        The study on the stability of cavity in rock and mechanical behavior of its surrounding rock mass provides the fundamental basis for developing underground space. When the isotropic rock contained the multi-circular holes under the uniaxial compression, the gradient changes were 8 steps to the compression direction, a = 0˙, 15˙, 20˙, 30˙,35˙, 45˙, 60˙, 90˙ and the parameters were presented as follows : 1. 3 steps of hole diameter change : φ = 3, 5, 7 ㎜φ 2. 3 steps of displacement rate change : C = 0.05, 0.1, 0.2 ㎜/min On this study, we worked to determine the fracture process and strength properties. The results wore summerized as follow ; 1) From this test, stress-strain curves were divided into 5 types, and influenced by gradient change. Namely, a - 0˙ and Intact were contained with Type Ⅰ , a = 15˙∼ 35˙ were contained with Type Ⅱ, a = 45˙ and 60˙ were contained with Type Ⅲ, a = 90˙ was contained with Type Ⅳ. Except for hole diameter 3 ㎜φ, a = 15˙∼ 60˙ were contained with Type Ⅴ. 2) Fracture type of specimen was divided into 4 types by gradient change mainly. Type (a) was influenced by Tension Force. Type (b) and Type(c) were influenced by mainly Tension Force and Compression Force, also they accompanied with shear stress of working along holes gradient direction on the specimen center. Type (d) was broken to perpendicular of the load direction by Compression Force. It was corresponded to relationship between stress-strain curves of 1) and fracture type 2).

      • Helicobacter pylori 항원을 이용한 면역우유 생산 및 항체특성에 관한 연구

        본 연구는 위염, 위궤양, 위림프종 및 위암과 같은 소화기 질환의 원인균으로 알려진 Helicobacter pylori균을 항원으로 하여 젖소에 면역시킨 후 생산된 우유의 anti-H. pylorigkdcp의 생성능을 검토하고, 백신투여량과 항체 생성과의 관계, 항원 항체의 특이성, H. Pylori균 응집력, 항체의 산과 열에 대한 안정성, 그리고 백신투여가 젖소에 미치는 영향을 알아보았다. 1. 백신 투여량에 따른 혈청과 유청내의 anti-H. pylori항체의 함량은 10㎖, 20㎖, 30㎖ 백신투여 모든 군에서 대조구에 비해서 높은 양의 항체 생성을 확인하였다. 유의적인 차이는 없었으나 20㎖투여가 항체 생성이 가장 많은 것으로 조사되었다. 백신 투여량에 따른 유청내의 anti-H. pylori항체 생성량은 혈청에서 나타난 결과와 유사한 양상으로 형성되었다. 2. Anti-H. pylori항체의 SDS-PAGE에 의한 분자량측정 결과 heavy chain은 50kDa정도, light chain은 24kDa정도로 확인 되었다. 3. H. pylori항원 단백질의 분자량측정 결과 12개의 band가 형성되었다. Anti-H. pylori의 항원 특이성을 알아보기 위해 western bloting을 한 결과 혈청, 혈청정제, 유청, 유청 정제 모두 7개의 항원성 물질을 확인할 수 있었고, 주 항 원성 물질은 분자량이 97, 66, 34kDa 이었다. 4. 응집반응결과 유청속의 anti-H. pylori항체가 H. pylori균에 대해 1/10의 응집가를 나타내었다. 5. Anti-H. pylori항체의 산·알칼리에 대한 안정성 실험에서는 pH 5 ∼ pH 10 범위에서 안정한 상태로 100%의 활성을 나타내었다. 6. Anti-H. pylori항체의 열에 대한 안정성 실험에서는 60℃에서 60분간 안정한 상태를 보였고, 70℃에서도 비교적 안정한 상태였으나 60분 경과후 40%정도 활성이 감소하였다. 80℃에서는 4분간 처리했을 때 77%의 활성을 유지하였고, 100℃에서도 1분간은 비교적 안정한 상태였다. 7. 백신투요로 인하여 유량이 12% 감소하는 경향을 나타냈고, 최장 1주일 정도 지나면서 회복되었다. 8. 백신투여 후 젖소의 체온을 측정한 결과 대조구에 비해 정상적인 범위 내에서 체온이 상승하는 경향을 나타내었다. This study has been to examine bio-function of anti-H. pylori antibodies of milk produced from cows immune with antigen germ of Helicobacter pylori and search the relation between vaccine dosage volume and antibody formation, peculiarity of antigen antibody, cohesion of H. pylori germ, stability about add and heat of antibody, and impact of vaccine dosage on cows. The content of serum and Anti-H. pylori antibody within whey in accordance with vaccine dosage volume has confirmed the formation of high-quantified antibody compared to the controlled conditions in all groups vaccine dosages of 10㎖, 20㎖, and 30㎖. It has been turned out that the antibody was farmed most in 20㎖ dosage on while there was no attention difference. The molecular weight of Anti-H. pylori antibody measured by SDS-PAGE were turned out as about 50kDa in the heavy chain and about 24kDa in the light chain. 12 bands were formed as the result of measured molecular weight of antibody protein. The western blotting was performed in order to examine the antigen peculiarity of Anti-H. pylori that all 7 antigen substances including serum, serum refining, whey and whey refining could be confirmed and the main antigen substances were 97, 66, 34kDa of molecular weight. As a result of cohesive response Anti-H. pylori antibody in whey showed 1/10 cohesive rate about H. pylori germ. In stability test about acid and alkali of antibody there was 100% activated in the range of pH 5-pH 10. In stability test about heat it showed stable condition in 60℃: for 60 minutes and comparatively stable condition in 70℃, but reduced activation to 40% after 60 minutes. It maintained 77% activation in 80℃ for 4 minutes and comparatively stable in 100℃ for I minute. It was inclined to reduce to 12% of flow caused by vaccine injection, but recovered after about maximum 1 week. In measurement of body temperature of cows after vaccine injected, it was inclined to rise with the normal scope in comparison with the controlled conditions.

