        The Working Group on the revision of Clinical Research Methodology for Acupuncture met in Seoul, Republic of Korea from 24 to 26 August 2005. The main objectives of the meeting were to review the regional publication guidelines for Clinical Research on Acupuncture, to make necessary revisions in the guidelines, to make recommendations on further collaboration and activities in the field of research on acupuncture, and to discuss scientific evidence-based approaches in the clinical research on acupuncture. Fourteen members from the eight Member States, one secretariat staff from the WHO Regional Office for the Western Pacific and ten observers from the Republic of Korea attended the meeting. The members presented their papers to review the current status of clinical research on acupuncture. The drafts of proposed revision to the previous guidelines for clinical research methodology on acupuncture were discussed extensively. The issues covered during the discussion included: the definition of the new terms; reorganization of clinical research design; revision and update of contents (e.g., control group); introduction of Institutional Review Board (IRB) on the section of ethical approval; additional outcomes including health-related quality of life (HRQOL), qualitative measures, etc. In the course of these discussions, the working group developed the revised guidelines for clinical research on acupuncture and made recommendations for promoting the dissemination of the revised guidelines.

        멸균방법에 따른 탈회동종골의 세포독성 여부에 관한 실험적 연구

        Procurement, cutting, cleansing, freezing, freeze-drying, and demineralization of the allogeneic bone must be made under the germ-free stable condition without bacterial and/or viral contamination. Even thought the bone is procured under the germ free condition, we must have confidence on disinfection of all the solutions that come in contact with tissue during the whole procedure. Lots of antibacterial agents have been introduced for chemical sterilization. Recently ethylene oxide gas sterilization or radiation sterilization is frequently selected as a secendary sterilization procedure. The biological and biochemical response of the graft material differs with the type and concentration of the sterilizing agents, and various toxic reactions have been reported due to the graft material itself and the substance released by the chemicals. The authors conducted the Millipore filter test to observe the toxic effect on L929 fibroblasts according to the effect on activity of succinate dehydrogenase, during the secondary sterilization of the demineralized allogeneic bone powder with irradiation or ethylene oxide gas. The result were as follows: 1. Around the copper disk, positive control group, 10mm diameter discoloration was observed. 2. As same as the negative control group, the disk showed no discoloration. 3. The demineralized allogeneic bone which was sterilized with ethylene oxide gas or irradiation showed no cytotoxicity. 4. From this results, it is suggested that treatment with ethylene oxide gas or irradiation should be effective to sterilize the deminineralized allogeneic bone.

        레진 시멘트를 이용한 레진 파이버 강화 레진포스트의 치근 상아질에 대한 미세인장결합강도

        Object The purpose of this study were to evaluate the microtensile bond strength of resin fiber reinforced post to radicular dentin using resin cement according to various dentin surface treatment and to observe the interface between post and root dentin under SEM. Material and Method A total 16 extracted human single rooted teeth were used. A lingual access was made using a #245 carbide bur in a high-speed handpiece with copious air water spray. The post space was mechanically enlarged using H-file(up to #60) and Gates Glidden bures(#3). This was followed by refining of the canal space using the calbrating drill set provided in ER Dentinpost(GEBR, BRASSELER GmbH&Co. KG). The 16 teeth were randomly distrubuted into 4 group of 4 teeth. Group 1 teeth had their post space prepared using 10% phosphoric acid as root canal surface treatment agent during 20s. The canal was then rinsed with saline and dried with paper point. Group 2 teeth had their post space prepared using 3% NaOCI as root canal surface treatment agent during 30min. The canal was then rinsed with saline and dried with paper point. Group 3 teeth had their post space prepared using 17% EDTA as root canal surface treatment agent during 1min. The canal was then rinsed with saline and dried with paper point. Group 4 teeth had their post space prepared using 17% EDTA as root canal surface treatment agent during 1 min. After rinsing with saline, the canal was rinced 10ml of 3% NaOCI for 30min. After drying with paper point, the post(ER Dentinpost, GEBR, BRASSELER GmbH&Co. KG) was placed in the treated canals using resin cement. Once the canal was filled with resin cement(Super bond C&B sunmedical co. Ltd.), a lentulo was inserted to the depth of the canal to ensure proper coating of the root canal wall. After 24 hours, acrylic resin blocks(10·15·50mm) were made. The resin block was serially sectioned vertically into stick of 1·1mm. Twenty sticks were prepared from each group. After that, tensile bond strengths for each stick was measured with Microtensile Tester. Failure pattern of the specimen at the interface between post and dentin were observed under SEM. Results 1. Tensile bond strengths(meen±SD) were expressed with ascending order as follows ; group 4, 12.52±6.60 ; group 1, 7.63±5.83 ; group 2, 4.13±2.31 ; group 3, 3.31±1.44. 2. Tensile bond strengths of Group 4 treated with 17% EDTA +3%NaOCI were significant higher than those of group 1, 2 and 3 (p<0.05). 3. Tensile bond strengths of Group 1 treated with 10% phosphoric acid were significant higher than those of group 2 (p<0.05). Tensile bond strengths of Group 4 treated with 17% EDTA +3% NaOCI was significant higher than those of other groups.

