        The purpose of this study was to estimate the proportion of user charges out of total treatment costs and to investigate the occuring status of the charge of non-benefit service. The data were collected from 6 hospitals in 3 cities (Seoul 2, Incheon 3, Shihung 1), which containing 1,752 discharge-bills of discharged patients insured by health insurance. The data were analyzed after standardization of the items of non-benefit services. The result are as follows; 1. the average percent of the cost-sharing of discharged patients was 38.1%(benefit : 15.5%, non-benefit : 22.6%) and the proportion of the cost-sharing due to non-benefit services was greater than that due to benefit services. 2. The occurrence rate and the occurred number of non-benefit services were different among hospitals and showed characteristic occurrence rate in individual hospitals. 3. In acute appendicitis, the characteristics of the occurrence rate and the occurred number of non-benefit services by individual hospitals was similar to those of hospitals. It suggest that the hospitals intentionally applied non-benefit item to patients in order to increase their profit. These findings suggest that the burden of cost-sharing is still high especially due to non-benefit services, so it is necessary to extend the coverage of insurance benefits and to develop management system for the appliance of non-benefit services. Regarding the discharge-bill, all service charges should be included in the bill.

        조직문화가 공무원의 적극행정 인식에 미치는 영향에 관한 연구: 조직공정성의 매개효과

        본 연구는 중앙행정기관 소속 국가공무원 247명을 대상으로 조직문화가 조직공정성을 매개하여 적극행정에 미치는 영향을 실증적으로 분석하였다. 연구방법은 조직 간 문화 차이를 Cameron & Quinn(1999)의 경쟁가치모형을 토대로 집단문화, 위계문화, 발전문화, 합리문화로 분류하여 조직문화, 조직공정성, 적극행정 세 변수의 관계를 위계적 회귀분석 방법을 통해 검증하였다. 분석결과 첫째, 합리문화, 발전문화는 적극행정에 유의한 정(+)의 영향을 미치는 것으로 나타났다. 둘째, 조직공정성은 적극행정에 유의한 정(+)의 영향을 미쳤다. 셋째, 조직공정성의 매개효과를 살펴본 결과 합리문화와 적극행정의 관계에서는 부분매개 효과를, 집단문화와 적극행정의 관계에서는 완전매개 효과를 확인하였다. 통제변수와 관련하여 연령이 높을수록 적극행정 인식이 유의하게 높았으며, 중앙행정기관 본부보다 소속기관에 근무하는 공무원의 적극행정 인식이 더 높은 것으로 분석되었다. 본 연구는 조직문화와 조직공정성, 적극행정의 관계에 대한 실증연구가 부족한 실정에서 이들 변수간의 관계를 실증적으로 분석하고, 연령, 기관 특성 등이 적극행정 인식에 영향을 미칠 수 있음을 확인하여 후속 연구의 필요성을 제기하였다는 점에서 의의가 있다. 이와 더불어 실증분석 결과를 토대로 외부지향적 문화의 강화와 조직공정성 개선을 위한 내부홍보 및 교육 등의 정책적 대안을 제시하였다. This study analyzed the impact of organizational culture on active administration by mediating organizational fairness, targeting 247 state public officials. Organizational culture classifies into hierarchical culture, collective culture, rational culture, and development culture based on the competitive value model of Cameron & Quinn (1999), and hierarchical regression on the relationship between variables was conducted. As a result of the analysis is as follows. First, rational culture, and development culture, have a significant positive(+) effect on active administration. Second, organizational fairness has a significant positive effect on active administration. Third, as a result of test the mediating effect of organizational fairness, a partial mediating effect was confirmed in the relationship between rational culture and active administration, and a complete mediating effect was confirmed in the relationship between collective culture and active administration. Among the control variables, age and institutional characteristics significantly impact on active administration. This study contributes to literature in that it explored the relationship between the three variables of organizational culture, organizational fairness, and active administration. Future research direction was also discussed. In addition, based on empirical evidences, policy alternatives such as internal public relations and education were presented to strengthen external-oriented culture and organizational fairness.

        <P>Identification of the cellular mechanisms that mediate cancer cell chemosensitivity is important for developing new cancer treatment strategies. Several chemotherapeutic drugs increase levels of the posttranslational modifier ISG15, which suggests that ISGylation could suppress oncogenesis. However, how ISGylation of specific target proteins controls tumorigenesis is unknown. Here, we identified proteins that are ISGylated in response to chemotherapy. Treatment of a human mammary epithelial cell line with doxorubicin resulted in ISGylation of the p53 family protein p63. An alternative splice variant of p63, δNp63α, suppressed the transactivity of other p53 family members, and its expression was abnormally elevated in various human epithelial tumors, suggestive of an oncogenic role for this variant. We showed that ISGylation played an essential role in the downregulation of δNp63α. Anticancer drugs, including doxorubicin, induced δNp63α ISGylation and caspase-2 activation, leading to cleavage of ISGylated δNp63α in the nucleus and subsequent release of its inhibitory domain to the cytoplasm. ISGylation ablated the ability of δNp63α to promote anchorage-independent cell growth and tumor formation in vivo as well to suppress the transactivities of proapoptotic p53 family members. These findings establish ISG15 as a tumor suppressor via its conjugation to δNp63α and provide a molecular rationale for therapeutic use of doxorubicin against δNp63α-mediated cancers.</P>