When the isotropic rock contained the multi-circular holes under the uniaxial compression, we have been studies on the fracture properties of rock mass then the gradient changes were 8 steps to the compression direction, α=0°, 15°, 20°, 30°, 35°, 45° ,60°, 90° and the parameters were presented as follows: 1. 3 steps of hole diameter change : Ø = 3, 5, 7mmØ 2. 3 steps of displacement rate change. : C = 0.05, 0.1, 0.2mm/mm As a result of this study, even if parameters(change of hole diameter and displacement rate) influenced to the properties of fracture strengths, they influenced less then gradient changes.

      • 불연속면을 갖는 암석의 역학적 거동에 관한 연구

        To construct a structure in rock, first of all, the mechanical behavior of the rock should be understood. Most of rocks in situ have contained mechanical weak planes which greatly influenced on the mechanical behavior of the rock. On this study, hard rock was used to investigate in influence of the weak plane on the peak strength and displacement behavior. The results were summarized as follows; 1. When the angle of a weak plane(α) was 30。, the lowest peak strength was appeared. And the peak strengths between 30。to 45。were increased with increasing angles of weak plane. But the anisotropy of peak strength by the angle of weak plane was not remarkable in the comparision with that of soft rock, Tuff. The reason is the influence of the angle of the internal friction of the weak plane. 2. Different from soft rock, the peak strength was higher α= 60。than 90。with the lowest rate of displacement (1㎛/sec) in case of hard rock. It shows that the peak strength of weak plane is chiefly influenced by coefficient of internal friction rather than cohesion.

        Nifedipine 투여로 증상 호전을 보인 일측성 다발성 피부 모평활근종 1예

        We report a case multiple piloleiomyoma in a 50-year-old man. About 30 years ago, multiple red colored firm papules with pain developed on the left lower leg. The skin lesions gradually increased in number and in and size arranging in a group. Some, lesion became more painful after cold exposure. About a few years ago, new lesions developed on the left back and flank abdomen, The histologic findings showed poorly demarcated and were composed of interlacing bundles of smooth muscle fibers with varying amounts of collagen bundles being intermingled. Masson's trichrome and smooth muscle actin stain showed typical leiomyoma, and s-100 stains revealed an increased amount of nerve fibers interlacing within muscle fibers and the surrounding tissue. We diagnosed our case as unilateral multiple piloleiomyoma arising from arrector pilorum muscle. Pain was ameliorated with oral nifedipine alone.

