      • Patterns of Upper Aero-digestive Tract Cancers in Kamrup Urban District of Assam: A Retrospective Study

        Background: The incidence of upper aero-digestive tract (UADT) cancers, including C00-C14, C30-C32, C15 and C16, is increasing rapidly in Kamrup Urban District (KUD) of Assam, North East (NE) India. According to the NCRP (2013) report 37.6% of all cancers in both sexes are UADT cancers in the NE region, accounting for 53.3% in males and about 27.5% in females of the total cases. Materials and Methods: A retrospective study was conducted for patient information from the period of 2008-2011. Age-standardized or age-adjusted rates (ASR or AAR) (per 100,000 person-years) were calculated using the World Standard Population as proposed by Segi and modified by Doll et al. The registry population area at risk was estimated using the 1991 and 2001 census population by sex, as well as the growth rate during that interval using the difference distribution method. Results: There were 5,638 cases registered during the last four years of the study (2008-2011) accounting for 56.7% (3,198/5,638) of the total in males and 43.3% (2,440/5,638) in females. The male: female ratio was 1.31:1.00. The overall age adjusted rates (AAR) were 179.4 and 153.8 per 100 000 males and females respectively. Cancer of the oesophagus was most common in both sexes, with most appreciable gender variation for tongue and hypopharynx, presumably reflecting differential expsoure to risk factors.

      • Molecular and Circulatory Expression of Insulin Growth Factors in Indian Females with Advanced Cervical Cancer

        Background: Recent studies have demonstrated an association between insulin growth factor (IGF) and insulin growth factor binding protein-3 (IGFBP-III) serum levels and increased risk for various cancers. However, little information is available on clinical implications of the IGF system in Indian patients with cervical cancer. This study explored associations by analyzing their expression profiles in cervical cancer cases. Materials and Methods: Totals of 50 patients with advanced cervical cancer and 40 healthy controls were enrolled. Human papillomavirus (HPV) and cervical biopsy sample were obtained from all participating women. Circulatory levels were estimated by ELISA and the tissue expression was assessed using RT-PCR and Western blotting. Results: Levels of IGF-I and II showed significant increase whereas IGFBP-III showed significant decline in all patients as compared to controls. Spearman correlation analysis between IGFs and HPV status showed significant correlations. Conclusions: We demonstrated elevated circulating levels and tissue expression of IGF-I and IGF-II in advancer cancer cervix patients, as compared with controls, with a converse trend being apparent for IGFBP-III. In future, associations of the IGF system and clinical outcome of cervical cancer patients in post treatment samples might point to significance in disease mapping as a prognostic marker after validation with a larger patient series.

      • Descriptive Report on Pattern of Variation in Cancer Cases within Selected Ethnic Groups in Kamrup Urban District of Assam, 2009-2011

        Background: The global burden of cancer is continuously increasing. According to recent report of the National Cancer Registry Programme (NCRP) on time trends it is estimated that future burden of cancer cases for India in 2020 will be 1,320,928. It is well known that knowledge of the incidence of cancer is a fundamental requirement of rational planning and monitoring of cancer control programs. It would help health planners to formulate public health policy if relevant ethnic groups were considered. North East-India alone contains over 160 Scheduled Tribes and 400 other sub-tribal communities and groups, whose cancer incidence rates are high compared to mainland India. As since no previous study was done focusing on ethnicity, the present investigation was performed. Materials and Methods: In this paper PBCR-Guwahati data on all cancer registrations from January 2009 to December 2011 for residents of the Kamrup Urban District, comprising an area of 261.8 sq. km with a total population of 900,518, including individual records with information on sex, age, ethnicity and cancer site are provided. Descriptive statistics including age adjusted rates (AARs) were taken as provided by NCRP. For comparison of proportional incidence ratios (PIR) the Student's t test was used, with p<0.05 considered as statistically significant. Results and Conclusions: Differences in leading sites of Kamrup Urban District since from the beginning of the PBCR-Guwahati were revealed among different ethnic groups by this study. The results should help policy makers to formulate different strategies to control the level of burden as well as for treatment planning. This study also suggests that age is an important factor of cancer among different ethnic populations as well as for overall population of Kamrup District of Assam.

