        Accuracy of cardiac output measurements during off-pump coronary artery bypass grafting: according to the vessel anastomosis sites

        Background: During beating heart surgery, the accuracy of cardiac output (CO) measurement techniques may be influenced by several factors. This study was conducted to analyze the clinical agreement among stat CO mode (SCO), continuous CO mode (CCO), arterial pressure waveform-based CO estimation (APCO), and transesophageal Doppler ultrasound technique (UCCO) according to the vessel anastomosis sites. Methods: This study was prospectively performed in 25 patients who would be undergoing elective OPCAB. Hemodynamic variables were recorded at the following time points: during left anterior descending (LAD) anastomosis at 1 min and 5 min; during obtuse marginal (OM) anastomosis at 1 min and 5 min: and during right coronary artery (RCA) anastomosis at 1 min and 5 min. The variables measured including the SCO, CCO, APCO, and UCCO. Results: CO measurement techniques showed different correlations according to vessel anastomosis site. However, the percent error observed was higher than the value of 30% postulated by the criteria of Critchley and Critchley during all study periods for all CO measurement techniques. Conclusions: In the beating heart procedure, SCO, CCO and APCO showed different correlations according to the vessel anastomosis sites and did not agree with UCCO. CO values from the various measurement techniques should be interpreted with caution during OPCAB.         가압가열 및 Microwave 처리가 생면의 품질에 미치는 영향

        중력분을 이용하여 제면한 후 가압가열, microwave 및 가압가열과 microwave 병행 처리한 후, 생면 및 삶은 면의 품질에 미치는 영향에 대하여 알아보았다. 생면의 pH는 microwave 1 min 처리구에서 무처리구와 유의적인 차이를 보이지 않았으며, 수분 함량은 autoclave 50 min 처리구에서 무처리구와 유의적인 차이를 보이지 않았고, 그 외의 처리구에서는 모두 수분 함량이 감소함을 확인하였다. 면의 색도 측정 결과는 명도는 무처리구에 비해 모든 처리구에서 감소하였으며, 적색도는 microwave 1 min 처리구는 감소한 반면, 나머지 처리구는 증가하였다. 황색도는 autoclave 50 min 처리구와 autoclave 50 min/microwave 1 min 병행 처리구에서 증가한 반면, 나머지 물리적 처리구에서는 감소하였다. 삶은 면의 색도에서는 명도, 적색도, 황색도 모두에서 무처리구에 비하여 물리적 처리구에서 증가한 값을 나타내었다. 물성 측정 결과는 경도, 부착성, 응집성, 검성, 복원성에서 무처리구와 비교시 microwave 1 min 처리구는 유의적으로 감소하였으며, 그 외 처리구에서는 유의적으로 증가한 값을 나타내었다. 탄력성과 전단력은 모든 처리구에서 유의적으로 증가하였으며, 인장력은 가압가열 30 min 처리구와 무처리구가 유의적인 차이를 보이지 않았으나, 나머지 물리적 처리구에서는 유의적으로 증가하였다. 관능 평가 결과는 생면의 경우 색 항목에서 물리적 처리구들이 유의적으로 낮은 점수를 받았다. 맛, 질감, 향 항목에서는 무처리구와 물리적 처리구간 유의적 차이를 보이지 않았다. 전체적인 기호도에서는 microwave 1 min 처리구가 가장 높은 점수를 얻었으며, 가압가열(50 min)과 microwave(1 min) 병행 처리구는 무처리구와 비슷한 점수를 얻었다. 따라서 제면 후 가압가열 및 microwave 처리가 생면 및 삶은 면에 있어서 색과 질감 항목을 보완, 개선시킨다면 알러겐성이 저감화된 면을 제품화하는데 적합할 것으로 사료되어진다. This study was conducted to determine the effects of physical treatments for quality of wet noodles. Noodles were being tried with a microwave (for 1 min), an autoclave (for 30 or 50 min), and both autoclave and microwave (for 30/1 min or 50/1 min). The results showed that the pH levels were slightly decreased after treatments of autoclave and autoclave/microwave. The moisture contents were considerably decreased as compared to the control except autoclave (50 min). After all treatments, the lightness was decreased in all samples, but, redness was increased (except microwave) and the yellowness was increased after autoclave (50 min) and autoclave/microwave (50/1 min). Texture was increased as compared to the control except microwave. In the sensory evaluation, the noodles treated with microwave, autoclave (50 min), and autoclave/ microwave (50/1 min) showed a high score in overall preference. From these results, both the autoclave and microwave methods can be applied to the wet noodles without diminishing its quality to a great extent.

