Background The role of 6 min-walk test (6MWT) and cardiopulmonary exercise test (CPET) for preoperative evaluation of non-small cell lung cancer (NSCLC) has not been well established in Asian population. This retrospective study investigated the correlation between the exercise tests and their effect on postoperative cardiopulmonary complications and treatment outcomes. Methods We included the subjects with stage I-II NSCLC who received lobectomy or greater extent surgery and underwent 6MWT and CPET simultaneously due to reduced pulmonary function, which was defined as predicted postoperative (ppo) FEV1 or ppoDLCO <60%. Study population was classified into four different groups according to the cut-values of the 6MWD and VO2max for risk stratification. Postoperative pulmonary complications (PPCs), postoperative cardiac complications (PCCs), and all-cause mortality were investigated. Results Between 2015 and 2020, a total of 191 subjects were enrolled. Median values of the 6MWD and VO2max were 462 m and 23.3 mL/kg/min, respectively and the 6WMD was well correlated with the VO2max (r=0.409, p<0.001). The cut-off values of 450 m for the 6MWD and 20 mL/kg/min for the VO2max were identified as optimal ones for risk stratification. The rates of PCCs and all-cause mortality were significantly different among the four groups (p=0.005 and p=0.001). However, no difference was found in PPCs among the four groups. On multivariate analysis, a greater exercise capacity (6MWD ≥450 m and VO2max ≥20 mL/kg/min) was significantly associated with a low rate of PCCs (adjusted OR, 0.40; 95% CI, 0.19-0.84; p=0.016), and a decreased all-cause mortality (aOR, 0.26; 95% CI, 0.08-0.79; p=0.017). Conclusions Exercise capacity was associated with PCCs and all-cause mortality in patients with NSCLC who underwent surgical resection. Indicators of 6MWT and CPET can help to select the optimal surgical candidates for NSCLC patients with reduced pulmonary function.

<P><B>Abstract</B></P> <P>Molecular drug susceptibility testing (DST) for antituberculosis drugs is important for improving the efficacy of multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB) treatment. In this study, we developed a molecular high-throughput assay system based on allele-specific primer extension (ASPE) and MagPlex-TAG microspheres, referred to here as TAG-ASPE, which can detect mutations related to resistance to injectable second-line drugs and fluoroquinolones. Target genes were amplified by multiplex PCR using DNA from H37Rv and 190 clinical <I>Mycobacterium tuberculosis</I> strains and extended by ASPE using 22 ASPE primers. ASPE products were then sorted on the TAG-ASPE array and detected using a Luminex 200 system. The performance of the TAG-ASPE method was compared with that of sequencing and phenotypic DST. Comparison of the TAG-ASPE method with sequencing showed that the sensitivity and specificity of the TAG-ASPE method were 100% [95% confidence interval (CI), 96.38–100%] and 100% (95% CI, 95.70–100%) for the <I>rrs</I> gene and 100% (95% CI, 96.90–100%) and 100% (95% CI, 95.07–100%) for the <I>gyrA</I> gene, respectively. Compared with phenotypic DST, the sensitivity and specificity of the TAG-ASPE method for detecting drug-resistance mutations against injectable second-line drugs were 92.52% (95% CI, 85.8–96.72%) and 98.7% (95% CI, 92.98–99.97%), respectively. Additionally, the sensitivity and specificity for fluoroquinolone-resistance detection were 85.4% (95% CI, 78.36–90.85%) and 100% (95% CI, 92.38–100%), respectively. The results of this study demonstrate that the TAG-ASPE method can effectively detect mutations conferring resistance to second-line antituberculosis drugs in numerous clinical specimens.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A high-throughput assay was developed to detect TB drug-resistance mutations. </LI> <LI> The assay employs allele-specific primer extension and MagPlex-TAG microspheres. </LI> <LI> The diagnostic method, as well as sequencing, detected drug-resistance mutations. </LI> </UL> </P>

Background: Advancement of technology has improved diagnostic sensitivity of radial endobronchial ultrasound (EBUS). Also, image characteristics of radial EBUS provide additional information of risk for malignancy of a lung nodule. However, when indeterminate radial EBUS guided biopsy results, such as atypical cell, fibrosis and inflammation are reported, there is no consensus among doctors whether to consider re-biopsy or surgery, or to observe with follow up. In this study, first, we identified malignancy associated risk factors from radial EBUS image findings, clinical and radiological factors. Second, based on identified risk factors, we constructed a nomogram to predict the risk of malignancy in indeterminate biopsy results. Methods: This retrospective designed study included 157 patients, diagnosed as indeterminate with radial EBUS guided biopsy from January 2017 to December 2018. Medical records, Chest Computed tomography (CT), radial EBUS images and final diagnosis results were reviewed. Patients were randomly divided into training and validation sets. The factors related to malignancy were identified through logistic regression analysis, and nomogram was constructed using the training set and subsequently applied to the validation set. Results: Through univariate and multivariate analysis, upper lobe location, spiculation, presence of satellite nodules, echogenicity, presence of dot or linear arc and patency of vessels and bronchus were identified as predictors. The nomogram was constructed based on predictors. The area under the receiver operating characteristic curve (AUC) value of constructed nomogram was 0.858 (0.786-0.930) using radiological factors, and improved to 0.952 (0.914-0.990) when radial EBUS factors was added. The calibration curve showed optimal agreement between the malignant probability as predicted by nomogram and the actual probability. Conclusions: The nomogram constructed in this study could be utilized in predicting the probability of malignancy in lung nodules diagnosed as indeterminate on radial EBUS, and may help in selecting patients who are at high risk for malignancy.
Treatment of patients with confirmed or suspicious tuberculous granulomas in resected lung nodule

