      • 소화성 궤양에 대한 Nizatidine의 임상효과

        Nizatidine is a newly introduced potent H2-receptor antagonist of acid secretion with long duration of action. For evaluation of clinical efficancy and safety, twenty one patients with endoscopically proved thirteen gastric ulcers and eight duodenal ulcers were treated with nizatidine 300mg at bed time via per os for 6 weeks. The results were as follows 1. The rate of complete endoscopic healing of duodenal ulcer 92.3% and 87.5% after 6 weeks. The overall healing rate of peptic ulcer was achieved in 89.9% at 6 weeks. 2. Clinical symptoms disappeared in 84.6%, 92.3% and 100% of patients with duodenal ulcer and 75.0%, 87.5% and 87.5% patients with gastric ulcer at 2, 4 and 6 weeks respectively. 3. There were not developed any symptoms and signs suggesting side effect of drug. In conclusion, data from this study suggest that nizatidine 300mg p.o. at sleeping time is effective and safe in treatment of peptic ulcer disease, and is well tolerated on a short-term basis.

        Recently, there will be need to make up construction site on the reclamation ground in considering effective using the small country and expansion of urban site. therefore, there is coming many troubles such as effluence of seepage-out water and differential settlement as constructional structure is made on the soft reclamation ground. This study, examined behavior of foundation, property and behavior of pollution matter and the stable term of reclamation ground when reclamation ground is used to construction site, indicate that there are necessary fact for maintenance-management to investigate 10 case of reclamation construction for the analysis of troubles and their cause. Usually, the reclamation ground is constructing on the soft soils and lubricate action of pollution matter cause the lateral flow pressure to foundation pile. if there were considered to differential settlement pipe-line-arrangement of underground structure must be constructed with flexible joint and reclamation will be required to aseismic design as the shearing force of reclamation ground is suddenly decreased by vibration owing to construction property

        To preserve the basis of milk production in Korea and alleviate the invasion of Korean market by western countries′milk products under GATT, Korean dairy industry has to lower the price and improve the quality of raw milk and milk products until 2005 when Korea will have to cut much more the custom duties imposed on the milk products imported from foreign countrys. First of all Korea has to concentrate it's interst on pasteurized market milk and yoghurt that can be stored under refrigeration only for a short period and have to be made with fresh raw milk to guarantee high quality. These two products have high possibility to compete with western countries′milk products if the quality of raw milk from Korean dairy farm is improved to that of western country and Korean consumer is illuminted for the nutritional superiority of milk products made with fresh raw milk. Also we have to strive to find out and develop milk products having competence. It will be very difficult and time consuming to insure the perfect competitive power of Korean milk products to western countries′milk products, all Korean in dairy industry must do their best in close cooperation together. Especially considering conditions of dairy industry in Korea, there will be a limit in lowering price, therefore we have to accentuate our effort on the development of technology that can overcome the disadvantage of price.

        봉지종류 및 괘대시기가 ‘원황’ 및 ‘화산’ 배 과실의 수확기 품질에 미치는 영향

        We tested fruits quality and skin coloration of two pear cultivar ‘Wonhwang’ and ‘Whasan’ produced by bagging with different kinds of paper bags in the Cheonan area, Chungnam Province, Korea. The fruits were bagged at 35, 45, 55 or 65 days after full bloom (DAFB) with 3 kinds of double layer paper bag which consisted of grey color, yellow color and newspaper for outer layer. The great extent of light interruption was observed in grey colored outer paperbag among three kinds of paperbag which showed only 0.46% of lowest light transmittance when compared with 43.7% and 40.0% of yellow paper and newspaper, respectively. The development of skin redness (a*) increased with the delay of bagging time in two pear cultivars. But the fruits treated at 65 DAFB showed uneven coloration and excessive development of redness which represented low fruit external appearance value. Uniform flesh firmness was attained at the fruits bagged with grey outer color paperbag regardless of bagging time, although the fruit quality indices including soluble solids and titratable acidity did not change significantly by using different kind of fruit bags and bagging time in two pear cultivar. Based on our results, it was explained that the grey-colored fruit bag had positive effect on the development of skin coloration without any detrimental effect on fruit quality factors in ‘Wonhwang’ and ‘Whasan’ pears.

