        교정용 브라켓에 가해지는 힘의 방향에 따른 결합강도의 비교

        본 연구는 교정용 금속 브라켓에 다양한 방향에서의 복합적인 응력을 가하여 힘의 방향과 적용점에 따른 교정용 브라켓의 결합강도를 비교하고, 브라켓의 유지력을 평가하는데 기준이 되는 최소결합강도의 특성에 대해 알아보고자 시행되었다. 일정한 표면특성을 갖는 금속봉에 Micro-Loc base, Chessboard base, Non-etched Foil-Mesh base 등 서로다른 기저면 형태를 가지는 3가지 종류의 금속 브라켓을 부착시키고, 0˚, 15˚, 30˚, 45˚, 60˚, 75˚, 90˚의 peel 결합강도(_(0)PBS, _(15)PBS, _(30)PBS, _(45)PBS, _(60)PBS, _(75)PBS, _(90)PBS 및 전단결합강도(SBS)와 인장결합강도(TBS)에 대한 브라켓의 결합강도를 측정하고, 각 브라켓의 기저면 면적을 고려하여 단위면적당 결합강도를 산출하여 비교한 결과 다음과 같은 결론을 얻었다. 1. Micro-Loc base와 Chessboard base 및 Non-etched Foil-Mesh base 브라켓 모두에서 전단결합강도(SBS)가 가장 컸다(p<0.01). 2. Peel 응력의 방향 변화에 따른 peel 결합강도(PBS)의 변화양상은 Micro-Loc base와 Chessboard base, Non-etched Foil-Mesh base 브라켓 모두에서 유사하였으며(p>0.05), peel 응력의 적용 각이 증가할수록 peel 결합강도(PBS)는 감소하였고 60˚에서 최저값을 보였다(p<0.05). 3. Micro-Loc base에서 최저 peel 결합강도(_(60)PBS)는 전단결합강도(SBS)의 29% 수준이었으며, 인장결합강도(TBS)에 대해서는 52% 수준이었고, Chessboard base에서 최저 peel bond strength(_(60)PBS)는 전단결합강도(SBS)의 34% 수준이었으며, 인장결합강도(TBS)에 대해서는 61% 수준이었으며, Non-etched Foil-Mesh base에서 최저 peel 결합강도(_(60)PBS)는 전단결합강도(SBS)의 34% 수준이었으며, 인장결합강도(TBS)에 대해서는 55% 수준이었다. 4. 단위면적당 결합강도에 있어서 전단결합강도(SBS)와 인장결합강도(TBS) 및 75˚와 90˚ peel 결합강도는 Micro-Loc base와 Chessboard base에서 차이가 없었으며 Non-etched Foil-Mesh base에서 가장 작았고(p<0.05), 0˚, 15˚, 30˚, 60˚ peel 응력을 적용한 결과 Chessboard base에서 가장 큰 peel 결합강도를, Non-etched Foil-Mesh base에서 가장 작은 결합강도를 보였다(p<0.05). The purpose of this study was to evaluate the bond strength of orthodontic brackets bonded to metal bar with chemically cured adhesive (Ortho-one, Bisco Co, USA) in various types and directions of force application. Three types of metal bracket with different bracket base configurations; Micro-Loc base(Tomy Co, Japan), Chessboard base(Daesung Co, Korea), Non-etched Foil-Mesh base(Dentaurum, Germany); were used in this study. Peel, shear, tensile bond strengths were measured by universal testing machine and compared each other. The peel force directions applied were 0˚, 15˚, 30˚, 45˚, 60˚, 75˚, 90˚. And then, in consideration of the different surface area of the bracket bases, the bond strength per unit area were calculated and compared. The results obtained were summarized as follows : 1. The bond strengths according to the types and the directions of the forces were greatest at the shear forces in all three bracket base configuration groups(p<0.01). 2. As the peel force direction grew higher in degree, peel bond strength decreased. The patterns of peel bond strength change according to force direction was similar in all three bracket base configurations. The minimum bond strength was 60 degree-peel bond strengths in all three bracket base configurations. 3. In Micro-Loc base group, minimum peel bond strength(_(60)PBS) was in 29% level of shear bond strength and 52% level of tensile bond strength. In Chessboard base group, _(60)PBS was in 34% level of shear bond strength and 61% level of tensile bond strength. In Non-etched Foil-Mesh base group, _(60)PBS was in 34% level of shear bond strength and 55% level of tensile bond strength. 4. The bond strengths per unit area were lowest in Non-etched Foil-Mesh base group and highest in Chessboard base group(p<0.05). However, there were no differences in shear bond strength, tensile bond strength, _(75)PBS and _(90)PBS per unit area between Micro-Loc and Chessboard base groups.

