IgG4-related systemic diseases are characterized by a diffuse or mass forming inflammatory reaction rich in lymphocytes and IgG4-positive plasma cells (lymphoplasmacytic infiltration), fibrosclerosis of variable organs and obliterative phlebitis. They usually involve various organs including the pancreas, bile duct, gallbladder, salivary gland, retroperitoneum, kidney, lung, and prostate. However, most of them are accompanied by autoimmune pancreatitis, and good response to steroid treatment is one of the hallmarks of this disease. We report a case of an 67-year-old man with IgG4 associated sclerosing cholangitis, who was diagnosed by endoscopic retrograde cholangiopancreatography and successfully treated with steroid therapy. The Effects of Mutant p21 Protein in the Cultured Colon Cancer Cell Lines

The products of the ras oncogenes (H-rns, K-ras and N-ras) are proteins of 188 or 189 amino acids and approximate 21,000 molecular weight, termed p21?? or, more simply p21 proteins. But the exact roles of c-ras proteins in cell proliferation and differentiation as well as in neoplastic transformation are little understood. Both viral and cellular p21 products bind guanine nucleotides and reside at the inner side of the plasma membrane and attachment of p21 proteins to the cell membrane takes place through lipid and this membrane bound protein is thought to be involved in signal transduction in cellular proliferation and differentiation pathways. Three biochemical activities speeific for guanine nucleotides have been identified in ras proteins: GTP or GDP binding, GTPase activity and autophosphorylation. Single point mutation at codons 12, 13 and 61 residues can convert the members of ras family to its active oncogenes. Many oncogenic mutants of p21 are reduced in GTPase activity led to the proposal that celli proliferation and differentintion by p21 is the result of abnormal levels of p21 in an active, GTP-bound state with point mutation. Either a structural or a quantitative change increase in the cellular ras p21 expressed in a cell may induce phenotypic transformation. The purpose of this study is to investigate the function of the p21 protein to the human colon cancer cell lines according to the exposure time and dosage of p21, the authors divided tumor cell lines into 5 groups as follows; group Ⅰ(control, colon cancer cell lines without administration of p21 or polyclonal antibody), group Ⅱ (administration of p21 with labelling of "H-thymidine and leucine), group Ⅲ (simultaneous administration of p21 protein and polyclonal antibody antibody with labelling of ??H-thymidine and ??H-leucine). After deciding the most effective dose of p21 protein and culture time with target cells in preliminary studies, the morphologic changes of target cells with administration of p21 protein and the p21 expression and interaction with anti-p2l polyclonal antibody were examined by phase contrast microscopy each other. 1. The most effective dose of the p21 wi1h the colon cancer cell which resulted in increase uptake of ??H-thymidine and ??H-leucine were 50ng but there were some differences in culture time of the ??H-leucine; 96 hours in SBA-1, 72 hours in HT-29 and 120 hours in SW-1116. 2. The increase uptakes of the ??H-thymidine by the different dosage of p21, 50ng vs 200ng were 131%(50ng), 160%(200ng) in SBA-1, 203%(50ng), 123%(200ng) in HT-29, and 127%(50ng), 189% (200ng), in SW-1116; and increase uptakes of ??H-leucine were 130%(50ng), 159%(200ng) in SW-1116. 3. There were some cellular proliferation and morphological changes of the colon cancer cells such as ruffling of the cell membrane, rounding of the cell after administration of the mutant p21, but such changes were not observed after simultaneous administration of the mutant p21 and anti-p21 polyclonal antibody.

