      • Poster Session:PS 1189 ; Cardiology : A Case of Stress Induced Cardiomyopathy Accompanied by Panhypopituitarism

        Stress induced cardiomyopathy, otherwise known as Takotsubo cardiomyopathy(TC), is a cardiac syndrome characterized by transient LV dysfunction in the absence of coronary artery disease. We report an unique TC case that accompanied panhypopituitarism. A 52 year old male patient visited our emergency room due to dyspnea. The patient had been on hormone replacement therapy after undergoing transsphenoidal adenectomy for pituitary adenoma 18 months ago. Patient reported that he skipped hormone therapy for 7 days and it led him to have secondary adrenal insuffi ciency, thyroid hormone defi ciency, hyponatremia, and hyperkalemia. After admitting to ER, his dyspnea and hypoxic status rapidly worsened and his initial EKG showed sinus bradycardia that progressed to pulseless electrical activity. His spontaneous circulation returned after 2 minute cardiopulmonary resuscitation. Thereafter he was emergently treated with glucocorticoid hormone replacement and appropriate hormone replacement therapy was performed accordingly. Transthoracic echocardiography revealed typical pattern of stress induced cardiomyopathy and coronary angiography result was normal. We treated both his hormonal defi ciency status and heart failure. Consequentially, his clinical status improved and follow up echocardiography showed normal heart function in two months. We report the unique case of stress induced cardiomyopathy caused by acute adrenal crisis owing to secondary adrenal insuffi ciency in patient with panhypopituitarism.

        Antiphospholipid Antibody Positivity and the Clinical Outcomes of Patients with Systemic Lupus Erythematosus

        Objective. To identify the prevalence of antiphospholipid antibodies (aPL) in systemic lupus erythematosus (SLE) patients and determine the relationship between aPL and the clinical outcomes. Methods. SLE patients with aPL test results within 2 years of enrollment were selected from Korean lupus network study. They were classified into two groups: aPL (+) group, patients positive for at least one aPL, and aPL (-) group, patients without an aPL. The clinical characteristics of the two groups were compared and the role of aPL in the risk of chronic kidney disease (CKD) in SLE patients was examined. Results. Among the 469 SLE patients, 69 (14.7%) had at least one aPL. The prevalence of cerebrovascular disease and CKD was higher in the aPL (+) group than in the aPL (-) group (10.1% vs. 1.8% and 13.8% vs. 5.1%, p<0.05). Multivariable regression analysis showed that the aPL positivity (odds ratio=3.93, 95% confidence interval=1.48∼10.47) was associated with the risk of CKD after adjusting for age, disease duration, and lupus nephritis history. Conclusion. The prevalence of aPL in Korean SLE patients is 14.7%, and it is associated with a higher prevalence of cerebrovascular disease and CKD in SLE patients. The aPL positivity is independently associated with the risk of CKD in SLE patients. (J Rheum Dis 2018;25:239-247)

        The publication of genetic epidemiology meta-analyses has increased rapidly, but it has been suggested that many of the statistically significant results are false positive. In addition, most such meta-analyses have been redundant, duplicate, and erroneous, leading to research waste. In addition, since most claimed candidate gene associations were false-positives, correctly interpreting the published results is important. In this review, we emphasize the importance of interpreting the results of genetic epidemiology meta-analyses using Bayesian statistics and gene network analysis, which could be applied in other diseases.

        The usefulness of trabecular bone score in patients with ankylosing spondylitis

        Backgrounds/Aims: This study was performed to reveal the usefulness of the trabecular bone score (TBS) in assessing bone strength in patients with ankylosing spondylitis (AS) in comparison with dual-energy X-ray absorptiometry (DXA) methods. Methods: A total of 215 AS patients (75.8% male) were enrolled from a single university hospital in Korea. Demographic and clinical information were assessed. Patients completed X-rays of the cervical and lumbar spine (L-spine), and spinal ankyloses were quantified using the modified Stoke AS Spine Score (mSASSS). Hip, anteroposterior and lateral L-spine bone mineral density (BMD) and TBS were assessed by DXA methods. Clinical characteristics and bone strength measurement results were compared between male and female AS patients. The accuracy of each bone strength evaluation method in predicting Fracture Risk Assessment Tool (FRAX) scores indicating moderate or higher fracture risk was compared by receiver operating characteristic curves in patients aged ≥ 40 years. Correlations between each bone strength measurement method and mSASSS were examined. Results: Male patients showed higher mSASSS and less prevalent peripheral joint involvement compared to female patients (p < 0.05). TBS, hip BMD, and L-spine lateral BMD showed comparably high areas under the curve (AUCs) for predicting FRAX-major osteoporotic fractures (MOF) ≥ 10% (AUC ranged 0.72 to 0.76). TBS negatively correlated with mSASSS in both male and female patients (p < 0.01). Conclusions: TBS could predict the risk of MOF and is not influenced by spinal osteoproliferation in AS patients, even in those with advanced spinal changes.