        Plasma arc light를 이용한 금속 브라켓의 부착시 광조사 방향과 중합시간이 전단결합강도에 미치는 영향

        본 연구는 Plasma arc light를 이용하여 금속 브라켓의 부착시 광조사 방향, 중합시간 그리고 경화시간이 전단결합강도에 미치는 영향을 평가하기 위해서 시행되었다. 240개의 발거된 소구치를 15군으로 나눈 후, 광중합기(Plasma arc light와 Halogen light), 중합시간(Plasma arc light 2, 4, 6초와 Halogen light 20초), 그리고 광조사 방향(Vertical 방향과 Oblique 방향)을 다르게 하여 브라켓을 부착하였다. 광중합 접착제는 Transbond XT를 이용하였으며 브라켓 부착 후 5분과 24시간 후에 만능물성시험기를 이용하여 전단결합강도를 측정하였다. 결과는 다음과 같다 광중합기의 종류와 중합시간에 관계없이 Vertical군과 Oblique군 간의 전단결합강도는 유의한 차이가 없었다 (p>0.05). Plasma arc light를 이용하여 2초 동안 광중합한 군들의 전단결합강도는 다른 중합시간의 군들보다 현저히 낮았다 (p<0.05). 부착 5분 후의 전단결합강도는 24시간 후보다 현저히 낮았다 (p<0.05). ARI 점수는 군들 간에 통계적으로 유의한 차이가 없었다. 이상의 결과는 plasma arc light를 사용한 금속 브라켓의 부착시 광조사 방향은 전단결합강도에 영향을 끼치지 않으며, Halogen light와 유사한 결합강도를 얻기 위해서는 4초 이상의 중합시간이 필요함을 시사한다. The purpose of this study was to compare the effects of different light direction. exposure times and setting times when using plasma arc light on shear bond strength of metal brackets. 240 extracted human premolars were randomly assigned to one of 16 groups. Standardized brackets were bonded to enamel using different light curing units (Plasma arc light and Halogen light), exposure times (Plasma arc light 2, 4, 6 seconds and Halogen light 20 seconds), and light directions (Vertical direction [V] and Oblique direction [O]). 8 groups were tested after 5 minutes and the remaining 8 groups after 24 hours. The metal brackets were bonded with Transbond XT. Shear bond strength was measured by a universal testing machine. The results were as follows: There were no differences between the shear bond strengths of the Vertical groups (V) and Oblique groups (O), regardless of exposure times and types of light curing units (p>0.05), The shear bond strength of the group with 2 seconds of plasma light were significantly lower than other exposure time groups (p<0.05). The shear bond strength tested after 5 minutes was lower than after 24 hours (p<0.05). The Adhesive Remment Index (ARI) score showed no statistically significant difference among the different groups. The results of this study suggested that the light direction of plasma arc light had no influence on the shear bond strength of metal brackets to enamel, and exposure times more than 4 seconds produced shear bond strengths similar to those produced with a conventional halogen curing light.

        Self-etching primer/adhesive를 사용한 교정용 브라켓의 접착시 오염이 전단결합강도에 미치는 영향

        이 연구는 self-etching primer와 adhesive를 사용하여 법랑질 표면에 교정용 브라켓을 접착할 때 수분과 타액, 그리고 혈액에 의한 오염이 브라켓의 전단결합강도에 어떤 영향을 미치는지 알아보고자 시행되었다. 96개의 발거된 치아를 4개의 군으로 나누어서 self-etching primer와 adhesive를 사용하여 다음의 4가지 방법에 따라 교정용 브라켓을 접착하였다. 첫 번째 군은 치면을 건조시킨 상태에서 부착하였고, 두 번째, 세 번째, 네 번째 군은 각각 증류수, 인공타액, 혈액을 적용한 후 브라켓을 접착하였다. 각 시편의 전단결합강도를 측정하였고 debonding 후에 브라켓과 치아 표면은 stereomicroscope을 사용하여 관찰하였다. 각 군에서 4개씩의 시편을 선택하여 표면처리된 법랑질 표면과 접착계면을 주사전자 현미경으로 관찰하여 다음과 같은 결과를 얻었다. 건조군과 증류수군의 전단결합강도는 각각 15.22 ± 2.86 MPa과 16.20±3.85 MPa로 혈액군의 전단결합강도(12.56 ± 2.94 MPa)보다 높았다(p<0.05). 건조군과 증류수군, 그리고 인공타액군간에는 전단결합강도에 통계적 차이가 없었다(p>0.05). 혈액군은 다른군에 비해서 debonding 후 법랑질 표면에 잔류한 레진의 양이 더 적었다(p<0.05). self-etching primer로 처리된 치면의 주사전자현미경 소견에서 건조군과 증류수군의 치면에서는 인공타액군과 혈액군에 비해서 더욱 뚜렷한 산부식 구조를 볼수 있었다. 이상의 결과로서 self-etching primer/adhesive를 사용한 교정용 브라켓의 접착시 수분이나 타액의 오염은 결합력에 거의 영향을 미치지 않음을 알수 있으며, 혈액으로 오염시에는 결합력에 영향을 미치지만, 임상적으로 유용한 최소 결합강도(5.9-7.8 MPa) 이상을 보이고 있음을 알 수 있었다. The purpose of this study was to investigate the influence of water, saliva, and blood contamination on the bonding strenght of metal brackets with a self-etching primer/adhesive to enamel. Ninety-six extracted human teeth were divided into four groups. The brackets were bonded to enamel with a self- etching primer (3M/Unitek Dental Products, Monorovia, California) according to one of four protocols. The teeth were bonded in a dry condition (group D) or in contamination with distilled water (group W), artificial saliva (group S), or fresh human blood (group B). Shear bond strengths were tested using an Instron Universal testing machine. After debonding, bracket and tooth surfaces were examined with a stereomicroscope. In each group, four samples were selected and examined with a Scanning electron microscope of the prepared enamel surface and resin-enamel interface. The results obtained were summarized as follows: Shear bond strength in group D (15.22 ± 2.86 MPa) and W (16.20 ± 3.85 MPa) were higher than in group B (12.56 ± 2.94 MPa) (p<0.05). There were no statistical differences in the shear bond strengths between groups D, W and S (p>0.05). There was a tendency to have less residual adhesive remaining on the enamel surfaces of group B than group D. The SEM morphology of group D and W showed a more roughened etching pattern than group S and B. Water or saliva contamination on bonding of orthodontic brackets with Transbond plus self etching primer had almost no influence on bond strength. In this study, the blood contaminated group showed the lowest bond strength, but it was above the clinically acceptable bond strength (5.9-7.8 MPa, Reynold, 1975). The results of this study suggest that acceptable clinical bond strengths can be obtained in wet conditions when self-etching adhesives are used.