        주요우울증이 근로자의 생산성에 미치는 영향 : WHO-HPQ(Health and Work Performance Questionnaire)를 이용한 예비연구

        Objectives : Major depressive disorder (MDD) causes patients' distress and makes socioeconomic burden, both directly and indirectly. We used the concept of lost productive time (LPT) to estimate the indirect costs and calculated both absenteeism and presenteeism among workers with MDD. Mcthods : Depression group was recruited from workers visiting psychiatric outpatient clinic who had MDD without major physical or mental disorders (N= 106). Age and sex matched healthy control group was also recruited through advertisement (M=100). All participants completed a interview using WHO Health and Work Performance Questionnaire (HPQ), Job Stress Measurement Scale for Korean Employees, and Hamilton Rating Scale for Depression. Statistical analysis was performed with independent t-test or χ² test as characteristics of values (p=0.05). Results : The number of absence (0.94-day/month vs. 0.10-day/month, P=0.015) andthe numberofearly leaving (2.56-day/month vs. 0.24-day/month, P<0.001) were significantly higher in the depression group. Depression group evaluated their Perfor-mance level much lower than controls with significant value (5.16 vs. 7.62, P<0.001). In addition, depression group estimated their performance level during the last 4 weeks lower compared to the level of past 1-year (5.16 vs 6.63, P<0.001). The estimated costs of absenteeism in depression group were higher than controls by 2,520,000 Korean Won per year, and those of presenteeism were also higher by 4,880,000 Korean Won per year. The total costs of LPT in depression group were higher than controls by 7,400,000 Korean Won, which corresponds to 26% ofmean annual salary. In addition, the level of occupational stress, such as high demand and interpersonal conflict, was higher in the depression group. Conclusion : Major depressive disorder costs substantial productivity loss to workers and their company. Presenteeism imposes more time cost than absenteeism. Effectiveness trials are needed to devise cost-effective programs for the early detection and treatment of depression at the workplace.

        한국인 주요우울장애 환자에서 혈중 ACTH, Cortisol 농도와 해밀턴 우울 평가 척도의 신체증상 항목과의 상관관계

        Objectives:Hypothalamic-Pituitary-Adrenocortical(HPA) system dysregulation plays an important role in the pathophysiology of major depressive disorder. Medically unexplained somatic symptoms comprised the predominant complaints of korean patients with major depressive disorder. It might be related to dysregulation of HPA system and somatic symptoms. The aim of this study was to investigate the association between Hamilton Depression Rating Scale severity and the Plasma Adrenocorticotropic Hormone(ACTH) & Serum Cortisol level in Korean patients with major depressive disorder. Methods:Our study design was prospective. A total 111 hospitalized patients with major depressive disorder (DSM-IV) at Anam Hospital, Korea University Medical Center between Jan 2009 and May 2011 were selected. Hamilton Depression Rating Scale was measured by psychiatrist. Plasma ACTH, Serum Cortisol levels were analyzed at the time of admission. Results:There are significant correlations(p<0.05) between Hamilton Depression Rating Scale of somatic component(HAM-D 11-15 item) and Plasma ACTH and Serum Cortisol level. However, other HAM-D sub items were not significantly correlated with Plasma ACTH and Serum Cortisol level except somatic component. Conclusion:Our Study results suggest that hyperactivity of HPA axis might be correlation with somatic symptoms in korean patients with major depressive disorder.

        Imaging of Activated Cortical Areas after Light and Electrical Stimulation of the Rabbit Retina: F-18 FDG PET-guided Brain Mapping

        Purpose Using 18F- fluorodeoxyglucose positron emission tomography (FDG PET), we aimed to assess the cortical areas activated by light and electrical stimuli in rabbits and to determine the nature of the electrically evoked visual perception by comparing the 2 activated cortical images. Methods Three adult white rabbits were used, and 5 18F -FDG PET scans were performed on 5 different days for each rabbit: a resting image, 2 light stimulation images, and 2electrical retinal stimulation images by suprachoroidal electrodes of the right and left eyes. A small-animal PET scanner (resolution: 1.18 to 1.45 mm) was used for image acquisition. Activated cortical areas were determined by voxel-wise statistical analysis of the difference between the stimulation and resting PET images, and three-dimensional brain imaging of functional activation was constructed. Results After visual and electrical stimulation of the rabbit retina, increased metabolism was found in 2 distinct cerebral cortical areas: (1) the ipsilateral frontal cortex (frontal eye field) and (2) the contralateral occipito-parietal cortex (visual cortex). Cortical areas activated after electrical retinal stimulation were nearly identical to the visually activated cortical areas. Conclusions Electrical stimulation of the rabbit retina using suprachoroidal electrodes induced a similar cortical activation response as that induced by light stimulation. Brain PET images obtained from stimulation of the retina with light and electric current increase our understanding of cortical perceptions by both stimuli and can be a useful methodology for retinal prosthesis research.

        Case Reports : Resolution of Recalcitrant Uveitic Optic Disc Edema Following Administration of Methotrexate: Two Case Reports

        A 13-year-old male and a 15-year-old female presented with optic disc edema associated with chronic recurrent uveitis. While the ocular inflammation responded to high doses of oral prednisolone, the disc edema showed little improvement. After oral administration of methotrexate, the disc edema and ocular inflammation were resolved, and the dose of oral corticosteroid could be reduced.
