Background and Objectives : Palindromic rheumatism (PR) is known to be triggered without any obviously inciting events in most patients, although, in a few cases, attacks have been noted to relate to another events such as weather change, childbirth, ingestion of certain foods, or over-exercise. The aim of this study was to characterize the triggering factors on palindromic attacks. Materials and Methods : We evaluated sixty-four patients with PR for the presence of triggering factors and the relationship between attacks and triggering factors by interview of patients. We considered that there was an probable association between episodes and PR, if episodes within 24 hours prior to PR had triggered at least 50% of all attacks and a definite association between episodes and PR, if episodes within 24 hours prior to PR had triggered 100% of all attacks. Results : In 15 patients (35.9%), there was a possible association between episodes of PR and initiating events. In 8 patients (12.5%), there was a definite association between episodes of PR and initiating events. The triggering factors were physical over-activity including exercise (19/64: 29.7%), foods, such as rawfish, crab, and chicken, including alcohol (19/96: 19.8%). However, there were no difference between the groups with and without the triggering factor in sex ratio, duration and onset of PR, the positive rate of rheumatoid factor and involved sites. Conclusion : Our observations showed that PR was triggered by physical over-activity more than in previous reports and hypersensitivity to foods was a causative factor in some patients with PR. However, patients with triggering factors had a similiar clinical profile to patients without triggering factors.

The accuracy and applicability of numerical simulation model for land-sea breezes are studied by the computative study between analytical model and numerical simulation results. In the model, Defant's equation which has analytical solution was selected. Both numerical and analytical solutions represented well land and sea breezes under the condition of various diffusion coefficients and hydrostatic and nonhydrostatic conditions. By the results, the numerical simulation method was proved as an appliable technique to simulate accurate land-sea breezes.

<P><B>Purpose</B></P><P>To evaluate the incidence of colon cancer as a second primary cancer (CCSPC) and the survival outcomes of women with and without CCSPC after the diagnosis of endometrial cancer (EC).</P><P><B>Methods</B></P><P>The standardized incidence ratio (SIR) of CCSPC and survival outcomes of EC survivors with and without CCSPC were analyzed using data from January 1 1993 to December 31 2011, obtained from the Korea Central Cancer Registry.</P><P><B>Results</B></P><P>Of 14,797 EC survivors, 147 (0.99%) developed CCSPC after an average interval of 5.5 years. The SIR of CCSPC among EC survivors was 2.56, higher than that of colon cancer in the general population. The SIR of CCSPC was highest for the ascending (3.77), followed by the transverse (3.45), descending colon (2.06), and rectum (1.99). The risk of a proximal site of CCSPC was high, especially within 5 years after the diagnosis of EC in the ascending (SIR, 4.37) and transverse (4.91) colon, and in young survivors (< 60 years) in the ascending (5.19) and transverse (3.82) colon. The 5- and 10-year overall survival rates were 84.8 and 80.4% among survivors with EC only and 89.2 and 76.3% for survivors with CCSPC, respectively.</P><P><B>Conclusions</B></P><P>The risk of CCSPC among EC survivors increases especially in the proximal colon in young survivors. These results could be used for surveillance and counseling of EC survivors.</P>

Background: Plasmablastic lymphoma (PBL) is a rare type of aggressive lymphoma that is not well known regarding treatment and outcome. The aim of this study is to analyze the clinical characteristics and treatment outcome of adult PBL, focusing on the outcome by chemotherapeutic regimens. Methods:We have retrospectively analyzed the outcome of adult patients with PBL diagnosed and treated at Seoul National University Hospital in Seoul, Korea, between Jan. 2007 and Feb. 2014. Results: fifteen patients were diagnosed during the period. The median age was 54 years (range 32-78), and 12 (80.0%) patients were male. Two patients were human immunodefi ciency virus (HIV) positive. None of the patients had received organ transplantation, and none of HIV negative patients had history of receiving any immunosuppressive treatment. Nine (60.0%) patients had extranodal disease, primary sites being maxilla/ mandible in 3, hypopharynx/pharynx in 2, nasal cavity in 1, pelvic bone in 1, gall-bladder in 1 and heart in 1. In situ hybridization for Ebstein-Barr virus was performed in all 15 patients and were positive in 6 (40.0%). Thirteen patients received chemotherapy and outcome data were available in 12 patients for analysis. Of the 12, 6 were treated with CHOP (cyclophosphamide 750mg/m2 iv D1, Adriamycin 50mg/m2 iv D1, vincristine1.4mg/m2 iv D1 and prednisolone 100mg p.o D1-5) and 6 with EPOCH (etoposide 50mg/ m2 iv D1-4, adriamycin 10mg/m2 iv D1-4, vincristine 0.4mg/m2 iv D1-4, cyclophosphamide 750mg/m2 iv D5 and prednisolone 60mg/m2 p.o D1-5). EPOCH group showed signifi cantly better progression-free survival (not reached vs. 1.2mo, P=0.039) compared to CHOP. Overall response rate (100.0% vs. 50.0%, P=0.273) and OS (not reached vs. 11.4mo, P=0.104) seemed better in EPOCH group than CHOP group. Conclusions: Chemotherapy more aggressive than CHOP may benefit adult patients with PBL. EPOCH can be a good chemotherapy option.

