Acute intermittent porphyria is a disorder that results from partial porphobilinogen deaminase deficiency and accumulation of overproducted porphyrin and its precusors. This makes variable manifestations such as abdominal pain, nausea, vomiting, contipation, peripheral neuropathy, seizure, psychotic symtoms, respiratory failure, tachycardia, and hypertension. When acute paroxysmal hypertension presents, it is difficult to differentiate from pheochromocytoma. We experienced a case of young female with acute intermittent porphyria presenting paroxysmal recurrent headache, palpitation, chest discomfort and paroxysmal hypertension. Initially blood pressure was 160/100 mmHg and checked high paroxysmally on follow up. Rhythm was sinus tachycardia. She did not complain any other specific symptoms. Although her clinical manifestations were suspicious for pheochromocytoma, laboratory findings were not compatible to it. In searching other causes of secondary hypertension mimickng ph대chromocytoma, elevated urine porphobilinogen level was detected and we could make diagnosis of acute intermittent porphyria. If AIP was not beared in mind as a cause of secondary hypertension, diagnosis would be delayed more.

심장내로 연장된 정맥내 평활근종증 1예 : 심장내 평활근종증 intracardiac leiomyomatosis-case report and literature review

정맥내 평활근종증은 병리학적으로는 양성 질환이나 진행 양상은 악성으로 모든 종양의 적출술이 시행되어야 하며, 자궁적출술시 병리학적으로 정맥내 평활근종증을 진단받은 경우 정기적인 검사를 통해 종양의 재발이나 하대정맥과 심장내로의 연장을 확인하여야 한다. 우측 심장내 종양이 발견된 경우는 점액종을 포함한 원발성 심내 종양 외에도 다른 원인 질환을 확인하여야 하며, 반드시 정맥내 평활근종증을 감별진단하여야 한다. Intravenous leiomyomatosis is a rare benign vascular tumor defined as the extension into venous channels of a histologically benign smooth muscle tumor arising either from a uterus or from the walls of uterine vessels, and about 10% spread to the heart. The treatment of choice is complete resection of the tumor. Hormonal therapy should be considered in cases of unresectable residual tumor. A 46-year-old woman was admitted for abdominal discomfort and pain. She was found to have intravenous leiomyomatosis of the uterus with extension into inferior vena cava and right atrium. The patient underwent surgery employing simultaneous sternotomy and laparotomy. Radical excision was achieved using cardiopulmonary bypass. We herein describe a patient in whom complete removal of intravenous leiomyomatosis with cardiac extension was successfully performed.

21세기의 정보화 산업사회와 지식기반 사회에서 우리의 아이들이 자신의 목소리로 세계 속에서 살아가게 하기 위해서는 다른 사람과 다른 아이디어를 낼 수 있도록 교육의 틀을 바꾸지 않으면 안된다. 다라서 21세기의 미래 사회를 살아갈 학생들을 가르치는 교육의 현장 역시 창의적인 인간을 길러낼 수 있도록 변해야 한다. 즉, 미래 사회를 대비하기 위해서는 각 개인의 개성을 존중하는 분위기에서 창의적인 능력을 지닌 인간을 양성해야 한다는 것을 의미한다. 따라서, 학교 안에서의 교육뿐만이 아니라 학교 밖에서도 다양한 학습 경험을 통해서 교육 과정이 추구하는 인간상을 구현할 수 있도록 활동할 수 있는 기회가 학생들에게 제공되어야 한다. 창의력을 발휘하여 문제를 해결하는 능력을 기르기 위해서는 개별적이 아닌 여러 명의 동료가 주어진 과제를 해결할 수 있는 기회를 갖고 짧은 시간이 아닌 장시간 동안 고민하고 그 해결책을 찾는 활동을 가질 기회가 필요하다. In order to educate our students with their special talents for the informational and knowledge based age expected in the 21st century, we must improve our educational programs such that students are able to create their own fresh ideas. Hence the present educational environment should be changed such that they are willing to respect others' characters and personality with their own original ideas. Therefore, we must make them have the opportunity that they can acquire many experiences through the school education and the social education program, so that they may grow up to the person with the humanity and creativity. In this study we have suggested new educational program where students engage themselves in long term research projects with collaboration of the team and creativity of individuals.

관상동맥색전증에 의한 급성심근경색증 4예

Acute myocardial infarction(AMI) due to coronary embolism can occur in patients with normal coronary artery. Coronary embolism is believed to be a cause of acute myocardial infarction when emergent coronary angiography shows perfectly normal coronary trees, except for the finding of coronary artery occlusion. In this report, we describe four patients with AMI due to coronary embolism who were successfully treated using a thrombus aspiration device and anticoagulation, and those patients had intracardial thrombus arising from the left atrium due to chronic atrial fibrillation and valvular heart disease. Patients with chronic or paroxysmal atrial fibrillation can present with AMI due to coronary embolism, and thrombus aspiration with percutaneous aspiration device is a feasible treatment option for these patients along with anticoagulation the rapy.