        척골 지단 신경의 전기생리학적 연구

        The ulnar nerve extends down the arm, across the elbow, and into the hand. It provides sensation to the little and ring fingers and activates many of the small muscles in the hand. The determination of peripheral nerve conduction velocity is an important part of ulnar nerve evaluation. The electrodiagnostic value as neurophysiologic investigative procedure has been known for many years but normal value of digital nerve was not reported in korea. The purpose of this investigation was to measure the digital nerve conduction velocity of ulnar nerve for obtain clinically useful reference value and compare difference in each fingers and then compare with the other countries. 71 normal Korean volunteers (age, 19-65 years; 142 hands) examined who has no history of peripheral neuropathy, diabetic mellitus, chronic renal failure, endocrinedisorders, anti-cancer medicine, anti-tubercle medicine, alcoholism, trauma, radiculopathy. Nicolet Viking Ⅱ(EMG machine) was use for detected conduction velocity and amplitude of digital nerve in ulnar nerve. Data analysis was performed using SPSS. Descriptive analysis was used for obtain mean and standard deviation and independent t-test was used to compare with ring and little finger. Conduction velocity of the right ring finger was 57.44m/sec and little finger was 55.32msec. The left ring finger was 55.55msec and little finger was 54.11msec. Amplitude of the right ring finger was 30.28µV and little finger was 48.36µV. The left ring finger was 30.67µV and little finger was 57.76µV. There were significantly difference between ring and little in amplitude (p<.05) but there were no statistically difference between conduction velocity of ring and little finger (p>.05). The amplitude of little finger are greater than ring finger. The present results revealed that electodiagnosis can easily perform in little finger for digital nerve of ulnar nerve study.

        정중운동신경과 척골운동신경의 전기생리학적 연구

        The determination of peripheral nerve conduction velocity is and important part to electrodiagnosis. Its value as neurophysiologic investigative procedure has been known for many years but normal value of median and ulnar motor nerve was poorly reported in Korea. To evaluate of median and ulnar motor nerve terminal latency, amplitude of CMAP(compound muscle action potential), conduction velocity and F-wave latency for obtain clinically useful reference value. 71 normal volunteers(age,19-65 years;142 hands) examined who has no history of peripheral neuropathy, diabetic mellitus, chronic renal failure, endocrine disorders, anti-cancer medicine, anti-tubercle medicine, alcoholism, trauma, radiculopathy. Nicolet Viking Ⅱ was use for detected terminal latency, amplitude of CMAP, conduction velocity and F-wave latency of median and ulnar motor nerve. Data analysis was performed using SPSS. Descriptive analysis was used for obtain mean and standard deviation, independent t-test was used to compare between Rt and Lt side also compare between different in genders. The results are summarized as follows: 1. Median motor nerve terminal latency was right 3.00ms, left 2.99ms and there was no significantly difference between right and left side and genders. 2. Median motor nerve amplitude of CMAP was right 17.26mV, left 17.50mV and there was no significantly differences between right and left side and genders. 3. Median motor nerve conduction velocity was right 57.89m/sec, left 58.03m/sec and there was no significantly difference between right and left side and genders. 4. Median motor nerve F-wave latency was right 25.74ms, left 25.59ms and there was significantly differences between genders. 5. Ulnar motor nerve terminal latency was right 2.38ms, left 2.45ms and there was significantly differences between right and left side. 6. Ulnar motor nerve amplitude of CMAP was right 15.99mV, left 16.02mV and there was no significantly differences between right and left side and genders. 7. Ulnar motor nerve conduction velocity was right 60.35m/sec, left 59.73/sec and there was no significantly differences between right and left side and genders. 8. Ulnar motor nerve F-wave latency was right 25.53ms, left 25.57ms and there was significantly differences between genders.

        다양한 피판을 이용한 비결손 재건의 치험례

        The nose plays a signifcant role in the determination of man`s appearance and is also a highly developed functional organ. Functional reconstruction of the nose is pointed to the maintenance of an adequate air passage into the nasopharynx. For the aesthetic reconstrction of the nose, tissue should be available with possible good match in terms of color, texture, size and shape with surrounding tisseus. We experienced four different methods of nasal reconstruction; midline forehead flap, scalping flap, free radial forearm flap and expanded forehead flap to reconstruct subtotal or total nasal defects in four patients. We present their results and review their indications, advantages and limitations.

