        용접공들의 면역능에 관한 연구

        The authors examined human immunity indicators such as CD4, CD8, Natural Killer(NK) cell, IgG, IgA, and IgM, and heavy metals such as Pb, Cr, Cd to evaluate the health hazard of welders. We selected total 126 male subjects, 60 welders who had more than 5 year working history as an exposure group, and 66 office workers and medical college students as a control group. This immunity evaluation approach is the first newly designed study that has never been tried in past studies on welder's heath evaluation. And the results were as follows. 1. Total lymphocyte count of peripheral blood was significantly higher in welder group, 2,615±650 ea/㎕ compared with 2,368±681 ea/㎕ in control group(p>0.05). 2. The proportion of CD4, CD8, NK cell in welder group was 37.5±9.4%, 24.0±8.8%, and 21.0±9.5%, respectively, and 35.5±6.5%, 25.0±6.9%, and 19.6±8.8% in control group. CD4 and NK cell were slightly higher, and CD8 was slightly lower in welder group. but there was no statistical significance between two groups(p≥0.05). 3. IgG, IgA, and IgM in welder group were 15.9㎎/㎗, 2.7㎎/㎗ and 1.5㎎/㎗, respectively, and 15.6㎎/㎗, 2.9㎎/㎗ and 1.7㎎/㎗ in control group. There was no statistical significance between two groups(p≥0.05). 4. Blood lead, cadmium and chromium concentrations were 4.2±1.8㎍/㎗, 0.4±0.38㎍/㎗, 1.2±0.6㎍/㎗ in welder group and 3.2±1.7㎍/㎗, 0.44±0.29㎍/㎗, 1.1±0.6㎍/㎗ in control group, respectively, and these value were within normal ranges.

      • Scabies presenting as atypical vesicular manifestations

        Scabies is a contagious infestation caused by the mite, Sarcoptes scabiei. The common presentations of scabies are linear burrows, pruritic papules, and inflammatory nodules. Vesicular and bullous lesions are rare in scabies. Because of its clinical and histopathologic non-specific nature, vesicular scabies may be difficult to diagnose early. We report two cases of scabies presented with vesicular lesions. A 35-year-old male with quadriplegia due to subarachnoid hemorrhage presented with multiple erythematous vesicles and pustules that appeared on his hands 2 weeks ago. Biopsy specimen from the right palm showed subcorneal blister and eosinophilic infiltration in the dermis. He treated with topical steroids for 3 months, but new inflammatory nodules and pruritic papules developed on his whole body, especially on flexural areas. Then, the KOH test was performed and scabies mites were found. Second case was a 13-year-old boy with itchy multiple vesicles on whole body which have lasted 2 months. Histopathologic examination from a vesicle of the left lower leg revealed subepidermal cleft and DIF showed negative finding. A KOH test was performed and several mites were found. After topical permethrin and oral antihistamine treatments for both patients, their skin lesions showed marked improvement. Therefore, scabies should be considered as a differential diagnosis if itchy vesicular skin lesion of unexplained etiology occurs.