      • 디지털 음악 및 아날로그 음악이 악력에 미치는 영향 평가

        This study evaluated the effect of the music type (analog and digital) on the grip strength. Five male and five female healthy college students participated in the experiment. The independent variables were music type (analog, digital, no music) and listening time (5 min., 10 min., and 15 min.). The classic LP disc was used as the analog music and the LP was converted to mp3 to make the digital music source. Grip forces were measured every 5 minutes until 15 minutes using Jamar strain guqge. Mean forces and peak forces were used as dependent variables. The analysis of variance showed that there was no difference between analog and digital music (p>0.05). Only, the gender effect was statistically significant (p<0.05). These results should be verified using different types of music (not only classical music) or preferred music, during different listening time duration (>15 minutes).

      • Chlorpropamide 2-hydroxylation is catalysed by CYP2C9 and CYP2C19 <i>in vitro</i>: chlorpropamide disposition is influenced by CYP2C9, but not by CYP2C19 genetic polymorphism

        <P>Aims</P><P>We evaluated the involvement of cytochrome P450 (CYP) isoforms 2C9 and 2C19 in chlorpropamide 2-hydroxylation <I>in vitro</I> and in chlorpropamide disposition <I>in vivo</I>.</P><P>Methods</P><P>To identify CYP isoforms(s) that catalyse 2-hydroxylation of chlorpropamide, the incubation studies were conducted using human liver microsomes and recombinant CYP isoforms. To evaluate whether genetic polymorphisms of CYP2C9 and/or CYP2C19 influence the disposition of chlorpropamide, a single oral dose of 250 mg chlorpropamide was administered to 21 healthy subjects pregenotyped for CYP2C9 and CYP2C19.</P><P>Results</P><P>In human liver microsomal incubation studies, the formation of 2-hydroxychlorpropamide (2-OH-chlorpropamide), a major chlorpropamide metabolite in human, has been best described by a one-enzyme model with estimated <I>K</I><SUB><I>m</I></SUB> and <I>V</I><SUB>max</SUB> of 121.7 ± 19.9 µ<SMALL>M</SMALL> and 16.1 ± 5.0 pmol min<SUP>−1</SUP> mg<SUP>−1</SUP> protein, respectively. In incubation studies using human recombinant CYP isoforms, however, 2-OH-chlorpropamide was formed by both CYP2C9 and CYP2C19 with similar intrinsic clearances (CYP2C9 <I>vs.</I> CYP2C19: 0.26 <I>vs.</I> 0.22 µl min<SUP>−1</SUP> nmol<SUP>−1</SUP> protein). Formation of 2-OH-chlorpropamide in human liver microsomes was significantly inhibited by sulfaphenazole, but not by <I>S</I>-mephenytoin, ketoconazole, quinidine, or furafylline. In <I>in vivo</I> clinical trials, eight subjects with the <I>CYP2C9</I>*<I>1/</I>*<I>3</I> genotype exhibited significantly lower nonrenal clearance [*<I>1/</I>*<I>3 vs.</I>*<I>1/</I>*<I>1</I>: 1.8 ± 0.2 <I>vs.</I> 2.4 ± 0.1 ml h<SUP>−1</SUP> kg<SUP>−1</SUP>, <I>P</I> < 0.05; 95% confidence interval (CI) on the difference 0.2, 1.0] and higher metabolic ratios (of chlorpropamide/2-OH-chlorpropamide in urine: *<I>1/</I>*<I>3 vs.</I>*<I>1/</I>*<I>1</I>: 1.01 ± 0.19 <I>vs.</I> 0.56 ± 0.08, <I>P</I> < 0.05; 95% CI on the difference − 0.9, − 0.1) than did 13 subjects with <I>CYP2C9</I>*<I>1/</I>*<I>1</I> genotype. In contrast, no differences in chlorpropamide pharmacokinetics were observed for subjects with the <I>CYP2C19</I> extensive metabolizer <I>vs.</I> poor metabolizer genotypes.</P><P>Conclusions</P><P>These results suggest that chlorpropamide disposition is principally determined by CYP2C9 activity <I>in vivo</I>, although both CYP2C9 and CYP2C19 have a catalysing activity of chlorpropamide 2-hydroxylation pathway.</P>

