The Efficacy of Microvascular Decompression in Hemifacial Spasm over the Course of Time

Objective : Microvascular decompression[MVD] at root exit zone[REZ] of the facial nerve has been largely popularized and it has become the standard treatment for patients with hemifacial spasm[HFS]. This sturdy is performed to evaluate the efficacy of MVD over the course of time. Methods : From 1994 to 2003, 50 patients with HFS who underwent MVD were followed up for more than 6 months. We retrospectively analyzed results with medical records and telephone researches. Results : The mean age of patients at the time operation was 57.6 years and 84% of the patients were female. The mean duration of follow-up after operation was 3.4 years [range $0.5{\sim}7.8\;years$]. One day after MVD, 54% of patients had complete relief of spasm immediately. Continuous improvements of HFS were observed during the follow-up period and these improvements were statistically significant with time (P< 0.05). Until 6 months after operation, complete relief of spasm was observed in 84% of patients. The delayed relief of spasm was observed in 35.7% of our patients who experienced complete relief. Conclusion : The efficacy of MVD in HFS is improves with time. Continuous follow-up evaluations for the duration of more than at least 6 months after MVD are important for the decision of its clinical results because delayed relief of spasms occurs.

Cytoplasmic male sterility (CMS) and fertility restoration have been utilized as valuable tools for F_1-hybrid seed production in many crops despite laborious breeding processes. Molecular markers for the selection of CMS-related genes help reduce the expenses and breeding times. A previously reported genomic region containing the Ppr-B gene, which is responsible for restoration of fertility and corresponds to the Rfo locus, was used to develop gene-based or so-called "functional" markers for allelic selection of the restorer-of-fertility gene (Rfo) in F_1-hybrid breeding of radish (Raphanus sativus L.) Polymorphic sequences among Rfo alleles of diverse breeding lines of radish were examined by sequencing the Ppr-B alleles. However, presence of Ppr-B homolog, designated as Ppr-D, interferes on specific PCR amplification of Ppr-B in certain breeding lines. The organization of Ppr-D, resolved by genome walking, revealed extended homology with Ppr-B even in the promoter region. Interestingly, PCR amplification of Ppr-D was repeatedly unsuccessful in certain breeding lines implying the lack of Ppr-D in these radishes. Ppr-B could only be successfully amplified for analysis through designing primers based on the sequences unique to Ppr-B that exclude interference from Ppr-D gene. Four variants of Rfo alleles were identified from 20 breeding lines. A combination of three molecular markers was developed in order to genotype the Rfo locus based on polymorphisms among four different variants. These markers will be useful in facilitating F_1-hybrid cultivar development in radish.

Background: Unilesional mycosis fungoides (MF) is a rare variant of mycosis fungoides, characterized clinically as having solitary skin lesion, and histopathologically as having mycosis fungoides. Objectives: To evaluate clinicopathological, immunohistochemical, and molecular features of unilesional MF. Methods: This study was conducted on 19 patients with a solitary lesion who diagnosed with MF in the authors`` hospital. Results: The patients’ ages ranged from 11 to 68 years. They all had early-stage MF (IA type). Histopathologically, 13 cases of 19 cases were presented as conventional MF. Two cases revealed papillomatosis and hyperkeratosis which are features of verrucous variant of MF. Three cases had features of folliculotropic MF. Another 3 cases revealed prominent epidermotropism of atypical lymphocytes, marked epidermal hyperplasia, and acanthosis, and were diagnosed as Woringer-Kolopp disease. Two of 3 patients of Woringer-Kolopp disease had a CD4/CD8 ratio<1. PCR detection of TCR γ gene rearrangement was conducted in 15 patients and showed monoclonality in 10 patients. All patients showed complete response after treatment. Fourteen of 19 patients were treated with either ultraviolet A1 (9 patients) or photodynamic therapy (5 patients). Conclusion: Unilesional MF is a rare variant of MF, and appears to have a favorable prognosis. Ultraviolet A1 and photodynamic therapy seems to be effective and safe therapy for unilesional MF.

<P>Cytogenetic and hematological analysis was performed on the peripheral blood lymphocytes (PBLs) obtained from Korean native goats bred in two nuclear power plants (Wolsong and Uljin) and a control area. The frequencies of gamma-ray-induced micronuclei (MN) in the cytokinesis-blocked (CB) lymphocytes at several doses were measured in three Korean native goats. The measurements performed after irradiation showed dose-related increases in the MN frequency in each of the donors. The results were analyzed using a linear-quadratic model with a line of best fit of y = 0.1019D + 0.0045D<SUP>2</SUP> + 0.0093 (y = number of MN/CB cells and D = irradiation dose in Gy). The MN rates in the goats from the Wolsong and Uljin nuclear power plant, and the control area were 9.60 ± 2.88, 6.83 ± 1.47 and 9.88 ± 4.32 per 1,000 CB lymphocytes, respectively. The apparent difference is not statistically significant. The MN frequencies of PBLs from goats bred in three areas means that the values are within the background variation in this experiment. The MN frequencies and hematological values were similar regardless of whether the goats were bred in the nuclear power plant or the control area.</P>
