        Case Reports : Basal Cell Carcinoma on the Pubic Area: Report of a Case and Review of 19 Korean Cases of BCC from Non-sun-exposed Areas

        Basal cell carcinoma (BCC) is one of the most commonly diagnosed malignant skin tumors and develops characteristically on sun-exposed areas, such as the head and neck. Ultraviolet light exposure is an important etiologic factor in BCCs, and BCCs arising from non-sun- exposed areas are, therefore, very rare. In particular, the axilla, nipple, the genital and perianal areas are not likely to be exposed to ultraviolet light; thus, if BCC develops in these areas, other predisposing factors should be considered. Herein, we report a case of BCC arising on the pubic area in a 70-year-old man. We also performed a survey of the literature and discussed the 19 cases of BCC from non-sun-exposed areas reported to date in Korea.

      • 중·고등학교 청소년의 자살사고 및 자살시도의 유병률과 자살시도의 위험요인에 관한 연구

        연구목적 : 지역사회에 거주하고 있는 중·고등학교 청소년들을 대상으로 자살 사고 및 자살시도의 빈도, 성별과 연령(13∼15세 vs 16∼18세)에 따른 자살사고 및 시도의 빈도에 차이가 있는지 여부, 자살사고 및 자살시도가 있는 청소년의 특징, 그리고 자살시도의 위험요인을 알아보고자 하였다. 방 법 : 부천시에 소재하고 있는 중학교 24개교 및 고등학교 20개교의 청소년 71,102명중에서 무작위 집락 표본 추출범으로 청소년 2,203명(남자 1,105명, 여자 1,098명)을 조사대상으로 선정하였다. 자살시도 및 자살사고의 정의에 관한 항목은 Moscicki가 사용한 자살행동에 관한 4항목 중 2항목을 사용하였다. 우울증에 대한 조사도구는 the Center for Epidemiologic Studies Depression Scale, 알코올 의존에 대한 조사도구는 Composite International Diagnostic Interview의 알코올 의존 관련 항목을 사용하였다. 조사자료 통계는 빈도, 백분율, chi-square test, 단변인 및 다변인 로지스틱 회귀분석을 사용하였다. 결 과 : 본 조사연구의 분석결과를 통계적으로 유의한 차이를 보인 항목으로 살펴보면 다음과 같다. 1) 전체 대상자의 자살 사고률은 30.7%, 자살 시도률은 5.3%인 것으로 조사되었다. 남자 대상자의 자살 사고률과 시도률은 26.9%, 3.4%이었고, 여자 대상자의 자살 사고률과 시도률은 34.9%, 7.3%인 것으로 조사되었다. 성별에 따라 자살 사고률과 시도률에 통계적으로 유의한 차이가 있었다. 2) 연령(학년)을 13∼15세(중학교)와 16∼18세(고등학교)의 두 연령층으로 조사하였을 때, 중학생의 자살 사고률과 시도률은 30.1%, 4.3%이었고, 고등학생의 자살 사고률과 시도률은 31.6%, 6.4%인 것으로 조사되었다. 연령에 따른 자살 사고률에는 통계적으로 유의한 차이가 없으나 자살 시도률에는 유의한 차이가 있었다. 3) 전체 조사 대상자를 대상으로 시행한 다변인 로지스틱 회귀 분석 결과에 의하면, 자살 시도의 위험요인이 여자, 알코올 의존, 중등도 및 중증 우울 증상인 것으로 조사되었다. 남녀별로 시행한 단변인 로지스틱 회귀 분석 결과에 의하면, 남자는 알코올 의존이 있을 때, 여자는 알코올 의존, 중등도 및 중증의 우울 증상이 있을 때 자살시도의 위험성이 높았다. 결 론 : 본 연구에서 자살 사고률 및 자살 시도률이 각각 29.6%(남자 25.6%, 여자 33.7%)와 5.31%(남자 3.3%, 여자 7.3%)인 점과 자살시도의 위험요인이 우울 증상의 정도, 알코올 의존인 점을 고려할 때 알코올 의존 및 우울 증상의 조기발견과 치료, 그리고 자살 사고자 및 자살 시도자에 대한 정신의학적 접근과 추적관찰에 대한 방안이 강구되어야 할 것으로 생각된다. Objectives : Aims of this study were 1) to estimate prevalence rates of suicidal ideation and suicide attempt, 2) to find out whether the gender difference and age difference(13-15 years vs 16-18 years) for suicide behaviors exist, 3) to analyse correlates of suicide attempts among junior and high school adolescents aged 13-18 years old. Methods : A self-administered questionnaire was used in a cluster random sample of 2,203 school adolescents(male 1,105, female 1,098). Data were obtained on a wide range of constructs including suicidal ideation and attempts during past 2 weeks, depressive symptoms, symptoms of DSM-IV alcohol dependence, family structure, socioeconomic status, smoking, satisfaction with school achievements, and subjective health status. Chi-square test were done for analysing gender difference and age difference of suicide behaviors. Statistical analyses including uni-variate and multi-variate logistic regression analysis were done for each sex, respectively. Results : 1) The prevalence of suicide ideation during past 2 week was 30.7% in the total sample. The prevalence rates of male and female ideators were 26.9% and 34.9% respectively. The prevalence of suicide attempt during past 2 week was 5.3% in the total sample. The prevalence of suicide attempt in males was 3.4% and 7.3% in females. 2) Gender difference was significant. That is, female respondents showed higher rate of suicidal ideation and suicide attempt. In our study age difference was found only at the rate of suicide attempt. That is, older adolescents(16-18 years) showed only statistically significant higher rate of suicide attempt than younger adolescents(13-15 years). 3) Multi-variate logistic regression analysis revealed that the correlates for suicide attempt were female, moderate to severe depressive symptoms, alcohol dependence in the total sample. In males the correlate for suicide attempt was alcohol dependence. In females, were moderate to severe depressive symptoms, and alcohol dependence. Conclusion : Depressive symptoms, alcohol dependence, presence of suicidal ideation were strong predictors for suicide attempts. This suggest that early detection of treatment of depression and alcohol dependence is important and psychiatric approach and follow-up be needed for the prevention of suicide.

