P058 : Diagnostic accuracy of dermoscopy of non- melanocytic skin tumors

Background: There is no systematic analysis of the diagnostic accuracy of a dermoscopy in various non-melanocytic skin tumors. Objectives: To investigate the diagnostic accuracy of dermoscopy for various non-melanocytic skin tumors. Methods: Total 865 patients with 18 different types of non-melanocytic skin tumors, which can be differentiated by a distinct dermoscopic pattern, was included in this study. All patients were evaluated by naked eye and dermoscope, then received a pathologic diagnosis. The diagnostic accuracy of the clinical diagnosis or the dermoscopic diagnosis agreed to the pathologic diagnosis were analyzed. Results: The overall accuracy of the dermoscopic diagnosis compared to the clinical diagnosis was significantly increased from 53.5% to 75.1%. Morever, the pathologic agreement with the dermoscopic diagnosis in actinic keratosis(AK), basal cell carcinoma(BCC), squamous cell carcinoma(SCC), seborrheic keratosis(SK), nevus sebaceus(NS), dermatofibroma(DF) and granuloma pyogenicum(GP) was significantly higer than the clinical diagnosis with naked eye. The sensitivity for the diagnosis of AK, BCC, SCC, SK, NS, DF, GP and pilomatricoma and the specificity of all disease groups other than dermatofibrosarcoma significantly increased after dermoscopy (p<0.05). Conclusion: Dermoscopy can improves the accuracy of diagnosis and thus facilitate appropriate management decision for non-melanocytic skin tumors.

Extramammary paget disease (EMPD) is a heterogenous condition representing either an in situ malignancy with apocrine differentiation or pagetoid spread of an underlying adnexal tumor or a non-cutaneous internal neoplasm. Concurrent involvement of the same area by EMPD and benign adnexal neoplasm is extremely rare. We describe a case of EMPD of the vulva with a benign underlying syringoma. A 69-year-old woman presented with a pruritic erythematous lesion on the left side of the vulva, which had been present for 3 years. Histopathological examination of biopsy specimen from the lesion revealed the presence of large pale cells with prominent nuclei, which were randomly dispersed throughout the epidermis. The dermis showed dilated cystic spaces lined by two layers of cuboidal cells and epithelial strands of similar cells with no evidence of atypia, and with no demonstrable continuity with epidermis. Some of the cysts had small comma-like tails, which gave them the appearance of tadpoles. A diagnosis of combined vulvar Paget’s disease and syringoma was rendered. This case is the first reported example of EMPD with a benign underlying syringoma, which is highly suspected to be coincidental association.
Characteristics of Legionella Species Isolated from the Public Facilities in Busan, 2013-2022

Analysis of the distribution of Legionella species in public water systems andinvestigation of the degree of contamination are the best ways to establish apreemptive management system that can respond quickly in the event of anoutbreak. In this study, distribution of Legionella species isolated from publicwater systems in Busan facilities from 2013 to 2022 was analyzed. Culture andPCR methods were performed for 15,161 environmental water samples collectedfrom public water systems including buildings, hotels, shopping centers, hospitals,public baths, and welfare facilities. Of these samples, a total of 2,156 strains(14.2%) of Legionella species were detected. A total of 1,512 (70.1%) isolates ofL. pneumophila were obtained and non-L. pneumophila species were identifiedin 468 strains (21.7%) with 176 (8.2%) strains showing co-detection. Legionellaspecies were detected in different facilities, including public baths (19.4%),hotels (19.3%), ships (18.6%), buildings (14.2%), hospitals (12.1%), welfarefacilities (9.8%), others (9.3%), and shopping centers (9.0%). Detection resultsfor water sources included hot water (19.9%), cooling tower water (15.3%),tank water (11.3%), cool water (8.2%), and others (7.6%). Legionella specieswere detected mainly in summer, although statistically significant differences indetection among different seasons were not found. High colonization was morefrequently detected in water sources from cooling tower water in buildings andhot water in public baths. These results might be useful for preventing andcontrolling future outbreaks of legionellosis.
