      • Distribution of BRCA1/2 mutation and clinical outcomes in epithelial ovarian, peritoneal, fallopian tube cancer: based on multicenter real-world data

        Objective: This study aimed to review multicenter real-world data for BRCA1/2 gene test and clinical outcomes in epithelial ovarian, peritoneal, fallopian tube cancer. Methods: We reviewed the patients who underwent primary surgery and adjuvant treatment between January 2009 and December 2021 in Seoul, Yeouido, and Incheon St. Marys hospital, the college of medicine, the catholic university of Korea. The following data were retrospectively obtained for analysis: clinical factors such as patient age, levels of tumor markers (CA 125, CA 19-9); International Federation of Gynecology and Obstetrics stage at diagnosis, histological types, and BRCA 1/2 gene test. Results: In total, 652 patients were evaluated and 237 patients (36.3%) were tested BRCA 1/2 mutation. Among all patients who received BRCA 1/2 test, 62 patients (26.2%) were noted BRAC 1/2 mutation. The other of 136 patients (57.4%) were resulted wild type and 39 patients (16.4%) were done variants of uncertain significance. In BRCA1/2 mutation group, 53 had high-grade serous ovarian cancer (HGSOC, n = 53) and 9 had non-HGSOC, consisting of endometrioid carcinoma (n = 5), clear cell carcinoma (n = 2), and mixed carcinoma (n = 2). The portion of HGSOC in BRCA 1/2 mutation group was significant higher than that of BRCA wild type (85.8% versus 61.0%, p<0.001). Overall survival and level of CA 125 of BRCA1/2 mutation group were significantly higher than those of BRCA 1/2 wild group (60.4 vs. 42.9 months, 1750 vs. 1272 mg/dl, respectively). However, only BRCA 1/2 result was reported significantly difference in multivariate logistic regression (Odd ratio: 4.73, 95% confidential Interval, 1.53 to 14.6, p=0.007). Although 26 patients of BRCA 1/2 mutation group were treated PARP inhibitor for maintenance therapy, the use of PARP inhibitor was not significant meaning in multivariate logistic regression. Conclusion: BRCA 1/2 mutation test was essential for epithelial ovarian, peritoneal, and fallopian tube cancer regardless of histologic findings.

      • Cognition of female immigrants in Korea about necessity of antenatal folic acid

        Objective: We aimed to assess how immigrant women in fertility and pregnancy perceive the importance of folic acid and further to evaluate the percentage of taking folic acid in immigrant women in pregnancy. Methods: We designed the study for women in fertility (from 15-49 years old) visiting Ahn San Vincent hospital located in Ahn San. We surveyed women who answered the survey from October, 2018 to April 2019. Results: Total 67 women replied the survey. Average age was 29.9+-6.88 years old, 42 were pregnant women, while 25 were not. Nationalities were Uzbekistan (15), Cameroon (10), Philippines (9), Kazakhstan(8), and Russia(7). Group which answered they were aware of folic acid was 29 females which was 43.3%. 54.3% of pregnant women answered they were aware of the role of folic acid while in non-pregnant women it was only 24% (p-value = 0.001). 64.3% of pregnant women answered they were aware how folic acid worked its role during pregnancy while in non-pregnant women, it was only 40% (p-value = 0.001). 18 out of 42 were pregnant women with planned pregnancy and among them, only 7 women took folic acid before pregnancy, which was 16.7%. For those who answered with planned pregnancy, 88.8% answered they were aware of the importance of folic acid while it was only 42.8% in unplanned pregnancy (p-value = 0.001). However, even when it was planned pregnancy, only 27.7% took folic acid beforehand. Conclusion: The percentage of taking folic acid in immigrant women in Korea was low as we expected. We assume there must be multiple factors such as low socio-economic class, education level, and linguistic barrier.

      • A case of uterine prolapse during pregnancy

        Prolapse uteri during pregnancy is a rare event. We demonstrate a case of a patient with uterine prolapse during pregnancy. The patient was treated with vaginal pessary to avoid maternal and fetal risks, which would be fatal. We removed the pessary at 36+5wks of gestation and the patient delivered vaginally after pessary was removed. At postpartum follow-up uterine prolapse recurred and vaginal pessary was reinserted.

      • Hobnail-like shallow Electrosurgical Excision Prosedure(HEEP) for the treatment of HSIL: prevention of IIOC

        Objective: We knew that conventional conization can make the incompetent internal os of cervix, which can induce the abortion and preterm delivery. So, we try to minimize the removed volume after conization. Methods: 34 young patients with HSIL ( final diagnosis: CIS or cervical cancer FIGO stage IA1 ) were enrolled in the Department of Obstetrics and Gynecology of St. Vincent\’s Hospital from Jan. 2014 to Aug. 2018. We treated those patients with Hobnail-like shallow Electrosurgical Excision Prosedure(HEEP) of uterine cervix for the treatment HSIL. Results: 4 women experienced 5 pregnancies. 4 of 5 pregnancies were kept more than 34 gestation weeks. Furthermore, all of 5 pregnancies gave healthy live births. Conclusion: HEEP could be considered as a treatment of choice for HSIL.