        Fullerene의 유전독성을 평가하기 위하여 Chinese hamster유래의 난소유아세포(CHO-K1 cell)를 이용하여 직접법(-S9)과 대사활성화법(+S9 mix)의 염색체이상시험을 실시하였다. 시험물질은 1% CMC 나트륨염의 현탁액(l% CMC 용액)에 희석하여 조제하였다. 대사활성화를 시키지 않은 직접법의 염색체이상시험에서 24시간 투여군은 8단계의 농도(0.078, 0.156, 0.313, 0.625, 1.25, 2.5, 5, 10mM)로 투여하여 실시하였다. 투여 농도 증가에 따른 염색체이상의 빈도가 증가하는 양상이 나타나지 않았다. 48시간의 투여군에서는 8단계의 농도(0.078, 0.156, 0.313, 0.625, 1.25, 2.5, 5, 10mM)로 투여하여 실시하였는데 투여 농도 증가에 따른 염색체이상의 빈도가 증가하는 양상이 나타나지 않았다. 배수체의 염색체이상은 직접법에서 관찰되지 않았다. 대사활성화법을 이용하여 6시간 시험물질을 투여한 시험에 있어서는 8단계의 용량단계 (0.078, 0.156, 0.313, 0.625, 1.25, 2.5, 5, 10mM)를 설정하였는데 투여 농도가 증가함에 따른 염색체이상빈도의 증가양상이 관찰되지 않았다. 이상의 결과를 종합할 때 본 시험물질은 본 시험 조건하에서 CHO-K1세포에서 대사활성화를 시켰을 때 염색체 이상을 유발하지 않는 것으로 판단된다.

        태양돛 기술 동향

        태양돛은 태양 광자들의 운동량 변화를 이용하여 추력을 얻는 우주선이다. 매우 작지만 연속적인 가속도를 얻을 수 있어 태양돛은 다양한 임무를 수행할 수 있다. 효과적인 임무 개발을 위해서는 태양돛의 형상, 돛의 재료 그리고 전개 장치에 따른 구조/재료적 특성뿐만 아니라 위성의 임무 궤적에 따른 자세제어 방법들을 고려해야 한다. 본 논문에서는 태양돛의 기술발전, 태양돛 제작에 필요한 소요기술 그리고 태양돛을 이용한 향후 개발 가능한 위성 임무에 대하여 살펴보고자 한다. Solar sail spacecrafts can gain propulsion using the momentum change through reflecting the photon packets of energy from the Sun. The sail slowly but continuously accelerates to accomplish a wide-range of potential missions. To develop the potential mission of the solar sail, the configuration, the film characteristics and the deployment devices should be carefully considered. In this paper, recent development and activities of the solar sail are introduced and design technology of the sail subsystem is investigated.

