        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

        한국철학 : 삼봉 정도전의 벽이단론과 그 해석 문제 -심문천답과 심기리편을 중심으로

        이 논문은 조선전기 삼봉三峰 정도전鄭道傳(1342∼1398)의 벽이단闢異端(Excluding Heresy) 문제와 그 해석을 다룬 글이다. 조선 건국의 이념과 제도와 문물의 이론적 초석을 제공한 정도전의 대표적인 벽이단서로 간주된 일련의 저술 심문천답心問天答(1375), 심기리편心氣理篇(1394), 불씨잡변佛氏雜辨(1398), 특히 앞의 두 저술에 나타난 벽이단 문제를 다루었다. 정도전의 벽이단 특히 불교비판 문제에 대한 이해와 왜곡은 삼봉 사후부터 현대까지 계속되어 왔다. 정도전의 세 저술에 나타난 벽이단론의 변화는 유교의 자기 반성, 도교와 불교의 정치적 비판과 유교 중심적 수용, 그리고 정치적 필요성에 따른 교세의 축소를 위한 정책적 비판의 방식으로 전개된다. 그럼에도 불구하고 위의 세 저술이 그의 대표적 도교와 불교비판서로 간주된 것은 정도전의 글 그 자체에 기인한 것이라기보다, 오히려 권근의 주석과 서문에 나타난 평가에 기초한 것이다. 설상가상으로 연구자들이 갖고 있던 오해의 소지는 심에 대한 개념이 변화되었음에도 불구하고 불씨잡변에 나타난 불교비판을 전제로 앞의 두 저술에 무비판적으로 이해하였고, 심기리편에 나타난 개념을 심문천답에까지 소급 적용시켜 해석하였기 때문이다. 심문천답에 나타난 개념은 성리학적 심 개념으로 허령지각한 마음이 천리를 지각하고 선을 행할 수 있도록 하는 상식적 판단의 주체였다면, 심기리편에 나타난 마음은 불교의 일체의 인연화합이 일어나는 장인 주관적이면서도 초탈적인 심을 은유한 것이었다. 그리고 정도전의 일련의 저술에 일관되게 나타난 벽이단의 정신은 다름 아닌 인욕적 인심에 대한 천리天理이었으며, 주관적 마음과 육체적 기질에 대한 생리生理이자 의리義理였고, 인연화합의 허구적 심리현상에 대한 실리實理라는 기준이었다. 즉 정도전이 비판한 대상은 성리학적 리理의 필요충분조건을 충족시키지 못한 당시의 세태에 대한 비판이었다. 그리고 그 내용은 유교의 자기비판이었고, 유교를 통해 도교와 불교의 폐해를 극복하려 하였으며, 유교의 공고화와 부흥을 위해 불교의 폐단을 비판하는 것이었다. 이 고찰을 통하여 정도전의 벽이단론은 성리학적 정체성 확립을 위한 유학의 자기 비판적 태도인 동시에 도교와 불교의 포용 혹은 조화라는 융복합적 가능성이 포함되어 있으며, 정치적 이유로 불교 비판의 목소리가 점진적으로 강해진 것임을 알 수 있을 것이다. 부차적으로 거시적 안목에서 현대사회에서 유교의 벽이단론이 갖는 의미를 반추해 보는 한편, 과거의 종교대립과 갈등, 그리고 화합의 모색 문제를 재해석함으로써 종교다원주의 사회 속에 나타난 종교적 갈등 문제를 해결할 수 있는 방안을 모색해 볼 수 있을 것이다. This thesis focuses itself on understanding of anti-heresy view of Early Chosun Neo-Confucianism - viewpoint of Confucianism toward Buddhism and Taoism. More specifically, this thesis discusses the criteria and of heresy while focusing on the understanding of Sambong Jeong, Do-Jeon (1342~1398), who contributed to foundation of Chosun dynasty. Jeong, Do-Jeon`s writings on philosophy, economics and politics, political exclusion of Buddhism, and his writing on criticism of Buddhism show different attitudes. Former studies suggested that Jeong, Do-Jeon criticized Buddhism and Taoism. However, this theory would like to add another view point that Jeong, Do-Jeon also suggested harmony and tolerance. The contemporary reaction of buddhists shall also be given in this theory. Through this study, it can suggest a way to solve religious conflicts and make harmony by re-interpreting religious conflicts of past, as well as ruminate the meanings of anti-heresy of Neo-Confucianism in current society`s view Anti-heresy of Neo-Confucianism is a view point that excludes heresy to protect the original. Anti-heresy, which developed in early Chin dynasty (an ancient Chinese empire) by Confucius, became magnified in Neo-Confucianism while criticizing Buddhism and Taoism. Scholars of Neo-Confucianism in late Korea dynasty and Chosun dynasty used anti-heresy in political and scholarly matters. Jeong, Do-Jeon was representative scholar of anti-heresy. His writings also are considered representative books of Early-Chosun anti-heresy, especially focusing on criticizing Buddhism. Jeong, Do-Jeon aimed for ideal society where ideas of Neo-Confucianism are realized. He