        Case Report : The First Case of Novel Influenza A (H1N1) Fatality in Korea

        Here we report the first fatality caused by H1N1 influenza virus infection with acute respiratory distress syndrome in Korea. A 55-year-old man presented at our emergency department with dyspnea, fever, diffuse myalgia and malaise. Bilateral lung air-space consolidation was detected on his initial chest radiograph combined with severe hypoxemia. He was supported by mechanical ventilation and treated with antibiotics. A nasopharyngeal aspirate was positive for influenza A rapid antigen and oseltamivir was started on day 3 of admission. The nasal swab sample was positive for influenza H1N1 virus by real-time reverse-transcriptase polymerase chain reaction. Despite aggressive treatment, he had refractory hypoxemia and uncontrolled septic shock. On day 5 of admission he went into cardiac arrest and expired.

        Degradation Behavior of TiN Coatings with Different Thicknesses after a Pulsed Laser Thermal Shock Test

        The degradation behavior of TiN coatings with different thicknesses deposited on Inconel 617 substrates was investigated in terms of surface and interface microstructures after pulsed laser thermal shock. The specimens with coating thicknesses of 1.0 and 4.5 ?m were exposed to repetitive laser shock using a pulsed laser ablation system (LSX-213). The TiN coating with a 1.0 ?m thickness contained cracks along the columnar grains in the coating layer, and spalling and delamination with interface cracks appeared with an increase in the number of ablations. In contrast, the 4.5 ?m thick TiN coating still maintained adhesion to the substrate until the substrate elements dissolved out to the surface. Surface and cross-sectional observation showed that the pulsed laser seems to simultaneously induce normal and shear stress to the surface at the ablated spots with high thermal stress.         전신부종으로 내원하였던 환자로 항핵항체와 항-dsDNA 항체가 음성이면서 ACR 진단기준을 만족시키지 못하였으나 신장 조직검사 상에서 SLE가 진단되었고 스테로이드와 면역억제제 치료 후에 증상이 호전되었다. 이러한 비전형적인 혈청학적 검사 소견을 보이는 환자의 진단을 위해서는 의심되는 임상증상이 있는 경우 지속적인 추적관찰이 필요할 것으로 사료된다. Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by the production of diverse autoantibodies with various systemic organ involvements. In patients with SLE, autoantibodies, such as antinuclear antibody (ANA) and anti-dsDNA antibody, play an important role not only in diagnosing the disease, but also representing the pathogenesis of the disease. ANA is the main screening tool in diagnosis and serum complement levels and anti-dsDNA antibody level are closely related to the disease activities. Nevertheless, exceptionally, some patients represent with negative ANA and/or anti-dsDNA antibody leading to difficulties in diagnosing the disease. Here, we report a case of 37-year old female SLE patient with negative ANA, negative anti-dsDNA antibody, and positive anti-Ro/SSA antibody, which manifested with nephrotic syndrome.

      • Insulin-sensitizing activities of tanshinones, diterpene compounds of the root of <i>Salvia miltiorrhiza</i> Bunge

        <P><B>Abstract</B></P><P>In this study, the effects of the extract and four tanshinone compounds from the dried root of <I>Salvia miltiorrhiza</I> Bunge (Labiatae) on the tyrosine phosphorylation of the insulin receptor (IR) β-subunit and the downstream signaling were examined in Chinese-hamster ovary cells expressing human insulin receptors (CHO/IR cells) as well as in 3T3-L1 adipocytes. In addition the translocation of the glucose transporter 4 was investigated in 3T3-L1 adipocytes. Total extract of Danshen (1–10μg/ml) and the four tanshinones (10μM) did not show any activity, but the total extract and the tanshinone I, IIA and 15, 16-dihydrotanshinone I except cryptotanshinone enhanced the activity of insulin (1nM) on the tyrosine phosphorylation of the IR as well as the activation of the downstream kinases Akt, ERK1/2, and GSK3β. In the adipocytes the same IR-downstream signaling and the translocation of glucose transporter 4 were demonstrated by the three tanshinones in the presence of insulin. These insulin-sensitizing activities of tanshinones may be useful for developing a new class of specific IR activators as anti-diabetic agents.</P>
