUP>-P<SUB>i</SUB> uptake was dependent on the length of the hydrocarbon chain, and it resulted from the binding of one molecule of alcohol, as indicated by the Hill coefficient (n) of 0.8-1.04. Catalase significantly prevented the inhibition, but superoxide dismutase and hydroxyl radical scavengers did not alter the ethanol effect. A potent antioxidant DPPD and iron chelators did not prevent the inhibition. Pyrazole, an inhibitor of alcohol dehydrogenase, did not attenuate ethanol-induced inhibition of Na<SUP>⁢</SUP>-P<SUB>i</SUB> uptake, but it prevented ethanol-induced cell death. These results suggest that ethanol may inhibit Na<SUP>⁢</SUP>-P<SUB>i</SUB> uptake through a direct action on the carrier protein, although the transport system is affected by alterations in the lipid environment of the membrane.

The crystal structures of 1, 4, 6, and 7 determined by X-ray diffraction reveal distorted square-pyramidal geometries around the Al atoms. Complexes 1–7 are all air-stable in both the solid and solution states and have high thermal stability (decomp 313–338 °C). Differential scanning calorimetric analyses show that they can form amorphous glasses with glass transition temperatures of 95–132 °C depending on the C5 substituent. UV/Vis absorption spectra of the complexes exhibit major bands at λ=338–413 nm assignable to salen-centered π–π* transitions with a gradual red shift of the absorption maximum wavelengths as the substituent is varied from an electron-withdrawing (NMe<SUB>3</SUB>) to an electron-donating group (NMe<SUB>2</SUB>). The maxima in the emission spectra of 1–7 occur over the entire visible region, ranging from λ=438 nm for 7 to λ=599 nm for 6, with high fluorescence quantum efficiencies of up to Φ=0.40 for 4 in solution. DFT calculations suggest that the low-energy electronic transitions in 1–7 are characterized by HOMO<SUB>−i</SUB>–LUMO<SUB>+1</SUB> (i=1 for 1–6 or i=4 for 7) transitions localized on the salen moiety, with much involvement of the C5 position in the HOMO<SUB>−i</SUB>. Thus, the electronic alteration at the C5 position of the phenoxide ring, which mainly affects the HOMO<SUB>−i</SUB> energy levels of salen–Al luminophores, is responsible for the observed emission color-tuning properties over the entire visible region.</P> <B>Graphic Abstract</B> <P>Showing their true colors?         We report here on a case of diffuse sclerosing papillary carcinoma (DSPC), which is a subtype of papillary carcinoma of thyroid, in a 27-year-old female. The ultrasound images showed diffuse enlargement of the thyroid lobes and this was associated with underlying diffuse scattered microcalcifications and a heterogeneous hypoechoic background parenchyma that was without any masses. The CT image showeddiffuse scattered dot-like hyperdensities with bilateral multiple metastastic lymph nodes. Because making the cytological diagnosis of DSPC is still challenging, the radiological findings that show this disease's characteristic features may be important clues for making the correct diagnosis. <B>(Ko</B><B></B><B>rean J Endocrine Surg 2008;8:43-47)</B>

        Determination of Malignant and Invasive Predictors in Branch Duct Type Intraductal Papillary Mucinous Neoplasms of the Pancreas: A Suggested Scoring Formula

        <P>Prediction of malignancy or invasiveness of branch duct type intraductal papillary mucinous neoplasm (Br-IPMN) is difficult, and proper treatment strategy has not been well established. The authors investigated the characteristics of Br-IPMN and explored its malignancy or invasiveness predicting factors to suggest a scoring formula for predicting pathologic results. From 1994 to 2008, 237 patients who were diagnosed as Br-IPMN at 11 tertiary referral centers in Korea were retrospectively reviewed. The patients' mean age was 63.1 ± 9.2 yr. One hundred ninty-eight (83.5%) patients had nonmalignant IPMN (81 adenoma, 117 borderline atypia), and 39 (16.5%) had malignant IPMN (13 carcinoma in situ, 26 invasive carcinoma). Cyst size and mural nodule were malignancy determining factors by multivariate analysis. Elevated CEA, cyst size and mural nodule were factors determining invasiveness by multivariate analysis. Using the regression coefficient for significant predictors on multivariate analysis, we constructed a malignancy-predicting scoring formula: 22.4 (mural nodule [0 or 1]) + 0.5 (cyst size [mm]). In invasive IPMN, the formula was expressed as invasiveness-predicting score = 36.6 (mural nodule [0 or 1]) + 32.2 (elevated serum CEA [0 or 1]) + 0.6 (cyst size [mm]). Here we present a scoring formula for prediction of malignancy or invasiveness of Br-IPMN which can be used to determine a proper treatment strategy.</P>
