        수종의 광독성 약물투여 후 시간에 따른 피부농도 및 최소 광독량에 관한 연구

        N/A The Skin Concentration and Minimal Phototoxic Dose Following Administration of Phototoxic Drugs as a Function of Time Yoon-Kee Park, M.D., Moo Yon Cho, M.D., Saung Kyung Hann, M.D., Sungbin im, M.D. Department of Dermatology Yonsei Univeraity College of Medicine, Seoul, Korea Psoralen photochemotherapy(PUVA) is defined as a treatment that depends on the interaction of a photosensitizing substance, psoralen, and long wave jltraviolet(UVA) radiation that results in therapeutically beneficial effects. The rationale of its use is to clear skin disease by repeated controlled phototoxic reactions. The efficacy of PUVA depends on the drug, the UVA dose and wave length, the individual sensitivity to the phototoxic reaction and on the disease to be treated. However, the major factor is the amount of psoralen at the site of photochemical reaction in skin. Daspite the extensive experiences which have now accumulated in the use of PUVA, the relationship between the therapeutic response and phototoxicity as it occurs in normal skin is not established. Comparative data on different psoralen molecules and their skin sensitizing ability are required. From studies on the skin concentration using HPLC and minimal phototoxic dose following oral administration, intraperitoneal administration, a bath of phototoxic drugs as a function of time in guinea pigs, the following results are obtained. 1. The skin concentration of phototoxic drugs after oral administration peaked at 1.5 hours, and the concentration of 8-methoxypsoralen(8-MOP) was 3.7 times greater than that of 5-methoxypsoralen(5-MOP). The skin concetration of 4,5,8-trimethylpsoralen(TMP) was not detected in this study(limit of sensitivity 5ng/g). 2. The skin conentration of phototoxic drugs after intraperitoneal administration peaked at 30 minutes, and the concentration of 8-MOP was slightly greater than that of 5-MOP. The skin concentration of TMP was not detected in this study(limit of sensitivity 5ng/g). 3. The skin concentration of phototoxic drugs after bathing peaked immediately after bathing, and the skin concentrations of phototoxic drugs after bathing decreased in the order of 5-MOP, TMP, and 8-MOP. 4. The minimal phototoxic dose of phototoxic drugs after oral administration was lowest at 1.5 hours, and the minimal phototoxic dose of 8-MOP was 8 times less than 5-MOP, 24 times less than TMP. The phototoxicity of 8-Mop after oral administration was strongest. The time of lowest phototoxic dose and highest skin concentration was the same. 5. The minimal phototoxic dose of phototoxic drugs after intraperitoneal administration was lowest at 30 minutes, and the minimal phototoxic dose of 8-MOP was 1.3 times less than 5-MOP, 5.7 times less than TMP. The phototoxicity of 8-MOP after intraperitoneal administration was strongest. The time of lowest phototoxic dose and highest skin concentration was same. 6. The minimal phototoxic dose of phototoxic drugs after bathing was lowest at 15 minutes. The minimal phototoxic dose of 8-MOP was not significantly different but was significantly greater than that of 5-MOP.

        N/A The Effect on the Recovery of Epidermal Langerhans Cells in C3H Mice after Single and Fractionated Exposure of Ultraviolet B Irradiation Sang-Wahn Koo, M.D., Seung Kyung Hann, M.D., Yoon-Kee Park, M.D., Moo Yon Cho, M.D. Department of Dermatology, Yonsei University College of Medicine Seoul, Korea The study was undertaken to see the recovery in epidermal Langerhans cells in relation to time after UVB irradiation through different amounts and ways of exposure in C3H mice. We irradiated the ears of C3H mice with UVB 200mJ/㎤ and 400mJ/㎤ in a single dose at one time and 5 fractionated doses for 5 days and 10 fractionated doses for 10 days and performed biopsies on the ears of the control and experimental groups after, 2, 7, 14, 21 days of irradiation and stained them by the immunoperoxidase method. The results are summarized as follows. 1. A more significant decrease in the number of Ia-positive epidermal Langerhans cells was observed in single-dose-exposed group than fractionated-dose-exposed groups on the second day of irradiation. 2. On the seventh day of exposure, the number of the Ia-positive epidermal cells in single-dose-exposed group returned to normal, but in fractionated-dose-exposed groups the number of Langerhans cells decreased most remarkably. 3. In fractionated-dose-exposed groups, the number of Langerhans cells returned to normal on the 14th or 21st day of expposure. (Kor J Dermatol 29(4) : 459-465, 1991)