      • Water-soluble mononuclear cobalt complexes with organic ligands acting as precatalysts for efficient photocatalytic water oxidation

        <P>The photocatalytic water oxidation to evolve O<SUB>2</SUB> was performed by photoirradiation (<I>λ</I> > 420 nm) of an aqueous solution containing [Ru(bpy)<SUB>3</SUB>]<SUP>2+</SUP> (bpy = 2,2′-bipyridine), Na<SUB>2</SUB>S<SUB>2</SUB>O<SUB>8</SUB> and water-soluble cobalt complexes with various organic ligands as precatalysts in the pH range of 6.0–10. The turnover numbers (TONs) based on the amount of Co for the photocatalytic O<SUB>2</SUB> evolution with [Co<SUP>II</SUP>(Me<SUB>6</SUB>tren)(OH<SUB>2</SUB>)]<SUP>2+</SUP> (<B>1</B>) and [Co<SUP>III</SUP>(Cp<SUP>*</SUP>)(bpy)(OH<SUB>2</SUB>)]<SUP>2+</SUP> (<B>2</B>) [Me<SUB>6</SUB>tren = tris(<I>N</I>,<I>N</I>′-dimethylaminoethyl)amine, Cp<SUP>*</SUP> = <I>η</I><SUP>5</SUP>-pentamethylcyclopentadienyl] at pH 9.0 reached 420 and 320, respectively. The evolved O<SUB>2</SUB> yield increased in proportion to concentrations of precatalysts <B>1</B> and <B>2</B> up to 0.10 mM. However, the O<SUB>2</SUB> yield dramatically decreased when the concentration of precatalysts <B>1</B> and <B>2</B> exceeded 0.10 mM. When the concentration of Na<SUB>2</SUB>S<SUB>2</SUB>O<SUB>8</SUB> was increased from 10 mM to 50 mM, CO<SUB>2</SUB> evolution was observed during the photocatalytic water oxidation. These results indicate that a part of the organic ligands of <B>1</B> and <B>2</B> were oxidized to evolve CO<SUB>2</SUB> during the photocatalytic reaction. The degradation of complex <B>2</B> under photocatalytic conditions and the oxidation of Me<SUB>6</SUB>tren ligand of <B>1</B> by [Ru(bpy)<SUB>3</SUB>]<SUP>3+</SUP> were confirmed by <SUP>1</SUP>H NMR measurements. Dynamic light scattering (DLS) experiments indicate the formation of particles with diameters of around 20 ± 10 nm and 200 ± 100 nm during the photocatalytic water oxidation with <B>1</B> and <B>2</B>, respectively. The particle sizes determined by DLS agreed with those of the secondary particles observed by TEM. The XPS measurements of the formed particles suggest that the surface of the particles is covered with cobalt hydroxides, which could be converted to active species containing high-valent cobalt ions during the photocatalytic water oxidation.         정책 과정론 관점에서 외고·자사고 정책 논의

        이 논문은 정책이란 다양한 수준의 정책 참여자들 간 역동과 정책 문서에 대한 상황적 해석을 통해 독창적으로 집행될 수 있다는 정책 과정론 관점에서 국내 고교유형 다양화 정책 과정을 탐색하였다. 특히 문재인 정부의 출범과 함께 폐지위기에 놓인 외고·자사고 정책의 정책 과정에 초점을 두고 해당 정책을 둘러싼 맥락적 요인, 정책 참여자들의 인식 및 인식의 변화, 상충하는 목표 달성을 위한 전략들을 조망하였다. 이를 위해 정책을 과정으로 바라보는 다양한 정책이론들에 대한 면밀한 탐색에 기초해 분석 틀을 고안하였으며, 분석을 통해 외고·자사고라는 정책 하위체제는 사회 구조적 맥락 하에서 다른 정책 하위체제들과 긴밀하게 연결되어있으며, 정책 문서에 사용된 공식적 규칙 및 정책 용어들에 대한 해석의 차이는 논란의 원인이 될 수 있다는 점과 정책 행위자들의 담론 간 모순을 확인하였다. 이를 통해 궁극적으로 추후 일반고가 나아가야 할 방향에 대한 시사점을 제언하였다. Weible et al. (2012:1) once claimed that one of the most frequently asked questions from students, researchers, and public policy officials is “how to influence the policy process.” In that regard, this research touches on the controversial impending abolishment of elite high schools, and it started out with the curiosity of a foreign language high school student who questioned why the students themselves are not asked for their opinions concerning the issue. In this paper, we adopted a descriptive approach—as opposed to a normative approach—and on a practical level, this standpoint acknowledges the impact of the actors, the different values and strategies, and the varying interpretations of policy documents that are involved in the policy process. Based on this theoretical inquiry, we devised an analytical framework that includes three elements: structure, actors, and political behaviors. This framework was used to thoroughly examine the policy process related to the contended impending abolishment of elite high schools. Through this, we proposed the problems that need to be resolved beforehand in preparation of the imminent discussion of the direction of the high school type policies and also deduced implications.

        현재 판매 중인 대부분의 그룹웨어는 큰 규모의 기업 위주로 설계되어 과도하게 많은 기능이 탑재되어 있다. 이로 인해 사용자들이 쉽게 적응할 수 없어 사용하기 어려운 기능들은 대부분 방치된 채 비용만 들어가고 있다. 이 논문에서는 직원 관리, 업무 내용 공유 및 관리, 보고 및 결재의 필수적인 기능만을 가진 그룹웨어를 설계·개발하여, 비용 절감과 효율적인 업무 관리를 가능하게 한다. Most of the groupware currently on sale is designed mainly for large companies and is equipped with excessively many functions. As a result, most of the functions that are difficult to use because users cannot easily adapt are left unattended and only costs money. In this paper, we design and develop groupware that has only essential functions of employee management, work content sharing and management, reporting, and approval, enabling cost reduction and efficient work management.
