Background: Activins are dimeric growth and differentiation factors which belong to the transforming growth factor-β (TGF-β) superfamily. Objectives: To know the mechanism how activin regulates transcription of lipopolysaccharide (LPS)-induced Toll-like receptors (TLRs), cytokines, and inducible nitric oxide synthase (iNOS) in human melanocytes, and the involvement of nuclear transcription factor-κB (NF-κB) and mitogen-activated protein kinase (MAPK) pathways. Methods: Normal human melanocytes were pretreated with activin A before exposure to LPS. Total RNAs were purified and real-time PCR was performed. Also we conducted immunoblot analysis to know the expression levels of proteins. Results: LPS increased mRNA expressions of TLRs (1-10) and cytokines (IL-1β, IL-6, IL-8, IL-10, TNF-α), and enhanced mRNA and protein expression of iNOS. Activin inhibited LPS-induced mRNA expressions of TLRs and cytokines, and decreased mRNA and protein expression of LPS-induced iNOS. Also activin suppressed NF-κB p65 activation and blocked IκBα degradation in LPS-stimulated melanocytes, and reduced LPS-induced p38 MAPK and MEK/ERK activations. Conclusion: Activin inhibited expression of genes of TLRs, cytokines, and iNOS in LPS-activated normal human melanocytes. Moreover, anti-inflammatory effect of activin was mediated through suppressing activation of NF-κB and MAPKs signaling pathways in LPS-activated normal human melanocytes, resulting in reduced expression of TLRs, cytokines, and nitric oxide.
P088 : Analysis of immune parameters in the patients of psoriasis treated with low dose cyclosporine

Background: Cyclosporine (CsA) is a potent immunosuppressive drug which inhibits the expression of cytokines such as interleukin-2 (IL-2) and proliferation of T cells. It also has been reported that CsA inhibited the lymphocyte proliferative response in a dose dependent fashion. Objectives: To examine the impact of low dose CsA therapy on immune system in patients with psoriasis by the number of T cell, B cell and NK cell. Methods: We analyzed psoriasis patients who had been treated at our hospital between January 2009 and June 2014. They were divided into CsA-treated group and the group who did not receive any immunosuppressant treatments. The counts of CD4 and CD8 T cells, B cell and NK cell from blood sample were analyzed and compared using unpaired t-test. Results: 11 patients had not been treated with any immunosuppressive drugs and 56 patients had received oral CsA medication with 100 mg to 200 mg per day for average 17 months before laboratory test. In patients without anyimmunosuppressive drugs, CD4 is 858±261/ul, CD8 is 610±169/ul, B cell is 260±144/ul and NK cell is 249±167/ul. After treated with low dose cyclosporine, CD4 is 846±308/ul, CD8 is 585±212/ul, B cell is 250±124/ul and NK cell is 346±203/ul. There was no significant difference between the two groups. The whole blood count and lymphocyte percentage of both groups were in the normal range. Conclusion: These results suggest that low-dose CsA therapy in patients with psoriasis does not affect patient’s immune system.
P065 : Treatment of rosacea using intense pulsed light

Background: Rosacea is a chronic disease that affects esthetic appearance of face by erythema and telangiectasias. The use of intense pulsed light (IPL) has be known effective for the treatment of rosacea. Objectives: The aim of this study was to investigate the effectiveness of IPL in treating facial vascular lesion in rosacea patients. Methods: The 20 severe vascular rosacea patients were treated with using the IPL. The derma-spectrometer was used for photodocumentation and analysing of skin lesion. Results: 28 Patients received IPL at 2 week intervals. The performance of IPL assessed by dividing the result to 4 categories: 4 patient was no improvement (0%~25%), 8 patients was mild improvement (25~40%), 12 patients was moderated improvement (40~70%), and 4 patients were marked improvement (70~100%). And there is statistically difference in erythema index on Derma-spectrometer after treatment (p<0.05). Postinflammatory hyperpigmentation was found in 3 patients after IPL treatment and other side effect was not reported. About 3 month after IPL treatment, recurrence of rosacea was noted in 3 patients. Conclusion: This study demonstrated that IPL treatment on facial rosacea is transient effective in obtaining clearance of erythema and telangiectasia, but there is a possibility of recurrence that may due to aggravating factors of rosacea. So IPL could be good treatment option on facial vascular rosacea.
P276 : The impact of different fluence for intense pulsed light in the treatment of melasma

Background: Melasma is a common acquired symmetric brownish macules with irregular borders. Various treatment protocols for melasma have been suggested, but the treatment efficacy and safety varies according to the reports. Objectives: This study was aimed to investigate effect of low fluence IPL for the treatment of melasma. Methods: Korean adults with bilateral melansma randomly assigned 2 grouped and treated with low fluence of IPL weekly during 6 weeks. Melanin and erythema index was scored using a Spectrophotometer. Results: 10J and 13J IPL treatment groups decreased melanin index after 3 weeks. Effect of IPL in melasma is slightly more effect in subjects treated with 13J of IPL than 10J during 9 weeks. 10J and 13J IPL treatment groups decreased melanin index after 3 weeks. Conclusion: We demonstrated that low fluence of IPL effect in melasma after 3 weeks and statistically difference from 3 weeks to 9 weeks than baseline. And slightly more effect in patients treated with 13J of IPL than 10J treatment. We suggest that low fluence IPL treatment for melasma is therapeutic effect with minimal side effects.

Background: Various methods have been used to improve transdermal drug delivery, still but penetration through the stratum corneum limits effectiveness. Objectives: The purpose of this study was to investigate in vitro skin delivery and in vivo protoporphyrin IX accumulation of topically applied 5-aminolevulinic acid(ALA), and assess the degree of enhancement by fractional radiofrequency(RF) pretreatment and sonophoresis post-treatment. Methods: Three male domestic swine were used, and the skin of pig was exposed to fractional RF alone, followed by topical 5-ALA application with and without sonophoresis. Fluorescence intensities were measured for porphyrin fluorescence. Results: Fluorescence intensities of porphyrin ware increased after pretreatment with fractional RF and after sonophoresis post-treatment in the epidermis and dermis. Conclusion: We confirmed that pre-fractional RF and post-treatment with sonophoresis increased transdermal drug penetration.
Atypical presentation of primary cutaneous diffuse large B cell lymphoma on the leg

Primary cutaneous lymphomas are defined as lymphocytic neoplasias that present themselves clinically in the skin without extracutaneous disease at diagnosis and up to 6 months. The skin lesion of primary cutaneous B-cell lymphomas have variable clinical, histopathologic, and immunologic phenotypes. Primary cutaneous lymphomas often show a completely different clinical behavior and prognosis than histologically similar systemic lymphomas, which involve the skin secondarily. A 50-year-old man presented with solitary 3x1 cm sized cresent-shape well demarcated erythematous plaque with tingling sensation on the right lower leg. The skin lesion is gradually increasing from 3 month ago and severe pain was accompanied. Histopathologic findings shows dense infiltration of lymphocyte with atypical features in dermis. Immunohistochemical finding shows positive for CD3, CD20 and BCL-2 but negative for CD 56. Laboratory test was negative result in EBV and normal range of CD4 and CD8 cells. Computed tomography imaging of the chest, abdomen, and pelvis failed to show an associated systemic lymphoma. So he was referred to hematooncology department to further evaluation and to receive R-CHOP (rituximab, cyclophosphamide, hydroxydaunorubicin, Oncovin and prednisolone) which is considered to be the first line of treatment for these lymphomas. We report a case of atypical presentation of cutaneous diffuse large B cell lymphoma occurring on the leg.