Abstract: After surgical resection for the diagnosis of indeterminate lung nodule, tuberculous (TB) granulomas are often suspected or diagnosed based on the pathologic reports. There is little information about treatment patterns among physicians in patients with confirmed or suspicious TB granuloma that are negative in culture and have only lung nodules on chest computed tomography (CT). Method : From January 2010 to March 2013, patients with pulmonary nodules that pathologically identified granuloma were retrieved ant reviewed retrospectively. Cases in which a cause was evident other than pulmonary tuberculosis at the time of initial evaluation were excluded. The analysis cohort comprised 263 patients of resected lung nodules with diagnosed or suspicious TB granuloma. Clinical and laboratory information was abstracted from medical records. Result : Caseous necrosis in histology accounted for 80 cases (30.4%). TB-PCR and AFB culture in the tissue samples was performed in 40 cases (15%) and 23 cases (8%), respectively. A total of 68 patients were diagnosed as TB granuloma by definition, 61 (89%) of whom received standard TB therapy and 60 (88%) completed the treatment. There were no patients treated with LTBI, and 7 (10%) were followed up without medication. A total of 166 patients were diagnosed with suspected TB granuloma, of which 32 (19.2%) received standard TB therapy and 30 (18.0%) completed treatment. Only one patient was treated with LTBI and 133 (80.1%) were observed without medication. Conclusion : Among the patients with pathologically confirmed granuloma after lung resection surgery, approximately 25.8% of patients were diagnosed with TB granuloma, and most of them received standard TB treatment. The management for 74.1% of patients suspected of tuberculosis granulomas but without clear evidence of tuberculosis differed depending on the physicians they manage.
Celastrol binds to ERK and inhibits FcεRI signaling to exert an anti-allergic effect

<P><B>Abstract</B></P><P>The role of celastrol, a triterpene extracted from the Chinese “Thunder of God Vine,” in allergic inflammation was investigated. Celastrol decreased the secretion of β-hexosaminidase, decreased the release of histamine, decreased the expression of Th2 cytokines and decreased calcium influx and cell adhesion in antigen-stimulated RBL2H3 cells. Exposure to celastrol decreased the phosphorylation of extracellular regulated kinase (ERK) and the ERK kinase activity was decreased in RBL2H3 cells. A molecular dynamics simulation showed binding of celastrol to a large pocket in ERK2, which serves as the ATP-binding site. Exposure to celastrol inhibited the interaction between immunoglobulin Fc epsilon receptor I (FcεRIγ) and ERK and inhibited interaction between FcεRIγ and protein kinase C δ (PKCδ). Antigen stimulation induced an interaction between Rac1 and ERK as well as an interaction between Rac1 and PKCδ. Inhibition of ERK decreased Rac1 activity and inhibition of Rac1 decreased ERK activity in antigen-stimulated RBL2H3 cells. Celastrol regulated the expression of epithelial–mesenchymal transition (EMT)-related proteins through inhibition of PKCα, PKCδ, and Rac1 in antigen-stimulated RBL2H3 cells. Exposure to celatrol inhibited PKCδ activity in antigen-stimulated RBL2H3 cells. Celastrol exerted a negative effect on FcεRIβ signaling by inhibiting the interaction between heat shock protein 90 (hsp90) and proteins, such as, FcεRIβ, Akt and PKCα. Celastrol exerted a negative effect on <I>in vivo</I> atopic dermatitis induced by 2, 4-dinitrofluorobenzene (DNFB), which requires ERK. Celastrol also showed an inhibitory effect on skin inflammation induced by phorbol myristate acetate (PMA) in Balb/c mice. In summary, celastrol binds to ERK and inhibits FcεRI signaling to exert an anti-inflammatory effect.</P>

In this paper, a 3-bridge LLC resonant converter with auxiliary switches for a wide output voltage control range is presented. The proposed converter can be controlled in the following two ways to achieve a wide controllable output voltage control range of Vo~3Vo. The first control mechanism is done through pulse width modulation (PM) of the auxiliary switches and the primary switching devices. The second control mechanism is done through frequency modulation (FM) of the primary switching devices configured to operate in the fullbridge switching modes, when the auxiliary switches are 50% duty ratio or turned-off. The feasibility of the proposed converter is verified with experimental results of a 5kW prototype.