        Background and Objectives : The localization of platelet-derived growth factor-α receptor (PDGF-α R) was commonly restricted to oligodendrocyte progenitors during late embryonic and postnatal development. However, several studies recently demonstrated that mature neurons could also synthesize PDGF-α, Materials and Methods : In the present study, to analyze the distributional pattern of PDGF-αR during postnatal development of the canine cerebral cortex, we used immunohistochemistry on sections of canine brain tissue. Results : We found that neurons of various regions of cerebral cortex exhibited the immunoreactivity to PDGF-αR as early as postnatal day 0, and slightly decreased after postnatal day 14. Whereas neuronal PDGF-αR were maintained at all ages, the oligodendroglia-like expression of PDGF-αR could not be confirmed. Conclusion : The localization of PDGF-αR in immature and mature neurons supports the several roles of PDGF during development, protection and survival of neurons.

        Chronic neutrophilic leukemia(CNL) is a very rare myeloproliferative disease, characterized by sustained mature neutrophilic leukocytosis with granulocytic bone marrow infiltration, high NAP(neutrophilic alkaline phosphatase) score and absence of philadelphia chromosome, It is frequently accompanied by hepatosplenomegaly, elevated serum vitamin B_12 and uric acid level. For the diagnosis of CNL, the leukemoid reaction, especially secondary to neoplasia, infection and autoimmune diseases, should be excluded. Since Tuohy's first description in 1920, more than 50 cases fullfilling the above criteria have been reported worldwide, and 4 cases in Korea. Several authors have demonstrated the defect of intracellualr killing in the mature neutrophil and this finding correlates well with the very high incidence of fatal infection, The hemorrhagic diasthesis in CNL is caused by functional abnormality of the platelet. This disease has tendency to transform to blastic crisis and acute leukemia as in other myeloproliferative disease, but characteristically shows frequent coexistence with multiple myeloma. Until now , the therapeutic trials in CNL have been disappointing. Hydroxyrea and busulfan can control hyperleukocytosis. On the basis of functional defect in neutrophil, alpha-2b-interferon has been tried and several reports have demonstrated the clinical and functional effect of interferon on CNL. CNL is very rate hematologic disease and there are few report about general aspect of disorder. We report here a typical CNL case presenting with splenomegaly and leukocytosis with a review of the literature.

      • 乾癬治療를 위한 자외선 B照射와 光化學療法(PUVA)의 비교 연구

        Abstract Thirty nine patients with stable plaque type psoriasis were treated with the combination therapy of mineral oil plus UV-B or oral methoxsalen plus longwave UV light (PUVA) therapy. The mean number of treatment, average exposure dose, and accumulated exposure dose of each group needed for achieving each grade were evaluated according to the lesion site. Average exposure time and accumulated exposure time were also calculated. No difference was found between these croups in the number of exposure for clearing of over 95% of the lesions. But, for the exposure time, mineral oil plus UV-B treated group took much lese than the PUVA treated group. Based on these findings and considering the general advantages of UV-B phototherapy, the combination therapy of mineral oil plus UV-B can be recommended as a useful method for the treatment of psoriasis.

        악골에 발생한 석회화 치성낭의 임상 및 방사선학적 고찰

        Nine cases presented with a calcifying odontogenic cyst to the Dental Hospital of the College of Dentistry, Yonsei University, from January 1987 to September June, 1996, Clinical or histopathological findings were observed according to each radiologic criteria. The results obtained are as follows: 1. The male to female ratio was 2:1 with a mean age of 25 years. 2. The radiographic appearences of nine cases were well-defined radiolucent lesions in which eight cases were shown unilocular lesions and only one lesion was seen multilocular lesion. 3. Radiologically, three of nine cases were pure radiolucent lesions and others, six cases were radiolucent contained a variable amount of radiopaque material. 4. Histologically, two cases were classified simple cyst, five were cyst associated odonoma, two were neoplastic type. 5. Histlolgical findings according to the radiological classification, two of three radiolucenct lesions were simple cysts; another was a cyst associated with odontoma. While, four of six radiologic mixed lesions turned out to be a cyst associated with odontoma, two was the neoplastic type.

