<P>Argonaute (Ago) is the catalytic core of small RNA-based gene regulation. Despite plenty of mechanistic studies on Ago, the dynamical aspects and the mechanistic determinants of target mRNA binding and dissociation of Ago–guide strand remain unclear. Here, by using single-molecule fluorescence resonance energy transfer (FRET) assays and <I>Thermus thermophilus</I> Ago (<I>Tt</I>Ago), we reveal that the 3′-end of the guide strand dynamically anchors at and releases from the PAZ domain of Ago, and that the 3′-end anchoring of the guide strand greatly accelerates the target dissociation by destabilizing the guide–target duplex. <P>Shear wave elastography (SWE) is an emerging technique which can obtain quantitative elasticity values in breast disease. We therefore evaluated the diagnostic performance of SWE for the differentiation of breast masses compared with conventional ultrasound (US). Conventional US and SWE were performed by three experienced radiologists for 158 consecutive women who had been scheduled for US-guided core biopsy or surgical excision in 182 breast masses (89 malignancies and 93 benign; mean size, 1.76??cm). For each lesion, quantitative elasticity was measured in terms of the Young's modulus (in kilopascals, kPa) with SWE, and BI-RADS final categories were assessed with conventional US. The mean elasticity values were significantly higher in malignant masses (153.3??kPa??±??58.1) than in benign masses (46.1??kPa??±??42.9), (P??<??0.0001). The average mean elasticity values of invasive ductal (157.5??±??57.07) or invasive lobular (169.5??±??61.06) carcinomas were higher than those of ductal carcinoma in situ (117.8??kPa??±??54.72). The average mean value was 49.58??±??43.51 for fibroadenoma, 35.3??±??31.2 for fibrocystic changes, 69.5??±??63.2 for intraductal papilloma, and 149.5??±??132.4 for adenosis or stromal fibrosis. The optimal cut-off value, yielding the maximal sum of sensitivity and specificity, was 80.17??kPa, and the sensitivity and specificity of SWE were 88.8% (79 of 89) and 84.9% (79 of 93). The area under the ROC curve (Az value) was 0.898 for conventional US, 0.932 for SWE, and 0.982 for combined data. In conclusion, there were significant differences in the elasticity values of benign and malignant masses as well as invasive and intraductal cancers with SWE. Our results suggest that SWE has the potential to aid in the differentiation of benign and malignant breast lesions.</P>

Recent animal studies have shown that aggressive volume replacement toward more than normotension causes a marked increase in blood loss from the site of vascular injury and pro-duces significant mortality in animal model of near-fatal(40-50ml/kg bleeding) uncontrolled hemorrhagic shock(UCHS). The purpose of this study was to establish near-fatal UCHS model in rat and to find the optimal level of preperative fluid resuscitation in near-fatal UCHS. We could establish UCHS model in rat by transsecting the terminal branch of ileocolic artery and vein. The mean survival time was 54 munutes with no fluid resuscitation. We used the UCHS model to compare the effects of resuscitation to mean arterial pressure of 60 to 80, 100mmHg, Fourty-one mature rats(350 to 500gm) were instrumented and subjected to intraperitoneal bleeding. Ten minutes after transsecting ileocolic vessel, normal saline infusion was begun at 10ml/100g/hr except control group(no fluid resuscitation, n=12) and continued as needed to maintain the fol lowing endpoints : group I(MAP=60mm Hg, n=11), group I(MAP-60mm Hg, m=11), group II(MAP=80 mmHg, n=12), and group III(MAP=100 mmHg, n=6). When mean arterial pressure was changed to ±5mmHg from each desired mean arterial pressure, normal saline infusion was ceased or maintained. All animals were observed for 120 minutes or until death. No animal survived until 120 minutes. Mean survival time was 48.9minutes, 95.8 minutes, 75.8 minutes, and 75.8 minutes for control group, group I, II and, III, respectively. Intraperitoneal hemorrhage volume for four groups was 2.5ml/100g,6.2ml/100g,6.7ml/100g, and9.8ml/100. In setting of UCHS we conclude that maintenance of the hypotensive state(MAP=60mmHg) with normal saline infusion causes less blood loss and may be preferable before definitive surgical repair of bleeding site.

Intramural esophageal dissection is a rare but clinically important condition in the field of gastroenterology. Classically, intramural esophageal dissection rarely occurs in patients who are anticoagulated or have poor medical condition, and its clinical presentation may include chest pain, dysphagia and hematemesis. Herein, we present a case of intramural esophageal dissection in an alcoholic hepatitis patient that was diagnosed by endoscopy and successfully treated with conservative management.(Korean J Gastroenterol 2016;67:35-38)

Vesicovaginal fistula may be a complication of gynecologic and obstetric procedures, of which abdominal hysterectomy remains as the most common cause. Although there are a number of transvaginal or transabdominal surgical techniques for the correction of this condition, we performed a laparoscopic repair to reduce the morbidity of the transabdominal approach. A 44-year-old woman presented with urinary incontinence after undergoing Cesarean hysterectomy for uterine atony eight years ago. After confirming her vesicovaginal fistula, laparoscopic repair was performed. The bladder and vaginal walls were mobilized by meticulous dissection and repaired by intracorporeal stitches in 2 layers followed by omental interposition. Total operative time was 3 and half hours and estimated blood loss was about 400 ml. Foley catheter was removed at 23rd day after operation. Patient was discharged at 26th day after confirming the complete correction of the fistula by cystogram. Laparoscopic repair of a vesicovaginal fistula appears to be a safe and effective procedure. We report this case with a brief review of literature.

<P>The role of cereblon (CRBN) in T cells is not well understood. We generated mice with a deletion in Crbn and found cereblon to be an important antagonist of T-cell activation. In mice lacking CRBN, CD4(+) T cells show increased activation and IL-2 production on T-cell receptor stimulation, ultimately resulting in increased potassium flux and calcium-mediated signaling. CRBN restricts T-cell activation via epigenetic modification of Kcna3, which encodes the Kv1.3 potassium channel required for robust calcium influx in T cells. CRBN binds directly to conserved DNA elements adjacent to Kcna3 via a previously uncharacterized DNA-binding motif. Consequently, in the absence of CRBN, the expression of Kv1.3 is derepressed, resulting in increased Kv1.3 expression, potassium flux, and CD4(+) T-cell hyperactivation. In addition, experimental autoimmune encephalomyelitis in T-cell-specific Crbn-deficient mice was exacerbated by increased T-cell activation via Kv1.3. Thus, CRBN limits CD4(+) T-cell activation via epigenetic regulation of Kv1.3 expression.</P>
