        Background: Extramammary Paget`s disease (EMPD) is a rare cutaneous, intraepithelial adenocarcinoma. Diagnosis is often delayed because of the various clinical presentations and the rarity of the disease. Dermoscopy improves diagnostic accuracy of pigmented and non-pigmented skin tumors. However, dermoscopic features of EMPD had not been thoroughly investigated. Objectives: To evaluate the clinical and dermoscopic characteristics of EMPD. Methods: Clinical and dermoscopic images of 32 histopathologically confirmed EMPD were analyzed. Results: EMPDs were clinically characterized by erythematous patches (100%), scales (62.5%), ulcer (37.5%), nodules (12.5%), focal hyperpigmentation (18.8%), and hypopigmentation (9.4%). Dermoscopically, the following features were detected: erythematous background coloration (100%), surface scales (96.9%), vascular patterns (90.6%), ulcers (43.8%), structureless white areas (12.5%), shiny white lines (12.5%), and pigmentary structures (34.4%). Among vascular patterns, dotted vessels were frequently observed (53.1%), followed by glomerular vessels (21.9%), and polymorphous vessels (12.5%). Conclusion: The results of our study suggest that there are distinctive clinical and dermoscopic features of EMPD. Recognition of the characteristic features of EMPD can facilitate accurate diagnosis of EMPD, and may reduce misdiagnosis.

      • Poster Presentations : P15 ; Change of Epidermal Barrier Function after Ablative and Non-Ablative Fractional Photothermolysis

        Background: Fractional lasers are commonly used for skin rejuvenation and scar correction. Though applying moisturizers has been considered as a usual step for post-laser care to improve physiologic function of skin, little is known about their efficacy. Objective: To investigate the changes in epidermal barrier function with or without the use of moisturizer after fractional photothermolysis in human skin Methods: A total of 29 healthy volunteers, age ranging from 27 to 35 years were enrolled. All subjects underwent 10,600nm ablative fractional CO2 laser and 1,550nm non-ablative fractional erbium-doped yttrium aluminum garnet laser on the inner surface of both arms with or without the use of palmitoylethanolamide (Physiogel AI repair cream®, GSK Korea, Seoul, South Korea). To elucidate whether the moisturizer could improve barrier impairment after laser irradiation, palmitoylethanolamide was applied 3 times a day for 1 week on one side of arm. Epidermal barrier function including transepidermal water loss (TEWL), water capacitance and surface colorwere assessed during the recoveryperiod. Results: Both types of fractional laser led to an immediate increase of TEWL, level of erythema and pigmentation, and these parameters recovered within 1 week. On the contrary, water capacitance was decreased after laser irradiation and remained lower than the baseline for similar period of time. TEWL was increased to a lesser extent when palmitoylethanolamide was regularly applied after 10,600nm ablative fractional CO2 laser. However, after irradiating 1,550nm non-ablative fractional erbium-doped yttrium aluminum garnet laser, there was no difference of TEWL, water capacitance and surface color regardless of using moisturizer. Conclusion: Our study suggests that the use of moisturizer after ablative fractional CO2 laser helps to restore impaired epidermal barrier function. However, applying moisturizer after fractional laser irradiation is not enough to compensate skin reactions including erythema and pigmentation.

        Background: Sometimes the clinical differentiation between seborrheic keratosis(SK) and verruca plana(VP) could be challenged. However, there have been no studies for this issue to date. Objectives: To elucidate clinical and dermoscopic differences of two diseases. Methods: The patient who has the lesions clinically considered as VP or SK was target of our study. We took clinical and dermoscopic photos with informed consent and conducted a questionnaire. HPV-DNA chip testing was also done in volunteer patients. Treatment modalities and prognosis were investigated retrospectively. Results: Thirty-four patients were enrolled in our study. Thirteen patients were finally diagnosed as SK(38.2%) and 21patients as VP(61.8%). The average age of first occurrence was older in SK(p= 0.192) than VP, and koebnization rate was higher in VP patients(p=0.143). The percentage accompanying itch was similar in two groups(p=0.923), but size variability and homogeneity of the shape, color, and surface texture showed some differences. Spontaneous resolution was also observed more in VP and dermoscopic photo showed more vascular pattern in VP. Conclusion: This study seems to be unique in that this is the first trial to find the clinical and dermoscopic differences in clinically ambiguous cases between SK and VP.We suggest that not only the age, koebnization, and spontaneous resolution but also homogeneity of color, and vascular patterns in dermoscopic view could be good clues to differentiate two diseases.

      • P090 : Assessment of characteristics of itch in patients with hand eczema

        Background: Hand eczema is a common inflammatory dermatosis which influences the quality of life especially when it comes to accompanying itching. However, there is no research regarding the characteristics of itch and neurologic association in hand eczema so far. Objectives: To objectify the influence on quality of life in patients with hand eczema and to investigate the association with cutaneous nerve, Methods: we conduct a hand eczema severity scoring using HECSI score, a questionnaire contains Leuvin itch scale and dermatology life quality index(DLQI), and a neurologic exam using von-Frey filament and neurometer. Results: Forty-four patients were enrolled the study and 36 patients were also done neurologic examination. Itch occurred at least once daily in all study patients, and finger and palm were the most commonly affected itch areas (25.0%). The Leuvin itch scale was directly proportional to HECSI score (p=0.272), and DLQI was inversely proportional to it (p=0.019). The sensory threshold force measured by von-Frey filament was significantly higher in lesion than normal skin (p<0.05) and the pain threshold using neurometer was also significantly decreased in lesion (p<0.05). Conclusion: This study is a unique trial which describes the characteristics of itch experienced in hand eczema and investigates the relationship between hand eczema and cutaneous nerve. We thought it could be a good basis in development of future therapeutic modalities such as neurotransmitters in hand eczema.