strictly applied such ideas in founding principle, national system, and culture. As Neo-Confucianism of Chinese Song dynasty focused on criticizing Buddhism and Taoism, Jeong, Do-Jeon kept the criticism. However, his first two writings have certain issues on being classified as anti-heresy writings. That is because the two books rather focus on enlightenment and convergence instead of exclusion. Nevertheless, the two books are considered as anti-heresy writings because Kwon, Keun judged so in his annotation and preface. Sim-Mun-Chun-Dab(心問天答) is a book of question and answer, not inter-criticism between Buddhism and Confucianism. This book actually criticized personal desires that set national security and stability of people`s lives away. Therefore, this book is a book that focuses on criticizing people who follow personal desire and set Confucian principles away. Sim-Ki-Li-Pyeon(心氣理篇) was thought of as the first major criticism toward Buddhism and Taoism as it was written after Jeong, Do-Jeon`s exclusion of Buddhism in 1391. However, this book is rather a book of enlightenment than a book of criticism. Sim, Gi, and Ri represent Buddhism, Taoism, and Confucianism respectably. This book was composed of three chapters, which are Sim-Nan-Ki(心難氣), Ki-Nan-Sim(氣難心) and Li-Yu-Sim-Ki (理諭心氣). The chapters` contents are Criticism of Taoism by Buddhism, Criticism of Buddhism by Taoism, and Enlightenment of Buddhism and Taoism by Confucianism. Therefore, the book is about leading Buddhism and Taoism toward Confucianism by converging them in the view point of Confucianism. Bul-Ssi-Jab-Byeon (佛氏雜辨) is a book that criticizes Buddhism, and was written only a little prior to his death. However, due to political reason, this book was not published for certain period of time. Although it has the form of criticism of Buddhism, the content actually focuses on criticism of negative phenomenon of Buddhism rather than the theory and concept of Buddhism. Thus, it would be more valid to regard criticism of Buddhism as what was derived from realistic and political reason for tactical purposes. Therefore, it is more valid to regard this criticism as what was overestimated due to Jeong, Do-Jeon`s political standpoint. Also, such evaluation was more from Kwon, Keun`s annotation and preface, instead of from the book`s original message. It would be more likely to understand that Jeong, Do-Jeon rather tried to find way to keep Taoism and Buddhism in check, and make way to future together in harmony. There is an idea that the three books have in common, which is that Confucianism is a theory of loyalty and pursuits live principles, and the that heresy is bounded to gain and loss and pursuits hollow ideas. In general term, heresy that he suggested seems to be Taoism and Buddhism from historical background. However, heresy can be any group that focuses on profit instead of loyalty, and fixed principle instead of live principle. From such view point, it can be considered that his criticism not only was made toward Taoism and Buddhism, but Confucianism itself was also target of criticism.