      • 국내 토양오염 및 광주광역시 토양오염의 통합적인 정량화에 따른 중금속의 변화

        이 연구는 국내 토양오염과 광주광역시 토양 오염도의 통합적인 정량화에 따른 중금속의 변화에 관한 연구이다. 연구결과 국내의 토양오염과 광주광역시의 토양 오염도의 정도는 1998, 1999, 및 2000년을 비교하여 본 결과 공장요지 및 철도 용지에서는 카드뮴, 구리, 납의 오염도가 다른 토지용도에 비하여 높게 나타난 것이 특징적이며, 광주광역시는 1998및 1999년도를 비교하여 본 결과 구리와 비소만이 약간 증가하였고 나머지 중금속들은 감소하였다. 광주광역시는 하남공단과 본촌공단 내에서의 유류 유출은 없는 것으로 판단되며, 밭 토양에서 약간의 유류가 검출되었는데 이는 주변의 주유소의 영향으로 생각되어진다.그러나 전국적인 평균에 비한다면 아주 청정한 토양이다. 전반적으로 토양오염 측정망이 가동된 기간이 매우 짧아서 정확한 판단을 내리기는 어려우나 납의 변화량이 가장 큰 변위를 나타내는데 측정망 원년에 가장 높은 수치를 나타내며 1998년에 약간 감소하다 1999년에 상승하고 2000년에는 다시 감소한는 경향을 보이고 있다. 카드뮴은 증가 추세를 보이고 있고 수은과 시안은 감소 추세를 보이고 있다. This study is a change of heavy metals according to integrative fixed quantity on soil pollution level in domestic area and Gwangju city. As a result of the study which compared with the 1998,1999 and 2000 years data, it is characterized that Cd, Cu, Pb, pollution level of national and GwangJu metropolitan city in factiry sites and railway sites are higher than other sites. The study also showed that only Cu and As were slightly increased and other heavy metals was decreased. It is believed that there is no evidence of oil spill pollution in Hanam and Bonchon industrial complex of Gwangju city. But some oil was detected by chemical analysis in field area, it is thought as the effect of gas stations in the neighborhood. On the whole it is difficult to make an accurate conclusion because of the very short term of operating soil pollution survey net. The change amount of lead shows the biggest displacement. It shows the highest figure the first year of survey, and it has a little decline in 1998. The figure goes up in 1999, and drops again in 2000. Cd is on an increasing trend, but Hg and CN is the opposite situation.

      • 성인형 당뇨병에서의 심혈관 합병증과 지질대사와의 관계

        Hypwelipiswmia is commonly observed in the diabetics, especially with cardiovascular complications. We studied lipid profiles and cardiovascular complication in the 191 adult onset DM patients. The results are as follows: 1. The age distribution was from 24 years old to 82 years old. Male patients were 99 persons, famale patients were 92 persons. The mean disease duration was less than 2 years in the 77 patients, more than 5 years in the 79 patients. 2. Upon the classification between sexes, the mean age was 55±1(SEM) years in the male group, 53±1(SEM) years in the female group. The mean disease durtion was 6.4±1(SEM) years in the female group, longer in the male group (P<0.05). The triglyceride level was 210±16(SEM)㎎% in the male group, 214±13(SEM)㎎% in the female group. The fasting blood sugar level was 147±6(SEM)㎎/dl in the male group, 158±8(SEM)㎎/dl n the female group. The cholesterol level was 229±5(SEM)㎎/dl in the male group, 248±5(SEM)㎎/dl in the female group, higher in the female group (P<0.05). 3. The cardiovascurlar complicatioin (Hypertension, Myocardial infarction, Myocardial ischemia, Angina, Gangrene, Proliferative retinopathy) were present in the 60 patients from 191 patients. Hypertension was present in the 48 patients, and the most prevalent. The mean age and carrdiovascular complication were higher in the more than 5 years of mean disease duration group(mean age 57.99±1.14(SEM)㎎ year) than less than 5 years group(mean age 52.12±0.10(SEM)㎎ year) (P<0.001, P<0.01). 4. The mean disease duration (7.40±1.01(SEM) years), mean triglyceride levels(271±1.01(SEM)㎎%), and mean cholestrerol levels( 257±8㎎.dl) in the cardiovascularly complicated group were higher than those of non-complicated group (P<0.01). 5. The triglycerdie level was high in the obese group (230±14SEM㎎%) than non-obese group (184±15SEM㎎%) (P<0.05). The obese patients were more prevalent in the female group than male group significantly. 6. Considering obesity and cardiovascular complications, the triglyceride value was higher in the obese complicated group(300±30SEM㎎%) than obese non-complicated (197±14SEM㎎ %) or non-obese non-complicated (168±13SEM㎎ %) group (P<0.001). The cholesterol level was higher in the obese complicated group (267±10SEM㎎/dl) than obese non-complicated (230±5SEM㎎/dl) or non-obese non-complicated group(229±7SEM㎎/dl) (P<0.05). 7. Classifying upon insulin response pattern, the obese patients were found in the response pattern, low response pattern, flat type pattern(no response pattern) in the decreasing number of order (P<0.01). The cardiovascular complications were found in the low response group, response group, and flat type pattern (no response) in the decreasing number of order ( P<0.05). The combined cases of obesity and cardiovascular complications were found in the low response group, response group and flat type group(no response) pattern in the decreasing number of order (P<0.01). The mean disease duration was longer in the flat type group (no response) (4.48±0.6SEM years) than the response group(2.43±0.90SEM years) (P<0.05). The mean triglyceride value was higher in the response group (267.38±154.75SEM㎎%) than the flat type(no response group) (198.35±154.05SD㎎%) (P<0.05). So, we conclude that in the NIDDM, obesity control(die control, ideal body weight mainterice, and hyperlipidemia control) is essential for better DM control and cardiovascular complication control.