저자들은 계명대학교 의과대학 피부과학 교실에서 약3년간 파장 10,600nm의 CO₂레이저를 이용하여 여러 피부질환을 치료하여 얻은 결과를 바탕으로 여러 피부질환을 치료하여 얻은 결과를 바탕으로 피부과 영역에서 CO₂레이저 사용의 실제적 적응과 발생될 수 있는 부작용을 살펴본 바 20가지의 피부질환에 대하여 CO₂레이저치료가 시행되고 있으며 질환별로는 색소성 모반 118명, 사마귀(심상성, 편평, 수장, 족저 사마귀 및 첨규 콘딜롬 포함) 80명, 흑자 65명, 여드름반흔 42명, 한관종 32명, 지루각화증 28명 등의 순이었다. 이중 대부분의 질환에서 치료효과가 양호이상의 판정을 받았으며 치료시 레이저광의 출력이나 pulse 그리고 치료횟수는 질환에 따라 그리고 병변의 깊이와 넓이에 따라 큰 차이를 보였다. 부작용으로는 과색소침착, 저색소증, 반흔형성, 위축, 감염등이 있었으나 대부분의 경우 국소치료와 항생제등의 경구투여로 조절할 수 있었고 레이저 시술후의 스테로이드경구투여와 항생제 연고 도포가 부작용의 방지에 도움이 되었다. 그러므로 레이저시술에 앞서 시술에 이용되는 CO₂레이저의 특성을 잘 이해하고 병변의 종류와 범위 그리고 환자의 피부상태를 면밀히 파악하여 적절한 pulse와 출력으로 치료하고 추적관찰하여 부작용이 발생할 경우 조기에 치료하는 것이 중요하다고 생각된다. CO₂laser emits energy in the mid portion of the infrared spectrum at a wavelength of 10,600nm. The CO₂laser is the workhorse laser in the treatment of cutaneous disease With water as the chromophore (light-absorbing compound), CO₂laser light is highly absorbed in biologic tissue in which water comprises between 75-90%. The CO₂laser is superficially absorbed by tissue water, with little scatter, limiting penetration into the skin to less than 0.1 mm. The beam can be delivered with a large spot size by using silk touch(TM)-computerized scanner and with little thermal damage to the tissues by using superpulsed mode recently. The purpose of this study is to evaluate the variety of cutaneous diseases treated by CO₂laser and the therapeutic effects of the CO₂laser on various skin diseases. MATERIALS AND METHODS : 459 cases of various skin diseases treated by CO₂laser between 1996 and 1998 were subjected to detailed analysis. The CO₂laser is candela laser system (U.S.A) Model 20C. The laser produces power output that ranges from 0 to 30 W of continous wave and from 0.5 to 7.0 W of superpulsed mode. The effectiveness of treatment was measured into 4 groups-excellent, good, fair, poor by estimating the clinical improvement with initial image of photograph. RESULTS : 1. In 459 cases of various skin diseases treated by CO₂laser the case of pigmented nevus (118), wart (80), lentigo (65) and acne scar (42) comprise the large portion of patient compositon. 2. The excellent and good response above 50% of patient was observed in most of the cutaneous diseases. Especially in pigmented nevus, wart, acne scar and xanthelasima palpebrum the excellent and' good response was observed above 90% of patient. CONCLUSION : The CO₂laser is actually used in treatment for wide ranges of cutaneous disorders such as pigmented nevus, wart, acne scar and lentigo even including basal cell carcinoma and hailey-hailey disease. The CO₂laser with silk touch(TM) and superpulse mode is a very effective modality for the various skin disorders such as pigmented nevus, wart, acne scar and lentigo.

<P><B>Abstract</B></P> <P>Recently, development of a human skin equivalent has been under active study. Construction of a human skin equivalent requires co-culturing multiple cell types in a 3D scaffold. The 3D skin equivalent offers advantages over 2D cell culture models, improving the physiological relevance of the model. Combination of microfluidics with 3D skin models have been studied, providing a novel <I>in vitro</I> platform. Here, we examined 3D skin equivalent in a microfluidic chip. Immunohistochemistry and H&E staining revealed that the skin cells proliferated and differentiated in a microfluidic environment. Our study demonstrates successful on-chip culture and differentiation of 3D skin equivalent.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>