      • 새로운 연노출의 효과지표로서의 혈장 δ-aminolevulinic acid 측정의 유용성

        To evaluate the usefulness of the measurement of plasma δ - aminolevulinic acid (ALAP) as a new effect indicator of lead exposure, the association of ALAP with blood lead, DMSA chelatable lead and bone lead with a special emphasis of genetic interaction of ALAD polymorphism was studied with 419 lead exposed workers and 85 non-lead exposed workers. Lead workers were recruited from storage battery industries, secondary smelting industries and other lead using industries and non-lead workers selected from electronic refrigerator manufacturing industry. Blood lead, DMSA chelatable lead and bone lead were chosen for parameters of lead exposure. Plasma and urinary ALA were selected for the effect parameter of lead intoxication. Information for age, smoking, drinking habit and body mass index were also obtained. Job duration for lead workers was also collected. The overall prevalence of the variant allele, ALAD type 1-2 or 2-2(ALAD2) in lead exposed workers was 10.0% (42 out of 419 lead workers) which was not differed from control workers (9.4% : 8 out of 85). The means blood lead and DMSA chelatable lead of lead workers with ALAD 1(ALAD type 1-1) were lower than those of lead workers with ALAD2, but the differences were not statistically significant. On the other hand, the mean of tibia lead of lead workers with ALADI was higher than that of lead workers with ALAD2, but the difference was not statistically significant. DMSA chelatable lead showed highest correlation with log transformed plasma ALA(LALAP)(r=0.682) and blood lead and tibia lead had r=0.650, r=0,402 correlation coefficient with LALAP respectively On the other hand. log transformed urinary ALA(LALAU) had lower correlation coefficient with all three parameters of lead exposure than LALAP. The equation of robustic simple linear regression of blood lead on LALAP was LALAP=2.0439+0.0165PbB in ALADI lead workers and LALAP=2.031+0.0121PbB in ALAD2 lead workers. ALAD genotype in the multiple regression analysis of blood lead with LALAP after adjusting for possible confounders(sex, ago, BMI, smoking and drinking status and job duration) showed significant main effect on LALAP resulting lower LALAP in ALAD2 lead workers than ALADI lead workers. On the other hand, effect modification of ALAD genotype was observed in the multiple regression analysis of tibia lead with LALAP after adjusting for possible confounders, but there was no significant effect of ALAD genotype in the multiple regression analysis of DMSA chelatable lead with LALAP after covariate adjustment. With above results, plasma ALA was found to be a useful effect parameter of lead exposure compared with urinary ALA. It was also observed that ALAD polymorphism made significant effect on the association of blood and tibia lead with ALAP. Thc lower plasma ALA in ALAD2 lead workers suggested supportive effect of ALAD2 in lead exposure.

        A 60-year-old man was admitted to our hospital complaining of fever with chillness and anarexia for 12 weeks. Physical examination revealed supraclavicular lymphadenopathy and left inguinal lymphadenopathy. Abdominal CT scan showed the evidence of maked lymphadenopathy on retraperitoneal, external iliac, obturator, left inguinal, and celiac lymph nodes. A pathologic diagnosis of Ki-1(+) lymphoma was made by the biopsed supraclavicular lymph node because the node consisted of large cells with pleomorphic nuclei, scanty cytoplasm, large basophilic nucleoli, atypical vacuoli and large cells were positive for leukocyte common antigen, Ber-H2, marker, and B-cell marker.