        The objective of this study is to predict the high risk group of coronary artery disease from body mass index, blood pressure, fasting blood sugar and serum lipids level using the method of discriminant analysis. Variables with significant difference between coronary artery disease group and normal group were HDS-C, LDL-C/HDL-C, LDL-C, and TG. But the acuity did not exceed 70% in any single variable. As the results of discriminant analysis, statistically significant discriminant variables were HDL-C, T-chol, FBS and TG. The discriminant equation was(Y) = -0.06759(HDL-C) + 0.01498(T-chol),+0.01923(FBS) - 0.01421(TG) + 0.5187, and the overall discriminant power using the above 4 variables was 79.3%

      • 좌측흉수로 발현된 Boerhaave 증후군 1예

        구토 후 발생한 토혈을 주소로 내원한 환자에서 좌측 늑막 삼출 동반 시 식도 파열의 가능성을 고려하여 흉수 천자 및 지속적인 흉부 단순 촬영의 추적관찰이 필요하며 식도 파열이 배제된 후 환자의 생체 징후 및 토혈 양을 파악하여 내시경 검사 시기를 신중히 결정하는 것이 좋을 것으로 생각된다. 본 증례는 토혈을 주소로 내원하여 Mallory-Weiss 증후군 의심 하에 내시경을 통한 치료 시행 후 추적 관찰에 의해 진단된 Boerhaave 증후군을 경험하였기에 보고하는 바이다. Boerhaave's syndrome is a spontaneous perforation of the esophagus most commonly resulting from a sudden increase in intraesophageal pressure combined with negative intrathoracic pressure caused by straining or vomiting. Surgery should generally be performed within 24 hours because mortality approached 30 to 50 percent in which surgery was delayed. Therefore, early diagnosis and prompt surgical repair are critical for survival. We have experienced a case of Boerhaave's syndrome in a patient presenting with hematemesis with left pleural effusion without any signs or symptoms of pneumomediastinum or pneumothorax.

        Successful Kidney Transplantation using Deceased Donor Kidney with Acute Tubular Necrosis

        We present here a case of success kidney transplantation from a deceased donor who showed a serum creatinine level of 5.5 mg/dl. A 52-year-old Asian female with blood type O was determined to be brain death after a compressive craniectomy. Her post-operative course was fatal. She had experienced acute renal failure with oliguria, and these conditions had persisted for 3 days, and the serum creatinine level was elevated up to 5.5 mg/dl. The random urine creatinine, sodium and osmolarity levels were 17.6 mg/dl, 116 mmol/L and 346 mOsmol/L, respectively. However, the ultrasonographic and pathologic findings did not show any chronic structural abnormality in the kidneys. Two female patients with blood type O were transplanted with these donor kidneys. Both recipients experienced 2∼3 days of delayed graft function, but the graft function was fully recovered at 4∼5 day after transplantation. Both recipients were discharged with full-functioning grafts (1.1 and 0.9 mg/dl of serum creatinine, respectively). This shows that a deceased donor with reversible kidney damage, and even one showing a high serum creatinine level, can be acceptable for transplantation if the absence of chronic renal damage is confirmed by pathologic study.

        Sirolimus/steroids Maintenance Therapy after Early Cyclosporine Withdrawal

        Purpose: Sirolimus has potent anti-rejection activity as well as the ability to prolong allograft survival and reduce nephrotoxicity. This study was designed to evaluate the efficacy and safety of sirolimus in Korean de novo renal transplantation. Methods: We included 79 patients who received sirolimus at nine Korean transplantation centers in the intention-to-treat and valid-for-safety analyses. The study was an open, single treatment arm multicenter trial with 12 months of patient follow-up. Initially, patients received 2 ㎎ of sirolimus (after 6 ㎎ of loading does) with cyclosporine and steroids. Sirolimus was administered for up to 12 months. Antibody induction was not used. At 3 months after transplantation, cyclosporine was progressively withdrawn over 4 to 8 weeks while sirolimus was adjusted to obtain trough concentrations within 15∼30 ng/㎖ up to 6 months and concentrations within 12∼24 ng/㎖ between 7 and 12 months. Results: The proportion of patients who completed the 12-month sirolimus medication per protocol was 74.7% (59/79). Cyclosporine withdrawal was possible in 64 recipients (81.0%). Fifteen patients discontinued sirolimus before cyclosporine withdrawal, and 5 recipients did so after successful cyclosporine withdrawal. Most common causes of sirolimus discontinuation were graft rejection (n=8). Incidence of biopsy-proven acute rejection within 6 months after transplantation was 15.2%. Patient and graft survival rates at 12 months post transplantation were 97.5% and 96.2%, respectively. During the study period, three graft losses occurred by patient death. Conclusion: Based on this study, cyclosporine and sirolimus induction followed by cyclosporine withdrawal at 3 months post-transplant is considered to be efficient and safe after primary renal transplantation.
