Background: Urine tissue inhibitor of metalloproteinases-2/insulin-like growth factor-binding protein 7 (TIMP-2/IGFBP7) (NephroCheck, Ortho Clinical Diagnostics, Raritan, NJ, USA) is a US Food and Drug Administration-approved biomarker for risk assessment of acute kidney injury (AKI) in critically ill adult patients in intensive care units; however, its clinical impact in the emergency department (ED) remains unproven. We evaluated the utility of NephroCheck for predicting AKI development and short-term mortality in the ED. Methods: This was a prospective, observational, five-center international study. We consecutively enrolled ED patients admitted with ≥30% risk of AKI development (assessed by ED physician: ED score) or acute diseases. Serum creatinine was tested on ED arrival (T0), day 1, and day 2 (T48); urine for NephroCheck was collected at T0 and T48. We performed ROC curve and reclassification analyses. Results: Among the 529 patients enrolled (213 females; median age, 65 years), AKI developed in 59 (11.2%) patients. The T0 NephroCheck value was higher in the AKI group than in the non-AKI group (median 0.77 vs. 0.29 (ng/m)2/1,000, P=0.001), and better predicted AKI development than the ED score (area under the curve [AUC], 0.64 vs. 0.53; P=0.04). In reclassification analyses, adding NephroCheck to the ED score improved the prediction of AKI development (P<0.05). The T0 NephroCheck value predicted 30-day mortality (AUC, 0.68; P<0.001). Conclusions: NephroCheck can predict both AKI development and short-term mortality in at-risk ED patients. NephroCheck would be a useful biomarker for early ruling-in or ruling-out of AKI in the ED.
Experimental and numerical studies of miniature bar-typed structural fuses with Teflon pads

Many studies have proved that structural fuses could improve the seismic performance of structures efficiently. A structural fuse named the miniature bar-typed structural fuse (MBSF) has been proposed and investigated by the authors, which consists of a central core bar, a confining tube. To further improve the mechanic performances of the MBSFs under compressive loadings, Teflon pads are introduced to adjust the contact and friction status between the core bar and the confining tube. Three groups of specimens were discussed including the specimen with a single cutting line (SC), the specimen with double cutting lines (DC), and the specimen with triple cutting lines (TC). The results show that the hysteretic performances of the fuses are improved with the help of Teflon pads. The compression strength adjustment factor declines when Teflon pads are appended. Numerical and theoretical analyses are also conducted which expounded the effect of the Teflon pads. Different plastic buckling deformation principles of the core bars are compared by the theoretical analysis. It is shown that Shanley’s theory fits the numerical results well, which is recommended for the theoretical calculation of the proposed MBSFs.

Background/AimsHypopharyngeal multichannel intraluminal impedance-pH (HMII-pH) technology incorporating 2 trans-upper esophageal sphincter impedance channels has been developed to detect pharyngeal reflux. We used the HMII-pH technique to validate the candidate pharyngeal acid reflux (PAR) episodes based on the dual-pH tracings and determined the interobserver reproducibility. MethodsWe conducted a cross-sectional study in tertiary centers in Taiwan. Ninety patients with suspected laryngopharyngeal reflux and 28 healthy volunteers underwent HMII-pH test when off acid suppressants. Candidate PAR episodes were characterized by pharyngeal pH drops of at least 2 units and reaching a nadir pH of 5 within 30 seconds during esophageal acidification. Two experts manually independently identified candidate PAR episodes based on the dual-pH tracings. By reviewing the HMII-pH tracings, HMII-pH-proven PAR episodes were subsequently confirmed. The consensus reviews of HMII-pH-proven PAR episodes were considered to be the reference standard diagnosis. The interobserver reproducibility was assessed. ResultsA total of 105 candidate PAR episodes were identified. Among them 84 (80.0%; 95% CI, 71.0-87.0%) were HMII-pH-proven PAR episodes (82 in 16 patients and 2 in 1 healthy subject). Patients tended to have more HMII-pH-proven PAR episodes than healthy controls (median and percentile values [25th, 75th, and 95th percentiles]: 0 [0, 0, 3] vs 0 [0, 0, 0], P = 0.067). The concordance rate in diagnosing HMII-pH-proven PAR episodes between 2 independent observers was 92.2%. ConclusionOur preliminary data showed that 80.0% (71.0-87.0%) of the proposed candidate PAR episodes were HMII-pH-proven PAR episodes, among which the interobserver reproducibility was good.

