폐렴구균 균혈증에 합병된 심내막염은 드문 경우로 페니실린이 사용된 1940년대 이후로 전체의 1% 이하로 감소하였고, 주로 알코올 중독자에게 잘 발생한다. 저자들은 부비동염과 폐렴구균에 의한 균혈증으로 내원하여 항생제 치료 중 폐렴 및 대동맥 판막에 세균성 심내막염이 발생하여 대동막 판막 치환술 및 승모판 성형술 시행을경험하였기에 문헌 고찰과 함께 보고하는 바이다. Streptococcus pneumoniae has been a rare cause of endocarditis in the postantibiotic era. The incidence of pneumococcal endocarditis now accounts for less than 3% of all cases and most often occur in patients with risk factors, especially alcoholism. We report the case of a 68-year-old male with acute infective endocarditis due to Streptococcus pneumoniae. We stressed the low frequency of this agent as a cause of endocarditis and the atypical evolution of this case.

이 논문에서는 I=(m)와 J=(n)가 well-ordered Euclidean semidomain R 위에서 Principal ideal이며 I, J가 m과 n의 l.c.m.c에 의해 generated될 때, I/I∩J와 (I+J)/J는 동형을 이룬다는 것을 보였다. This paper considers the second isomorphism theorem in well-ordered Euclidean semidomains. We show that if I and J are principal ideals generated by m and n respectively in well-ordered Euclidean semidomain R and c is the l.c.m. of m and n, then ??.

Nd-Yag pulsed laser(210mJ, 6ns) was focused on polymer to make craters in an atmosphere. The size of the crater has a trend to follow the ideal estimation. Laser-firing near the focal point in air could be an energy dissipation of laser and a possible cause of the crater size reduction. Multiple plasma image and normal eruption of the plasma was also studied with photographs.

BOD, 질소, 인 등이 다량으로 함유된 고농도 유기성 폐수는 BOB, 인, 질소 등 일부 오염물질의 농도가 현저하게 높거나 C/N비의 균형이 좋지 않은 특성 때문에 처리가 곤란할 뿐만아니라, 적정한 처리공정을 도입하는 것 또한 어려운 실정이다. 이에 따라 고농도 유기성 폐수 배출시설중 화학공장의 폐수를 처리대상 폐수로하여, 각각의 공정에서 배출되는 다른 성상의 폐수를 합한 원폐수를 초음파를 이용하여 무화(霧化 : fogginess)시킨 후 무화 전후의 BOD, COD, SS, N-H과중금속의 제거율을 분석한 결과, BOD, 55, N-H의 제거율은 높았으며 중금속의 제거에는 영향을 미치지 않는 것으로 나타났다. High concentration organic wastewater has been discharged out of industries. These industries are food industries, chemical industries, fiber industries, spinning factories, paper mill manufactories or night soil and stock breeding. Its wastewater was treated to fogginess using Ultrasonic Wave. Due to development of wastewater treatment technique, it is investigated by technique previous to the treatment. Acording to fogginess frequency, it was analysised variation of pollutants concentration. The conclusions are as fallows. Most pollutants were removed by fogginess using Ultrasonic Wave, while heavy metals were not removed Removal effect of BOD, COD, SS, N-H, T-N, T-P developed a tendency to decrease according to frequency.

초음파 반사 패턴과 주파수 대역 분할 신호처리를 이용한 다층구조인 노내 간극 측정

모의 원자로내 용융물 냉각 실증 실험 결과 용융물 Al₂O₃/Fe themite 와 lower head vessel사이에 간극이 발생된다. 간극을 정량적으로 측정하기 위한 기존의 초음파 방법은 구조물의 복잡성과 2300℃ 에서 용융되는 thermite Al₂O₃/Fe, Al₂O₃의 영향으로 발생되는 lower head의 열영향부위(HAZ)의 금속학적인 결정입자 크기의 변화로 결정 계면으로부터 발생되는 잡음신호와 초음파 산란, 감쇠, 모드 변환 등이 발생되어 신뢰성이 떨어지고 있다. 간극을 정량적으로 측정하기 위해서 thermite와 lower head 사이에 물이 체워진 형태의 즉 고체, 액체, 고체의 다층구조에서 초음파의 진행 및 반사 형태를 분류 및 분석하였다. 결정립의 잡음을 억제하기 위하여 주파수대역의 확률 처리 방식인 PT(polarity threshold) 알고리즘을 이용하여 약 6dB 신호 대 잡음비의 개선효과를 가져왔다.

Helicobacter pylori 편모 유전자의 클로닝 및 염기서열 분석

A λgt11 expression libary of H. pylori DNA in E. coli Y1090 was screened with flagellin-specific rabbit antiserum for molecular cloning of the flagellin gene of H. pylori. A positive clone, λHPF4, was obtained and the recombinant antigen expressed from λHPF4 was a fusion protein with the molecular weight of 168kd. Sequence analysis of antigen-encoding DNA showed that an open reading frame composed of 1,536 nucleotides encodes a polypepride with a oredicted molecular size of 54kd. This open reading frame did not show the homology with flaA gene encoding 56kd protein of H. pylori and was confirmed as a unique sequence through homoligy searching. Therefore, the cloned antigen is supposed to be the carboxy-terminal region of the other flagellin protein of H. pylori, flaB, with the molecular weight of 58kd.

Objective: To report linezolid-induced syndrome of inappropriate antidiuretic hormone (SIADH). Case summary: A 81-year-old woman was transferred to our hospital due to progression of malignant otitis externa extended to ipsilateral zygomaticotemporal bone. She was previously treated with teicoplanin against methicillin-resistant Staphylococcus aureus (MRSA) isolated from the ear tissue for 7 weeks, but not improved. She has taken buspirone and memantine for 4 years, with a medical history of old cerebral infarct and dementia. We started linezolid (600mg q 12 h, intravenously) against MRSA. From 13th day to 22th day of linezolid use, she developed severe hyponatremia with a decrease in serum Na levels from 126 to 118 mmol/L, but, she was asymptomatic. She was in euvolemic state. Her laboratory values showed serum osmolality (mOsm/kg of water) 250, urine osmolality 322, and urinary Na 78.7 mmol/L. Her thyroid and adrenal function were normal. These . ndings were consistent with SIADH. Her serum Na level rose to 130 mmol/L after a supplement of 3% hypertonic saline for 3 days. In 26th day of linezolid use, it was discontinued because of development of thrombocytopenia (80,000/uL). Three days after discontinuation of linezolid, her serum Na level increased to 137 mmol/L, and then remained stable. We excluded other potential etiologies or concomitant drugs that might cause the SIADH. Therefore, linezolid-induced SIADH is the most likely diagnosis for this patient. Discussion: An objective causality assessment using the Naranjo scale was done in this patient: A score of 6 revealed a probable causality between linezolid and SIADH. So far, only one linezolid .induced SIADH has been previously reported. The likely mechanism of this reaction is not known. Conclusions: Clinicians should consider the potential adverse effect of linezolid, when assessing a patient who receives linezolid and presents hyponatremia or SIADH.