        한국어 판 구조화 임상면담도구 개발 : 신뢰도 연구

        연구목적: 정신장애의 정확한 진단과 평가는 임상에서뿐만 아니라 신뢰성 있는 연구를 위해서도 매우 중요하다. Structured Clinical Interview for DSM-IV(SCID)는 임상전문가들이 사용하는 진단도구로서, 비교적 짧은 시간에 정확한 진단을 내릴 수 있다. 본 연구는 한국어판SCID를 개발하고 그 신뢰도를 평가하기 위해 수행되었다. 방법: 국문학자가 포함된 번역위원회를 통하여 연구용판 SCID를 번역한 후, 정신과 의사 2인과 임상심리학자 1인에게 한국어판SCID 실시방법을 교육시킨 뒤 한국어판SCID를 이용한 면담의 평가자간 신뢰도 (interrater reliability)를 검증하였다. 면담 대상은 1999년 2월에서 3월까지 2개 병원 정신과에 치료중인 환자 90명(남:41명, 여:29명)이었다. 결과: 현재 장애(current disorder) 평가에서 주요 우울장애, 기분부전장애, 정신분열병, 알코올 남용 및 의존, 기타 물질 남용 또는 의존, 여러 불안장애들 및 섭식장애 등과 같은 대부분 장애의 kappa값은 .70이상으로 매우 높았다. 그 이외의 양극성 장애, 망상장애, 광장공포증, 감별 불능 신체화 장애 및 건강염려증의 kappa값도 .69에서 .40사이로 수용할 수 있는 정도였다. 평생 장애(lifetime disorder)에서는 양극성 장애(k=.69)와 감별 불능 신체화장애(k=.59)를 제외한 다른 모든 장애의 kappa값이 .70이상이었다. K-SCID 면담시 Ⅰ축 질환에 대하여 흔하게 다중 진단이 내려졌으며, 평균 진단 수는 1.5∼1.7개로 나타났다. 결론: 한국어판 SCID는 신뢰도가 높은 진단도구로 생각되며, 향후 정신질환의 정확한 진단과 임상연구에 유용하게 사용될 수 있다. Objectives: Accurate diagnosis and assessment for psychiatric disorders is crucial for research, as well as for clinical practice. Structured Clinical Interview for DSM-Ⅳ(SCID-RV) is a less time-consumimg and more accurate structured diagnostic interview form. It can be used by clinical professions and is known for a reliable diagnostic tool. Present study was conducted to develop Korean version of SCID-RV and to test the inter-rater reliability. Methods: The authors have translated original SCID-RV into Korean, and revised in parallel with sociocultural background of Korea. Ninety patients from two psychiatric hospitals, both outpatient and inpatient, were interviewed and rated independently by three raters. Results: The kappa coefficients for most of illnesses, such as major depressive disorder, dysthymia, schizophrenia, alcohol abuse and dependency, anxiety disorder and eating disorder were excellent(>0.70) in the evaluation of current disorders. And the kappa coefficients for bipolar disorder, delusional disorder, agoraphobia, undifferentiated somatoform disorder, and hypochondriasis were acceptable(>0.40) in the evaluation of current disorders. In the evalua-tion of lifetime disorders, the concordant rates of all the diagnoses except bipolar disorder and undifferentiated somatoform disorder were excellent. Lack of hierarchy in DSM-Ⅳ allows for multiple Axis I diagnoses. Mean numbers of Axis I diagnoses per subject assigned by the three raters were 1.5-1.7. Conclusions: Our findings confirm that SCID-RV yields highly reliable diagnoses. SCID-RV is recommended for accurate diagnosis in clinical practice and research on psychiatric disorders.

