Background: Decompressive laminectomy alone for degenerative lumbar scoliosis (DLS) is not recommended because it can lead to further instability. However, it is uncertain whether instability at the decompressed segments is directly affected by laminectomy or the natural progression of DLS. The purpose of this study was to evaluate the surgical outcome of decompressive laminectomy alone for DLS with spinal stenosis and to determine whether the procedure leads to post-laminectomy instability (PLI). Methods: We retrospectively reviewed 60 patients with DLS. They were divided into 2 groups according to PLI criteria: stable group and PLI group. The PLI group was subdivided into 2 groups based on the level of PLI: the first group that showed PLI at the index laminectomy level (PLI-I) and the second group that showed PLI at another level other than the laminectomy level (PLI-NI). Radiological evaluation was performed to determine factors associated with the progression of DLS. Pain and disability outcomes were assessed. Results: There were 34 patients (56.7%) in the stable group and 26 patients (43.3%) in the PLI group. Twelve patients (20.0%) underwent revision surgery. Eleven patients (18.3%) showed PLI at the index segments (PLI-I group), and 15 patients (25%) showed PLI at the adjacent or cephalad segments, not related to the laminectomy site (PLI-NI group). Four patients underwent revision surgery in the stable group and 8 in the PLI group. Survivorship analyses revealed that the predicted survivorship of DLS was 90.0% at 12 months and 86.4% at 24 months after laminectomy. Conclusions: The development of PLI was not always related to laminectomy at the index level. However, PLI developed more rapidly at the index level, compared to the natural progression of the scoliotic curve at the adjacent segments.
Modulus Modeling of PP/GF Composites by X-ray Micro-CT and Comparison with Experimental Data

본 연구에서는 비파괴적 분석 방법인 X-선 마이크로-CT를 사용하여 폴리프로필렌(PP) 복합체로 사출 성형한 덤벨 시편 내부의 유리 섬유(GF) 분포들을 연구하였다. 폴리프로필렌 복합체의 기계적 물성은 섬유 길이 분포(FLD), 종횡비(AR) 그리고 섬유 배향 분포(FOD)에 영향을 받는다. 사출 몰드의 구조와 사출 방향에 따라 형성된 사출물 내부의 섬유들을 뚜렷하게 분석하기 위해 X-선 마이크로-CT로 2차원, 3차원적 이미지들을 얻었다. 우리는 섬유들이 분수 흐름을 형성하는 수지 흐름의 방향에 따라 기울어져 있다는 것을 확인하였다. 3차원 이미지 내의 섬유들에 대한 FLD, FOD 데이터를 정량화하였다. 정량화한 FLD, AR 그리고 FOD 데이터를 반영한 인장 탄성계수의 이론적 계산 값은 일방향으로 가정한 이론적 계산 값보다 실험치의 값과 유사하다는 것을 확인하였다. 따라서 기울어진 각도를 적용하여 인장 탄성계수를 다시 계산한 값이 실험치와 잘 부합함을 알 수 있었다. We investigated internal distributions of glass fibers (GFs) in injection molded dumbbell specimens of polypropylene (PP) composites by using the nondestructive analysis method. The mechanical properties of PP composites were mainly influenced by the fiber length distribution (FLD), aspect ratio (AR), and fiber orientation distribution (FOD), etc. The 2-D and 3-D images were generated by X-ray micro-CT to clearly analyze the fibers inside the injection mold formed according to the injection mold geometry and injection direction. We could confirm that the fibers were oriented toward the resin flow direction forming a fountain-like flow. The 3D images of the fibers were analyzed to obtain quantitative FLD and FOD data. The theoretical tensile modulus reflected by quantitative FLD, AR and FOD data turned out to be closer to experimental values than the theoretical tensile modulus values obtained only under unidirection. Therefore, the modulus values recalculated by adding tilt angles gave satisfactory result in accordance with the experimental data.