        췌십이지장절제술 후 발생한 지속적인 구토증과 Wernicke 뇌병증

        Wernicke encephalopathy is an acute neurologic disorder attributable to thiamine (vitamin B1) deficiency. We report the case of a 61-year-old female patient who presented Wernicke encephalopathy after surgery for pancreatic head cancer. From the ninth postoperative day, she had suffered from nausea and vomiting and had difficulties ingesting food, she was given total parenteral nutrition (TPN), but lacked adequate vitamin (thiamine) supplementation. After 28 days, she developed ataxia, ophthalmoplegia, and mental confusion. The magnetic resonance image showed pathologic changes in the medial thalamus,periaqueductal gray matter, medulla and mamillary bodies. The blood level of thiamine was very low. After intravenous and oral supplementation of thiamine (200 mg/day), consciousness was soon normalized and neurologic symptoms have gradually been improving. Nausea and vomiting disappeared after administration of a low dose of mirtazapine (7.5 mg/day). We emphasize the importance of thiamine supplementation to the patients who suffer from vomiting which hinders them from taking food and who require prolonged TPN.

      • 들기름의 酸敗測定方法에 關한 硏究

        Several methods of measuring rancidity were compared with respect to sensitivity against autooxidized perilla oil storing at 45℃. The results obtained were summarized as follows: 1. Peroxide value, hydroperoxide diene and triene levels, contents of acetaldehyde and hexanal in head space and refractive index of perilla oil were increased almost linearly with the incubation time tested at 45℃. 2. The best correlation with sensory test score was peroxide value(r=0.992**). Hydroperoxide diene value and acetaldehyde contents in head space also show highly significant correlationship with wensory test score. 3. Considering the rapidity and sensitivity, hydroperoxide diene method and peroxide method are the recommendable for measuring the rancidity of perilla oil.

      • 貧血症과 赤血球過多症에 있어서 血淸 Erythropoietin 活性度에 關한 硏究

        Erythropoietin (Ep) is a glycoprotein hormone of mostly renal origin that stimulates red cell production in response to tissue hypoxia. Serum Ep activities provide important clinical clues in making the differential diagnosis of some types of anemia and polycythemia. The study was performed in order to evaluate the diagnostic value of serum Ep activities in 29 normal subjects, 43 anemic patients and 15 secondary polycythemia patients and to find out the relationship between hemoglobin concentration and serum Ep activities in these patients using the Enzyme Immunoassay with Second International Preparation for Ep. The results obtained were as follows: 1. Ep activities in sera from 29 normal subjects among children ranged from 11 to 72 mU/ml, with a mean of 37.10±16.54 mU/ml. There was no significant difference between the serum Ep activities in male and female subjects (p>0.05). 2. Ep activities in sera from 22 patients with iron deficiency anemia ranged from 48 to 400 mU/ml, with a mean of 153.5 mU/ml which was about 4 times higher than normal value. Ep activities in sera from 18 anemic patients with acute infection ranged from 35 to 290 mU/ml, with a mean of 134.05 mU/ml which was about 3.5 times higher than normal value. Ep activities in sera from 3 patients with aplastic anemic ranged from 620 to 840 mU/ml, with a mean of 720 mU/ml which was about 20 times higher than normal value. 3. Ep activities in sera from 15 patients with secondary polycythemia ranged from 12 to 140 mU/ml. In 10 of 15 patients, the mean Ep activities was 87.50 ± 25.60 mU/ml, which was about 2 times higher than normal value. In 5 of 15 patients, Ep activities were within normal range as a mean of 35.80 ± 17.23 mU/ml. In summary, a significant negative correlation was found between the hemoglobin concentration and the logarithm of the serum Ep activities in 43 patients with anemia (r=0.73, Y=3.6766-0.1802x) and Ep activities in patients with aplastic anemia was higher than those in patients with iron deficiency anemia at comparable hemoglobin concentration. From the above results, it was concluded that serum Ep activities in anemic patients are not only inversely related to the hemoglobin concentration but also to the activity of the erythroid bone marrow. While no significant correlation was found between hemoglobin concentration, hematocrit percentages and Ep activities in 15 patients with secondary polycythemia (r=0.18, 0.39 respectively).
