Cutaneous and systemic plasmacytosis (CSP) is a rare disorder of unknown etiology characterized by cutaneous polyclonal plasma cell infiltrates associated with various extracutaneous involvement and polyclonal hypergammaglobulinemia. Here, we report on a 54-year-old male patient with chronic renal insufficiency who presented with disseminated reddish-brown macules and plaques on the face and trunk. In our evaluation, he was found to have lymphadenopathy, polyclonal hypergammaglobulinemia; benign plasma cell infiltration involving the skin, bone marrow, and retroperitoneal area; and renal amyloidosis. To the best of our knowledge, this is the first reported case of CSP associated with renal amyloidosis. (Ann Dermatol 27(6) 759∼762, 2015)

<P>When placed in an outdoor environment, organic electronic devices (OEDs) can degrade on exposure to moisture, UV light, and heat, owing to the chemical sensitivity and decomposition of the organic materials. Therefore, to protect OEDs from outdoor environments, thin-film passivation, which can block harmful elements from reaching organic materials, is required. To meet the demands and trends in encapsulation technologies, in this study, we developed a low-temperature, simple, and effective gas diffusion multibarrier (GDM), which is UV and heat reflective as well as corrosion resistant. The designed UV- and heat-reflective GDM (UHGDM) has a multistacked structure in the form of a UV filter/Ag/gas diffusion barrier (GDB)/polymer based on a dielectric/metal/dielectric (DMD) configuration. First, the DMD structure was used as a heat mirror for infrared reflectance. Second, the bottom dielectric layer of the DMD structure was used as the UV filter, and it consisted of a ZnS/LiF multistacked structure with large differences in refractive indexes. Third, a nanolaminate-based GDB barrier with multi-interfacial and defect-decoupling systems, which achieved a water vapor transmission rate of 1.58 × 10<SUP>-5</SUP> g/m<SUP>2</SUP>/day at a thickness of 60 nm, was used as the top dielectric layer of the DMD structure. Finally, an inorganic/organic hybrid polymer layer was coated on the DMD structure to provide corrosion-resistance and waterproofing properties. The fabricated UHGDM showed high transparency in the visible region and excellent reflectance in the UV and IR regions, resulting in excellent UV and heat rejection capability in practical UV and heat reflection tests. In addition to optical functionalities, the UHGDM maintained its functionality against harsh environmental conditions because of the GDB/polymer structure. Finally, the feasibility of the UHGDM was demonstrated using organic solar cells through water immersion and shelf lifetime tests.</P> [FIG OMISSION]</BR>

Background: Epinephrine can relieve severe symptoms of urticaria. There has been no previous study on the continuous infusion of epinephrine in uncontrolled acute urticaria. Objectives: This study was aimed to assess the efficacy and safety of continuous infusion of low-dose epinephrine for severe acute urticaria. Methods: From August 2010 to July 2014, 34 severe acute urticaria patients who were uncontrolled symptoms by conventional treatment, were treated with continuous infusionof low-dose epinephrine. They received 1mg epinephrine HCl mixed with 1L of normal saline for 40cc/hour. Blood pressure were checked on 0, 30, 60 and 90 minutes after onset of epinephrine. Results: Low-dose epinephrine significantly improved the clinical symptom. The mean duration of clinical symptom after epinephrine was 3.64 days. Adverse effects were observed in 2(5.8%) patients, and were palpitation & chest discomfort. The average variation of blood pressure after continuous infusion of epinephrine was -3.23mmHg. There were no significant side effects to suggest stopping epinephrine injection. Relapse of urticaria after injection of epinephrine was occurred at 1(2.9%) patient. Of the 34 patients who were followed for at least 1 month after completion of epinephrine injection, 31(91.1%) patients weresymptom free, and 3(8.8%) patients used antihistamine. Conclusion: This study shows that continuous injection of low-dose epinephrine is an efficacious and safe treatment option for treating acute urticaria.
Primary Cutaneous Nocardiosis Caused by Nocardia takedensis

Nocardia species are aerobic, gram-positive, filamentous, partially acid-fast actinomycetes which are found worldwide in soil and decaying organic plant matter. When they infect human beings, they generally enter through the respiratory tract and then disseminate systemically. Rarely has a primary infection occurred as the result of direct inoculation. Isolation of Nocardia from clinical specimens and identification of species are difficult. But, with the introduction of new genetic technologies, reports of novel species of Nocardia have increased. We describe a case of cutaneous nocardiosis caused by Nocardia takedensis in an 87-year-old woman who was diagnosed by bacterial culture and 16S ri-bosomal RNA sequencing. N. takedensis has been described as a new species. This report describes the first clinical isolate of N. takedensis from a skin specimen in Korea. (Ann Dermatol 29(4) 471∼475, 2017)
P128 : Treatment of recalcitrant warts with quadrivalent HPV vaccine

Background: Warts are benign epidermal proliferations cause by HPV. Although two-thirds of warts in children show spontaneous regression in two years, recalcitrant warts can be very difficult to treat with traditional modalities. Objectives: This is a study to investigate the effect of quadrivalent HPV vaccine injection for patients with recalcitrant wart. Methods: From August 2012 to March 2013, thirteen patients were enrolled in the study after informed consent. HPV vaccine was administered in 3 doses at 0, 2, and 6 months. Each visit, the grade of improvement and satisfaction in treatment were measured by patients and investigator. Results: Overall, the response rate of recalcitrant warts was 84.6%. Six patients (46.2%) had complete remission, five patients(38.5%) had partial remission and 2 patients(15.4%) had none remission. There are no statistically significant differences in sex(p=0.612), age(p=0.332), number of lesion(p=0.575), and history of previous therapy(p=0.222) and combination therapy(p=0.777) among the complete, partial and none remission groups. Adverse effects were observed in 1 patients, and were syncope. Conclusion: Quadrivalent HPV vaccine treatment is an effective alternative treatment for recalcitrant wart.

Three phase squirrel cage induction motor is generally adopted as a traction motor to drive high speed trains because of its robustness for surrounding environment and easy maintenance. In the design of traction motor, reduction of weight is very important in order to reduce kinetic energy to accelerate the vehicle. Therefore, in this paper, design variables of a preliminary designed traction motor to minimize its weight is determined using the optimization technique. Before the optimization process, rotor slot number is determined to reduce vibration and noise by the analysis of magnetic force. As a result, a design example to reduce weight by 12% than that of the preliminary designed motor is presented.