Post-crosslinking treatment is essential for improving the water tolerance of the electrospinning keratin/Poly(ethylene oxide)(PEO) nanofiber membrane to meet the requirement of the biomedical application. In this study, three ecofriendly low-costmethods, hot air, pure oxygen, and ultraviolet irradiation were employed as the post-crosslinking treatment to prepare theelectrospun keratin/PEO (90/10) nanofiber membrane with good water tolerant. A comparative study between the performancesof the nanofiber prepared by different cross-linking methods were carried out. The results showed the three postcrosslinkingmethods were all effective for transforming the water-soluble keratin/PEO nanofiber membranes to the waterinsoluble membranes. However, the mechanical property, water sorption and swelling rate, molecular structure, thermalproperty, and crystallinity of the nanofiber membranes were all different due to the different post-crosslinking methods. Afterheat treatment, the tensile strength of the nanofiber membrane can reach 2.29 MPa, while that of the oxygen crosslinking andultraviolet crosslinking are 0.99 and 1.68 MPa, respectively. The water absorption and swelling of the oxygen and ultravioletcrosslinking membranes are relatively large, and the swelling rate is about 21%. The oxygen crosslinking membrane possessedhigher hydrophilicity and the contact angle is only 21°. In vitro cell culture studies showed that the post-crosslinkednanofiber membrane all presented excellent biocompatibility and supported cell adhesion and proliferation. The three ecofriendlylow-cost post-crosslinking treatments are all reliable and safe to prepare keratin/PEO nanofiber membrane with goodwater tolerant, which is potential for tissue engineering and biomedical applications.

In this study, two soybean genotypes, i.e., aluminum-tolerant Baxi 10 (BX10) and aluminumsensitive Bendi 2 (BD2), were used as plant materials and acidic red soil was used as growth medium. The soil layers from the inside to the outside of the root are: rhizospheric soil after washing (WRH), rhizospheric soil after brushing (BRH) and rhizospheric soil at two sides (SRH), respectively. The rhizosphere bacterial communities were analyzed by high-throughput sequencing of V4 hypervariable regions of 16S rRNA gene amplicons via Illumina MiSeq. The results of alpha diversity analysis showed that the BRH and SRH of BX10 were significantly lower in community richness than that of BD2, while the WRH exhibited no significant difference between BX10 and BD2. Among the three sampling compartments of the same soybean genotype, WRH had the lowest community richness and diversity while showing the highest coverage. Beta diversity analysis results displayed no significant difference for any compartment between the two genotypes, or among the three different sampling compartments for any same soybean genotype. However, the relative abundance of major bacterial taxa, specifically nitrogen-fixing and/or aluminum-tolerant bacteria, was significantly different in the compartments of the BRH and/or SRH at phylum and genus levels, indicating genotype-dependent variations in rhizosphere bacterial communities. Strikingly, as compared with BRH and SRH, the WRH within the same genotype (BX10 or BD2) always had an enrichment effect on rhizosphere bacteria associated with nitrogen fixation

This study evaluated the value of procalcitonin (PCT) levels in pleural effusion to differentiate the etiology of parapneumonic effusion (PPE). Forty-one consecutive PPE patients were enrolled and were divided into bacterial and non-bacterial PPE. Blood and pleural effusion samples were collected for PCT measurement on admission and analyzed for diagnostic evaluation. PCT of pleural fluid was significantly increased in the bacterial PPE group (0.24 ng/mL) compared to the non-bacterial PPE group (0.09 ng/mL), but there was no significant difference for serum PCT. A PCT concentration of pleural fluid >0.174 ng/mL (best cut-off value) was considered positive for a diagnosis of bacterial PPE (sensitivity, 80%; specificity, 76%; AUC, 0.84). Pleural effusion PCT in the bacterial PPE is significantly different from those of the non-bacterial PPE and control groups, so the diagnostic use of PCT still warrants further investigation.
