      • Employment Status and Work-Related Difficulties in Lung Cancer Survivors Compared With the General Population

        OBJECTIVE:: To investigate the employment status of lung cancer survivors and the work-related problems they face. BACKGROUND:: Although the number of lung cancer survivors is increasing, little is known about their employment and work-related issues. METHODS:: We enrolled 830 lung cancer survivors 12 months after lung cancer curative surgery (median time after diagnosis, 4.11 years) and 1000 volunteers from the general population. All participants completed the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire, Core 30-item and a questionnaire that included items relating to their jobs. We used logistic regression analysis to identify independent predictors of unemployment. RESULTS:: The employment rate of lung cancer survivors decreased from 68.6% at the time of diagnosis to 38.8% after treatment, which was significantly lower than the employment rate of the general population (63.5%; adjusted odds ratio = 2.31, 95% confidence interval: 1.66–3.22). The posttreatment unemployment rate was higher for women than for men. Among survivors, employment was inversely associated with older age, household income, number of comorbidities, and poor social functioning. Fatigue (78.6%) was the most common work-related problem reported by survivors. CONCLUSIONS:: Lung cancer survivors experienced more difficulties in employment than did the general population. Age, monthly household income, number of comorbidities, and social functioning appear to be important factors influencing employment status. These findings suggest that lung cancer survivors need support to cope with the financial impact of cancer.

      • Mediastinal nodal staging of nonsmall cell lung cancer using integrated <sup>18</sup>F-FDG PET/CT in a tuberculosis-endemic country : Diagnostic efficacy in 674 patients

        <B>BACKGROUND</B><P>Integrated <SUP>18</SUP>fluorine fluorodeoxyglucose (<SUP>18</SUP>F-FDG) positron emission tomography/computed tomography (PET/CT) has shown somewhat variable sensitivity and specificity for mediastinal nodal staging in granulomatous disease endemic areas. The purpose of the study was to prospectively evaluate the efficacy of PET/CT for mediastinal nodal staging in nonsmall cell lung cancer (NSCLC) patients in a tuberculosis-endemic country.</P><B>METHODS</B><P>Prospective assessment of the diagnostic efficacy of integrated PET/CT for detecting mediastinal nodal metastasis was performed in 674 patients (M:F ratio = 502:172; mean age, 61 years) with NSCLC. Patients underwent an integrated PET/CT examination and subsequent surgical nodal staging (by mediastinoscopy only in 121 patients and by thoracotomy in 553). Nodes showing greater <SUP>18</SUP>F-FDG uptake than mediastinum at PET without benign calcification or high attenuation >70 household unit (HU) at unenhanced CT were regarded as being positive for malignancy. The histologic nodal assessment results were used as reference standards.</P><B>RESULTS</B><P>Of 2477 mediastinal nodal stations evaluated in 674 patients, 275 (11%) stations in 180 (27%) patients proved to be malignant. On a per-person basis, the overall sensitivity, specificity, and accuracy of PET/CT for mediastinal nodal staging were 61% (110 of 180), 96% (473 of 494), and 86% (583 of 674), respectively. On a per-nodal station basis, they were 46% (126 of 275), 98% (2154 of 2202), and 92% (2280 of 2477).</P><B>CONCLUSIONS</B><P>Integrated PET/CT provides high specificity and reasonably high accuracy, but somewhat low sensitivity for mediastinal nodal staging of NSCLCs. The high specificity is achieved at the expense of sensitivity by interpreting calcified nodes or nodes with high attenuation at CT, even with high FDG uptake at PET, as benign in a tuberculosis-endemic region.         피부 병원균에 대한 톱니모자반 추출물의 항균 시너지 효과

