Background: Picosecond neodymium-doped yttrium aluminium garnet Laser (ps-Nd:YAG) has emerged as a new alternative in the treatment of various dermatologic diseases. Objectives: The aim of this study was to evaluate the efficacy of a fractional 1064-nm ps-Nd:YAG for the improvement of atrophic scars. Methods: A retrospective chart review and clinical photo analysis were performed on subjects treated with a low-fluence 1064-nm ps-Nd:YAG laser with a micro-lens array (MLA). The Manchester Scar Scale (MSS), 5-point Global Assessment Score (GAS) were measured to evaluate the improvement of the scar. Results: A total of 18 Korean patients (7 males and 11 females; mean age of 27.94±13.66 years) were analyzed for the retrospective analysis. All patients were treated with a ps-Nd:YAG at the wavelength of 1064-nm, pulse duration of 450 or 750 ps, mean spot size of 5.95 mm (range 4-8 mm), mean fluence of 2 J/㎠ (range 0.3-4.8 J/㎠), and mean pulse rate of 6.13 Hz (range 2-10 Hz). The mean number of treatments was 4.05±2.19 (range: 2 -10), and mean total duration of treatment was 29.94±23.42 weeks (range: 4-68). After laser treatment, the average MSS score was statistically significantly reduced from 9.83 to 7.55 (p < 0.001). The average GAS was measured as 3.61±0.92, which revealed fair gross improvement. Conclusion: For the treatment of atrophic scars in Asian skin, the fractional low-fluence 1064-nm ps-Nd:YAG can be a effective modality.

Median raphe cysts are relatively rare congenital anomalies that may occur anywhere along the midline of the ventral side of the male genital area. It usually occurs as a single or a few cysts, but rarely can appear as a canaliform pattern. The pathogenesis of these cysts is not well understood, but is thought to be caused by defects in embryologic development. Most these cysts are asymptomatic until adulthood, but they can be detected earlier when trauma or secondary infections occur, accompanied by symptoms such as swelling, tenderness, and exudates. We report a case of a 5-year old boy with inflamed canaliform median raphe cysts. He presented with multiple yellowish cysts along a cord-like swelling running in the midline extending from the anterior margin of the anus to the lower pole of the scrotum. Especially, one painful scrotal lesion with exudative crusts occurred after swimming in the pool four days ago. Histopathologic findings showed tangentially sectioned invaginated keratin filled lesions with some coccoid form bacterial organisms and focal neutrophilic collections. Bacteroides fragilis was identified on the bacterial culture from the affected area. We treated his lesion with appropriate antibiotics and drainage dressing. Thus, we introduce this as a rare case of inflammatory canaliform raphe cysts in a pediatric patient.
Secondary infection of the umbilical keloid : report of three cases and literature review

As many surgical operations have been replaced by laparoscopic surgery, the number of umbilical keloid patients is gradually increasing. Generally, as keloids are painful in the disease itself, clinical differentiation with secondary infection could be difficult. If a thick keloid is formed, it may be a hard problem to confirm the fluctuation and heat sensation in the physical examination. For these reasons, rapid and appropriate surgical intervention can be delayed in numerous umbilical keloid patients. We experienced three cases of secondary keloid infection in our hospital. In two middle-aged women with tenderness of the keloid, an inflammatory epidermoid cyst was identified by incisional biopsy. Pus from the both patients was sent for microbiological examination, but any significant bacteria growth that might be a pathogenicity factor was not found. Both patients were treated by empirical antibiotics and pus drainage with sterile packing dressing. In the other patient, the cause of repeated keloid inflammation was found to be an iatrogenic small peritoneal defect due to previous surgery. She was sent to general surgery part for additional diagnostic work-up and proper management. From these, we could infer the inflammation of umbilical keloid can be caused by a lot of reasons and require much attention for diagnosis. Thus, we will discuss appropriate diagnostic and therapeutic approach for secondary infection of umbilical keloid.
Scalp white piedra caused by Rhodotorula species

White piedra is a superficial mycotic infection of the hair shaft, characterized by small, firm, irregular gritty nodules. This asymptomatic infection, also known as tinea nodosa and trichomycosis nodularis, is mainly caused by basidiomycetous yeasts in the genus Trichosporon. But rarely has this disease due to other fungi infection been reported and no case of white piedra in Korean was documented so far. A 22-year-old man presented with palpable translucent to white nodules along the left temporal scalp hair. He denied other symptoms like scalp irritation or pruritus, but complained the affected hairs became brittle and easily broke at points of attached nodules. On the contrary, the adjacent scalp skin was not affected. For the diagnosis, fungus culture from the affected hair on Sabouraud agar was performed and it revealed distinctive orange to red colonies, which were diagnostic of Rhodotorulla species. The patient was treated with oral itraconazole and isoconazole cream, and his symptom was clinically improved after a month with significant decrement of the concretions and brittle hairs. Thus, we will introduce this patient as a rare case of white piedra caused by Rhodotorula species.