<P><B>Purpose</B></P><P>Patients often complain of change of defecation pattern and it is necessary to quantify their symptoms. To quantify symptoms, use of questionnaire is ideal, so we adopted a simple and easily writable visual analogue scale for irritable bowel syndrome questionnaire (VAS-IBS). The aim of this study was to develop and validate the Korean version of VAS-IBS questionnaire (Korean VAS-IBS) that can adequately reflect the defecation pattern.</P><P><B>Methods</B></P><P>This study translated English VAS-IBS into Korean using the forward-and-back translation method. Korean VAS-IBS was performed on 30 patients, who visited the outpatient clinic and had no possibility of special defecation pattern. Detailed past medical history and Bristol stool chart was added to the questionnaire. The survey was conducted twice, and the median interval between the 2 surveys was 10 days (8–11 days). Cronbach α for internal consistency reliability and intraclass correlation coefficients for test-retest reliability were analyzed.</P><P><B>Results</B></P><P>Korean VAS-IBS achieved acceptable homogeneity with a Cronbach α coefficient of 0.66–0.79 showing adequate internal consistency reliability. In addition, intraclass correlation coefficients showed significant test-retest reliability with 0.46–0.80 except for the question assessing the “perception of psychological wellbeing.”</P><P><B>Conclusion</B></P><P>The Korean VAS-IBS is a valid and reliable questionnaire for the measurement of the symptoms of defecation pattern changes.</P>

<P>Photodetachment of aryl moieties from covalently functionalized SWNTs was studied by temperature-dependent and UV Raman spectroscopic tools. The sidewall functionalized SWNTs were prepared <I>via</I> diazonium reactions from 4-bromoaniline and isoamyl nitrite. <I>I</I>–<I>V</I> conductivity measurements were performed for the functionalized SWNTs, after purification from pristine SWNTs using their different solubility. Temperature dependent Raman data appeared to be in line with thermal gravimetric (TGA) data exhibiting removal of aryl moieties above 590 °C. Raman spectra of the functionalized SWNTs were examined using ultraviolet excitation at 244 nm (5.08 eV) and 325 nm (4.82 eV) as well as visible irradiation at 633 nm (1.96 eV). Our experimental results indicated that the disorder-induced D mode should change significantly, whereas the tangential G modes do not become different under UV irradiation. The D mode for the functionalized SWNTs was found weakly at ∼1410 cm<SUP>−1</SUP> at a low power of UV irradiation, whereas it was observed strongly at ∼1310 cm<SUP>−1</SUP> upon 633 nm irradiation. This is a case report of a 68-year-old man with hepatocellular carcinoma (HCC) accompanied by hereditary hemorrhagic telangiectasia (HHT), also known as Osler-Weber-Rendu disease, and hepatic vascular malformation. HHT is an autosomal dominant disorder of the fibrovascular tissue that is characterized by recurrent epistaxis, mucocutaneous telangiectasias, and visceral arteriovenous malformations. HHT is caused by mutation of the genes involved in the signaling pathway of transforming growth factor-β, which plays an important role in the formation of vascular endothelia1. Hepatic involvement has been reported as occurring in 30-73% of patients with HHT. However, symptomatic liver involvement is quite rare, and the representative clinical presentations of HHT in hepatic involvement are high-output heart failure, portal hypertension, nodular regenerative hyperplasia, and symptoms of biliary ischemia. Some cases of HCC in association with HHT have been reported, but are very rare. We present herein the characteristic radiologic and genetic findings of HHT that was diagnosed during the evaluation and treatment of HCC. (Korean J Hepatol 2011;17:313-318)
