      • 콘크리트 지하철 구조물의 비선형 지진응답해석

        1995년 Hyogoken-Nanbu 지진은 RC 지중 지하철 구조물에 지난 과거 전혀 경험치 못했던 심각한 피해를 일으켰다. 이같은 지진피해의 정확한 평가를 위하여, 주파수영역상에서의 지반-구조물 상호작용효과를 고려한 지하 구조물에 대한 선형지진응답해석이 수행되어왔으나, 보다 정확한 지진피해를 예측하기 위해서는 시간영역상에서 지반과 구조물의 이력감쇠와 강성변화를 고려한 비선형지진응답해석을 수행하는 것이 반드시 필요하다. 본 논문에서는 지반-구조물 상호작용을 고려한 비선형 지진응답해석을 시간영역상에서 수행하기 위하여 주파수영역상에서 지반-구조물 상호작용효과를 고려하기위해 개발된 KIESSI-2D와 유한요소 범용해석프로그램 ANSYS를 연계한 복합법을 사용하여 Hyogoken-Nanbu 지진시 피해가 많았던 고베의 가미사와 지하철역을 대상으로 비선형 지진응답해석을 수행하고 실제 발생한 피해상황과 비교 ·분석하였다. 해석시 등가 선형화기법에 근거한 대표적인 자유장 해석프로그램인 SHAKE를 사용하여 지반의 비선형성을 고려하였고, 콘크리트의 비선형성을 고려하기위해서 Hognestad 모델을 사용하였다. 해석결과 실제 발생한 중간기둥의 전단파괴를 매우 유사하게 예측할 수 있음을 보여주었다. During the 1995 Hyogoken-Nanbu earthquake, underground RC structures experienced significant damages which had never been experienced before. To evaluate these damages, a linear earthquake response analysis in frequency domain has often been used. For more accurate evaluation of the damages, however, a nonlinear earthquake response analysis in time domain must be employed by incorporating she continuous change of stiffness and hysteresis damping of soil-structure interaction systems. In this study, a hybrid method which combines a soil-structure interaction program, KIESSI-2D with a general purpose FEM package, ANSYS is developed to perform nonlinear earthquake response analysis of soil-structure interaction in time domain. As a verification of the hybrid method, nonlinear earthquake response analyses are performed on the Kamisawa subway station which was damaged severely during the Kobe earthquake and analytical results are compared with the real damages. In order to consider the nonlinearity of soil, SHAKE program is used, whereas Hognestad model is used to consider the nonlinearity of concrete. It is shown that the analytical prediction agrees fairly well with the actual damages which were caused by the shear failure of the center column of the station.

        Case Reports : High-dose Immunoglobulin Infusion for Thrombotic Thrombocytopenic Purpura Refractory to Plasma Exchange and Steroid Therapy

        The outcomes of the treatment of thrombotic thrombocytopenic purpura (TTP) have been shown to be improved by the administration of plasma exchange. However, treatment options are currently limited for cases refractory to plasma exchange. The autoantibodies that block the activity of ADAMTS13 have been demonstrated to play a role in the pathogenesis of TTP; therefore, high-dose immunoglobulin, which can neutralize these autoantibodies, may be useful for refractory TTP. However, successful treatment with high-dose immunoglobulin for TTP refractory to plasma exchange and corticosteroids has yet to be reported in Korea. Herein, we describe a refractory case which was treated successfully with high-dose immunoglobulin. A 29-year-old male diagnosed with TTP failed to improve after plasma exchange coupled with additional high-dose corticosteroid therapy. As a salvage treatment, we initiated a 7-day regimen of high-dose immunoglobulin (400 mg/kg) infusions, which resulted in a complete remission, lasting up to the last follow-up at 18 months. High-dose immunoglobulin may prove to be a useful treatment for patients refractory to plasma exchange; it may also facilitate recovery and reduce the need for plasma exchange.

        뇌전산화 단층촬영상의 임상적분석

        The brain computerized tomography, an epoch-making diagnostic procedure has caused a revolutionary change in the neurosurgical field; the correct localization of small space occupying lesion wherever it is in the brain without pain and harm to the patients. The brain CT findings of 3035 patients which were examined in this hospital during past 29 months from Oct. 1981 to March 1984 were analysed and the literature review was made. The results of analysis are as follows: 1) 37.1% of total 3035 cases were observed abnormal, including 29% of head injured patients, 42.4% of non-trauma cases, 57.6% of emergency cases' and 15.2% of non-emergency cases, 2) The rates of abnormal findings were proportional to the degree of consciousness disturbance in the head injured patients and severity of the neurological deficits in the other neurosurgical patients. 3) The follow up studies were performed for 287 patients among 1127 patients having abnormal findings in the first check and we found the improving process of various lesion and the resolving process of hematoma after operation. The follow up studies were performed for 138 patients having normal findings in the first check and 26% of these cases were observed abnormal, including 21.5% of head injured cases and 55% of the non-trauma cases. 4) 94% of patients with spontaneous subarachnoid hemorrhage showed bleedings chiefly in the basal cistern.

      • 괄루근 렉틴의 면역활성 및 세포응집에 관한 연구

        Since ancient times Trichosanthis Radix has been used in traditional herbdrung as an antitumor and immunologic stimulating agent. Lectin was purified to homogeneity from Trichsanthis Radix by ammonium sulfate precipitation, DEAE-Cellulose chromatography and Sephadex G-150 gel filltration. The major amino acid was tyrosine and some aspartic acid, glutamic acid and saccharides were also present. Trichosanthis Radix Lectin (TRL)strongly stimulated activity of human erythrocytes, lymphocytes, and thrombocytes. In addtion, agglutination of gram negative Escherichia coli and Pseudomonas ararugimosa was shown, but proteus, Hemophilus influenza and Staphylococcus aureus were not agglutinated. Folloaing injection of TRL into ICR mouse, granulocyte was increased by 87.5% and the number of lymphocyte was increased by 25-28%. These results suggest that TRL may have stimulating effects on the general immune systems.