Background/Aim: Many patients with lamivudine-resistant (LAM-R) chronic hepatitis B (CHB) had been treated by switching to entecavir (ETV) 1.0mg. Although rate of resistance to ETV (ETV-R) is reported high, current medical insurance system doesn`t pay-back for change from ETV to other agent in patients whose resistance to ETV was not identified. This study was aimed to stratify ETV therapy in LAM-R patients. Methods: One hundred and ten CHB patients who occurred LMV-R and received ETV 1.0mg up to 5 years were evaluated prospectively. At 12 months of switching to ETV, we divided subjects into non-detection group (HBV DNA<20 IU/mL) and detection group (HBV DNA≥20 IU/mL), which was subdivided into low viral load group (20≤HBV DNA<2,000 IU/mL) and high viral load group (2,000 IU/mL≤ HBV DNA). Virologic response rate (VR; HBV DNA<20 IU/mL) and ETV-R were evaluated as end point. Results: One hundred and ten patents were enrolled. The mean age was 45±11years, the proportion of male and HBeAgpositive patient was 71% (80/110) and 77% (85/110), respectively. The mean serum HBV DNA levels were 6.89±1.03, 3.26±1.81, 3.06±1.82, 2.49±1.53, 2.43±1.35 and 1.73±0.87 log10IU/ ml at baseline, month 12, 24, 36, 48 and 60, respectively. The VR (95% vs. 29%, P<0.001) was higher and ETV-R (10% vs. 54%, P=0.001) was lower in non-detection group than in detection group. The VR (27% vs. 29%, P=0.853) and ETV-R (45% vs. 57%, P=0.367) was not significantly different between low viral load and high viral load group. Resistance to ETV occurred at 26±10.3 months (median 24 months, 12-48 months) in detection group. Conclusion: Resistance rates were high in patients with detectable HBV DNA at 12 months of ETV therapy. Therefore, switching to or adding a potent nucleotide analogue (e.g. tenofovir) is warranted in LAM-R CHB patients whose HBV DNA is detected after 12 months of ETV therapy.
Differences between Patients with TB-Destroyed Lung and Patients with COPD Admitted to the ICU

Background: Although patients with tuberculous-destroyed lung (TDL) account for a significant proportion of those with chronic airflow obstruction, it is difficult to distinguish patients with airway obstruction due to TDL from patients with pure chronic obstructive pulmonary disease (COPD) on initial presentation with dyspnea. We investigated clinical features differing between (i) patients with TDL and airway obstruction and (ii) those with COPD admitted to the intensive care unit (ICU) due to dyspnea. Methods: We reviewed the medical records of patients with TDL who had a forced expiratory volume in 1 second (FEV1)/forced vital capacity (FVC) of <70% on a pulmonary function test (PFT; best value closest to admission) and patients with COPD without a history of pulmonary tuberculosis (TB) who were admitted to the ICU. Ultimately, 16 patients with TDL and 16 with COPD were compared, excluding patients with co-morbidities. Results: The mean ages of the patients with TDL and COPD were 63.7 and 71.2 years, respectively. Mean FVC% (50.4% vs. 71.9%; p<0.01) and mean FEV1% (39.1% vs. 58.4%; p<0.01) were significantly lower in the TDL group than in the COPD group. More frequent consolidation with TB (68.8% vs. 31.3%; p=0.03) and more tracheostomies (50.0% vs. 0.0%; p=0.02) were observed in the TDL than in the COPD group. Conclusion: Upon ICU admission, patients with TDL had TB pneumonia more frequently, more diminished PFT results, and more tracheostomies than patients with COPD.

The purpose of this study is to examine temporal structures of English words beginning with an /s/ plus stop sequence through production experiments with native speakers of Korean learning English and native speakers of English. According to the results of our production experiment, both a beginner and an advanced group of Korean English learners produced /s/ shorter than a following stop, while the opposite pattern was observed in English native speakers' production. An advanced group of Korean English learners were good at producing a stop after /s/ as unaspirated, but their production of a stop following /s/ was different from English native speakers' production in that the closure duration of the stop was much longer.

<P>We report the origin of the improvement of the power conversion efficiency (PCE) of hybrid thin-film solar cells when a soluble C<SUB>60</SUB> derivative, [6,6]-phenyl-C<SUB>61</SUB>-butyric acid methyl ester (PCBM), is introduced as a hole-blocking layer. The PCBM layer could establish better interfacial contact by decreasing the reverse dark-saturation current density, resulting in a decrease in the probability of carrier recombination.