        S. aureus, S. epidermidis, P. aeruginosa, P. acnes와 C. albicans는 사람의 피부에서 발생하는 질병과 밀접한 관련을 가지는 대표적인 병원성 미생물로 알려져 있다. 본 연구에서는 해조류 중에서도 항균 활성에 대한 연구가 미미한 모자반을 대상으로 피부 병원균에 대한 항균 효과를 조사하였다. 국내의 연해에 자생하는 7종의 모자반 추출물 중에서 disc diffusion assay와 MIC assay를 통해 가장 뛰어난 항균 효과를 나타낸 톱니모자반을 후속 연구를 위한 후보 물질로 선정하고 연구를 진행하였다. 톱니모자반 추출물의 유기용매 분획층 중에서, 노말-헥세인 분획층이 S. aureus, S. epidermidis, P. aeruginosa, P. acnes 및 C. albicans에 대한 MIC값이 32-256 μg/mL로 가장 뛰어난 항균 활성을 나타내었다. 이에 피부 병원균들에 대한 치료제로 사용되고 있지만 내성균의 출현으로 효능이 거의 없는 항생제들인 테트라사이클린, 에리트로마이신, 린코마이신과 플루코나졸과 항균 활성이 뛰어난 것으로 나타난 톱니모자반의 노말-헥세인 분획층과의 병용 사용에 의한 항균 시너지 효과를 조사하였다. 그 결과, 톱니 모자반 노말-헥세인 분획층과 이들 항생제와의 병용 사용에 의해 피부 병원균에 대한 항생제와 톱니모자반 노말-헥세인 분획층의 MIC값이 4-32배 감소되었고, 톱니모자반 노말-헥세인 분획층과 이들 항생제와의 병용 시 median FIC값이 0.26-0.55로 항균 시너지 효과를 나타내었다. 즉, 항생제와 톱니모자반 노말-헥세인 분획층과의 병용 사용은 피부 병원균에 대한 이들 항생제의 감수성을 회복시키는데 크게 기여하는 것으로 나타났다. The object of this study was to develop an alternative way to treat human skin pathogens using marine algae. During this study, we observed that the ethanolic extract of the edible brown algae [Sargassum serratifolium (C. Agardh) C. Agardh] exhibited potential antimicrobial activity against pathogenic commensal bacteria related with acne vulgaris (Propionibacterium acnes, Staphylococcus epidermidis, Staphylococcus aureus and Pseudomonas aeruginosa), and Candida albicans which causes cutaneous candidiasis. Among the solvent-soluble fractions from the ethanolic extract, a hexane-soluble fraction showed the strongest antimicrobial activity against all tested human skin pathogens with MIC values ranging from 32 to 512 μg/mL. In addition, the hexane fraction exhibited a synergistic antimicrobial activity with commercial antibiotics used in the treatment of acne vulgaris or cutaneous candidiasis. Thus, this study suggests that S. serratifolium extract could be a potential source of natural antimicrobial agents or a pharmaceutical component against human skin pathogens.

        Development of Biologically Active Compounds from Edible Plant Sources XIV. Cyclohexylethanoids from the Flower of Campsis grandiflora K. Schum.

        Campsis grandiflora K. Schum. flower was extracted with 80% aqueous MeOH, and concentrated extract was successively partitioned with EtOAc, n-BuOH, and $H_2O$. From n-BuOH fraction, two cyclohexylethanoids were isolated through repeated silica gel and Sephadex LH-20 column chromatographies. Based on physico-chemical data obtained from NMR, MS, and IR, chemical structures of compounds were determined as 1,4-dihydroxy-3,4-(epoxyethano)-5-cyclohexene (1) and cornoside (2). These compounds were isolated for the first time from C. grandiflora K. Schum flower.

        <P>Bioassay-guided isolation of an aqueous methanolic extract of <I>Vitis vinifera</I> using pancreatic lipase inhibitory activity led to isolation of seven stilbenoids, wilsonol C (<B>1</B>), heyneanol A (<B>2</B>), ampelopsin A (<B>3</B>), pallidol A (<B>4</B>), <I>cis</I>-piceid (<B>5</B>), <I>trans</I>-piceid (<B>6</B>) and <I>trans</I>-resveratrol (<B>7</B>). The structures were established on the basis of NMR and MS spectroscopic data interpretation. All isolates were evaluated for their inhibitory effects on pancreatic lipase, and stilbenoid <B>1</B> exhibited potent inhibitory effect on pancreatic lipase with IC<SUB>50</SUB> values of 6.7 ± 0.7 µM.</P>

        Stage T1 Non-Small Cell Lung Cancer: Preoperative Mediastinal Nodal Staging with Integrated FDG PET/CT--A Prospective Study.