        Modeling of CO₂ Emission from Soil in Greenhouse

        Greenhouse industry has been growing in many countries due to both the advantage of stable year-round crop production and increased demand for fresh vegetables. In greenhouse cultivation, CO₂ concentration plays an essential role in the photosynthesis process of crops. Continuous and accurate monitoring of CO₂ level in the greenhouse would improve profitability and reduce environmental impact, through optimum control of greenhouse CO₂ enrichment and efficient crop production, as compared with the conventional management practices without monitoring and control of CO₂ level. In this study, a mathematical model was developed to estimate the CO₂ emission from soil as affected by environmental factors in greenhouses. Among various model types evaluated, a linear regression model provided the best coefficient of determination. Selected predictor variables were solar radiation and relative humidity and exponential transformation of both. As a response variable in the model, the difference between CO₂ concentrations at the soil surface and 5-㎝ depth showed are latively strong relationship with the predictor variables. Segmented regression analysis showed that better models were obtained when the entire daily dataset was divided into segments of shorter time ranges, and best models were obtained for segmented data where more variability in solar radiation and humidity were present (i.e., after sun-rise, before sun-set) than other segments. To consider time delay in the response of CO₂ concentration, concept of time lag was implemented in the regression analysis. As a result, there was an improvement in the performance of the models as the coefficients of determination were 0.93 and 0.87 with segmented time frames for sun-rise and sun-set periods, respectively. Validation tests of the models to predict CO₂ emission from soil showed that the developed empirical model would be applicable to real-time monitoring and diagnosis of significant factors for CO₂ enrichment in a soil-based greenhouse.

        열전대(thermocouple)를 이용한 Buchanan Plugger 표면의 온도상승 분석

        This study was performed to evaluate the actual temperature rise on the surface of Buchanan plugger using thermocouple. The heat carrier system 'System B Heatsource'(Model 1005, Analytic Technologies, Redmond, WA, USA) and the Buchanan pluggers of F, FM, M and ML sizes are used for this study. The temperature was set to 200℃ on digital display and the power level on it was set to 10. Five thermocouples were placed in direct contact with the surface of each size of Buchanan's pluggers at 1 mm increments from the tip to the 4 mm length of shank. The heat control spring was touched for 5 seconds, and temperature rise on the surface of the pluggers were measured at 1 sec intervals for more than 5 seconds with an accuracy of 0.01 using Data Logger. The data were statistically analyzed by one-way ANOVA. The results were as follows. 1. The position at which the temperature peaked was approximately at 1~2 mm far from the tip of Buchanan plugger (p<0.01). 2. The peak temperature was 215.25±2.28℃ in F plugger, 185.94±2.19℃ in FM plugger, 169.51±9.12℃ in M plugger, and 160.79±1.27℃ in ML plugger and the peak temperature was highest in F plugger and followed by, in descending order, FM plugger, M plugger. ML plugger showed the lowest peak temperature (p<0.01). 3. The temperature on the pluggers was decreased with the increase of touching time. This results suggest that the actual temperature on the surface of the pluggers dose not correlate well with the temperature set on digital display. Heat concentrates around the up. The larger plugger reveals lower temperature rise relatively.

      • 축대칭 왕복엔진의 비정상 난류 유동에 대한 수치 해석

        A numerical simulation of unsteady axisymmetric turbulent flow was performed for a reciprocating engine including port/valve assembly. The governing equations based on a non-orthogonal coordinate formulation with Cartesian velocity components were used and discretised by the finite volume method with non-staggered variable arrangements. The modified k-ε turbulence model which included the effect of compressibility was used. The results of two-dimensional transient calculation for the axisymmetric configuration were compared with the experimental data. Although slightly low rms velocity was predicted compared to the experimental data, predicted velocity distributions at the valve exit and in-cylinder region showed good agreements with the experimental data. The flow at the valve exit was separated at the same valve lift position with experimental data. The mean flow pattern at the valve exit was found to be more sensitive to valve lift than mass flow rate or valve motion. Two vortices in-cylinder region were generated during the initial intake process. The clockwise main vortex became strong and moved upward to the top wall. The counter-clockwise second vortex became weak and stick to the upper left corner of the cylinder. After middle intake process, new vortex adjacent to upper cylinder wall appeared by the piston motion and therefore, the in-cylinder flow was formed into three vortices. The cylinder pressure just before bottom dead center of piston was higher than inlet pressure and then the reverse flow occured at the valve exit. The in-cylinder flow characteristics were strongly dependent on piston motion, but insensitive to valve motion.
