<P><B>Abstract</B></P><P>Tumor‐infiltrating lymphocytes (TILs) play critical roles in host antitumor immune responses. It is known that cancer patients with tumor‐reactive lymphocyte infiltration in their tumors have better prognoses, while patients with tumors infiltrated by immunosuppressive cells have worse prognoses. We found that administration of 6‐gingerol, which is a component of ginger, inhibited tumor growth in several types of murine tumors, such as B16F1 melanomas, Renca renal cell carcinomas and CT26 colon carcinomas, which were established by inoculating tumor cells on the flanks of mice. However, administration of 6‐gingerol did not lead to complete eradication of the tumors. 6‐Gingerol treatment of tumor‐bearing mice caused massive infiltration of CD4 and CD8 T‐cells and B220<SUP>+</SUP> B‐cells, but reduced the number of CD4<SUP>+</SUP>Foxp3<SUP>+</SUP> regulatory T‐cells. The CD8 tumor‐infiltrating T lymphocytes in 6‐gingerol‐treated mice strongly expressed IFN‐γ, a marker of activation of cytotoxic T lymphocytes (CTL) CD107a and chemokine receptors that are expressed on T<SUB>H</SUB>1 cells, such as CXCR3 and CCR5. To test whether 6‐gingerol could promote infiltration of tumor antigen‐specific CD8 T‐cells into tumors, we adoptively transferred CFSE‐labeled OT‐1 CD8 T‐cells into EG7 tumor‐bearing mice. We found that CD8 T cells isolated from 6‐gingerol pretreated OT‐1 mice, but not from control OT‐1 mice, massively infiltrated tumors and tumor draining lymph nodes and divided several times. Our results strongly suggest that 6‐gingerol can be used in tumor immunotherapy to increase the number of TILs.</P>
TSLP Polymorphisms in Atopic Dermatitis and Atopic March in Koreans

Background: Atopic march (AM) is the progression from atopic dermatitis (AD) to allergic rhinitis and asthma. The development of AD is as high as 20% in children worldwide and continues to increase. AD seems to be caused by both genetic and environmental factors. Recently, polymorphisms of the thymic stromal lymphopoietin (TSLP) gene associated with allergic disorders were reported in ethnic groups from various countries. Objective: Identification of TSLP polymorphisms in Koreans with AD or AM. Methods: Wholeexome sequencing was performed in 20 AD and 20 AM patients. Results: Nine single nucleotide polymorphisms (SNPs) of TSLP were detected (rs191607411, rs3806933, rs2289276, rs2289277, rs2289278, rs139817258, rs11466749, rs11466750, rs10073816). These SNPs have been correlated with susceptibility to allergic diseases in ethnic groups from China, Japan, Turkey, and Costa Rica in previous studies. Remarkably, one of 20 patients in the AD group lacked all SNPs, compared to six of 20 patients in the AM group. Odds ratios showed that Korean patients without the nine TSLP variants had an 8.14 times higher risk of moving from AD to AM. Two haplotype blocks were validated in 60 AD and 59 AM patients using Sanger sequencing. The haplotype blocks (rs3806933 and rs2289276) and (rs11466749 and rs11466750) were in high linkage disequilibrium, re-spectively (D’=0.97, D’=1). Conclusion: The increase of major allele frequency of respective nine TSLP variants may enhance the risk of AM. These data will contribute to improved genetic surveillance system in the early diagnosis technology of allergic disease. (Ann Dermatol 30(5) 529∼ 535, 2018)

Background: Beyond its current function as a rescue therapy in acute respiratory distress syndrome (ARDS), extracorporeal membrane oxygenation (ECMO) may be applied in ARDS patients with less severe hypoxemia to facilitate lung protective ventilation. The purpose of this study was to evaluate the efficacy of extended ECMO use in ARDS patients. Methods: This study reviewed 223 adult patients who had been admitted to the intensive care units of 11 hospitals in Korea and subsequently treated using ECMO. Among them, the 62 who required ECMO for ARDS were analyzed. The patients were divided into two groups according to pre-ECMO arterial blood gas: an extended group (n=14) and a conventional group (n=48). Results: Baseline characteristics were not different between the groups. The median arterial carbon dioxide tension/ fraction of inspired oxygen (FiO<sub>2</sub>) ratio was higher (97 vs. 61, p<0.001) while the median FiO<sub>2</sub> was lower (0.8 vs. 1.0, p<0.001) in the extended compared to the conventional group. The 60-day mortality was 21% in the extended group and 54% in the conventional group (p=0.03). Multivariate analysis indicated that the extended use of ECMO was independently associated with reduced 60-day mortality (odds ratio, 0.10; 95% confidence interval, 0.02-0.64; p=0.02). Lower median peak inspiratory pressure and median dynamic driving pressure were observed in the extended group 24 hours after ECMO support. Conclusion: Extended indications of ECMO implementation coupled with protective ventilator settings may improve the clinical outcome of patients with ARDS.
Survey of scrub typhus vectors at epidemic regions in Jeollabuk-Do, Korea 2016

A surveillance of chigger mites was performed to monitor the incidence of scrub typhus vectors at 5 environmental collection points of 3 locations (Muju, Namwon and Gimge) from April 2016 in Jeollabuk-Do, Korea. During the surveillance period, 3,292 chigger mites were collected and the predominant species were Leptotrombidium pallidum (42.9%). The high environmental collecting rates were recorded at reservoir bank (31.6%) and grassfield (29.7%). We detected Orientia tsutsugamushi from the samples collected in Jeollabuk-Do using nested polymerase chain reaction. The specific DNA of O. tsutsugamushi were detected in 11 pools among total 119 pools and its minimum positive rate was 0.35%. In order to clarify the relationship between chigger mites and scrub typhus cases, further survey will be needed.
Comparison of Bile Drainage Methods after Laparoscopic CBD Exploration

Purpose: T-tube is a major procedure that prevents complication by biliary decompression, but which is accompanied by complications. Therefore, several procedures such as ENBD, PTBD, and antegrade biliary stent have been attempted, but with controversies as to which procedure is superior. Also, there are no standard procedures after laparoscopic CBD exploration. We performed this study to ascertain the most appropriate biliary drainage procedure after laparoscopic CBD exploration. Methods: From March 2001 to December 2009, 121 patients who underwent Laparoscopic CBD exploration in Gunyang University were included for retrospective analysis. The patients were divided to 4 groups according to type of procedure, and we compared clinical parameters including age and gender, operation time, hospital stay, start of post-operative diet, and complications. Results: There was no difference in age, gender, mean operation time, postoperative diet between the 4 groups. Hospital stay in the Stent group was shorter than T-tube group. There were 10 (7%) complications that occurred. Two 2 occurred in the T-tube, 3 in PTBD, and 5 in the Antegrade stent group. There were more complications in Stent group but no significant statistical difference. In 5 cases with remnant CBD stone, a total of 4 (3 PTBD, 1 Stent) was performed by endoscopic CBD stone removal. One T-tube case was removed easily by choledochoscopy through the T-tube. Three migrated and the impacted stents were removed by additional endoscopy. Perioperative biliary leakage (1) and peritonitis (1) post t-tube removal were resolved by conservative treatment Conclusion: T-tube appears to be an appropriate method to patients who are suspected to have remnant CBD stones. Multiple procedures may be performed on a case by case basis such as performing PTBD first in a suspected cholangitis patient.