      • 업종에 따른 연 취급 사업장의 기중 연 농도 및 연 노출 수준 평가

        In order to obtain an useful information for health management and biological monitoring of lead exposed workers, Authors tried to investigate air lead level and the lead exposed level of workers in lead industry according to occupational category. The subjects in our study were 2074 workers in 7 lead-using industries, and study subjects were divided into 4 occupational categories such as storage battery industry (type 1), primary smelting industry (type 2), secondary smelting industry (type 3) and litharge making industry (type 4). Blood zincprotoporphyrin concentration (ZPP), blood lead concentration (PbB) and urinary δ -aminolevulinic acid (ALAU), hemoglobin (Hb), hematocrit (Hct) were selected as the indices of lead exposure. Personal variables such as age, work duration were also collected. The results were as follows. 1. The geometric mean air lead in 9 lead-using industry was 0.1133±4.3120 ㎎/㎥, and that in type 1, 2, 3 and 4 was 0.1038±3.4952 ㎎/㎥, 0.0429±3.4329 ㎎/㎥, 0.1877±2.5123 ㎎/㎥ and 0.9961±5.2910 ㎎/㎥, respectively. 2. The mean ZPP in 9 lead-using industry was 53.1±28.0 ㎍/㎗, and that in type 1, 2, 3 and 4 was 52.0±24.8 ㎍/㎗, 48.0±16.4 ㎍/㎗, 109.8±85.6 ㎍/㎗ and 74.3±37.8 ㎍/㎗, respectively. There was significant difference in ZPP according to occupational category (P<0.01). The percents of lead workers whose ZPP were above 100 ㎍/㎗ in type 1, 2, 3 and 4 were 4.0%, 1.7%, 34.3% and 21.6%, respectively. 3. The mean PbB in lead-using industry was 26.0±11.2 ㎍/㎗, and that in type 1, 2, 3 and 4 was 25.3±11.1 ㎍/㎗, 26.7±8.8 ㎍/㎗, 50.3±15.2 ㎍/㎗ and 36.4±11.0 ㎍/㎗, respectively. There was significant difference in PbB according to occupational category (P<0.01). The percents of lead workers whose PbB were above 40 ㎍/㎗ in type 1, 2, 3 and 4 were 10.4%, 7.7% 71.9% and 43.2%, respectively. 4. While the correlation of Hb corrected PbB with ZPP was higher than non corrected PbB, the correlation of log-transformed ZPP with PbB was higher than non corrected ZPP. 5. Simple linear regressions of PbB and corrected PbB as independent variable with ZPP, log-transformed ZPP and ALAU as dependent variable were statistically significant (P0.01). Coefficient of determination of corrected PbB with other variables was higher than non corrected PbB. As the result of this study, it was suggested that reconsideration of environmental and biological monitoring program was highly recommended for secondary smelting and litharge making industry.

        日帝强占期 群山의 都市景觀變遷에 關한 硏究

        The main purpose of this study is to find city identity factors of Kunsan. To find local identity in today, maps were analyzed to examine formation of landscape and buildings. To find out landscape transition in Kunsan, we have analyzed the important time of city organization development by using past maps and literature. The maps used in this study were made in 1899, 1902, 1906,1911, 1916, 1923, and 1935s. Teh results are as follows : Something that modernized Kunsan city was General Foreign Settlement after opening its port. This is because they needed to export rice to Japan. Japanese had monopoly in General Foreign Settlement. Residential areas were developed into two divisions and provincial office was moved to Middle street. It led the city to expand to east-south areas. For convenience of exporting rice, harbor facilities were filled up. And by making recourses, the city was expanded to east. Buildings were constructed in groups at Haemang-dong, Walmyong-dong, and Jungangro. Important buildings include the Kunsan custom house, the police station, Nakasaki 18 bank, Dongkik temple, Chosun bank, Lee,yong-chun’s house, the Hyros’s house, and Walmyong-dong Japanese style houses. This study on transition used using map analysis but the accurate boundary of important relics will be necessary in more deeply study.