        Korean physician prescription patterns for home oxygen therapy in chronic obstructive pulmonary disease patients

        Background/Aims: Hypoxemia in chronic obstructive pulmonary disease (COPD) leads to reduced ability to exercise, decreased quality of life, and, eventually, increased mortality. Home oxygen therapy in patients with severe COPD reduces distress symptoms and mortality rates. However, there have been few studies on physicians’ prescription behavior toward home oxygen therapy. Therefore, we investigated the respiratory specialists’ perspective on home oxygen therapy. Methods: In this cross-sectional, study, a questionnaire was completed by 30 pulmonary specialists who worked in tertiary hospitals and prescribed home oxygen therapy. The questionnaire consisted of 28 items, including 15 items on oxygen prescription for outpatients, four for inpatients, and nine on service improvement. Results: All physicians were prescribing less than 2 L/min of oxygen for either 24 (n = 10, 33.3%) or 15 hours (n = 9, 30.3%). All (n = 30) used pulse oximetry, 26 (86.7%) analyzed arterial blood gas. Thirteen physicians had imposed restrictions and recommended oxygen use only during exercise or sleep. Sixteen (53.3%) physicians were educating their patients about home oxygen therapy. Furthermore, physicians prescribed home oxygen to patients that did not fit the typical criteria for long-term oxygen therapy, with 30 prescribing it for acute relief and 17 for patients with borderline hypoxemia. Conclusions: This study identified the prescription pattern of home oxygen therapy in Korea. Respiratory physicians prescribe home oxygen therapy to hypoxemic COPD patients for at least 15 hours/day, and at a rate of less than 2 L/ min. More research is needed to provide evidence for establishing policies on oxygen therapy in COPD patients.

        Factors Influencing the Acrylamide Content of Fried Potato Products

        BACKGROUND: Acrylamide (CAS No. 79-06-1) is known to be a carcinogenic compound, and is classified as a Group 2A compound by the International Agency for Research on Cancer (IARC, 1994). Acrylamide can be generated during the browning process via the non-enzymatic Maillard reaction of carbohydrates such as reducing sugars and of amino acids such as asparagine, both of which occur at a temperature above $120^{\circ}C$. Potato tubers contain reducing sugars, and thus, this will affect the safety of processed potato products such as potato chips and French fries. In order to reduce the level of acrylamide in potato processed products, it is therefore necessary to understand factors that affect the reducing sugar content of potatoes, such as environmental factors and potato storage conditions, as well as understanding factors affecting acrylamide formation during potato processing itself. METHODS AND RESULTS: Potatoes were cultivated in eight regions of Korea; For each of these different environments, soil physico-chemical characteristics such as pH, electrical conductivity, total nitrogen, available phosphate, and exchangeable cation content were measured and correlations with potato reducing sugar content and potato chip acrylamide levels were examined. The reducing sugar content in potato during storage for three months was determined and acrylamide level in potato chip was analyzed after processing. The storage temperature levels were $4^{\circ}C$, $8^{\circ}C$, or $10^{\circ}C$, respectively. The acrylamide content of chips prepared from potatoes stored at $10^{\circ}C$ or $20^{\circ}C$ for one month was analyzed and the different frying times were 2, 3, 5, and 7 min. CONCLUSION: This study showed that monitoring and controlling the phosphate content within a potato field should be sufficient to avoid producing brown or black potato chips. For potatoes stored at low temperatures, a reconditioning period ($20^{\circ}C$ for 20 days) is required in order to reduce the levels of reducing sugars in the potato and subsequently reduce the acrylamide and improve chip coloration and appearance.