        USE OF QUANTUM MECHANICAL FRAGMENTATION APPROACH IN DECAY DYNAMICS OF 221FR NUCLEUS

        The ground state and excited state decay dynamics of 221Fr nucleus is studied using the quantum mechanical fragmentation theory (QMFT) based methodology. The decay paths of ground state 221Fr nucleus, i.e. α-decay, cluster emission and spontaneous fission (SF), are explored within the preformed cluster model (PCM). The relative contribution of these decay modes is investigated in terms of fragmentation potential and preformation probability P0. The calculations are made for the spherical as well as deformed choice of decaying fragments. Here, the deformation effects are included upto quadrupole (β2) shapes of nuclei with ‘hot-compact’ and ‘cold-elongated’ optimum orientations. The halflife times (T1/2) of α particle and 14C cluster are calculated using β2-deformed fragmentation, and find reasonable agreement with the experimental data. The spontaneous fission decay fragments are identified for 221Fr parent nucleus and corresponding decay half-life time is predicted. Further, the excited state decay of 221Fr compound nucleus formed in 19F+202Pt reaction is studied at excitation energy E*CN=10 MeV within the framework of the dynamical cluster-decay model (DCM). Note that DCM is an extended version of PCM, where temperature (T) and angular momentum (ℓ) are duly incorporated. The evaporation residue (ER) and fusion-fission (ff) cross-sections are calculated using hot-compact fragmentation approach. Finally, a relative study of spontaneous fission and induced fission is presented in terms of barrier characteristics and potential energy surfaces.

      • Cancer: Scenario and Relationship of Different Geographical Areas of the Globe with Special Reference to North East-India

        Background: Cancer is becoming the most important public health burden around the globe. As per the GLOBOCAN 2008 estimates, about 12.7 million cancer cases and 7.6 million cancer deaths were estimated to have occurred in 2008. The burden of cancer cases for India in the year 2020 is calculated to be 1,148,757 (male 534,353; female 614,404) compared to 979,786 in 2010. The pattern of cancer incidence is varying among geographical regions, esophageal cancer for example being high in China, lung cancer in USA, and gallbladder cancer in Chile. The question remains why? Is it due to the diversity in genome pool, food habits, risk factor association and role of genetic susceptibility or some other factors associated with it? In India, the North East (NE)-India region is seeing a marked increase in cancer incidence and deaths, with a very different cancer incidence pattern compared to mainland India. The genome pool of the region is also quite distinct from the rest of India. Northeastern tribes are quite distinct from other groups; they are more closely related to East Asians than to other Indians. In this paper an attempt was made to see whether there is any similarity among the pattern of cancer incidence cases for different sites of NE-India region to South or East-Asia. Materials and Methods: Principal Component Analysis (PCA), Hierarchical Cluster Analysis (HCA), Pearson Correlation coefficient test was assessed to evaluate the linkage of North-East India region to other regions. A p value <0.05 was considered as statistically significant. Results: The results clearly shows that there are similarities in occurrence of cancer incidence patterns for various cancer sites of NE-India with South and East-Asian regions, which may lead to the conclusion that there might be a genetic linkage between these regions.