Gh Partially Mediates α1-Adrenergic Receptor-Stimulated Cardiac Hypertrophy Independent of Gq Regulation

In cardiac myocytes, stimulation of α1-adrenoceptor(α1-AR) leads to hypertrophic phenomenon. The Gh protein is tissue type transglutaminase and transmits the intracellular signal with GTPase activity. Little is known about Gh-mediated intracellular signal transductions to develop the hypertrophy. The aim of this study was to elucidate how Gh mediates norepinephrine(NE)-stimulated intracellular signal transductions leading to the hypertrophic responses in neonatal rat cardiomyocytes. NE-induced protein/DNA ratio and ERKs activation were inhibited by a α1-AR blocker(prazosin), but not by a β,-AR blocker(propranolol). Gq mainly regulated protein/DNA ratio and genetic markers of hypertrophy but inhibition of Gq did not induce a decrease of hypertrophic response to the normal control. The treatment with siRNA for Gh suppressed NE-induced PKC/MEK1,2/ERKs activation. NE-induced β- MHC reorganization was inhibited by small interfering RNA targeting Gq or Gh. Cell surface area level of Gq inhibition was not the same as control but atrial natriuretic peptide(ANP), a cardiac hypertrophy marker, was inhibited by Gh and Gq suppression and coincidently inhibition of both Gh and Gq reduced the gene expression. These findings suggest that Gh is partially involved in the hypertrophic response induced by NE in cardiomyocytes through PKC/ MEK1,2/ERKs signal pathways via α1-AR/Gh.

Perfecting of Sa-Vi <寫意, ``expression of ideals``) is one of the highly pursued artistic achievements in East Asian painting tradition. At the core of those ideals expressed lies the concept of Do <道, ``the Truth``). As a way of effectively portraying and conveying the ideals of Do, the application of Yea-Baek <餘白, ``void space``) is regarded important. This article intends co examine some key issues in understanding the aesthetics of Yea-Baek. In-depth understanding of Yea-Baek style makes up a crucial part of the fine art of Dok-Hwa (讀畵, ``reading a painting``). According to Eastern philosophy, the Truth, or Do, cannot be represented in any language or symbol. This notion is well summarized as ``the ineffability of Do (道可道非常道)`` in Too Te Ching (道德經) by Lao Tzu <老子, early 5th century BCE), a leading Chinese thinker. Once you depict something, the depicted image is no longer of the object. In that sense, Yea-Baek or the void is something ``depicted without the act of depicting.`` Yea-Baek is never a fixed, stationary space, but one ``of dynamic, ever-changing nature, filled with energy`` (氣部生動). A specific method to present this characteristic of Yea-Baek is the technique of Hong-Woon-Tak-Wol(烘雲托月), in which, in order co reveal the moon, an artist paints only the surrounding space of the moon, leaving the moon itself untouched. The purpose of such rendering is to achieve ``the ultimate clear, lucid, and tranquil state`` (空靈). In the East Asian concept of ``reading a painting`` (讀畵), looking at the painter`s brushwork (筆墨) is virtually the same as looking at the empty space or Yea-Baek of the composition. The brushwork and the empty space are not two separate matters. They are closely interconnected with each other, both philosophically as well as aesthetically. Although often underestimated as mere ``background``, Yeo-Baek or the void space, which in many cases takes up a large pan of the pictorial space, plays a role as significant as that of the brushwork. Thus, the visually empty space in aesthetic sense becomes no longer ``empty.`` The Korean painting tradition has not only exemplified the features of Yea-Baek, an aesthetic ideal shared in the whole East Asian an tradition, but even went further to intensify and innovate it. In Korean paintings, the effect of Yea-Baek was made prominent by maximizing the effect of harmonization of a larger space (e.g. one as a main background) and smaller spaces (e.g. ones within objects such as mountains, rocks, tree trunks, and so on). Using a mixture of larger and smaller spaces in a painting composition not only creates the aesthetic effect of providing breezy, fluid, and open feelings to the painting, but also expresses the philosophical notion that each object is not separate from the space or other objects, and that everything, including the space, is all interconnected. It is crucial to note that the Korean painting tradition not merely shares ideals common among the East Asia cultures, such as Sa-Vi and Yeo-Baek, but at the same time, also demonstrates a good number of unique quality and characteristics entirely of its own.