        한국의 발톱진균증 환자의 삶의 질에 관한 연구

        Background: Onychomycosis, especially toenail onychomycosis has become one of the common fungal infection and has historically been regarded as a cosmetic rather than medical problem by many patients, even by physicians. Recently, however, there are several reports that is a refractory disease which may cause a deleterious effect on patiets quality of life (QOL). Objective: The purpose of this study was to investigate the impact of toenail onychomycosis on QOL in Korea and to assess the chandes of QOL after treatment. Methods: Total 1004 patients with toenail onychomycosis which was confirmed by clinical findings and KOH preparation were were enrolled at 47 dermatologic centers in Korea, and interviewed with standardized QOL questionnaire before and after sytemic antifungal treatment. Responses to the questionnaire were scored by 5-point scale (1∼4) and averaged, and were analyzed for 5 dimensions of emotional impact, social impact, symptorn and functional impact, patients views concerning treatment, and relationship with doctor. Results: 1. Before and after treatment, the most serious impact was emotional dimension showing 1.90 and 1.30 in average score (AS), and social (AS: 1.14 and 0.83) and symptom and functional impact (AS: 1.05 and 0.92) was also affected. 2. In female rather than male, statistically more significant impact on patients QOL was observed in all dimensions. 3. After treatment, 3 of 5 dimensions were improved significantly - emotional dimension (AS : from 1.90 to 1.30), social dimension (AS: from 1.14 to 0.83), patients view concerning treatment (AS: from 1.34 to 1.02). 4. The drgree of patients satisfaction at the therapeutic effect was very high- 62.4% (immediately after treatment) and 65.8% (9 months affer initiation of treatment) of patients answered excellent or good. Conclusion: This study confirms that toenail onychomycosis has significant impact on the overall QOL of patients. Also the effect of antifungal therapy on patients QOL were watisfactory. Therefore, both doctor and patient should pay more attention to the treatment of onychomycosis. [Kor J Med Mycol 3(1): 115∼124]

      • A novel POK family transcription factor, ZBTB5, represses transcription of p21CIP1 gene.

        <P>Transcriptional repression through chromatin remodeling and histone deacetylation has been postulated as a driving force for tumorigenesis. We isolated and characterized a novel POZ domain Krüppel-like zinc finger transcription repressor, ZBTB5 (zinc finger and BTB domain-containing 5). Serial analysis of gene expression (SAGE) analysis showed that ZBTB5 expression is higher in retinoblastoma and muscle cancer tissues. Immunocytochemistry showed that ZBTB5 was localized to the nucleus, particularly nuclear speckles. ZBTB5 directly repressed transcription of cell cycle arrest gene p21 by binding to the proximal GC-box 5/6 elements and the two distal p53-responsive elements (bp -2323 approximately -2299; bp -1416 approximately -1392). Chromatin immunoprecipitation assays showed that ZBTB5 and p53 competed with each other in occupying the p53 binding elements. ZBTB5 interacted with co-repressor-histone deacetylase complexes such as BCoR (BCL-6-interacting corepressor), NCoR (nuclear receptor corepressor), and SMRT (silencing mediator for retinoid and thyroid receptors) via its POZ domain. These interactions resulted in deacetylation of histones Ac-H3 and Ac-H4 at the proximal promoter, which is important in the transcriptional repression of p21. MTT (3-(4,5-di meth yl thi azol-2-yl)-2,5-diphenyltetrazolium bromide) assays and fluorescent-activated cell sorter analysis revealed that ZBTB5 stimulated both cell proliferation and cell cycle progression, significantly increasing the number of cells in S-phase. Overall, our data suggest that ZBTB5 is a potent transcription repressor of cell cycle arrest gene p21 and a potential proto-oncogene stimulating cell proliferation.</P>

      • KAISO, a critical regulator of p53-mediated transcription of <i>CDKN1A</i> and apoptotic genes

        <P><B>Significance</B></P><P>Transcription factor KAISO (POZ/BTB family protein, ZBTB33) expression is induced by genotoxic stress in a tumor suppressor p53-dependent manner. KAISO then interacts with p53 and the acetyltransferase p300 to modulate p300 acetylation of p53 and imposing upon p53 a “code,” i.e., acetylation at K320 and K382, and inhibition of acetylation at K381. This coded p53 shows increased DNA binding to p53 response elements in the promoters of <I>CDKN1A</I> (cyclin-dependent kinase inhibitor 1) and apoptosis genes, subsequently inducing cell cycle arrest and potent apoptosis. KAISO is a critical regulator of DNA damage responses in multiple cell types and carries out this function by regulating p53-mediated cell cycle arrest and apoptosis.</P><P>An unresolved issue in genotoxic stress response is identification of induced regulatory proteins and how these activate tumor suppressor p53 to determine appropriate cell responses. Transcription factor KAISO was previously described to repress transcription following binding to methylated DNA. In this study, we show that <I>KAISO</I> is induced by DNA damage in p53-expressing cells and then interacts with the p53–p300 complex to increase acetylation of p53 K320 and K382 residues, although decreasing K381 acetylation. Moreover, the p53 with this particular acetylation pattern shows increased DNA binding and potently induces cell cycle arrest and apoptosis by activating transcription of <I>CDKN1A</I> (cyclin-dependent kinase inhibitor 1) and various apoptotic genes. Analogously, in <I>Kaiso</I> KO mouse embryonic fibroblast cells, p53-to-promoter binding and up-regulation of <I>p21</I> and apoptosis gene expression is significantly compromised. KAISO may therefore be a critical regulator of p53-mediated cell cycle arrest and apoptosis in response to various genotoxic stresses in mammalian cells.</P>