      • The effects of secondary task on the control of vehicle under driving simulator: a study among 50s taxi drivers

        Objective: The aim of this study is to quantitatively evaluate the effects of the secondary task during simulated driving using the variable indicating control of vehicle. Background: There are so many researches for simulated driving and performance. For instance, some researches show that the effects of cell phone conversations on simulated driving. Cell phone conversations impaired explicit recognition memory. Method: Participants included 50s taxi drivers; 15 males and 15 females. All subjects were instructed to keep a distance (30m) from the car ahead and a constant speed (80km/hr or 100km/hr). Sending text message (STM) and Searching navigation (SN) were selected as the secondary task. Experiment consisted of rest for 3min., driving alone for 1min. and driving with secondary task for 1 min.. We were analyzed anterior-posterior coefficient of variation (APCV) and medial-lateral coefficient of variation (MLCV). Results: APCV was increased by 175.4% in STM and 223.8% in SN at 80km/hr. MLCV was increased by 242.3% in STM and 323.4% in SN at 100km/hr. There was no significant difference between genders. Conclusion: Consequently, performing secondary task during driving impaired the control of vehicle.

      • 순환학습모형을 적용한 토의 수업이 중학생의 힘과 운동 개념변화에 미치는 효과

        The purpose of this study was to investigate middle school student's conceptual change about force and motion through the debating work based on the learning Cycle model. Misconceptions about physics concepts are hardly changed into scientific ones. Therefore, teachers have to guide in order that the students have scientific concepts for themselves through cognitive conflict if they have misconceptions. The debating work based on the Learning Cycle model was developed after the analysis of student's misconceptions and implemented to them. 20 students were tested before and after learning through the debating work. The test tools have two kinds of questions, one is asking about the direction of force acting on th ball thrown in the air when it is rising, when at the top, and when falling, the other is asking about the magnitudes and directions of forces acting on a car when it is accelerating, when in uniform velocity, and when decelerating. The results are as follows : In case of vertical motion, 3 students(15%) had scientific conceptions before learning, but after learning 17 students (85%) have scientific ones. In case of horizontal motion, only 1 student(5%) had scientific conceptions before learning, but after learning 11 students (55%) have scientific ones.

      • 家畜排泄物 콤포스트化 有效利用에 關한 硏究

        家畜排泄物의 堆肥化 작업체계의 工學的인 局面은 畜産經營의 畜産物 生産性, 에너지保存, 公害防力및 勞動力 節約에 密接한 關係를 갖고 있는 바이다. 그러나 韓國에 있어서 現在 畜産業의 飼養規模와 飼育頭數는 每年 增加하는 傾向을 나타내고 있으나, 傳統的인 家畜排泄物의 堆肥化處理 加工作業上의 問題點에 關한 硏究資料는 거의 없는 實情으로 堆肥化 작업체계 改善確立이 切實히 要求되고 있다. 따라서, 本硏究의 目的은 家畜排泄物 堆肥化 작업체계상의 工學的인 諸問題點을 調査分析하고 이에 對한 現況을 評價하여 改善方案을 模索하기 위한 基本資料를 提供하기 위해 遂行되었으며 調査分析 結果를 要約하면 다음과 같다. 1. 畜産農家의 家畜排泄物 處分方法으로는 大部分이 自然乾燥, 堆積, 嫌氣的 固形堆肥化에 依하여 遂行되고 있어 家畜糞尿에 의한 公害가 深刻해 지고 있었다. 2. 野積堆肥化 가공작업체계에 있어서는 트랙터에 附着된 前後作動式 로우더가 바람직하다. 3. 好氣性 野積堆肥化 加工作業에 따른 良質의 腐熟堆肥를 얻기 위해서는 外部氣溫에 따라 適正通氣量을 維持하기 위해 3∼7日 間隔으로 混合攪拌해야 되며 氣象條件에 의한 影響을 防止하기 위하여 堆肥作業物質의 表面을 비닐 等으로 덮거나, 더운날에는 加水를 해야할 것이 要望된다. 4. 堆肥化 加工作業體系의 基本段階別로 이루어지는 畜産農家는 거의 찾아 볼 수가 없었다. 5. 堆肥化加工 過程은 畜産農家周圍의 콤포스트센타에서 全體農家가 地力增進과 公害防止를 위해 參與할 수 있는 總合的이고 協同的인 加工作業體系로서 一貫性있게 遂行되어야 할 것으로 思料된다. There was almost no livestock farms to follow the basic stage in composting operation system. Composting disposal process should be performed successively an integrate cooperative operation system at compost center which is participated in all farmers near the livestock farms for the purpose of land improvement and pollution abatement. Well matured compost which is resulted from aerobic windrow solid composting should be turned at 3 to 7 days to insure abequate aeration according to the ambient temperature. The compost material should be covered with vinyl sheets etc. to avoid the effect of climatic condition.