Background: Rosacea is a chronic inflammatory skin disease with various clinical manifestations. Its cause and pathophysiology are not fully understood, but recently it has been shown to have an association with numerous systemic diseases. Objectives: To investigate (1) the association between rosacea and systemic comorbidities and (2) if the subtypes and the severity of rosacea have an effect on comorbidities. Methods: A matched case-control study was conducted using health care center data for three years (2014-2017). Rosacea group was identified by clinical photo review of two dermatologists. Rosacea-free control group was matched to case by age and sex. By using SPSS version 19.0., the data measured in health check-up program were analyzed. Results: Among 1829 subjects who visited our center, 229 (case) and 458 (control) were identified and selected for analysis. Body mass index(BMI), family history of liver disease and past medical history of thyroid cancer, hypertension, and kidney disease are associated with rosacea. Conclusion: Rosacea is associated with several systemic comorbid diseases.

Background: 532 nm wavelength of Q-switched neodymium: yttrium-aluminum-garnet (QSNY) laser is widely used for the treatment of solar lentigines. Recently, picosecond (PS) laser is emerging as a better treatment option for solar lentigines. Objectives: To evaluate the clinical efficacy and safety of 532 nm PS and QSNY laser in a split-face design. Methods: A prospective, randomized, split-face study of 532 nm PS laser and 532 nm QSNY laser was designed. Two laser sessions were performed at 2-week intervals. Clinical improvement was assessed by the number of lentigines, melanin index from 3D camera analysis, and 5-point quartile improvement scale (QIS). Development of PIH were also assessed. Results: A total of 24 subjects (23 females and 1 male) with Fitzpatrick skin types III to IV, mean age 50 years, were enrolled. The reduction of lentigo counts was not significantly different between the two groups, but the melanin index change showed a more significant improvement in the pico-arm. (p < 0.05). QIS showed 532 nm PS laser was more effective than 532nm QSNY laser. (p < 0.05) The incidence of PIH was 5% in 532nm PS laser, and 30% in 532nm QSNY laser. Conclusion: Both 532 nm PS laser and 532 nm QSNY laser are effective for the treatment of solar lentigines, however, 532 nm PS laser removed solar lentigines more effectively with less PIH.
A case of CD30+ T-cell lymphoproliferative disorder following trauma on the scalp

CD30+ lymphoproliferative disorders are the second most common primary cutaneous T-cell lymphoma, which include lymphomatoid papulosis and primary cutaneous anaplastic large cell lymphoma. Recently, CD30+ inflammatory reactions have been reported in a variety of infectious or inflammatory diseases, such as arthropod bites, herpes simplex infection, drug reactions, and atopic dermatitis. Among them, post-traumatic atypical CD30+ T-cell proliferation is an extremely rare case of these entities. It can mimic a cutaneous lymphoma, but clinical presentation or prognosis might quite different. It is important to distinguish these cases from other diseases. A 17-year-old male presented with a solitary 2 cm-sized nodule with yellowish discharge on the vertex. After bumping the scalp four days ago, there was a sore lump that increased in size. He denied other constitutional symptoms except for those associated with skin lesion. On the basis of histopathologic examination, large collected atypical vesicular lymphoid cells and geographic necrosis were observed under the ulcerative epidermis. In immunohistochemical analysis, tumor cells were positive for CD43, CD45, CD4, CD3, CD5, and CD30. The lesion was treated with surgical excision and the postoperative site has been stable without any recurrence. Thus, we report this patient with a reactive CD30+ cells infiltration following trauma.
조선후기 가사의 대중화 양상 연구: 이운영의 가사 작품을 중심으로

연구 목적: 이 연구는 이운영의 가사 작품을 중심으로 조선후기 가사의 대중화 양상을 살피는 데 있다. 연구 방법: 이운영 가사의 담론 특성을 파악하기 위해 작품이 창작된 생성 메커니즘에 역점을 두어 담론 차원에서 <세장가>와 <수로조천행선곡>을 분석하였다. 연구 내용: 담론 차원에서 파악된 각각의 작품은 잠재된 트라우마를 해소하는 방식에서 18세기 시정문화의 변화상을 반영하며 담론화되었다. 결론 및 제언: <세장가>는 민간 가요를 엮어 짜는 방식으로 내면에 잠재된 트라우마를 해소하고, <수로조천행선곡>은 민간 가악과 엮는 방식으로 문화적 트라우마를 해소하기 위해 가창되었음을 살필 수 있었다. 이처럼 이운영의 가사 작품은 조선후기 가사 장르의 유연성과 역동성을 동시에 확보하며 다양한 대중문화적 욕구를 충족하는 방향으로 텍스트화 되었다는 데서 그 문학사적 의의를 지닌다고 할 수 있다. The purpose of this study is to look into the popularization aspect of the Gasa in the late Joseon Dynasty, focusing on Lee Yun-Yeong’s Gasa work. In order to understand the discourse characteristics of Unyeong Lee’s Gasa, Saejangga and Suro Jocheon Haengseongok were analyzed from the level of discourse by focusing on the creation mechanism by which the works were created. In the dimension of discourse, each of the works was discoursed, reflecting the change of civic culture in the 18th century in the way of resolving latent trauma. Sejangga relieves the latent trauma inside in the way of weaving folk songs, and Suro Jocheon Haengseongok was sung along with a desire for culture in order to relieve cultural trauma with folk songs. In this way, Lee Yun-Yeong’s Gasa work has significance in the historical literature in that it was textualized in the direction of at the same time securing the flexibility and dynamism of the late Joseon Dynasty Gasa genre and satisfying various popular cultural desires.