      • Gelatin 액화세균의 생장억제에 관한 연구

        Gelatin 을 재료로 이용하는 식품의 폐기물에서 gelatin 을 액화시키는 능력이 특히 뛰어난 4종의 세균을 분리·동정하였으며, 배지내에 NaCI, KI·I₂, CuSO₄등의 억제제를 농도별로 처리했을때 나타나는 4균주의 생존도 및 액화능의 변화를 비교·검토하였고, 생존도에 미치는 pH의 영향도 조사하였다. 또한 MIC(Minimal Inhibitory Concentration) Test를 시행하여 항생제에 대한 4균주의 MIC도 조사하였다. 그 결과는 다음과 같다. 1. 4균주중 H strain은 Serratia liquefaciens로, Y₁strain은 Enterobacter속으로, Y₂와 Y₃strain 은 Pseudomonas 속으로 동정되었다. 2. 배지내에 억제제로 NaCl을 처리했을 경우, 7% 농도에서 생장이 100% 억제되었으며, KI·I₂처리시에는 20ppm 에서부터 생장이 억제되기 시작하여 100ppm에서는 4개의 균주 모두 100% 생장이 억제되었다. CuSO₄처리시에는 100ppm에서부터 억제효과를 나타내기 시작하여 300ppm에서는 거의 100% 억제 효과를 보였다. 생존도에 미치는 pH의 영향을 살펴본 결과, pH 5.0, 5.5, 6.0에서는 대조군과 동일한 생존도를 나타냈으나 pH 4.0, 4.5 에서는 100% 생장이 억제되었다. 분리 균주들의 액화능 비교에 있어서는 H strain 이 가장 뛰어난 액화능을 나타내었다. CuSO₄와 KI·I₂를 각각 3ppm, 10ppm의 저농도로 처리했을 경우 4균주의 액화능은 대체로 10∼20% 감소되었다. 3. 액화 능력이 가장 뛰어난 H strain 의 MIC 는 Penicillin G; 1∼5 ㎍/ml, Spectinomycin; 12.5∼25㎍/ml, Ampicillin; <0.05㎍/ml, Cefobid; <1㎍/ml, Claforan; 0.02∼0.3㎍/ml, Tetra-cycline; >10㎍/ml로 나타났다. Tetracycline을 제외한 5가지 항생제에 대하여 H strain이 Y strain들에 비해 민감한 것으로 나타났다. 그러나 Tetracycline의 경우에는 H strain이 Y strain들에 비하여 저항성을 갖는 것으로 나타났다. Four strains of gelatin-liquefying bacteria were isolated from the sewage of the food using gelatin as the material. Treated with various concentration of NaCl, KI-I₂and CuSO₄in the culture medium and with various gradient of pH, viability of the four strains was examined and compared with each other. And also, gelatin liquefying ability was investigated in the normal culture condition and in accordance with the concentration of the inhibitor. Morever, MIC(Minimal Inhibitory Concentration) test for these strains was also carried out. The results were as follows: 1. Of the four isolated strains, the H strain was identified as Serratia liquefaciens, Y₁as the genus Enterobacter, Y₂and Y₃strains as the genus Pseudomonas. 2. Treating the culture medium with NaCl as the growth inhibitor, 7% concentration of NaCl inhibited completely the viability of all the isolated strains. Adding KI-I₂solution to the medium at the level of 20ppm, viability began to decrease and the inhibitory effect was shown 100% at 100ppm. Treated with CuSO₄at the level of 10ppm, viability began to decrease, and at 300ppm growth inhibition showed almost 100%. There was no significant difference in viability according to the pH gradient at the range of 5.0-6.0. However, at pH 4.0 and 4.5, 100% inhibitory effect on the growth was shown. Considered with the ability of gelatin liquefaction, the H strain among the four strains had the most rapid and strong ability in gelatin liquefaction. (That is, about 23 times more effective in gelatin liquefaction than that of Y₁strain) 3. Considered the MIC test, the H strain was more sensitive to Penicillin G(1-5 ㎍/ml), Spectinomycin(12.5-25㎍/ml), Ampicillin(<0.05㎍/ml), Cefobid(<1㎍/ml) and Claforan (0.02-0.3㎍/ml) than to those of the strains. Compared with the other strains, the H strain was more insensitive up to the level of 10㎍/ml to the minimal inhibitory concentration of Tetracycline.

        In an effort to optimize tissue culture responses of zoysiagrass (Zoysia japonica Steud.) for genetic transformation, factors affecting callus induction and plant regeneration were investigated. MS medium containing 3 ㎎/L 2,4-D was optimal for embryogenic callus induction from mature seed. The plant regeneration frequency of 73.3% was observed when embryogenic calli induced in this medium were transferred to N6 medium supplemented with 0.1 ㎎/L 2,4-D and 5 ㎎/L BA. Among several basic media, MS and N6 medium were optimal for callus induction and plant regeneration, respectively. Regenerated plants were grown normally when shoots transplanted to the soil. A rapid and efficient plant regeneration system established in this study will be useful for molecular breeding of turfgrass through genetic transformation.
