실험적 항기저막 사구체신염에 재조합 Uteroglobin 투여가 미치는 영향

목 적 : 면역복합체 침착으로 인한 사구체신염은 우리나라 말기신부전의 중요한 원인이다. 최근 면역조절기능을 가진 uteroglobin(UG)을 유전적으로 결핍시킨 동물모델에서 인체의 IgA 신염과 같은 사구체신염의 발생이 보고된 바 있다. 저자들은 생쥐에서 항기저막 사구체신염(aGBMGN)을 발생시키고 인체 재조합 UG를 이용하여 신염의 발생에 UG가 미치는 영향을 확인하고 신염 억제기전을 파악하고자 하였다. 방 법 : 토끼에서 생산한 Anti-GBM Ab 4.5 ㎎을 8주령의 생쥐(C57BL/6)에 정맥주사하여 aGBM-GN을 유발시킨 후, UG를 투여한 치료군, 질환대조군, 정상대조군에서 신염의 정도를 비교하였다. 또한 생쥐의 메산지움세포를 LPS로 자극한 후 UG에 의한 세포증식의 저해정도를 비교하였다. 결 과: 신염 유도 후 7일째 치료군은 정상대조군에 비해 요단백량/creatinine 값은 차이를 나타내지 않았지만, 질환대조군에 비해서는 유의한 감소를 나타내었다. 이러한 UG의 효과는 사구 체신염이 유도된 14일째까지 유지되었으며, 21일째에는 치료군과 질환대조군에서의 요단백/creatinine 값이 정상대조군에 비해 감소하였다. 조직학적으로 신염이 유도된 7일째, 질환대조군에서는 메산지움세포의 증식과 사구체의 50% 이상에서 세포성 반월상이 관찰되었으나, 치료군은 정상대조군과 유사한 조직학적 소견을 보였다. 생쥐 메산지움세포를 LPS로 자극한 후 UG를 투여 하였을 경우 LPS에 의한 세포 증식효과는 유의한 감소를 보였다. 결 론: 실험적으로 유도된 항기저막 사구체신염에 인체 재조합 UG를 투여함으로써 항기저막 사구체신염의 발생을 억제하였으며, 그 기전으로 메산지움세포 활성화 억제 가능성을 제시하였다. 이러한 결과는 신질환 진행 고위험군 집단의 유전자치료에도 이용될 수 있는 근거를 제시하였다고 할 수 있다. Glomerulonephritis(GN) is characterized by cognate immune responses against self or non-self antigen. It is suggested that the crescentic GN is a manifestation of cell-mediated immune response akin to delayed type hypersensitivity. Uteroglobin(UG) is a steroid-dependent, immunomodulatory, and cytokine-like protein. It was reported that UG prevented fibronectin(Fn) deposition in the glomeruli of normal mice to form Fn-UG heterodimers that competed with Fn self-aggregation. We hypothesized that UG would prevent the development of experimental GN induced by anti-glomerular basement membrane globulin(anti-GBM Ab) in mice through immunomodulatory properties. GN was induced by intravenous injection of 4.5㎎ rabbit anti-GBM Ab to mice(C57BL/6). Renal injury was evaluated at 7, 14, and 21 days thereafter. UG-treated mice(n=10) were received for 3 days(0.5 ㎎/mouse/day) beginning 1 hour after anti-GBM Ab injection. Also, disease-control mice(n=10) were received PBS for 3 days after anti-GBM Ab. Proteinuria was significantly reduced in the mice treated with UG when compared with the disease-control mice after 7 and 14 days of anti-GBM Ab injection. The amount of proteinuria was similar between UG treated and normal control mice. The mesangial matrix expansion and cellular crescent were markedly attenuated by the injection of UG. The proliferative responses of mesangial cells(C57BL/6) to LPS were blunted with the addition of UG in dose-dependent manner. In this study, we revealed the preventive effects of UG in the experimental model of glomerulonephritis. This result in turn could provide the basis for the treatment of human disease such as chronic glomerulonephritis.

The comminuted intercondylar fracture of the distal humerus is rather uncommon injury. Because of anatomic complexity of the distal humerus. any incongrousness makes loss of function of the elbow joint. Hence, for the complete restoration of the articular surface and joint motion, wide exposure is necessary, while stable internal fixation and early post operative exercise should bc conducted. With the frequency of comminution and displacement. this intraarticular fracture is difficult to treat. But the fabrication of new implants and development of surgical approach method has increased the reliability of operative stabilization. The authors compared 16 patients of distal humeral fracture treated with transolecranon approach and Campbell's posterior approach at The Department of Orthopaedic Surgery. College of Medicine, Soonchunhyang University from Dec. 1991 to Oct. 1994 and following results were obtained: In transolecranon approach, the operation time was slightly longer with technical difficulties. However, we had excellent exposure of posterior aspect of lower end of the humerus and had a good range of motion. especially in flexion contracture compared with posterior approach. Also, we hardly observed complications of fracture of the olecranon in tansolecranon approach.
Injection Molding of Vertebral Fixed Cage Implant

A vertebral cage is a hollow medical device which is used in spine forgery. By implanting the cage into the spine column, it is possible to restore disc and relieve pressure on the nerve roots. Most cages have been made of titanium alloys but they detract the biocompatibility. Currently PEEK (polyether ether ketone) if applied to various implants because it has good properties like heat resistance, chemical resistance, strength, and especially biocompatibility. A new shape of vertebral cage is designed and injection molding of PEEK is considered for production. Before injection molding of the cage, it is needed to evaluate process conditions and properties of the final product. Variables affecting the shrinkage of the cage are considered, e.g., injection time, packing pressure, mold temperature, and melt temperature. By using the numerical simula-tion program, MOLDFLOW, several cases are studied. Data files obtained by MOLDFLOW analysis are used for stress anal-ysis with ABAQUS, and shrinkage and residual stress fields are predicted. With these results, optimum process conditions are determined.

Spindle epithelial tumor with thymus-like differentiation (SETTLE) is a very rare tumor of the thyroid gland mostly occurring in young patients. The imaging findings of SETTLE tumors are yet to be defined. However, they are usually described as well-defined heterogeneously enhanced masses on CT scan. The current case has the potential growth as compared with a 2009 chest radiography. We took into account the possibility of SETTLE in the case of a bulky mass in patients over 70 years old, particularly in the lower neck. Herein, we report a case of the oldest patient so far. The patient underwent a right lobectomy of the thyroid and mass excision. Follow-up CT scans after 6 months revealed no local recurrence. Surgery is the gold standard treatment for SETTLE. Chemotherapy and radiotherapy could be another possible option for patients with advanced stage SETTLE.

Congenital hypohidrotic ectodermal dysplasia is a rare genodermatosis. It is characterized by hypohidrosis hypotrichosis, dental hypoplasia and characterstic facial features, which reflect a wide constellation of developmental defec of tissue from the ectoderm. We have encountered three cases of congenital hypohidrotic ectodermal dysplasia in a 28-year-old female, her new-born baby, and a 10-month-old boy with a family history. All of the three patients had hypohidrosis, hypotrichosis, defective dentition, and characterstic facial features, which were characterstic features of this disorder. In addition, they showed dry skin, sparse and thin hairs. Histopathologic findings of previous cases revealed no eccrine gland structure in the dermis with routine and immunohistochemical stainning such cytokeratin and filaggrin. We report three typical cases of hypohidrotic ectodermal dysplasia with the review of literature.