        Clinical impacts of hazardous alcohol use and obesity on the outcome of entecavir therapy in treatment-naïve patients with chronic hepatitis B infection

        Background/Aims: The aim of this study was to analyze the clinical impacts of obesity and hazardous alcohol use on the outcome of entecavir (ETV) therapy in chronic hepatitis B (CHB) patients. Methods: The medical records of 88 treatment-naïve patients who were diagnosed with CHB and received ETV between March 2007 and September 2009 were analyzed retrospectively. Body mass index (BMI) values and Alcohol Use Disorders Identifi cation Test (AUDIT) scores were obtained at 6 months after the initiation of ETV (0.5 mg daily) treatment. Results: A BMI of 25 kg/m2 or more was recognized as an indicator of obesity, and a total AUDIT score of 8 or more was recognized as an indicator of hazardous alcohol use. Of the cohort, 24 patients (27.3%) were obese and 17 (19.3%) were hazardous alcohol users. The rate of seroconversion, alanine aminotransferase (ALT) normalization, and hepatitis B virus (HBV)-DNA negativity (<300 copies/mL) at 3, 6, and 12 months of treatment did not differ significantly between the normal-BMI and high-BMI groups. Moreover, the rate of seroconversion and HBV-DNA negativity at 3, 6, and 12 months of treatment did not diff er signifi cantly between the nonhazardous and hazardous alcohol users. However, the frequency of ALT normalization at 12 months was signifi cantly lower among hazardous alcohol users (91.5% vs. 70.6%; P =0.033). Conclusions: Obesity and hazardous alcohol drinking have no significant impact on the outcome of ETV treatment. However, the ALT normalization rate at 12 months after initiation of ETV treatment was signifi cantly lower among the hazardous alcohol users.         Characteristics and Management of Cyclosporine-associated Adverse Gastrointestinal Events

        Background: Cyclosporine (CS) is widely used to treat various skin diseases. Gastrointestinal (GI) discomfort is the most common adverse effect of orally administered CS for dermatologic indications. However, few studies on CSassociated adverse GI events have been conducted. Objective: In this study, we aimed to describe the major features of adverse GI events associated with CS therapy using a validated symptom questionnaire, and to investigate the factors contributing to their development. We also evaluated the effectiveness of three empirical GI medications in relieving adverse GI events. Methods: This study consisted of 2 phases. Phase I was a prospective observational cohort study to investigate the characteristics of CS-associated adverse GI events in 942 consecutive patients treated with CS. Phase II was a randomized controlled trial to evaluate the efficacy of three different classes of GI medications. Results: CS-associated adverse GI events occurred in 119 patients (12.6%). GI complications were more common in female patients (p=0.04), patients with a history of GI disorders (p=0.02), and patients whose initial CS doses were greater than 3 mg/kg/day (p=0.05). In patients treated with any one of the three GI medications, the mean Gastrointestinal Symptom Rating Scale scores significantly decreased (p<0.001). Conclusion: This study demonstrated that adverse GI events are common during early CS treatment, especially in women, patients receiving high doses of CS, and those with a history of GI disorders. Our results suggest that new-onset CS-associated GI side effects can be effectively managed with the addition of GI medications. (Korean J Dermatol 2019;57(10):608∼616)