        Circulating Eosinophil and Neutrophil Counts Correlate with Disease Severity in Bullous Pemphigoid

        Background: Bullous pemphigoid (BP) is an autoimmune subepidermal blistering disease characterized by tissue- bound and circulating autoantibodies directed against BP180 and/or BP230 antigens. Various inflammatory cells are involved in the development of blister in BP. Objective: The aim of this study was to evaluate the correlation between peripheral leukocyte counts and BP severity. Methods: We retrospectively included 60 patients with BP, who had not been treated with systemic steroid at the time of blood sampling. The patients were classified into two groups, those with admission history (admission group) and those without admission history (non-admission group). Disease severity was evaluated using three parameters: admission history, initial steroid dosage, and modified version of a pemphigus scoring system. We evaluated the correlation between peripheral leukocyte counts and disease severity measured by the three parameters. Results: The admission group showed a significant increase in disease severity measured by initial steroid dosage and severity score compared with the non-admission group. Additionally, the admission group had increased total leukocyte, eosinophil, and neutrophil counts. In the correlation study, the peripheral eosinophil and neutrophil counts showed positive correlation with BP severity evaluated by both initial steroid dosage and the pemphigus scoring system. Conclusion: Peripheral eosinophil and neutrophil counts can be used as a marker in predicting disease severity in patients with BP. (Ann Dermatol 30(5) 544∼549, 2018)

      • The effect of HIF-2α inhibitor in DNFB-induced contact hypersensitivity

        Background: Allergic contact dermatitis (ACD) is caused by exposure of foreign material and various cytokines are induced by direct stimulation of hapten on keratinocytes or by sensitized T lymphocytes. T cells have an important role in the pathogenesis of ACD providing rapid response to previously encountered pathogens. Hypoxia-inducible factors (HIFs) are the main regulator of cellular response to reactive oxygen species and T cells are expected to be suppressed by HIF-2α antagonist. Objectives: The purpose of this study is to evaluate cytokine profile and the effect of HIF-2α antagonist in contact hypersensitivity mouse model induced by DNFB. Methods: Contact hypersensitivity was induced in C56BL/6 mouse by DNFB. HIF-2α antagonist was treated by oral gavage in contact hypersensitivity group. We measured level of ear thickness, epidermal thickness and CD4+ T cells by immunohistochemical staining. RNA level of various cytokines was evaluated using real-time PCR. Results: Ear thickness, epidermal thickness, and CD4+ T cell percentage were increased in contact hypersensitivity group. Proinflammatory cytokines (IL-1α, IL-1β, TNF), alarmin (IL-33), Th1 (IFN-γ, CXCL-10) and Th2 (IL-4, CCL-11, CCL-24) cytokines, and EPAS1 were elevated, but no significant difference was identified when treated by HIF-2α antagonist. Conclusion: Various cytokines were elevated in contact hypersensitivity mouse model induced by DNFB, but no significant differences were found when treated by HIF-2 α antagonist.

      • Increased frequency of Tfh cytokines in serum of patients with autoimmune bullous diseases

        Background: Autoimmune bullous disease (AIBD), characterized by the blister formation in the skin and/or mucosa, produces autoantibodies reactive to desmosome and hemidesmosome. Although follicular helper T (Tfh) cells and its cytokines are essential for the formation of the pathogenic antibodies from B cells, the precise role of Tfh cells in AIBD remains unknown. Objectives: The aim of this study was to evaluate the difference and correlation in the level of Tfh cytokines in the serum of AIBD patients. Methods: We evaluated serum cytokine levels of IL-6, IL-10, tumor necrosis factor (TNF), and IL-21 in 14 epidermolysis bullosa acquisita (EBA) patients, 10 bullous pemphigoid (BP) patients, 10 pemphigus vulgaris (PV) patients, 10 pemphigus foliaceus (PF) patients, 6 paraneoplastic pemphigus (PNP) patients, and 18 healthy controls using cytometric bead array. The AIBD patients with moderate to severe diseases were selected by clinical and serological evaluation. Results: Overall, all AIBD groups showed significant increase in IL-6 and IL-10 compared to healthy control group. Additionally, PF group showed increased TNF and IL-21, and EBA group showed increased IL-21 compared to healthy controls. TNF and IL-21 were positively correlated in EBA, BP, PF, and PV groups; IL-10 and TNF in BP and PF groups; IL-6 and IL-10 in PV and PNP groups. Conclusion: Increased level of Tfh cytokines was observed in serum of AIBD patients, which suggests that Tfh cells have a possible role in AIBD pathogenesis.

      • 지하수 오염원 플럭스(flux) 측정 기법 소개

        Measuring groundwater contaminant flux is increasingly being recognized as crucial in order to prioritize contaminated site cleanups, estimate the efficiency of remediation technologies, measure rates of natural attenuation, and apply proper source terms to model groundwater contaminant transport. Flux measurement methods can be categorized as either point methods or integral methods. Point methods(transect and passive flux meter method) measure flux at a specific point or points in the subsurface while integral methods(IGIM, IPT method) use one or more pumping wells to interrogate large volumes of the subsurface. Recent work has investigated the development of an integral method(TRW method) that does not require extraction of contaminated water from the subsurface. Significance and importance of measuring groundwater contaminant mass flux will be introduced at first in this paper. We then review groundwater contaminant flux measurement methods and conclude with a qualitative comparison of the various flux measurement methods.