        Background: The use of musculoskeletal ultrasound (US) in rheumatology has rapidly advanced over the past decade, demonstrating its usefulness in the diagnosis and assessment of arthritis. Targeted Ultrasound Initiative (TUI) is an international research network which aims to promote US as a standard outcome measure in patients with rheumatoid arthritis. In collaboration with the TUI, to conduct the fi rst study in Korea to investigate current practices in US use among Korean rheumatologists. Methods: We translated the global TUI survey into Korean and augmented it to ascertain the current status and use of US by Korean rheumatologists. In February 2013, we sent e-mails to all Korean College of Rheumatology (KCR) members; survey ran from March until June 2013; paper surveys sent to KCR members who did not respond to the e-mail survey. Results: This study enrolled 108 rheumatologists which covered about a half of the total Korean rheumatologists. Rheumatologists in hospitals and private clinics use US to examine between one to fi ve patients daily; they use US for diagnosis more than monitoring and receive compensation of about US$30-50 per patient. There are marked differences in the rates of US usage between rheumatologists who work in private practice compared with tertiary hospitals. Korean rheumatologists not currently using US in their practice appear eager to do so. Most current US users reported using it primarily for diagnosing either suspected arthritis (77. 8%) or undifferentiated arthritis (64. 4%). The most commonly cited barrier was insuffi cient time for examination. Conclusions: This survey provides important insights into the current status of US in rheumatology in Korea and highlights several priorities; specifi cally, greater provision of formal training, standardization of reporting, and accrual of greater experience among US users. If these needs are addressed, all rheumatology departments in Korea are likely to use US or have access to it in the future.

        범뇌하수체저하증의 호르몬 대체요법 중단 후 발생한 스트레스 심근병증

        Stress induced cardiomyopathy (SC) is characterized by transient left ventricular (LV) dysfunction in the absence of coronary artery disease. We report on a patient with panhypopituitarism who developed SC resulting from withdrawal of hormonal replacement therapy (HRT). A 52-year-old male visited our hospital for progressively worsening dyspnea. The patient had discontinued HRT 7 days ago, which had been administered for 18 months after transsphenoidal adenomectomy for pituitary macroadenoma. Initial electrocardiogram showed marked sinus bradycardia. Transthoracic echocardiography showed apical ballooning with an LV ejection fraction of 25%. No significant obstructive lesions were observed on coronary angiography. With a clinical diagnosis of SC associated with panhypopituitarism, HRT was restarted, including glucocorticoid and thyroxine, along with standard heart failure management. His LV function had normalized at 2-month follow-up. He remains asymptomatic and administration of beta-blocker and angiotensin converting enzyme inhibitor were discontinued He currently only requires HRT.

        Yield prediction is one of the most important topics in semiconductor manufacturing. Especiall y, for a fast-changing environment of the semiconductor industry, efficient forecasting techniq ues are required. In this study, we propose a procedure to predict wafer yield using classificat ion algorithms. The proposed procedure addresses imbalance problems frequently encountered in semiconductor processes so as to construct reliable prediction models. The effectiveness and applicability of the proposed procedure was demonstrated through a real data from a leading semiconductor industry in South Korea.

        일반논문 : 광양읍성의 공간 구조에 관한 연구

        지금의 광양시 광양읍 지역은 조선시대 광양 지역의 읍치였다. 읍치의 중심이었던 광양읍성은 조선시대 읍성가운데 성곽의 안쪽 둘레가 853m 정도에 지나지 않는 소규모읍성이었다. 광양읍성 지역은 일제강점기에 성곽이 철거된 이후에, 급격한 시가지 확산으로 지금은 그 위치를 추정하기조차 어려울 정도로 그 흔적을 찾기가 어렵다. 본 논문은 지역문화의 시각화를 해석 주민들의 가치관 형성의 토대를 마련하고자, 조선후기 광양읍성의 공간구조와 그 변화에 대한 연구가 이루어졌다. 이 연구를 통해서 과양읍성의 축조 배경과 과정을 일부나마 확인할 수 있었다. 연구의 내용을 정리하면 광양읍성은 기름진 농토를 관리하기 위해 죽조되었고, 읍성의 지형적 약점을 극복하기 위해 전통 숲이 함께 조성되었다. 광양읍성은 성문이 3개소로 “T”형 도로망을 갖고 있었고, 도로 개설에 의해 성곽의 훼철이 시작되었다. 광양읍성의 대표적인 경관이었던 건통 숲 은 읍성 차폐에서 보안림으로, 그리고 도시공원으로 그 활용이 바뀌었음을 알 수 있었다. Present region of Kwangyang City and town was the Traditional Eup Towns in of Kwangyang in Chosun Era. Kwangyang-Upsung, the core Traditional Eup Towns, was asmall town castle whose inside circumstance was no more than 853m, compared with those in Chosun Era. The region of it is difficult to find the traces which can be assumed as the location these days owing to the rapid urban spreading after the castle was removed during the period of Japan`s forced occupation. But the study on the spacial construction and change about the latter period of Chosun come true to form the basis on sence of values on village people through the visualization of local cultures. On this study, the background of construction and process can be identitied even some paets of them. According to this study, Kwangyang-Upsung was constructed to manage the fertile farm and traditional forest was made together to compensate for the topographcial disadvantage. It consisted of three gates of castle and “T” type road network. The destruction of the castle was begun by the road building. Traditional forest which had represented Kwangyang-Upsung was turned into erserved forest city park fark from the town castle shelter in use.
