Giant keratoacanthoma is an uncommon variant of keratoacanthoma, which may increase to a diameter of several centimeters. Although keratoacanthomas usually resolve spontaneously, giant keratoacanthoma can be invasive and destructive. A 49-year-old man presented with a 5-year history of multiple large hyperkeratotic and crusted plaques and nodules on sun-exposed areas such as the face, ear, hand, and forearm. Some lesions involuted spontaneously, whereas others became rather enlarged. The biopsy specimen revealed horn-filled crater formation, epidermal extending resembling a buttress, and an eosinophilic glassy appearance in the keratinocytic cytoplasm. We treated the lesions with acitretin, and they almost completely resolved after 13 weeks. Here we describe a case of multiple giant keratoacanthoma treated with acitretin. A numerical study have been performed onthe effects of spacers on the heat transfer across a horizontal annulus under basic conditions; Ra=10⁴,Pr=0.7 D??/D??=2.6, K??K=48, ??/D??=0.06 and Nu??=122.5. The mode of heat transfer in the annulus is changed from conduction to convection at Ra??10³. The maximum dimensionless temperature districution along (r-r??)/L at Θ=50?? are founded for vertical spacers but at Θ=120??, for horizontal spacers, and the minimum dimensionless temperature distribution along (r-r??)/L at Θ=50?? are founded for horizontal spacers but Θ=120??, for no spacers. The maximum dimensionless angular velocity at Θ=50??, at (r-r??)/L??0.2 near the inner tube and at(r-r??)/L??0.8 near the outer cylinder but at Θ=120??, it shows at (r-r??)L??0.15 near the inner tube and at (r-r??)L??0.7 for no spacers and vertical spacers and at (r-r??)/L??0.8 for horizontal spacers near the outer cylinder. Finally, the heat transfer by natural convection in the plain annulus is maximum but in the annulus with horizontal spacers in munimum.
실내사무환경의 환경성담배연기(ETS)의 지표물질에 관한 연구

The purpose of this research is to evaluate indoor office environment using the concentrations of nicotine, 3-enthenyl-pyridine(3-EP), and respirable suspended particulate(RSP), which are indicators for environmental tobacco smoke(ETS) and the correlations between indicators and environmental conditions(smoking density, smoking index). The mean air charge per hour (ACH) in smoking rooms was 10.4 and most of the smoking rooms showed non-compliance withe ASHRAE standard value except only one smoking rooms. The concentrations of RSP, 3-EP, nicotine showed log=normal distributions, and became different statistically depending on smoking condition(p〈0.01). The geometric mean concentration of RSP in smoking room was 441.7 ug/㎡ that is far exceeded environmental standard(150 ug/㎡). This implies that fine particulate in smoking room should be carefully controlled considering smoking density and ventilation fate. The mean concentrations of nicotine and 3-EP were 93.4 ug/㎥, respectively. The correlation coefficients between RSP and SI, 3-EP and SI, and Nicotine and SI were 0.67, 0.84 and 0.74, respectively. The correlation coefficient between nicotine and 3-EP, nicotine and RSP, and RSP and 3-EP were 0.76,0.78 and 0.57 respectively.

Recently, the quantities of waste asphalt concrete at construction sites have much increased greatly, but maintaining a filling-up and final disposal place is a difficult problem. Therefore, we are faced with a worsening environmental problem brought about present illegal measures. One of the most effective recycling method is cold recycling. This method will satisfies treatment and recycling of construction waste is a very important question in the preservation of environmental and natural resources. So, in this paper, we dealt cold recycling modified mixtures using emulsified asphalt to concern the effect of adding reclaimed asphalt concrete 30, 40, 50%, and using SBR Latex for modified mixtures.
1. 06㎛ Nd : YAG Laser를 이용한 요추간판탈출증의 경피적 수핵감압술

The 1.06㎛ Neodymium-YAG Laser(800 micron hemisherical) is revealed a useful tool for percutaneous laser disc decompression(PLDD). Especially PLDD would be applied to the management of lumbar disc herniation with minimal intervention. Total irradiated energy to one level of disc space was bout 1000 to 2000 joules. We performed this method at 10 levels, in 7 cases of patients, consisting of 4 males and 3 females with an average age of 37.7 years. Five cases showed excellent and good results, were observed. In two cases, open surgery was required because of unsatisfactory results. The best results were obtained in the protrusion type of disc herniation. We have summarized some advantages of PLDD: a) safe and simple procedure in an outpatient setting, b) avoiding general anesthesia, late spinal instability, c) possibility of repetitive use.
Seborrheic keratosis like lichen planus-like keratosis

