간문부 담관암 환자에서 경피경관 양측성 금속 배액관의 효과

Background: The purpose of this study was to determine the effectiveness of percutaneously place self-expanding metallic stent for the relief of biliary obstruction in patients with hilar cholangiocarcinoma. Materials and Methods: From November 2001 to December 2004, 48 patients with hilar cholangiocarcinoma were prospectively studied. After percutaneous placement of bilateral self-expanding, uncovered metallic stents, follow-up evaluation was carried out until July 2005. Results: There were 4 cased of Bismuth type Ⅱ,21 cases of Bismuth typeⅢa 8 cases of Bismuth type Ⅲb and 15 cases of Bismuth type Ⅳ. Stent placement was technically successful in all patients All patients had satisfactory biliary drainage, resulting in one week drainage rate of 72.8% and final drainage of 91.9% There were 12 cases (21.3%) of abdominal pain requiring analgesics and 1 case (7.1%) of cholangitis; both were successfully managed with conservative treatments. Late complications occurred in four patient (8.3%), including two patients with cholangitis, one patient with liver abscess, and one patient with biloma; all were appropriately managed by percutaneous drainage. The average length and median durations of stent patency and median survival time were 303 days (range, 60~815) and 338 days (range, 60 ~1175), respectively. Conclusion: Placement of a percutaneous metallic stent is an effective and safe method of palliation of patients with hilar cholangiocarcinoma.
성인형 당뇨병 환자에서 피부생검을 이용한 표피하 모세혈관 기저막 두께의 변화에 관한 연구

Thickening and proliferation of capillary basement membrane is a generalized phenomenon in diabetes mellitus and has been described in many organs including heart, kidney, pancreas, retina etc. While such changes are specific, it is difficult to obtain specimens from those organs. We took the specimens from the medial surface of the thing of 33 diabetics and 4 healthy persons as controls by means of punch biopsy. The measurement carried out by normogram and electron microscopic technique. HbA_(1c) values were also measured simultaneously. 1. The HbA_(1c) values are higher in diabetics than in control group (p<0.01). 2. The subepidermal capillary basement membrane thickness of the diabetics : 30% of the 5 th decade population, 53.9% of the 6 rh decade population and 83.3% of the 7th decade population showed more rhan 3,000 A. Whereas all the controls showed below 3,000 A. 3. The subepidermal capillary basement membrane thickness did nor increase significantly with the duration of the disease. 4. In cases of high subepidermal capillary basement membrane thickness, HbA_(1c) showed significant increase. (p<0.01).
화목피 추출물이 Streptozotocin 으로 유발시킨 흰쥐의 당뇨와 Free Radical 에 미치는 효과

Type I diabetes is caused by destruction of the pancreatic beta cells responsible for insulin-secretion. It can be induced by the drugs alloxan or streptozotocin (STZ). Both an increase in reaction oxygen free radical species and a decrease in the antioxidant defense mechanism lead to the increase in oxidative stress in diabetes mellitus. Oxygen radicals scavengers are effective in preventing diagetes in animal models. Experiments were conducted by two system. The Sprague-Dawley rats were treated with STZ intravenous injection after oral administrated Betula's extract and quercetin for a week, and were administrated them for a week. In vitro, Betula's extracts and quercetin showed a tendency towards inhibition of free radical oxidation in all. In vivo, Betula's extracts group exhibited suppression morphologic change in pancreatic islet compare quercetin group on light microscopy. The Betula's extract significantly inhibited formation of MDA compared to the STZ-induced diabetic group in pancreas.They also significantly inhibited formation of Phosphatidylcholine hydroperoxide(PCOOH) compared to the STZ-induced diabetic group in pancreas and plasma, which considered defense against destruction in beta cells. The protection by Betula's extracts may be due to scavenging of deleterious and highly reactive free radical which is generated by STZ. In streptozotocin-induced diabetic rats, the effect of Betula's extract are greater than that of quercetin. Based on this result, we concluded that Betula's extract has the ability to effectively suppress in vitro and in vivo oxidation, thereby providing additional evidence for their potential beneficial type I diabetes.

A two years old intact female Minature Schnauzer was referred to the Veterinary Medical Teaching Hospital of Chungnam National University with an intermittent seizure by traffic accident. No specific finding was found by radiography and computer tomography (CT). Hematologic and blood chemical examination revealed that PCV (62.5%), BUN (72.6 mg/dl), CK (1769 U/l), ALT (169 U/l) and ALP (353 U/l) were increased. This dog was recovered by medical treatment.
Shape Optimization Using the Navier-Stokes Equations in an e-Science Environment

In this research, we implement an aerodynamic design shape optimization technique to a 3- dimensional wing in an e-Science environment. As an advanced computing toolkit, the Globus is used as a middleware connecting the geographically-distributed and heterogeneous computing resources. Before solving the grand problem in this environment, we experimented with geometrically simple 2- and 3-dimensional wing design problems in the e-Science environment. We introduce a brief overview of e-Science and an efficient design optimization algorithm. The experiments were performed on a simple Grid computing testbed consisting of Linux PC clusters. On the basis of this work, an aerodynamic design shape optimization technique will be implemented to a large-scale complicated design problem. The Gird is a communication service that collaborates dispersed high performance computers, large-scale databases and modern equipments so that those can be shared and worked together. In this paper, CFD research based on National Grid project is discussed. To validate the Grid technology, the flow past ONERA M6 wing and the flow past infinite wing are simulated on the National Grid testbed.

In this paper, we suggest how to build the future oriented education environment including the advanced R&D results, that is basically a simulation based learning environment called EDISON (EDucation-research Integration through Simulation On the Net). EDISON is like a cyber education environment, science gateway, or portal for the research and education of the computational science communities. The purpose of this system is to accelerate research and advance education by building and providing the converged cyber environment for the research and education with the research outcomes and developed softwares from the academia and research labs. The EDISON system structure consists of Science-AppStore (Science-Application Store) for managing simulation codes and analyzing program information, the PSE(Problem Solving Environment) in which users can carry out simulation, the cyber infrastructure for providing services to use various computing resources which comprises supercomputers, high performance networks. This system can be used in teaching and researching. With the advantages, the system has been used in CFD classes(e.g. homework, term project and paper publication) in at least 16 universities and more than 1,000 students in 2011.
공간 및 시간 정확도 향상을 위한 최적의 삽간영역 구성에 관한 연구

The spatial accuracy becomes first-order when second-order conservation schemes including the non-conservative interpolation in general Chimera method are used. To ensure the solution accuracy, the discontinuities must be located away from the overlapped regions, and the length of overlapped region also must be proportional to the grid spacing. In this paper, a proposed method, cut-paste algorithm, is used to satisfy above constraints. The cut-paste algorithm can generate the optimal mesh interface region automatically. To validate the spatial and temporal accuracy due to the non-conservative interpolation, inviscid and viscous problems are tested.