        Cone-beam CT를 이용한 제II급 부정교합 아동의 상기도 형태에 관한 3차원적 연구

        Objective: The aim of this study was to evaluate the volumes and areas of the upper airways in children with Class II malocclusion, using three dimensional cone-beam computed tomography (CBCT) and to compare the volumetric and cross-sectional measurements and cephalometric variables to investigate possible relationships between the upper airway and facial morphology. Methods: CBCT scans were obtained from 37 subjects (17 boys and 20 girls; average age, 11.02 years). The upper airway volumes and areas were measured, and compared with cephalometric variables. Results: The area of the PNS-posterior plane (SPP) was significantly smaller in the Class II malocclusion group (p < 0.05). Also, the volumetric and cross-sectional measurements were lower in Class II than in Class I malocclusion groups, although the differences were not significant between the two groups (p > 0.05). The Class II malocclusion group showed significantly smaller values of PFH, mandibular body length, pog to N perp and showed larger values of FMA, ANB, and facial convexity than the Class I malocclusion group. The volume of the upper airway in front of PNS point (WN) showed negative correlation with ANB (p < 0.05). Conclusions: The Class II malocclusion group had a narrower upper airway associated with a decreased posterior facial height and a divergent growth pattern than the Class I malocclusion group. 성장기 아동의 상기도 협착은 두개안면 성장에 영향을 미칠 수 있다. 본 연구는 3차원 cone-beam computed tomography (CBCT)를 이용하여 제II급 부정교합 아동의 상기도 부피 및 단면적 계측치와 측모두부계측치를 비교하여 상기도와 안면 형태와의 연관성을 알아보고자 하였다. 37명의 아동(남자 17명, 여자 20명, 평균 연령 11.02세)의 CBCT를 촬영하였다. 연구 결과, 상기도의 PNS-posterior plane에서의 단면적(S_(PP))은 제II급 부정교합군에서 유의하게 작았다 (p < 0.05). 상기도 부피 계측치는 제II급 부정교합군에서 제I급 부정교합군보다 작았으나, 두 군 간 통계적으로 유의한 차이는 없었다. 제II급 부정교합군은 후안모고경(PFH), mandibular body length (p < 0.05), Pog to N perp (p < 0.01)에서 제I급 부정교합군보다 유의하게 작은 값을 보였고, FMA (p < 0.05), ANB, facial convexity (p < 0.01)는 유의하게 큰 값을 보였다. 제II급 부정교합군의 PNS 전방의 상기도 부피(W_(N))는 ANB difference 각도와 음의 상관관계를 보였다 (p < 0.05). 제II급 부정교합 아동은 상대적으로 좁은 상기도 부피를 가지며 이에 따른 감소된 후안모고경(PFH)과 하악골의 divergent한 안면 성장 양상을 보였다.