        The Outcomes of Concomitant Chemoradiotherapy Followed by Adjuvant Chemotherapy with Temozolomide for Newly Diagnosed High Grade Gliomas : The Preliminary Results of Single Center Prospective Study

        Objective : Malignant gliomas are the most common primary cerebral neoplasms in adults. Despite multimodality treatments, the prognosis for patients with malignant glioma remains poor. However, recently, the effectiveness of concomitant chemoradiotherapy (CCRT) with temozolomide (TMZ) has been reported. We report for the first time preliminary results of the treatment with CCRT of newly diagnosed malignant gliomas in Korean people. Methods : Thirty-two patients over the age of 17 years with newly diagnosed and histologically confirmed high-grade gliomas (HGG), from June 2004 to August 2007 were the subjects of this study. There were 17 men and 15 women, with a median age of 53.5 years (range, 17-74). Pathologically, glioblastoma, anaplastic astrocytoma, anaplastic oligodendroglioma, and gliomatosis cerebri had been diagnosed in eighteen, eight, four, and two patients, respectively. These 32 patients were treated with CCRT with TMZ. Results : The median follow-up period was 12.5 months (range 3-48). At the time of this analysis, 13 patients died and three patients had been lost to follow-up. There was no mortality caused by drug toxicity. The median progression-free survival (PFS) of these patients was 9.0 months, and the six-month PFS rate was 72.4%. The median overall survival (OS) was 26 months, and the one-year OS rate was 83.6%. The 18 patients with glioblastoma were analyzed separately from the other patients with HGG, and the median OS was 18 months, and the one-year OS rates were 81.8%. The median PFS was seven months, and the six-month PFS rate was 75.0%. Conclusion : Our results are consistent with many other reports, confirming that CCRT with TMZ achieves good clinical outcomes in the treatment of HGG. Therefore, we suggest that CCRT with TMZ as adjuvant chemotherapy be considered as a standard therapy for patients with HGG.

        Clinical Significance of Monitoring Circulating CD4 <sup>+</sup> CD25 <sup>+</sup> Regulatory T Cells in Kidney Transplantation during the Early Posttransplant Period

        <P>The CD4<SUP>+</SUP>CD25<SUP>+</SUP> T regulatory cells (Tregs) play an important role in immune tolerance in experimental transplantation but the clinical significance of circulating Tregs in the peripheral blood is undetermined. In 50 kidney transplant (KT) recipients, 29 healthy controls and 32 liver transplant (LT) recipients, the frequency of Tregs was measured with flow cytometry before and after transplantation. In the KT recipients, IL-10 secretion was measured with an enzyme-linked immunospot (ELISPOT) assay. The median frequency of circulating Tregs before KT was similar to that in healthy controls but significantly lower than that in LT patients before transplantation. The frequency of Tregs was significantly decreased in patients with subclinical acute rejection compared with those without subclinical acute rejection. Calcineurin inhibitors (CNIs) and anti-CD25 antibody decreased the frequency of Tregs but mTOR inhibitor did not. The frequency of donor-specific IL-10 secreting cells did not correlate with the number of Tregs. The frequency of circulating Tregs in KT recipients is strongly affected by CNIs and anti-CD25 antibody, and a low frequency of Tregs is associated with subclinical acute rejection during the early posttransplant period.</P>