        Effects of zolpidem on sleep parameters in patients with cirrhosis and sleep disturbances: A randomized, placebo-controlled trial

        Background/Aims: The aim of this study was to study the efficacy and safety of zolpidem for sleep disturbances in patients with cirrhosis. Methods: Fifty-two Child-Turcotte-Pugh (CTP) class A or B cirrhotics with Pittsburgh Sleep Quality Index >5 were randomized to either zolpidem 5 mg daily (n=26) or placebo (n=26) for 4 weeks. Results: The therapy of 4 weeks was completed by 23 patients receiving zolpidem (3 stopped treatment due to excessive daytime drowsiness) and 24 receiving placebo (2 refused to continue the study). In the zolpidem group, after 4 weeks of therapy, there was significant increase in total sleep time (TST) and sleep efficiency compared to baseline and improvement in polysomnographic parameters of sleep initiation and maintenance (i.e., decrease in sleep latency time, decrease in wake time, and decreases in number of arousals and periodic limbs movements per hour of sleep), without any significant change in sleep architecture. Conclusions: Four weeks of 5 mg daily zolpidem in CTP class A or B cirrhosis patients with insomnia led to significant increases in TST and sleep efficiency and improvement in polysomnographic parameters of sleep initiation and maintenance without any significant change in sleep architecture.

        Unplanned 30-day readmission rates in patients undergoing endo-urological surgeries for upper urinary tract calculi

        Purpose: To see the 30-day unplanned readmission rates in patients underdoing endo-urological surgeries for upper urinary tract calculi we conducted this retrospective study at King George's Medical University, Lucknow, India. Unplanned readmissions not only add to healthcare costs but also are bothersome for the patients. There are many studies on 30-day unplanned readmissions in general surgical patients. Although similar studies have been done in certain urological procedures, no study has reported readmission rates or its risk factors in patients undergoing surgeries for upper urinary tract calculi. Materials and Methods: We retrospectively reviewed our prospectively maintained database from 1st January 2009 to 31st December 2017, for the patients who underwent endo-urological procedures for upper urinary tract calculi and identified the patients who were re-admitted within 30 days of discharge. Results: Out of the total 3,209 patients undergoing endo-urological procedures for upper urinary tract calculi 56 were re-admitted. The readmission rate was 1.74% over the study period. The most common etiology for readmission was sepsis followed by hematuria. The significant risk factors for readmission in bivariate analysis included male gender, age >65 years, current smoking, chronic obstructive pulmonary disease, diabetes mellitus, bleeding disorder, prior cardiac disease, and American Society of Anesthesiologists (ASA) class ≥3. In multivariate risk adjusted logistic regression analysis ASA class ≥3 was the only independent risk factor for readmission. Conclusions: The readmission rates in endo-urological procedures for urolithiasis are less compared to other procedures. ASA class ≥3 is the most important independent predictor of unplanned 30-day readmissions.

      • Performance Analysis of Different Wavelet Families Over Fading Environments for Mobile WiMAX System

        Orthogonal Frequency Division Multiplexing (OFDM) has made tremendous improvement possible in wireless technology. Wavelet analysis is used as an alternative to Fourier analysis in OFDM system to transform and map data onto orthogonal subcarriers, as it is a time-frequency domain operation thus it offers the advantages of optimal flexibility with less complexity. This paper presents performance analysis in terms of Bit Error rate (BER) of different wavelet families in OFDM System under the joint effect of path loss, multipath fading and noisy environment for Mobile WiMAX. The results obtained show that the performance of the mobile-multipath environments for 16-QAM modulation schemes is enhanced as Daubechies and Biorthogonal wavelet families offers lower BER with different Path loss model. The performance measurements and analysis was done in simulation developed in MATLAB.

      • Static and harmonic analysis of moderately thick square sandwich plate using FEM

        In this paper, sandwich plate, constructed with orthotropic and isotropic composite materials, is analyzed to obtain the static and harmonic behavior. The analysis is done by using ANSYS APDL FEM tool. A solid-shell 190 and an 8-node solid 185 elements are employed for face and core material respectively to analyze the plate. Results was attained by using Reissner-Mindlin theory. Effect of increasing thickness ratio of face sheet to depth of the plate is presented on static, vibration and harmonic response on the sheet and the results are discussed briefly. Published work in open domain was used to validate the results and observed excellent agreement. It can be stated that proposed model presents results with remarkable accuracy. Results are obtained to reduce the weight of the plate and minimizing the vibration amplitudes.