Lichen planus-like keratosis consists of non-pruritic solitary papule or slightly indurated plaque with smooth or verrucous surface. It has brown to erythematous color and occurs predominantly on sun-exposed trunk and upper extremities of adult between the 5th and the 7th decades. A 51-year-old woman presented with about 1 cm x 1 cm sized solitary slightly indurated and pigmented plaque on left forearm with onset of early adulthood. At first visit, our impression was seborrheic keratosis, because it showed sharply demarked brownish plaque with verrucus surface. For definitive diagnosis, skin punch biopsy was done and the epidermis showed focal parakeratosis, hyper granulosis and a few necrotic keratinocytes. Lichenoid infiltrate of lymphocytes obscured the dermoepidermal junction. Small number of plasma cells and solar elastosis were found in the upper dermis. Clinically, lichen planus-like keratosis is confused with seborrheic keratosis. For differential diagnosis, histologic study is necessary. Characteristic histologic features of lichen planus-like keratosis are focal parakeratosis, necrotic keratinocyte and lichenoid infiltration. Although clinical feature of her skin lesion was confused with seborrheic keratosis, histologic findings were consistent with lichen planus-like keratosis. We report a case of seborrheic keratosis like lichen planus-like which is clinically apt to be misdiagnosed with seborrheic keratosis.

Natural convection from a horizontal annulus with spacers has been studied on the effects of Rayleigh numbers and position of spacers by experimental method. In case of vertical spacers, the maximum local Nusselt number appears at θ=150° in a conducting tube and θ=30° in a cylinder. The local spacer Nusselt numbers show positive values on the lower spacer, but negative values on the upper spacer. In case of horizontal spacers, the flow over the spacer is more active than that of under the spacer as the Rayleigh number increase. The maximum local Nusselt number appears at θ=180° In a tube and at θ=0° in a cylinder. The local spacer Nusselt number show positive values on the upward surface of spacer, but negative values on the downward surface of spacer. The mean tube Nusselt numbers and mean cylinder Nusselt numbers with vertical spacer are increased 11% And 2.7%, respectively by those of horizontal spacer.
Optimization and Evaluation of HA filler reinjection after hyaluronidase injection in a rat model

Background: In recent years, hyaluronic acid (HA)-based fillers have become the popular material for use in soft tissue and dermal correction. HA fillers are generally accepted today to have replaced collagen fillers as the material of choice and its uses have subsequently grown extensively. Objectives: We investigated the duration of effect of hyaluronidase in vivo to provide the optimal time interval between hyaluronidase injections and HA filler reinjections. Methods: This study is comprised of 6 male Sprague-Dawley rats and used single monophasic HA filler and single hyaluronidase. Sequential reinjection of HA filler with different time interval. After 1 week, 600 IU of hyaluronidase was injected subdermally. All injection site were completely excised and taken biopsy sample simultaneously. Biopsy specimens were stained with hematoxylin-eosin stain and alcian blue stain to highlight the HA within the sections. All specimens were reviewed by a board-certified pathologist. Results: In 6 h intervention group, reinjected HA filler showed almost its original volume and was no significant difference from positive control group in macroscopically and microscopically. Conclusion: Our data suggest that the hyaluronidase lose its effect in dermis and subcutaneous tissue within 3 to 6 hours after injection and proper engraftment of reinjected HA filler can be accomplished after 6 hours.

<P>We have recently seen many successful applications of recurrent neural networks (RNNs) on electronic medical records (EMRs), which contain histories of patients' diagnoses, medications, and other various events, in order to predict the current and future states of patients. Despite the strong performance of RNNs, it is often challenging for users to understand why the model makes a particular prediction. Such black-box nature of RNNs can impede its wide adoption in clinical practice. Furthermore, we have no established methods to interactively leverage users' domain expertise and prior knowledge as inputs for steering the model. Therefore, our design study aims to provide a visual analytics solution to increase interpretability and interactivity of RNNs via a joint effort of medical experts, artificial intelligence scientists, and visual analytics researchers. Following the iterative design process between the experts, we design, implement, and evaluate a visual analytics tool called RetainVis, which couples a newly improved, interpretable, and interactive RNN-based model called RetainEX and visualizations for users' exploration of EMR data in the context of prediction tasks. Our study shows the effective use of RetainVis for gaining insights into how individual medical codes contribute to making risk predictions, using EMRs of patients with heart failure and cataract symptoms. Our study also demonstrates how we made substantial changes to the state-of-the-art RNN model called RETAIN in order to make use of temporal information and increase interactivity. This study will provide a useful guideline for researchers that aim to design an interpretable and interactive visual analytics tool for RNNs.</P>