        <P>Purpose: To prospectively evaluate the sensitivity and specificity of integrated fluorine 18 fluorodeoxyglucose (FDG) positron emission tomography (PET) and computed tomography (CT) (PET/CT) for the preoperative diagnosis of mediastinal nodal metastasis in stage T1 non-small cell lung cancer (NSCLC), with surgical and histologic results as reference standards. Materials and Methods: Institutional review board approval and informed consent were obtained. From June 2003 to February 2005, 150 patients (89 men and 61 women; mean age, 59 years) with stage T1 NSCLC at stand-alone CT underwent integrated PET/CT and surgical staging. Two observers (one radiologist and one nuclear medicine physician) evaluated prospectively and in consensus the mediastinal nodes by analyzing both PET (functional) and CT (anatomic) images. Nodal stages were determined by using the American Joint Committee on Cancer staging system and surgical and histologic findings as the reference standard. Statistical evaluation of malignant lymph nodes was performed on per-nodal-station and per-person bases. Results: A total of 568 mediastinal nodal stations were evaluated. Nodes were positive for malignancy in 34 (23%) of 150 patients and 55 (10%) of 568 nodal stations. For depiction of malignant nodes, the respective sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of integrated PET/CT were 42% (23 of 55), 100% (513 of 513), 100% (23 of 23), 94% (513 of 545), and 94% (536 of 568) on per-nodal-station basis and 47% (16 of 34), 100% (116 of 116), 100% (16 of 16), 87% (116 of 134), and 88% (132 of 150) on a per-patient basis. Conclusion: Integrated FDG PET/CT provides high specificity and positive predictive value of mediastinal nodal staging in stage T1 NSCLC, although the sensitivity is low. (c) RSNA, 2006.</P>

        Studying the Genetic Diversity and Phenetic Relationships of Porphyra yezoensis Populations in Korea Using Random Amplified Polymorphic DNA (RAPD)

        김(Porphyra yezoensis)은 김속의 홍조류이다. RAPD (random amplified polymorphic DNA) 마커를 이용하여 한국 내 네 집단의 표현형과 유전적 다양성을 조사하였다. 전체적으로 20 시발체로 김에서 55분절이 관찰되었다. 이들 밴드 중 30개(54.5%)는 다형성을 나타내었다. OPA-18-02 밴드는 낙동 김 집단에서만 증폭되었다. OPA-20-02 밴드는 서천 김 집단에서만 증폭되었다. 이 두 밴드는 특별한 집단을 구별해주는 특이밴드로 판정되었다. 대립유전자좌위의 수(Ae)는 1.161에서 1.293로 평균은 1.366였다. 서천 김 집단이 가장 높은 다형성을 나타내었다(0.163). 다른 집단과 격리되고 조간대에 위치한 낙동 김 집단은 가장 낮은 다형성을 나타내었다(0.092). 샤논의 표현형 다양성(I)은 서천 김 집단이 가장 높았다(0.238). 전체 유전적 다양도(HT)는 0.132(OPA-02)에서 0.420(OPA-19)로 나타났다. 대립유전자좌위에서 유전적 다양성(HS)은 0.059(OPA-18)에서 0.339(OPA-19)였다. 대립유전자좌위에 근거에서 전체 유전적 다양도에서 집단 간 차이(GST)는 0.012(OPA-11)에서 0.762(OPA-18)이였으며 평균은 0.415였다. 이는 전체 변이의 약 42%는 집단 간에서 발견된다는 것을 의미한다. 종 내 다양도의 58.5%는 집단 내에 있었다. 유전자 흐름(Nm)은 0.705로 낮았다. Porphyra yezoensis is a red algal species in the genus Porphyra. The phenetics and genetic diversity of four populations of P. yezoensis in Korea were reconstructed using random amplified polymorphic DNA (RAPD) markers. Overall, 55 fragments were generated among the tested P. yezoensis array with 20 OPERON primers. A total of 30(54.5%) of these bands were polymorphic. The OPA-18-02 band was amplified in the samples of Nakdong population and absent in them of other three populations. The OPA-20-02 band was only amplified in the Seocheon population. Both bands exhibited distinctive patterns in specific populations. The effective number of alleles per locus (Ae) ranged from 1.161 to 1.293 with a mean of 1.366. The Seocheon population had a high expected diversity (0.163). The Nakdong population was an isolated endemic and intertidal zone. Thus the narrow distributed Nakdong population had a low expected diversity (0.092). Shannon’s index of phenotypic diversity (I) of the Seocheon population (0.238) was the highest among all populations. Total genetic diversity (HT) varied between 0.132 for OPA-02 and 0.420 for OPA-19. The interlocus variation of genetic diversity (HS) was 0.059 for OPA-18 and 0.339 for OPA-19. On a per locus basis, the proportion of total genetic variation due to differences among populations (GST) ranged from 0.012 for OPA-11 to 0.762 for OPA-18 with a mean of 0.415, indicating that 42% of the total variation was found among these populations. In an assessment of the proportion of diversity present within this species, 58.5% (100%-41.5%) of genetic variation resided within the populations studied. The Nm was estimated to be low (0.705).
